    Diminishing benefits of urban living for children and adolescents’ growth and development

    AbstractOptimal growth and development in childhood and adolescence is crucial for lifelong health and well-being1–6. Here we used data from 2,325 population-based studies, with measurements of height and weight from 71 million participants, to report the height and body-mass index (BMI) of children and adolescents aged 5–19 years on the basis of rural and urban place of residence in 200 countries and territories from 1990 to 2020. In 1990, children and adolescents residing in cities were taller than their rural counterparts in all but a few high-income countries. By 2020, the urban height advantage became smaller in most countries, and in many high-income western countries it reversed into a small urban-based disadvantage. The exception was for boys in most countries in sub-Saharan Africa and in some countries in Oceania, south Asia and the region of central Asia, Middle East and north Africa. In these countries, successive cohorts of boys from rural places either did not gain height or possibly became shorter, and hence fell further behind their urban peers. The difference between the age-standardized mean BMI of children in urban and rural areas was &lt;1.1 kg m–2 in the vast majority of countries. Within this small range, BMI increased slightly more in cities than in rural areas, except in south Asia, sub-Saharan Africa and some countries in central and eastern Europe. Our results show that in much of the world, the growth and developmental advantages of living in cities have diminished in the twenty-first century, whereas in much of sub-Saharan Africa they have amplified.</jats:p

    Worldwide trends in underweight and obesity from 1990 to 2022: a pooled analysis of 3663 population-representative studies with 222 million children, adolescents, and adults

    Background Underweight and obesity are associated with adverse health outcomes throughout the life course. We estimated the individual and combined prevalence of underweight or thinness and obesity, and their changes, from 1990 to 2022 for adults and school-aged children and adolescents in 200 countries and territories. Methods We used data from 3663 population-based studies with 222 million participants that measured height and weight in representative samples of the general population. We used a Bayesian hierarchical model to estimate trends in the prevalence of different BMI categories, separately for adults (age ≄20 years) and school-aged children and adolescents (age 5–19 years), from 1990 to 2022 for 200 countries and territories. For adults, we report the individual and combined prevalence of underweight (BMI 2 SD above the median). Findings From 1990 to 2022, the combined prevalence of underweight and obesity in adults decreased in 11 countries (6%) for women and 17 (9%) for men with a posterior probability of at least 0·80 that the observed changes were true decreases. The combined prevalence increased in 162 countries (81%) for women and 140 countries (70%) for men with a posterior probability of at least 0·80. In 2022, the combined prevalence of underweight and obesity was highest in island nations in the Caribbean and Polynesia and Micronesia, and countries in the Middle East and north Africa. Obesity prevalence was higher than underweight with posterior probability of at least 0·80 in 177 countries (89%) for women and 145 (73%) for men in 2022, whereas the converse was true in 16 countries (8%) for women, and 39 (20%) for men. From 1990 to 2022, the combined prevalence of thinness and obesity decreased among girls in five countries (3%) and among boys in 15 countries (8%) with a posterior probability of at least 0·80, and increased among girls in 140 countries (70%) and boys in 137 countries (69%) with a posterior probability of at least 0·80. The countries with highest combined prevalence of thinness and obesity in school-aged children and adolescents in 2022 were in Polynesia and Micronesia and the Caribbean for both sexes, and Chile and Qatar for boys. Combined prevalence was also high in some countries in south Asia, such as India and Pakistan, where thinness remained prevalent despite having declined. In 2022, obesity in school-aged children and adolescents was more prevalent than thinness with a posterior probability of at least 0·80 among girls in 133 countries (67%) and boys in 125 countries (63%), whereas the converse was true in 35 countries (18%) and 42 countries (21%), respectively. Interpretation The combined burden of underweight and obesity has increased in most countries, driven by an increase in obesity, while underweight and thinness remain prevalent in south Asia and parts of Africa. A healthy nutrition transition that enhances access to nutritious foods is needed to address the remaining burden of underweight while curbing and reversing the increase in obesity

    Biogeochemical processes and the diversity of NhecolĂąndia lakes, Brazil

    The Pantanal of NhecolĂąndia, the world's largest and most diversified field of tropical lakes, comprises approximately 10,000 lakes, which cover an area of 24,000 kmÂČ and vary greatly in salinity, pH, alkalinity, colour, physiography and biological activity. The hyposaline lakes have variable pHs, low alkalinity, macrophytes and low phytoplankton densities. The saline lakes have pHs above 9 or 10, high alkalinity, a high density of phytoplankton and sand beaches. The cause of the diversity of these lakes has been an open question, which we have addressed in our research. Here we propose a hybrid process, both geochemical and biological, as the main cause, including (1) a climate with an important water deficit and poverty in Ca2+ in both superficial and phreatic waters; and (2) an elevation of pH during cyanobacteria blooms. These two aspects destabilise the general tendency of Earth's surface waters towards a neutral pH. This imbalance results in an increase in the pH and dissolution of previously precipitated amorphous silica and quartzose sand. During extreme droughts, amorphous silica precipitates in the inter-granular spaces of the lake bottom sediment, increasing the isolation of the lake from the phreatic level. This paper discusses this biogeochemical problem in the light of physicochemical, chemical, altimetric and phytoplankton data.<br>O Pantanal da NhecolĂąndia Ă© o maior e mais diversificado campo de lagos da regiĂŁo tropical do planeta, com cerca de 10.000 lagos de variadas salinidade, pH, alcalinidade, cor, fisiografia e atividade biolĂłgica dispostos em uma ĂĄrea de 24.000 kmÂČ. Os lagos hipossalinos tĂȘm pH variĂĄvel, baixa alcalinidade, macrĂłfitas e baixa densidade de fitoplĂąncton. Os lagos salinos tem pH acima de 9 ou 10, elevada alcalinidade, alta densidade de fitoplĂąncton e praias de areia. A causa da diversidade desses lagos Ă© uma questĂŁo ainda em aberto que Ă© abordada nesta pesquisa. PropĂ”e-se como principal causa um processo hĂ­brido, geoquĂ­mico e biolĂłgico, baseado em (1) clima com um dĂ©ficit hĂ­drico importante e pobreza em Ca2+na ĂĄgua superficial e do freĂĄtico e (2) na elevação do pH durante floraçÔes de cianobactĂ©rias. Estes dois aspectos desestabilizam a tendĂȘncia geral de pH neutro para as ĂĄguas superficiais da Terra. Este desequilĂ­brio resulta em aumento do pH e dissolução da areia quartzosa do fundo dos lagos salino-alcalinos. Durante secas extremas hĂĄ precipitação de sĂ­lica amorfa nos espaços inter-granulares dos sedimentos de fundo destes lagos, aumentando seu isolamento do freĂĄtico.     Preliminary investigation of Culicidae species in South Pantanal, Brazil and their potential importance in arbovirus transmission Investigação preliminar das espécies de culicídeos do Pantanal Sul brasileiro e sua potencial importùncia na transmissão de arbovírus

    In view of the high circulation of migratory birds and the environmental and climatic conditions which favor the proliferation of arthropods, the Brazilian Pantanal is susceptible to circulation of arboviruses. However, the amount of data concerning arbovirus vectors in this area is scarce; therefore the aim of this study was to conduct a preliminary investigation of Culicidae species in the NhecolĂąndia Sub-region of South Pantanal, Brazil and their potential importance in the arbovirus transmission. A total of 3684 specimens of mosquitoes were captured, 1689 of which caught in the rainy season of 2007, were divided into 78 pools and submitted to viral isolation, Semi-Nested RT-PCR and Nested RT-PCR, with a view to identifying the most important arboviruses in Brazil. Simultaneously, 70 specimens of ticks found blood-feeding on horses were also submitted to the same virological assays. No virus was isolated and viral nucleic-acid detection by RT-PCR was also negative. Nevertheless, a total of 22 Culicidae species were identified, ten of which had previously been reported as vectors of important arboviruses. The diversity of species found blood-feeding on human and horse hosts together with the arboviruses circulation previously reported suggest that the NhecolĂąndia Sub-region of South Pantanal is an important area for arbovirus surveillance in Brazil.<br>RegiĂ”es como o Pantanal brasileiro, que apresentam fatores como riqueza de fauna silvestre incluindo circulação de aves migratĂłrias e condiçÔes ambientais e climĂĄticas favorĂĄveis Ă  proliferação de artrĂłpodes estĂŁo potencialmente sujeitas Ă  circulação de arbovĂ­rus. Entretanto, poucos trabalhos foram realizados acerca da presença de arbovĂ­rus em potenciais vetores no Pantanal. Neste sentido o principal objetivo deste trabalho foi conduzir uma investigação preliminar para presença de arbovĂ­rus em amostragens de culicĂ­deos capturados na Sub-regiĂŁo da NhecolĂąndia no Pantanal Sul. Um total de 3684 mosquitos foi capturado, dos quais 78 grupos compondo uma amostragem de 1789 espĂ©cimes foram submetidos Ă s tĂ©cnicas de isolamento viral e RT-PCR para os mais importantes arbovĂ­rus no Brasil. Simultaneamente, 70 espĂ©cimes de carrapatos capturados durante hematofagia em cavalos tambĂ©m foram submetidos Ă  pesquisa viral. NĂŁo houve isolamento viral em nenhuma amostra analisada e os resultados de detecção de ĂĄcido nuclĂ©ico viral foram tambĂ©m negativos. Entretanto, foram identificadas 22 espĂ©cies de culicĂ­deos, dez das quais previamente reportadas como vetores de importantes arbovĂ­rus.     Colonization and Succession of Zooplankton After a Drought: Influence of Hydrology and Free-Floating Plant Dynamics in a Floodplain Lake

    We analyzed zooplankton colonization and succession following a temporary drought in a vegetated lake from a floodplain wetland of the Lower Paranå River (Argentina). We took samples of zooplankton and phytoplankton at four habitats of the lake characterized by the presence-absence of emergent or free-floating macrophytes along a 18-month period after the drought. Microzooplankton dominated along the study and macrozooplankton showed seasonal increments in cold seasons. Temporal patterns of broad taxonomic zooplankton groups were associated to hydrology-driven factors, like changes in water depth, conductivity and edible phytoplankton and seasonal changes and not related with free-floating plants. Nevertheless, spatial and temporal patterns of species composition were closely linked to the dynamics of free-floating plants. A clear shift from pelagic to littoral zooplankton taxa was registered when free-floating plants colonized. This study expands the knowledge on the composition and dynamics of zooplankton from floodplain lakes after the disturbance caused by a severe drought. Our results highlight the influence of water level fluctuations as drivers of zooplankton abundance and the joint effect of seasonal changes and free-floating plant dynamics on zooplankton species composition and richness.Fil: Chaparro, Griselda Noemí.     The impact of surgical delay on resectability of colorectal cancer: An international prospective cohort study

    AimThe SARS-CoV-2 pandemic has provided a unique opportunity to explore the impact of surgical delays on cancer resectability. This study aimed to compare resectability for colorectal cancer patients undergoing delayed versus non-delayed surgery.MethodsThis was an international prospective cohort study of consecutive colorectal cancer patients with a decision for curative surgery (January-April 2020). Surgical delay was defined as an operation taking place more than 4 weeks after treatment decision, in a patient who did not receive neoadjuvant therapy. A subgroup analysis explored the effects of delay in elective patients only. The impact of longer delays was explored in a sensitivity analysis. The primary outcome was complete resection, defined as curative resection with an R0 margin.ResultsOverall, 5453 patients from 304 hospitals in 47 countries were included, of whom 6.6% (358/5453) did not receive their planned operation. Of the 4304 operated patients without neoadjuvant therapy, 40.5% (1744/4304) were delayed beyond 4 weeks. Delayed patients were more likely to be older, men, more comorbid, have higher body mass index and have rectal cancer and early stage disease. Delayed patients had higher unadjusted rates of complete resection (93.7% vs. 91.9%, P = 0.032) and lower rates of emergency surgery (4.5% vs. 22.5%, P ConclusionOne in 15 colorectal cancer patients did not receive their planned operation during the first wave of COVID-19. Surgical delay did not appear to compromise resectability, raising the hypothesis that any reduction in long-term survival attributable to delays is likely to be due to micro-metastatic disease
