    About a case report of giant hydronephrosis

    Introduction. Our objective is to report a case of an infrequent entity as the giant hydronephrosis. Case Report. We report the case of an 82-syear-old male referred for a poor general condition. A radiological study revealed a great left hydronephrosis secondary to an urothelial carcinoma. The patient died due to his poor general condition. A histological diagnosis revealed a transitional cell carcinoma of renal pelvis and ureter and atrophic renal parenchyma. Conclusion. Giant hydronephrosis represents a very often entity to be taken into account in cases with big cystic abdominal masses in absence of unilateral or bilateral kidney. Simple nephrectomy is the treatment of choice in most cases. Nevertheless, in cases of nonsubsidiary surgery, percutaneous drainage may be necessary

    Determination of Volatiles in Mouse Urine by Headspace Solid Phase Microextraction and Gas Chromatography-Mass Spectrometry

    The extraction and determination of volatile compounds in mice urine were performed using headspace solid-phase microextraction and gas chromatography-mass spectrometry. In order to optimize the extraction conditions, experimental design was applied. A sample volume of 108 µl, a temperature of 148.6°C, and a time of 94 minutes were found to be the optimal conditions. Samples of male and female mouse urine were analyzed to determine volatile compound profiles. A total of 36 organic compounds including ketones, aldehydes, and terpenes were detected. The results revealed that compounds such as 2-isopropyl-4,5-dihydrothiazole, which is considered a male sexual pheromone, were only detected in male urine samples, whereas others like benzaldehyde were especially abundant in female mouse urine.     Effectiveness of a mental health stepped-care programme for healthcare workers with psychological distress in crisis settings: a multicentre randomised controlled trial

Objective To evaluate the capacity of a mental health intervention in reducing anxiety and depression symptoms in HCWs, relative to enhanced care as usual (eCAU), amidst the COVID-19 pandemic. Methods We conducted an analyst-blind, parallel, multicentre, randomised controlled trial. We recruited HCWs with psychological distress from Madrid and Catalonia (Spain). The intervention arm received a stepped-care programme consisting of two WHO-developed interventions adapted for HCWs: Doing What Matters in Times of Stress (DWM) and Problem Management Plus (PM+). Each intervention lasted 5 weeks and was delivered remotely by non-specialist mental health providers. HCWs reporting psychological distress after DWM completion were invited to continue to PM+. The primary endpoint was self-reported anxiety/depression symptoms (Patient Health Questionnaire-Anxiety and Depression Scale) at week 21. Findings Between 3 November 2021 and 31 March 2022, 115 participants were randomised to stepped care and 117 to eCAU (86% women, mean age 37.5). The intervention showed a greater decrease in anxiety/depression symptoms compared with eCAU at the primary endpoint (baseline-adjusted difference 4.4, 95% CI 2.1 to 6.7; standardised effect size 0.8, 95% CI 0.4 to 1.2). No serious adverse events occurred. Conclusions Brief stepped-care psychological interventions reduce anxiety and depression during a period of stress among HCWs.     Mortality in patients undergoing open aortoiliac surgery : prognostic value of troponin T in tehe inmediate postoperative period

    Producción CientíficaBackground: Patients undergoing open aortoiliac surgery constitute a high-risk subgroup. The aim of this study was to evaluate the relationship between postoperative troponin T (TnT) elevation with the associated postoperative mortality, and mean hospital stay. Methods: This was a prospective observational study of consecutive patients who underwent open aortoiliac surgery during 2006. TnT levels in the first 72 hours after the operation, immediate mortality, postoperative care unit stay, and total postoperative hospital stay were recorded. Statistical analyses were performed with the program SPSS 14.0; the chi-square test (or the Fisher’s exact test) was used for qualitative variables and the ManneWhitney test for quantitative variables. Results: Of the 65 patients included in the study, postoperative TnT was elevated in 14 (21.5%) patients. No significant differences were found in age, sex, hypertension, dyslipidemia, smoking, diabetes mellitus, ischemic heart disease, heart failure, bronchopathy, or renal failure between groups. Mortality in patients with elevated TnT levels was significantly higher (42% compared with 3.92%; relative risk 10.93 ± 0.76; p ¼ 0.001). Likewise, their mean postoperative intensive care unit stay was significantly greater (23.21 ± 6.96 days compared to 2.86 ± 1.96; p < 0.001). This finding resulted in a significantly longer postoperative hospital stay (32.57 ± 25.38 days compared with 12.47 ± 2.21).     The Spanish version of the reflective functioning questionnaire: Validity data in the general population and individuals with personality disorders.

    Introduction Mentalization or reflective functioning (RF) is the capacity to interpret oneself or the others in terms of internal mental states. Its failures have been linked to several mental disorders and interventions improving RF have a therapeutic effect. Mentalizing capacity of the parents influences the children’s attachment. The Reflective Functioning Questionnaire (RFQ-8) is a widely used tool for the assessment of RF. No instrument is available to assess general RF in Spanish-speaking samples. The aim of this study is to develop a Spanish version of the RFQ-8 and to evaluate its reliability and validity in the general population and in individuals with personality disorders. Methods 602 non-clinical and 41 personality disordered participants completed a Spanish translation of the RFQ and a battery of self-reported questionnaires assessing several RF related constructs (alexithymia, perspective taking, identity diffusion and mindfulness), psychopathology (general and specific) and interpersonal problems. Temporal stability was tested in a non-clinical sub-sample of 113 participants. Results Exploratory and confirmatory factor analyses suggested a one-factor structure in the Spanish version of the RFQ-8. RFQ-8 understood as a single scale was tested, with low scorings reflecting genuine mentalizing, and high scorings uncertainty. The questionnaire showed good internal consistence in both samples and moderate temporal stability in non-clinical sample. RFQ correlated significantly with identity diffusion, alexithymia, and general psychopathology in both samples; and with mindfulness, perspective taking, and interpersonal problems in clinical sample. Mean values of the scale were significantly higher in the clinical group. Discussion This study provides evidence that the Spanish version of the RFQ-8, understood as a single scale, has an adequate reliability and validity assessing failures in reflective functioning (i.e., hypomentalization) in general population and personality disorders.The study received a Health Strategic Action fund from the Carlos III Health Institute of the Spanish Ministry of Science and Innovation.     Tratamiento quirúrgico del síndrome de atrapamiento de arteria poplítea : nuestra experiencia

    Producción CientíficaEl síndrome de atrapamiento de la arteria poplítea (SAAP) es una entidad poco frecuente cuya incidencia varía del 0,17 al 3,5 %. Objetivo: Revisamos a los pacientes tratados quirúrgicamente en nuestro servicio durante los últimos 15 años. Material y métodos: Estudio retrospectivo entre 1995 y 2009, en que se registraron variables demográfi cas, comorbilidad, clínica, exploración física, pruebas complementarias, tratamiento quirúrgico realizado, así como evolución a los 15 años. Resultados: Se intervino quirúrgicamente a 8 pacientes (el 50 %, varones) por SAAP con una mediana de edad de 41,5 (intervalo, 16-62) años; 2 pacientes (25 %) presentaron claudicación invalidante, 5 (62,5 %), dolor de reposo y en 1 caso (12,5 %), lesiones trófi cas. La prueba de elección en todos ellos fue la arteriografía, aunque también en 2 casos se realizó TC y en 3, angio-RM. El tratamiento quirúrgico fue miotomía con liberación de la arteria poplítea en 3 pacientes (37,5 %) (en uno de ellos se asoció injerto venoso), miotomía junto a trombectomía poplítea en 2 pacientes (25 %), bypass poplíteo-poplíteo con vena en otros 2 (25 %) y un caso de simpatectomía lumbar (12,5 %). La arteriografía intraoperatoria fue normal. Durante el seguimiento a 15 años (mediana de seguimiento 7,5 ± 3,98 años) el 50 % de los pacientes estaba asintomático, el 25 % presentaba claudicación no invalidante y otro 25 % falleció como consecuencia de su afección concomitante. Conclusiones: En nuestra experiencia, la liberación de la arteria poplítea mediante miotomía del tendón de inserción anómalo, asociada o no a la realización de un bypass poplíteo-poplíteo, es el tratamiento quirúrgico de elección

    Mindfulness-Based Social Cognition Training (SocialMind) for People With Psychosis: A Feasibility Trial

    Introduction: Difficulties in social functioning are common among people with psychosis. Negative symptoms such as blunted affect or social withdrawal are often linked to these difficulties and worsen real-life outcomes. One important dimension associated with social functioning is social cognition, which refers to the psychological processes that are necessary to perceive, encode, store, retrieve, and regulate social information. Mindfulness-based interventions for people with psychosis are safe and effective in improving anxiety and depressive symptoms; however, no mindfulness-based interventions addressing social cognition have yet been developed.Method: A pilot, single-arm, nonrandomized, noncontrolled feasibility trial is proposed. The main objectives are to assess the tolerability of mindfulness-based social cognition training (SocialMind) and to test the feasibility of a further randomized controlled trial.Results: A final sample of 25 outpatients with schizophrenia spectrum disorders was included. Attrition rate was lower than usual for this population, and most participants completed the training. No adverse effects were identified in terms of hospitalizations, emergency room visits, dissociative and psychotic symptoms, or state of anxiety during the sessions.Conclusion: This is the first implementation of SocialMind, which is the first mindfulness-based social cognition training. It is well tolerated by participants with schizophrenia spectrum disorders, and a further randomized controlled trial is proposed for people who have suffered their first episode of psychosis within the past 5 years.Clinical Trial Registration:www.ClinicalTrials.gov, identifier NCT03434405
