    PTU-112 Incidence and predictors of buried bumper syndrome following gastrostomy insertion: a systematic review

    Introduction Gastrostomies are widely used to provide enteral nutrition support. Buried Bumper Syndrome (BBS) is a recognised complication seen following gastrostomy insertion, describing the migration of the internal bumper along the stoma tract towards the skin. The consequences of BBS can be fatal. Currently, there is a paucity of knowledge about the incidence and predictors of BBS, which is addressed in this study by systematically reviewing the medical literature. Method A systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analysis Guidelines (PRISMA). Four databases (EMBASE, MEDLINE, CINHAL, Web of Science) were utilised, and studies identified by searching for key terms relating to gastrostomy and BBS (Jan 1980 -Dec 2016). Unpublished studies were identified from conference abstract booklets. Two independent reviewers screened studies to identify those that included description of adult patients with BBS. Data was extracted relating to patient demographics, incidence, time to presentation and factors associated with BBS. Results 434 studies were initially identified (database search and grey literature). Of these, 299 (68.9%) were excluded following screening review of title and abstract (Kappa coefficient of reviewers=0.753). Of the remaining 135 studies, 85 met inclusion criteria (35 case studies, 11 case series and 39 cohort studies). No controlled trials were retrieved. In total, 16 627 patients were described post gastrostomy placement. 473 cases of BBS were reported in the literature, giving a pooled estimate incidence of 2.8% (0.46%–8.80%). The median age of presentation was 66 years, with 59.1% being in males. BBS was most frequently associated with gastrostomy tubes with thinner internal bumpers (40.7% of all reported cases), however reporting bias may have heavily influenced this outcome. The time to BBS presentation ranged from 3 days to 7 years. Common predictors of BBS reported in the literature include tight external bumpers and increased external traction made directly by the patient or during gastrostomy care. Conclusion This is the first study to systematically review the incidence of Buried Bumper syndrome and assess factors predicting its development. The paucity of high quality studies identified in this systematic review, provides the impetus to develop a national gastrostomy registry accurately assessing gastrostomy outcomes. Healthcare professionals, patients and their caregivers need to be appropriately educated about BBS, considering factors such as tightness of external bumpers and minimising external traction

    The interaction of human microbial pathogens, particulate material and nutrients in estuarine environments and their impacts on recreational and shellfish waters

    Anthropogenic activities have increased the load of faecal bacteria, pathogenic viruses and nutrients in rivers, estuaries and coastal areas through point and diffuse sources such as sewage discharges and agricultural runoff. These areas are used by humans for both commercial and recreational activities and are therefore protected by a range of European Directives. If water quality declines in these zones, significant economic losses can occur. Identifying the sources of pollution, however, is notoriously difficult due to the ephemeral nature of discharges, their diffuse source, and uncertainties associated with transport and transformation of the pollutants through the freshwater–marine interface. Further, significant interaction between nutrients, microorganisms and particulates can occur in the water column making prediction of the fate and potential infectivity of human pathogenic organisms difficult to ascertain. This interaction is most prevalent in estuarine environments due to the formation of flocs (suspended sediment) at the marine-freshwater interface. A range of physical, chemical and biological processes can induce the co-flocculation of microorganisms, organic matter and mineral particles resulting in pathogenic organisms becoming potentially protected from a range of biotic (e.g. predation) and abiotic stresses (e.g. UV, salinity). These flocs contain and retain macro- and micro- nutrients allowing the potential survival, growth and transfer of pathogenic organisms to commercially sensitive areas (e.g. beaches, shellfish harvesting waters). The flocs can either be transported directly to the coastal environment or can become deposited in the estuary forming cohesive sediments where pathogens can survive for long periods. Especially in response to storms, these sediments can be subsequently remobilised releasing pulses of potential pathogenic organisms back into the water column leading to contamination of marine waters long after the initial contamination event occurred. Further work, however, is still required to understand and predict the potential human infectivity of pathogenic organisms alongside the better design of early warning systems and surveillance measures for risk assessment purposes

    Childhood Adversity Moderates Change in Latent Patterns of Psychological Adjustment during the COVID-19 Pandemic: Results of a Survey of U.S. Adults

    Emerging evidence suggests that the consequences of childhood adversity impact later psychopathology by increasing individuals’ risk of experiencing difficulties in adjusting to stressful situations later in life. The goals of this study were to: (a) identify sociodemographic factors associated with subgroups of psychological adjustment prior to and after the onset of the COVID-19 pandemic and (b) examine whether and to what extent types of childhood adversity predict transition probabilities. Participants were recruited via multiple social media platforms and listservs. Data were collected via an internet-based survey. Our analyses reflect 1942 adults (M = 39.68 years); 39.8% reported experiencing at least one form of childhood adversity. Latent profile analyses (LPAs) and latent transition analyses (LTAs) were conducted to determine patterns of psychological adjustment and the effects of childhood adversity on transition probabilities over time. We identified five subgroups of psychological adjustment characterized by symptom severity level. Participants who were younger in age and those who endorsed marginalized identities exhibited poorer psychological adjustment during the pandemic. Childhood exposure to family and community violence and having basic needs met as a child (e.g., food, shelter) significantly moderated the relation between latent profile membership over time. The multilocal programming theory relies on global optimization strategies combined with simple ideas that are inspired in deflection or stretching techniques to avoid convergence to the already detected local minimizers. The most used methods to solve this type of problems are based on stochastic procedures and a population of solutions. In general, population-based methods are computationally expensive but rather reliable in identifying all local solutions. In this chapter, a review on recent techniques for multilocal programming is presented.     Tensor polarization in elastic electron-deuteron scattering in the momentum transfer range 3.8≤Q≤4.6 fm-1

    The tensor polarization of the recoil deuteron in elastic electron-deuteron scattering has been measured at the Bates Linear Accelerator Center at three values of four-momentum transfer Q=3.78, 4.22, and 4.62 fm-1, corresponding to incident electron energies of 653, 755, and 853 MeV. The scattered electrons and the recoil deuterons were detected in coincidence. The recoil deuterons were transported to a liquid hydrogen target to undergo a second scattering. The angular distribution of the d→-p scattering was measured using a polarimeter. The polarimeter was calibrated in an auxiliary experiment using a polarized deuteron beam at the Laboratoire National Saturne. A Monte Carlo procedure was used to generate interpolated calibration data because the energy spread in the deuteron energies in the Bates experiment spanned the range of deuteron energies in the calibration experiment. The extracted values of t20 are compared to predictions of different theoretical models of the electromagnetic form factors of the deuteron: nonrelativistic and relativistic nucleon-meson dynamics, Skyrme model, quark models, and perturbative quantum chromodynamics. Along with the world data the structure functions A(Q) and B(Q) are used to separate the charge monopole and charge quadrupole form factors of the deuteron. A node in the charge monopole form factor is observed at Q=4.39±0.16 fm-1

    Longitudinal double-spin asymmetry and cross section for inclusive neutral pion production at midrapidity in polarized proton collisions at sqrt(s) = 200 GeV

    We report a measurement of the longitudinal double-spin asymmetry A_LL and the differential cross section for inclusive Pi0 production at midrapidity in polarized proton collisions at sqrt(s) = 200 GeV. The cross section was measured over a transverse momentum range of 1 < p_T < 17 GeV/c and found to be in good agreement with a next-to-leading order perturbative QCD calculation. The longitudinal double-spin asymmetry was measured in the range of 3.7 < p_T < 11 GeV/c and excludes a maximal positive gluon polarization in the proton.     High pTp_{T} non-photonic electron production in pp+pp collisions at s\sqrt{s} = 200 GeV

    We present the measurement of non-photonic electron production at high transverse momentum (pT>p_T > 2.5 GeV/cc) in pp + pp collisions at s\sqrt{s} = 200 GeV using data recorded during 2005 and 2008 by the STAR experiment at the Relativistic Heavy Ion Collider (RHIC). The measured cross-sections from the two runs are consistent with each other despite a large difference in photonic background levels due to different detector configurations. We compare the measured non-photonic electron cross-sections with previously published RHIC data and pQCD calculations. Using the relative contributions of B and D mesons to non-photonic electrons, we determine the integrated cross sections of electrons (e++e2\frac{e^++e^-}{2}) at 3 GeV/c<pT< c < p_T <~10 GeV/cc from bottom and charm meson decays to be dσ(Be)+(BDe)dyeye=0{d\sigma_{(B\to e)+(B\to D \to e)} \over dy_e}|_{y_e=0} = 4.0±0.5\pm0.5({\rm stat.})±1.1\pm1.1({\rm syst.}) nb and dσDedyeye=0{d\sigma_{D\to e} \over dy_e}|_{y_e=0} = 6.2±0.7\pm0.7({\rm stat.})±1.5\pm1.5({\rm syst.}) nb, respectively.Comment: 17 pages, 17 figure

    Evolution of the differential transverse momentum correlation function with centrality in Au+Au collisions at sNN=200\sqrt{s_{NN}} = 200 GeV

    We present first measurements of the evolution of the differential transverse momentum correlation function, {\it C}, with collision centrality in Au+Au interactions at sNN=200\sqrt{s_{NN}} = 200 GeV. {\it C} exhibits a strong dependence on collision centrality that is qualitatively similar to that of number correlations previously reported. We use the observed longitudinal broadening of the near-side peak of {\it C} with increasing centrality to estimate the ratio of the shear viscosity to entropy density, η/s\eta/s, of the matter formed in central Au+Au interactions. We obtain an upper limit estimate of η/s\eta/s that suggests that the produced medium has a small viscosity per unit entropy.Comment: 7 pages, 4 figures, STAR paper published in Phys. Lett.
