Continuity of carer â what does it mean and does it matter to midwives and birthing women?
This paper discusses key themes from a large-scale, long-term multi-perspective evaluation of caseload midwifery practice in the United Kingdom (UK). Caseload practice was introduced in several UK settings, on a pilot scale, following the publication of Changing Childbirth intended to put into practice its core principles of continuity, choice and control for women. A range of new midwifery practices were piloted and evaluated across the UK. Relatively little attention was given to defining and exploring the nature and meanings of the new models of practice, or to midwives in this new practice context. This has led to difficulty in comparing the impact of different models of practice.
We draw on two aspects of our work â an ethnographic study of the experiences of midwives (with and without caseloads) and a study of the women's experiences and responses to care â to explore in greater depth the concepts involved in the caseload model and their meaning for women and for midwives in practice. Interviews were conducted with 40 women and 36 caseload midwives. Other forms of data collection, including questionnaires and observation were used and were analysed thematically using grounded theory principles. Themes emerging included autonomy, confidence, reciprocity and relationship: the idea of knowing and being known. On this basis we have argued that continuity is not an end in itself but an important means towards the end of women-centred care and should be considered in relation to other key themes such as autonomy and environment

Technologies of birth and models of midwifery care
This article is based on a study of a reform in the organisation of maternity services in the United Kingdom, which aimed towards developing a more woman-centred model of care. After decades of fragmentation and depersonalisation of care, associated with the shift of birth to a hospital setting, pressure by midwives and mothers prompted government review and a relatively radical turnaround in policy. However, the emergent model of care has been profoundly influenced by concepts and technologies of monitoring. The use of such technologies as ultrasound scans, electronic foetal monitoring and oxytocic augmentation of labour, generally supported by epidural anaesthesia for pain relief, have accompanied the development of a particular ecological model of birth â often called active management â, which is oriented towards the idea of an obstetric norm. Drawing on analysis of womenâs narrative accounts of labour and birth, this article discusses the impact on womenâs embodiment in birth, and the sources of information they use about the status of their own bodies, their labour and that of the child. It also illustrates how the impact on womenâs experiences of birth may be mediated by a relational model of support, through the provision of caseload midwifery care

Dark Matter and Baryons in the Most X-ray Luminous and Merging Galaxy Cluster RX J1347.5-1145
The galaxy cluster RX J1347-1145 is one of the most X-ray luminous and most
massive clusters known. Its extreme mass makes it a prime target for studying
issues addressing cluster formation and cosmology. In this paper we present new
high-resolution HST/ACS and Chandra X-ray data. The high resolution and
sensitivity of ACS enabled us to detect and quantify several new multiply
imaged sources, we now use a total of eight for the strong lensing analysis.
Combining this information with shape measurements of weak lensing sources in
the central regions of the cluster, we derive a high-resolution,
absolutely-calibrated mass map. This map provides the best available
quantification of the total mass of the central part of the cluster to date. We
compare the reconstructed mass with that inferred from the new Chandra X-ray
data, and conclude that both mass estimates agree extremely well in the
observed region, namely within 400 / h_70 kpc of the cluster center. In
addition we study the major baryonic components (gas and stars) and hence
derive the dark matter distribution in the center of the cluster. We find that
the dark matter and baryons are both centered on the BCG within the
uncertainties (alignment is better than <10 kpc). We measure the corresponding
1-D profiles and find that dark matter distribution is consistent with both NFW
and cored profiles, indicating that a more extended radial analysis is needed
to pinpoint the concentration parameter, and hence the inner slope of the dark

Proper Sterol Distribution Is Required for Candida albicans Hyphal Formation and Virulence
Candida albicans is an opportunistic fungus responsible for the majority of systemic fungal infections. Multiple factors contribute to C. albicans pathogenicity. C. albicans strains lacking CaArv1 are avirulent. Arv1 has a conserved Arv1 homology domain (AHD) that has a zinc-binding domain containing two cysteine clusters. Here, we explored the role of the CaAHD and zinc-binding motif in CaArv1-dependent virulence. Overall, we found that the CaAHD was necessary but not sufficient for cells to be virulent, whereas the zinc-binding domain was essential, as Caarv1/Caarv1 cells expressing the full-length zinc-binding domain mutants, Caarv1C3S and Caarv1C28S, were avirulent. Phenotypically, we found a direct correlation between the avirulence of Caarv1/Caarv1, Caarrv1AHD, Caarv1C3S, and Caarv1C28S cells and defects in bud site selection, septa formation and localization, and hyphal formation and elongation. Importantly, all avirulent mutant strains lacked the ability to maintain proper sterol distribution. Overall, our results have established the importance of the AHD and zinc-binding domain in fungal invasion, and have correlated an avirulent phenotype with the inability to maintain proper sterol distribution

"Am iz kwiin" (I'm his queen): Combining interpretative phenomenological analysis with a feminist approach to work with gems in a resource-constrained setting
This article focuses on working with gems using a feminist approach to interpretative phenomenological analysis (IPA) in a resource-constrained setting. The research explores the experiences of maternal disclosure of HIV to children of HIV positive mothers in Kingston, Jamaica. A feminist approach helps recognise power imbalances within research relationships and the womenâs lived experiences. We present three âgemsâ which illuminate womenâs lived experiences and explore how popularised representations of womenâs sexuality and mothering influence disclosure discourses. We use emotion work as a conceptual resource to structure the womenâs narratives and challenge existing policy discourses, which arguably represent disclosure within a binary, rationalist, decision-making framework. This article adds to global literature on maternal HIV disclosure and problematises policy discourses by bringing into relief the emotion work women engage in when deciding if and how to communicate their HIV status to their children. It adds to the body of research using IPA, particularly in resource-constrained settings where IPA has thus far had little application
Estimates of the coverage of parameter space by Latin Hypercube and Orthogonal Array-based sampling
In this paper we use counting arguments to prove that the expected percentage coverage of a d dimensional parameter space of size n when performing k trials with either Latin Hypercube sampling or Orthogonal Array-based Latin Hypercube sampling is the same. We then extend these results to an experimental design setting by projecting onto a t < d dimensional subspace. These results are confirmed by simulations. The theory presented has both theoretical and practical significance in modelling and simulation science when sampling over high dimensional spaces. (C) 2017 Elsevier Inc. All rights reserved

Social richness, socio-technical tension and the virtual commissioning of NHS research
<p>Abstract</p> <p>Background</p> <p>This paper draws on a recent study that evaluated the process of commissioning NHS funded research using virtual committees. Building on an earlier paper that reported our evaluation, here we focus on the effects of asynchronous computer mediated communication (CMC) when used to support group work.</p> <p>Methods</p> <p>To do this the discussion focuses on how CMC affected three key group factors, building relationships, group cohesion and group commitment. The notion of socio-technical tension is elaborated and the paper explores how social richness can act to counter the socially impoverishing and time extending effects of asynchronous CMC.</p> <p>Results</p> <p>We argue that social richness in this context results from the presence of five principal influences. These are: a dynamic range of participant aspirations and personal agendas; participant commitment to and identification with the work and ideals of the group; a rich diversity of social, professional and work-related backgrounds; a website designed to enhance participation and interaction and the mediating effects of an effective chairperson.</p> <p>Conclusion</p> <p>If virtual work groups are to be used by the NHS in the future, then there is a need for more research into the role of social context and its relationship to the effectiveness of newly formed virtual groups. Equally as important are studies that examine the effects of socio-technical interaction on groups undertaking tasks in the real world of work.</p
