    Efficacy of the laser at low intensity on primary burning oral syndrome:a systematic review

    Primary burning mouth syndrome (BMS) is a chronic clinical condition of idiopathic mainly characterized by pain and a burning sensation in the oral cavity. The application of laser at low intensity therapy is a treatment option. This systematic review evaluated the efficacy of laser therapy in treating symptoms of burning mouth syndrome. The study was formulated according to the PRISMA and Cochrane guidelines. Seven databases were used as primary sources of research. Only randomized controlled clinical trials were included. The efficacy of the therapy was estimated comparing the values of the visual and numerical scales of pain before and after laser treatment, through qualitative analysis. The search resulted in 348 records and only eight filled the eligibility criteria and were included. All studies evaluated pain and / or a burning sensation considering a time interval of two to ten weeks. The total sample consisted of 314 patients submitted to treatment: 123 from the control group, who participated with laser off or with the tip blocked, and 191 from the intervention group, treated with low-level laser therapy. The female gender stood out and the average age of the participants was 60.89 years. The main symptoms reported were pain and a burning sensation in the oral mucosa and tongue. The parameters adopted by the authors for laser treatment were diverse and the variables were not fully described in the published studies. Visual analog and numerical scales were used to assess symptoms and only three studies showed statistical significance. It is suggested that laser therapy may be an effective alternative in the treatment of BMS. New randomized clinical trials should consider well-established protocols to better understand the efficacy of laser therapy without confounding the effects

    Nonodontogenic cysts of the oral and maxillofacial region - demographic profile in a Brazilian population over a 40-year period

    The aim of the study was to determine the distribution of histologically diagnosed nonodontogenic cysts (nOCs) over a 40-year period in a Brazilian population. Biopsy records from patients with nOC from the files of the Oral Pathology Service during the period of 1970–2009 were evaluated. Among 10,311 oral biopsies, 58 met the criteria of nOCs. The most frequent nOCs were nasopalatine duct cysts (32.8%), followed by epidermoid cysts (20.7%) and oral lymphoepithelial cysts (17.2%). Nasopalatine duct cysts showed predominance among females (68.4%). Epidermoid cysts were most commonly found in the floor of the mouth (36.4%), tongue (27.3%), and buccal mucosa (27.3%). Oral lymphoepithelial cysts exhibited female prevalence (80.0%) and were commonly located in the tongue (44.4%). The frequency of nOCs found in the population studied here is slightly different from those reported in other case series. Nasopalatine duct cysts, epidermoid cysts, and oral lymphoepithelial cysts were the most common nOCs found, accounting for 70.7% of all nOCs

    Physical properties of the litter in successional forests of interfluvial plateau and floodplain in the lower Guamá river, Eastern Amazonia

    The forest ecosystems of the Amazonia biome are indispensable for climate regulation and service provision. Among them, the fluvial plains (ESFpf) stand out, due to their role in erosion control and water quality. However, the limited information available on this ecosystem is evident when compared to interfluvial plateaus (ESFpi). Thus, this study aimed to evaluate and compare the litter stock and water storage capacity in successional ecosystems in the Eastern Amazonia. The study ecosystems (ESFpf and ESFpi) are located in the municipality of Belém, Pará, in the region of the lower Guamá River. In each ecosystem, 12 permanent plots (100 m²) were installed, where in the periods of higher and lower rainfall, three subunits of litter samples were randomly collected. Water storage in the rainy season did not differ between ecosystems, while in the less rainy season ESFpf (245.91 ± 73.19%) was higher than ESFpi (182.31 ± 77.25%). For the litter stock, there was no influence of seasonality and in both periods the ESFpi averages were higher. Infer that due to the favorable conditions for the action of the edaphic fauna in the ESFpf, there was a reduction in the contact area of the litter, increasing the water storage capacity. The smaller stock found in the ESFpf allow to infer that there was an efficient cycling of nutrients, since these ecosystems are characterized by high litter deposition. Therefore, the results contribute to the understanding of the functional processes of the Amazon river plains ecosystems

    The influence of teriparatide in induced tooth movement: a systematic review

    Teriparatide is a synthetic drug similar than PTH (parathyroid hormone), which is currently used as long-term treatment option for patients with bone chronic diseases, as osteoporosis; and this drug can interfere in a positive way in orthodontic movement. Objectives: The medical literature was assessed in the present systematic review in order to determine the level of scientific evidence supporting the influence of teriparatide in induced tooth movement. The PRISMA Checklist was followed in this systematic review. Four electronic databases (PubMed; Scopus; ScienceDirect; OpenGrey) were searched without implementing restrictions of year, status, and language of publications. The inclusion criteria consisted of selecting only experimental studies comparing the influence of teriparatide in tooth movement of male Wistar rats. The exclusion criteria consisted of experiments with female rats or other experimental animals, and animals with pathologic conditions. The eligible studies were evaluated based on methodological quality. Two trained examiners performed all the research steps. The initial sample comprised 700 studies, which was reduced to 664 after the exclusion of duplicates (n=36). Three articles were selected for the final qualitative analysis. The local administration of parathyroid hormone (PTH) 1-34 or PTH 1-84 revealed major effectiveness when compared with control groups and systematic administration. Additionally, the dilution of PTH 1-34 within methyl cellulose (MC) gel increased the time range for drug release, enabling to reduce the drug concentration without decreasing the effectiveness of tooth movement.     Frequência de condenações de carcaças com lesões sugestivas de tuberculose bovina em abatedouro-frigorífico em Santo Antônio de Jesus-BA e correlação com casos de tuberculose humana

    A tuberculose bovina (TB) é uma doença infecciosa causada pela bactéria Mycobacterium bovis e afeta principalmente o gado, mas também pode infectar outras espécies animais. A tuberculose humana (TH) é uma doença infecciosa e transmissível que afeta principalmente os pulmões humanos. O presente estudo teve como objetivo obter informações sobre a frequência de carcaças condenadas com lesões sugestivas de TB em um abatedouro-frigorífico na cidade de Santo Antônio de Jesus, Bahia, no período de janeiro de 2007 a dezembro de 2016. Além disso, buscou-se verificar estatisticamente a correlação entre a TH e TB nos municípios de origem dos animais abatidos. Os resultados revelaram uma baixa frequência (0,020%) de tuberculose bovina no abatedouro-frigorífico estudado e não foi observada uma correlação positiva significativa (p = 0,312) entre a TB e TH nos parâmetros analisados. O estudo destaca a importância de compreender e controlar a tuberculose bovina e humana, visando melhorar as medidas de prevenção e reduzir os efeitos na saúde pública e na produção agropecuária

    O estudo buscou analisar o comportamento da topologia treliçada (Howe) utilizada em cobertura de pavilhões industriais, para um vão de 20 metros, visando criar uma relação entre disposição geométrica dos elementos da treliça, comprimento total de barras e alteração na distribuição de esforços de acordo com a mudança na distância entre banzos. A geometria foi definida através do programa Autocad 2019, os carregamentos fixados e aplicados sobre os nós do banzo superior por meio do programa Ftool, onde foram analisados e comparados de acordo com as variações da altura da treliça. De acordo com os resultados obtidos nas análises, a variação da altura (distância entre banzos) de uma treliça do modelo Howe de banzo superior inclinado e banzo inferior horizontal, foi possível concluir que o aumento da mesma gera a redução de esforços nas barras, tornando-os mais uniformes nas montantes e diagonais, podendo assim melhorar sua eficiência estrutural, com aumento relativamente pequeno no comprimento das barras

    Expression of sugarcane genes induced by inoculation with Gluconacetobacter diazotrophicus and Herbaspirillum rubrisubalbicans

    Several Brazilian sugarcane varieties have the ability to grow with little addition of inorganic nitrogen fertilizers, showing high contributions of Biological Nitrogen Fixation (BNF). A particular type of nitrogen-fixing association has been described in this crop, where endophytic diazotrophs such as Gluconacetobacter diazotrophicus and Herbaspirillum spp. colonize plant tissues without causing disease symptoms. In order to gain insight into the role played by the sugarcane in the interaction between this plant and endophytic diazotrophs, we investigated gene expression profiles of sugarcane plants colonized by G. diazotrophicus and H. rubrisubalbicans by searching the sugarcane expressed sequence tag SUCEST Database (http://sucest.lad.ic.unicamp.br/en/). We produced an inventory of sugarcane genes, candidates for exclusive or preferential expression during the nitrogen-fixing association. This data suggests that the host plant might be actively involved in the establishment of the interaction with G. diazotrophicus and H. rubrisubalbicans.Diversos genótipos brasileiros de cana-de-açúcar são capazes de crescer com baixa adição de adubos nitrogenados, obtendo elevadas contribuições da Fixação Biológica de Nitrogênio (FBN). Um tipo especial de associação com bactérias fixadoras de nitrogênio foi descrito em cana-de-açúcar, onde as bactérias endofíticas, como Gluconacetobacter diazotrophicus e Herbaspirillum spp., colonizam o interior dos tecidos vegetais, sem causar sintomas de doença. Com o objetivo de tentar entender o papel da cana-de-açúcar nesse tipo de associação, nós investigamos os perfis de expressão gênica de plantas colonizadas pelos diazotróficos endofíticos, usando o banco de dados do SUCEST. Um catálogo com os genes de cana-de-açúcar que são candidatos a se expressar exclusivamente ou preferencialmente durante a associação foi gerado.     Vascular complications in patients undergoing early percutaneous coronary intervention via the femoral artery after fibrinolysis with tenecteplase: registry of 199 patients

    BACKGROUND: Fibrinolysis is often used in the treatment of acute coronary syndromes with ST segment elevation (STEMI). Major cardiac outcomes were reduced with antiplatelet therapy intensification, but with increased risk of bleeding. Our objective was to assess the risk of vascular bleeding in patients undergoing early percutaneous coronary intervention after thrombolysis. METHODS: Between February 2010 and December 2011, five public emergency rooms in the city of São Paulo and the Emergency Health Care Service (Serviço de Atendimento Móvel de Urgência - SAMU) used tenecteplase (TNK) to treat patients with STEMI. Patients were referred to a single tertiary hospital and were submitted to early cardiac catheterization during hospitalization. All examinations were performed via the femoral artery and BARC criteria were used to classify bleeding. RESULTS: We evaluated 199 patients, of whom 193 had no bleeding of vascular origin (group 1) and 6 (3%) developed this complication (group 2). The median time between the administration of the fibrinolytic agent and catheterization was 24 hours in group 1 and 14.7 hours in group 2. According to BARC criteria, 1 patient had type 3a bleeding (hematoma in the inguinal region with a hemoglobin decrease of 3-5 g/dL), 2 patients had type 3b bleeding (1 not related to vascular access and 1 retroperitoneal hematoma with a hemoglobin decrease ≥ 5 g/dL) and the remaining patients had type 1 bleeding (small inguinal hematomas). Blood transfusions were required in 2 patients. None of the patients died due to vascular complications after the intervention. CONCLUSIONS: In our study, early catheterization via the femoral artery as part of a pharmaco-invasive strategy, using TNK as a fibrinolytic agent, had a low vascular bleeding rate, comparable to that of elective angioplasties.INTRODUÇÃO: A fibrinólise é frequentemente utilizada no tratamento das síndromes coronárias com supradesnivelamento do segmento ST (SCCSST). Desfechos cardíacos maiores foram reduzidos com a intensificação do tratamento antiplaquetário, porém com aumento do risco de sangramento. Nosso objetivo foi avaliar o risco de sangramentos de origem vascular em pacientes submetidos a intervenção coronária precoce pós-trombólise. MÉTODOS: Entre fevereiro de 2010 e dezembro de 2011, 5 prontos-socorros municipais da cidade de São Paulo e o Serviço de Atendimento Móvel de Urgência (SAMU) utilizaram tenecteplase (TNK) para tratamento de pacientes com SCCSST. Os pacientes foram encaminhados a um único hospital terciário e submetidos a cateterismo cardíaco precoce durante a internação. Todos os exames foram realizados por via femoral e os critérios do BARC foram utilizados para a classificação dos sangramentos. RESULTADOS: Foram avaliados 199 pacientes, dos quais 193 não apresentaram sangramento de origem vascular (grupo 1) e 6 (3%) evoluíram com essa complicação (grupo 2). A mediana de tempo entre a administração do fibrinolítico e o cateterismo foi de 24 horas no grupo 1 e de 14,7 horas no grupo 2. Segundo os critérios do BARC, 1 paciente apresentou sangramento do tipo 3a (hematoma em região inguinal com queda de hemoglobina de 3-5 g/dl), 2 pacientes apresentaram sangramento do tipo 3b (1 não relacionado ao acesso vascular e 1 hematoma de retroperitônio, com queda de hemoglobina ≥ 5 g/dl), e os demais apresentaram sangramentos do tipo 1 (pequenos hematomas em região inguinal). Nesse grupo foram necessárias duas hemotransfusões. Nenhum paciente teve óbito relacionado à complicação vascular pós-intervenção.     Clinical, Laboratory and Ultrasonographic Findings of Diabetic Dog with Emphysematous Cystitis

    Background: Emphysematous cystitis is a rare inflammatory disease of the lower urinary tract characterized by the accumulation of gas within the wall and lumen of the urinary bladder. The clinical manifestations of emphysematous cystitis resemble those of bacterial cystitis, often hindering the differentiation between the two. In this work, we report a case of emphysematous cystitis in a diabetic dog. The diagnosis of cystitis was followed by ultrasonography for the early detection of emphysematous cystitis, which showed the presence of multifocal and irregular hyperechoic interfaces forming a distal reverberation artifact.                                                                                       Case: A 9-year-old female dog was referred to VETCLINIC Veterinary Hospital 24 hours with a history of urinary incontinence, polydipsia, polyuria, and diabetes mellitus. At first, the blood glucose was measured and found to be 376 mg/dL. Blood count, biochemical measurements of alkaline phosphatase (AF), urea, creatinine, and alanine aminotransferase (AAT), urinalysis, urine culture with antimicrobial susceptibility testing, and abdominal ultrasonography were performed. The hematological exams showed that the serum was lipemic and with hemolysis; the values of AAT, AF, and total plasma proteins were above the reference values; hematocrit was below the normal level; erythrocyte rouleaux and thrombocytosis with platelet aggregates were present. Urinalysis showed the presence of traces of proteins, glucose, and occult blood as well as granular and hyaline cylinders and transitional epithelial cells. In urine culture, growth of the aerobic bacteria Klebsiella pneumoniae was observed, being sensitive to most of the antimicrobials. Ultrasonography showed the presence of gas in the wall of the urinary bladder, besides a discrete thickening of the wall, compatible with the diagnosis of emphysematous cystitis.                                                                                                   Discussion: The first report of emphysematous cystitis in dogs was made in 1926 in a diabetic dog. Emphysematous cystitis is complicated, characterized by the presence of gas in the wall and lumen of the urinary bladder. It is usually reported in patients with diabetes mellitus. The patient presented with a very high glycemic index (376 mg/dL), in addition to having a history of urinary obstruction and presence of bladder stones, which may have acted as predisposing factors for the onset of emphysematous cystitis. In the present case, ultrasonography was the examination of choice. Hyperechoic reverberation-forming lines, identified as gas present in the topography of the urinary bladder, were easily visualized, as described in the literature. For the treatment of this condition, adequate management of the diet and the correct use of antimicrobials are of fundamental importance since the presence of diabetes mellitus in this patient can present serious complications in the future. This report shows the importance of the use of a combination of diagnostic tools to arrive at the correct diagnosis of the patient

    Estimulação cerebral profunda como tratamento na doença de Parkinson : Deep brain stimulation as a treatment in Parkinson's disease

    O presente trabalho teve como objetivo detalhar a utilização da estimulação cerebral profunda (ECP) como estratégia terapêutica no tratamento da doença de Parkinson (DP) avançada. O diagnóstico da DP tem crescido de forma alarmante nas últimas décadas, sendo, atualmente, a principal desordem do movimento e segunda doença neurodegenerativa mais comum no mundo. O tratamento farmacológico para essa doença é bem estabelecido, porém ineficaz ou insuficiente em algumas situações. Nessas condições, outros métodos terapêuticos para tratar as complicações motoras são indicados, entre eles a ECP, que é uma forma de neuromodulação onde eletrodos, conectados a um gerador de pulso, são cirurgicamente implantados em alvos específicos do cérebro permitindo a articulação dos padrões de condução do potencial de ação dos neurônios até determinadas regiões. Trata-se de um recurso terapêutico seguro e efetivo, melhorando disfunções motoras e reduzindo tremores, além de permitir o ajuste adequado para cada indivíduo, restringindo os efeitos colaterais associados à medicação. Ainda, assim, o desenvolvimento de mais pesquisas sobre o tema é de grande importância de forma a tornar esse tratamento mais acessível, com maior previsibilidade no longo prazo, além de reduzir os principais riscos associados
