    L'articolo cerca di restituire il senso della prospettiva ecologica di Ivan Illich, pensatore e critico radicale della modernitĂ , che egli cosidera l'esito del pervertimento del messaggio evangelico, operato dalla Chiesa romana. L'industrializzazione, conepita come effetto ultimo di quel tradimento, condanna l'essere umano a una condizione di dipendenza e subalternitĂ  rispetto alle merci prodotte sul mercato e ai servizi offerti dalle categorie di esperti competenti. In questo modo egli compromette, secondo Illich, il proprio rapporto con gli altri e con la Terra come sua dimora, rinchiudendosi nel guscio della propria individualitĂ , prigioniero dei bisogni creati dal mercato. Ristabilire l'equilibrio ecologico per Illich è possibile solo attraverso un cambiamento sostanziale dei ritmi produttivi, per restituire all'uomo la padronanza degli strumenti e con essa la propria autonomia. The article tries to give the sense of the ecological perspective of Ivan Illich, a radical thinker and critic of modernity, which he considers the result of the evangelical message's perversion, operated by the Roman Church. Industrialization, conceived as the ultimate effect of that betrayal, condemns the human being to a condition of dependence and subordination to the goods produced on the market and the services offered by the categories of competent experts. In this way, according to Illich, he compromises his relationship with others and with the Earth as his home, locking himself in the shell of his own individuality, a prisoner of the needs created by the market. For Illich, restoring the ecological balance is possible only through a substantial change in production rhythms, in order to restore man's mastery of the tools and his own autonomy

    Prevalence of metabolic syndrome and non-alcoholic fatty liver disease in a cohort of italian patients with spinal-bulbar muscular atrophy

    Spinal-bulbar muscular atrophy (SBMA), is an X-linked motor neuron disease caused by a CAG-repeat expansion in the first exon of the androgen receptor gene (AR) on chromosome X. In SBMA, non-neural clinical phenotype includes disorders of glucose and lipid metabolism. We investigated the prevalence of metabolic syndrome (MS), insulin resistance (IR) and non alcoholic fatty liver disease (NAFLD) in a group of SBMA patients. Forty-seven consecutive patients genetically diagnosed with SBMA underwent biochemical analyses. In 24 patients abdominal sonography examination was performed. Twenty-three (49%) patients had fasting glucose above reference values and 31 (66%) patients had a homeostatic model assessment (HOMA-IR) 65 2.6. High levels of total cholesterol were found in 24 (51%) patients, of LDL-cholesterol in 18 (38%) and of triglycerides in 18 (38%). HDL-cholesterol was decreased in 36 (77%) patients. Twenty-four (55%) subjects had 3 or more criteria of MS. A positive correlation (r = 0.52; p < 0.01) was observed between HOMA-IR and AR-CAG repeat length. AST and ALT were above the reference values respectively in 29 (62%) and 18 (38%) patients. At ultrasound examination increased liver echogenicity was found in 22 patients (92 %). In one patient liver cirrhosis was diagnosed. Liver/kidney ratio of grey-scale intensity, a semi-quantitative parameter of severity of steatosis, strongly correlated with BMI (r = 0.68; p < 0.005). Our study shows a high prevalence of IR, MS and NAFLD in SBMA patients, conditions that increase the cardiovascular risk and can lead to serious liver damage, warranting pharmacological and non-pharmacological treatment

    European Union (EU) Member States have made multiple commitments to progress towards universal health coverage (UHC), so that everyone can access quality healthcare without experiencing financial hardship. Yet, significant gaps in all three dimensions of health coverage (population coverage, user charges, and benefits packages) remain. This article highlights some of these gaps, looks at how access to healthcare has been addressed through the EU’s socioeconomic governance and funding instruments, and suggests ways in which the EU can further support national progress towards UHC
