    A Fixed-Bed reactor improved by selective water adsorption LTA zeolites is proposed as an alternative to the conventional methanation reactors. This reactor produces methane from renewable H2 (i.e. surplus electricity) and high CO2 concentration sources, as could be municipal wastes derived biogas (30%v CO2 + 70%v CH4).     La artroplastia de Neer en las fracturas complejas del húmero proximal

    Se estudian los resultados obtenidos después del tratamiento agudo de una serie de 32 pacientes diagnosticados de fractura en cuatro fragmentos del húmero proximal a los que se les implantó una hemiartroplastia tipo Neer I. Se llevó a cabo una evaluación clínica-radiológica de los pacientes y para la evaluación de los resultados se aplicaron los criterios de Neer. Los resultados globales obtenidos son de un 69% entre buenos y regulares y de un 31% con resultado malo. El anclaje de las tuberosidades concreta el mayor índice de complicaciones, que junto a otras totalizan un 18% de los casos. El éxito en dicho anclaje se ha correlacionado significativamente con el resultado funcional (p=0,0019). La técnica meticulosa junto con el adecuado protocolo rehabilitador se señalan como puntos esenciales para el éxito.We study the results obtained after acute treatment of a group of 32 patients diagnosed of four-fragment fracture of the proximal humerus in whom a Neer I hemiarthroplasty was used. The patients were assessed with clinic and radiological parameters, and for the evaluation of the results Neer criteria were used. The global results obtained were 69% of good and fair and 31% of bad results. The tuberosities attachment concentrate the great complication index, that together with other complications acconist for 18% of the cases. The success of the attachment had a positive correlation with functional results (p=0.0019). A meticulous technique joined with a good rehabilitation program is pointed as essential for being successful

    Impact of a transformation from flood to drip irrigation on groundwater recharge and nitrogen leaching under variable climatic conditions

    [EN] The sustainability of agriculture in the Mediterranean climate is challenged by high irrigation water demands and nitrogen fertilizer losses to the environment, causing significant pressure on groundwater resources and groundwater dependent ecosystems. Advanced irrigation technologies and improved fertilizer management have been promoted as key solutions to reduce the agricultural impact on aquatic systems. However, it remains unclear how different irrigation-fertilizer practices perform on the long-term under a highly variable climate, such as the Mediterranean one. Here, we conduct hydrological simulations over a fifty-year period to quantify the magnitude and dynamics of groundwater recharge and nitrogen leaching under five real-case irrigation-fertilizer practices observed in Valencia (eastern Spain). The Valencian Region is the largest citrus-producing region of Europe and current irrigation fertilizer practices reflect the ongoing transformation of irrigation systems from flood to drip irrigation. Our simulations highlight three major implications of the irrigation transformation for groundwater resources. First, the transformation from flood to drip irrigation reduces the recharge fraction (19% vs. 16%) and especially the nitrogen leaching fraction (33%vs. 18%) on the long term. Second, the long-term performance of the two irrigation practices is subject to substantial inter-annual differences controlled by precipitation variability. The sensitivity of recharge and nitrogen leaching to annual meteorological conditions is stronger in drip irrigation, which eventually leads to a similar performance of flood and drip irrigation in wet years if fertilizer inputs are similar. Third, we identify a pronounced year-to-year nitrogen memory in the soil, whereby an enhanced (decreased) nitrogen leaching is observed after anomalously dry (wet) years, affecting the performance of irrigation-fertilizer practices. Overall, the study demonstrates the highly variable nature of the performance of irrigation-fertilizer practices, and the major findings can guide future efforts in designing sustainable water management strategies for agricultural areas with a Mediterranean climate.The authors thank the Coop Research Program on Sustainability in Food Value Chains of the ETH Zurich World Food System Center and the ETH Zurich Foundation for supporting this project.     Analysis of the safety culture in a Cardiology Unit managed by processes

    La cultura sobre seguridad se perfila como uno de los requisitos para evitar la aparición de efectos adversos, sin embargo no se ha estudiado en el ámbito de la cardiología. El objetivo es evaluar la cultura de seguridad en una unidad de cardiología que tiene implantado y certificado un sistema integrado de gestión de calidad y riesgos para la seguridad del paciente. Método: Se realizó un estudio observacional trasversal en 2 años consecutivos utilizando la encuesta Hospital Survey on Patient Safety Culture de la «Agency for Healthcare Research and Quality» en su versión española (42 ítems agrupados en 12 dimensiones) sobre todo el personal. Se comparó el porcentaje de respuestas positivas de cada dimensión en 2014 y 2015, así como con los datos a nivel nacional y en EE. UU., siguiendo las recomendaciones establecidas. Resultados: La valoración global, sobre un máximo de 5, fue de 4.5 en 2014 y de 4.7 en 2015. Identificamos 7 dimensiones como fortaleza. Las peor valoradas fueron: Dotación de personal, Apoyo de la gerencia y Trabajo en equipo entre unidades. La comparación mostró superioridad en todas las dimensiones a nivel nacional, y en 8 respecto a los datos del registro americano. Conclusiones: La cultura de seguridad en una unidad de cardiología con un sistema integrado de gestión de calidad y riesgos y seguridad del paciente es elevada, superior a la nacional en todas sus dimensiones y en la mayoría de ellas respecto al registro de EE. UU.The safety culture is one of the requirements to prevent the occurrence of adverse effects, however has not been studied in the field of cardiology. The objective is to evaluate the safety culture in a cardiology unit has implemented and certified an integrated quality and risk management for patient safety system. Methods: A transversal observational study was made in 2 consecutive years using the survey “Hospital Survey on Patient Safety Culture” of the “Agency for Healthcare Research and Quality” in its Spanish version (42 items grouped into 12 dimensions) in all staff. The percentage of positive responses in each dimension in 2014 and 2015 were compared, as well as national data and United States data, following the established rules. Results: The overall assessment of a possible 5, was 4.5 in 2014 and 4.7 in 2015. We identified seven dimensions as a fortress. The worst rated were: staffing, management support and teamwork between units. The comparison showed superiority in all dimensions respect to national data, and 8 respect to American data. Conclusions: The safety culture in a Cardiology Unit with an integrated quality and risk management and patient safety system is high, higher than the national in all its dimensions and in most of them respect to the United States

    Saturated fatty acid-enriched small extracellular vesicles mediate a crosstalk inducing liver inflammation and hepatocyte insulin resistance

    [Background & Aims]: Lipotoxicity triggers non-alcoholic fatty liver disease (NAFLD) progression owing to the accumulation of toxic lipids in hepatocytes including saturated fatty acids (SFAs), which activate pro-inflammatory pathways. We investigated the impact of hepatocyte- or circulating-derived small extracellular vesicles (sEV) secreted under NAFLD conditions on liver inflammation and hepatocyte insulin signalling. [Methods]: sEV released by primary mouse hepatocytes, characterised and analysed by lipidomics, were added to mouse macrophages/Kupffer cells (KC) to monitor internalisation and inflammatory responses. Insulin signalling was analysed in hepatocytes exposed to conditioned media from sEV-loaded macrophages/KC. Mice were i.v. injected sEV to study liver inflammation and insulin signalling. Circulating sEV from mice and humans with NAFLD were used to evaluate macrophage–hepatocyte crosstalk. [Results]: Numbers of sEV released by hepatocytes increased under NAFLD conditions. Lipotoxic sEV were internalised by macrophages through the endosomal pathway and induced pro-inflammatory responses that were ameliorated by pharmacological inhibition or deletion of Toll-like receptor-4 (TLR4). Hepatocyte insulin signalling was impaired upon treatment with conditioned media from macrophages/KC loaded with lipotoxic sEV. Both hepatocyte-released lipotoxic sEV and the recipient macrophages/KC were enriched in palmitic (C16:0) and stearic (C18:0) SFAs, well-known TLR4 activators. Upon injection, lipotoxic sEV rapidly reached KC, triggering a pro-inflammatory response in the liver monitored by Jun N-terminal kinase (JNK) phosphorylation, NF-κB nuclear translocation, pro-inflammatory cytokine expression, and infiltration of immune cells into the liver parenchyma. sEV-mediated liver inflammation was attenuated by pharmacological inhibition or deletion of TLR4 in myeloid cells. Macrophage inflammation and subsequent hepatocyte insulin resistance were also induced by circulating sEV from mice and humans with NAFLD.     Association between use of enhanced recovery after surgery protocol and postoperative complications in colorectal surgery: the postoperative outcomes within enhanced recovery after surgery protocol (power) study

    Importance: enhanced Recovery After Surgery (ERAS) care has been reported to be associated with improvements in outcomes after colorectal surgery compared with traditional care. Objective: to determine the association between ERAS protocols and outcomes in patients undergoing elective colorectal surgery. Design, setting, and participants: the Postoperative Outcomes Within Enhanced Recovery After Surgery Protocol (POWER) Study is a multicenter, prospective cohort study of 2084 consecutive adults scheduled for elective colorectal surgery who received or did not receive care in a self-declared ERAS center. Patients were recruited from 80 Spanish centers between September 15 and December 15, 2017. All patients included in this analysis had 1 month of follow-up. Exposures: colorectal surgery and perioperative management were the exposures. Twenty-two individual ERAS items were assessed in all patients, regardless of whether they were included in an established ERAS protocol. Main outcomes and measures: the primary study outcome was moderate to severe postoperative complications within 30 days after surgery. Secondary outcomes included ERAS adherence, mortality, readmissions, reoperation rates, and hospital length of stay. Results: between September 15 and December 15, 2017, 2084 patients were included in the study. Of these, 1286 individuals (61.7%) were men; mean age was 68 years (interquartile range [IQR], 59-77). A total of 879 patients (42.2%) presented with postoperative complications and 566 patients (27.2%) developed moderate to severe complications. The number of patients with moderate or severe complications was lower in the ERAS group (25.2% vs 30.3%; odds ratio [OR], 0.77; 95% CI, 0.63-0.94; P¿=¿.01). The overall rate of adherence to the ERAS protocol was 63.6% (IQR, 54.5%-77.3%), and the rate for patients from hospitals self-declared as ERAS was 72.7% (IQR, 59.1%-81.8%) vs non-ERAS institutions, which was 59.1% (IQR, 50.0%-63.6%; P¿<¿.001). Adherence quartiles among patients receiving the highest and lowest ERAS components showed that the patients with the highest adherence rates had fewer moderate to severe complications (OR, 0.34; 95% CI, 0.25-0.46; P¿<¿.001), overall complications (OR, 0.33; 95% CI, 0.26-0.43; P¿<¿.001), and mortality (OR, 0.27; 95% CI, 0.07-0.97; P¿=¿.06) compared with those who had the lowest adherence rates. Conclusions and relevance: an increase in ERAS adherence appears to be associated with a decrease in postoperative complications

    P and R Wave Detection in Complete Congenital Atrioventricular Block

    Complete atrioventricular block (type III AVB) is characterized by an absence of P wave transmission to ventricles. This implies that QRS complexes are generated in an autonomous way and are not coordinated with P waves. This work introduces a new algorithm for the detection of P waves for this type of pathology using non-invasive electrocardiographic surface leads. The proposed algorithm is divided into three stages. In the first stage, the R waves located by a QRS detector are used to generate the RR series and time references for the other stages of the algorithm. In the second stage, the ventricular activity (QT segment) is removed by using an adaptive filter that obtains an averaged pattern of the QT segment. In the third stage, a new P wave detector is applied to the residual signal obtained after QT cancellation in order to detect P wave locations and get the PP time series. Eight Holter records from patients with congenital type III AVB were used to verify the proposed algorithm. Although further improvements should be made to improve the algorithm¿s performance, the results obtained show high average values of sensitivity (90.52 %) and positive prediction (90.98%)

    La evaluación en los grados según el EEES se realiza de forma continua en todas las asignaturas del curso. Desde la implantación de los grados, esta evaluación continua ha sido criticada por la carga de trabajo que supone tanto para el estudiante como el profesorado. En este sentido, hace unos años realizamos un proyecto colaborativo para la realización del calendario de evaluación continua por curso académico. Dicho calendario mostraba las evaluaciones y controles que se realizan en las asignaturas de cada curso y cada semestre, sin tener en cuenta las prácticas. Sin embargo, las actividades de evaluación han ido cambiando y en ocasiones no se detallan en la guía docente con el detalle adecuado, lo que no permite disponer de un calendario de evaluación real del curso. Por tanto, el objetivo de este proyecto ha sido coordinar todas las evaluaciones, controles, y actividades obligatorias o voluntarias de evaluación de todas las asignaturas del segundo curso de la titulación

Dichos datos están impactando en todos los ámbitos de la sociedad y de la economía de tal forma que no se entiende ningún sector productivo ni ninguna relación social sin dato; todos tenemos algún lugar en las redes sociales desde donde intercambiamos experiencias personales o profesionales. Si a este hecho se le suma el auge de la Inteligencia Artificial, se tiene un siglo en el que los avances tecnológicos van a ser totalmente disruptivos para todos nosotros
