Incidence, Clinical Characteristics and Management of Inflammatory Bowel Disease in Spain : Large-Scale Epidemiological Study

    Get PDF
    (1) Aims: To assess the incidence of inflammatory bowel disease (IBD) in Spain, to describe the main epidemiological and clinical characteristics at diagnosis and the evolution of the disease, and to explore the use of drug treatments. (2) Methods: Prospective, population-based nationwide registry. Adult patients diagnosed with IBD-Crohn's disease (CD), ulcerative colitis (UC) or IBD unclassified (IBD-U)-during 2017 in Spain were included and were followed-up for 1 year. (3) Results: We identified 3611 incident cases of IBD diagnosed during 2017 in 108 hospitals covering over 22 million inhabitants. The overall incidence (cases/100,000 person-years) was 16 for IBD, 7.5 for CD, 8 for UC, and 0.5 for IBD-U; 53% of patients were male and median age was 43 years (interquartile range = 31-56 years). During a median 12-month follow-up, 34% of patients were treated with systemic steroids, 25% with immunomodulators, 15% with biologics and 5.6% underwent surgery. The percentage of patients under these treatments was significantly higher in CD than UC and IBD-U. Use of systemic steroids and biologics was significantly higher in hospitals with high resources. In total, 28% of patients were hospitalized (35% CD and 22% UC patients, p < 0.01). (4) Conclusion: The incidence of IBD in Spain is rather high and similar to that reported in Northern Europe. IBD patients require substantial therapeutic resources, which are greater in CD and in hospitals with high resources, and much higher than previously reported. One third of patients are hospitalized in the first year after diagnosis and a relevant proportion undergo surgery

Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, individuals with clonal mosaic events (clonal mosaicism for chromosome alterations and/or loss of chromosome Y) showed an increased risk of COVID-19 lethality

Gasotransmission of nitric oxide (NO) at early plant developmental stages

    Get PDF
Libro: Gasotransmitters in Plants[EN]The versatility of nitric oxide (NO) as a free radical that mediates numerous biological functions within early plant development is widely accepted. NO action in seed germination and root developmental processes involves a complex signaling pathway that includes the cellular redox levels, the posttranslational modification of specific proteins by S-nitrosylation, and the interaction with other plant growth regulators (i.e., phytohormones) using similar molecular com ponents. Recent evidence indicates that changing levels of this reactive nitrogen species (NO) may also fine-tune the molecular mechanisms by which NO leads to changes in seed germination and root growth. This chapter briefly introduces the key processes for the NO transmission during seed germination and root development and focuses on the sensing mechanisms underlying the effects of NO and its interaction with other plant hormones linking these changes.Ministerio de Economía y Competitividad; European Comission; Junta de Castilla y León; Fundación Samuel Solórzano Barrus

The Female Sexual Function Index: Transculturally Adaptation and Psychometric Validation in Spanish Women

    No full text
Background: The evaluation of sexual function is an important outcome in women who suffer some pelvic floor disorders (PFD). The Female Sexual Function Index (FSFI) is the most widely used questionnaire to evaluate the sexual health in female population. This study presents the adaptation and psychometric validation of the FSFI for Spanish women with PFD. Methods: The Spanish version of the FSFI was developed through the forward and backward translation process. The psychometric properties of reliability, validity, responsiveness, and feasibility were conducted in Spanish women with PFD who were assigned to the case or control group (with or without sexual dysfunction respectively). Results: A total of 323 Spanish women with PFD were recruited. The cross-cultural adaptation of the Spanish FSFI achieved a good semantic, conceptual, idiomatic, and content equivalence. The test-retest reliability was shown to be high in all of the cases. The convergent validity showed high results in the domain intercorrelations between each domain and total FSFI. The discriminant validity showed statistically significant differences between sexual dysfunction and control groups. The responsiveness was shown to be moderate to good in the dimensions and excellent in the total FSFI. Conclusions: Spanish FSFI can be used as a reliable, valid, responsive, and feasible instrument for assessing sexual function in women

RADIA -- Radio Advertisement Detection with Intelligent Analytics

Radio advertising remains an integral part of modern marketing strategies, with its appeal and potential for targeted reach undeniably effective. However, the dynamic nature of radio airtime and the rising trend of multiple radio spots necessitates an efficient system for monitoring advertisement broadcasts. This study investigates a novel automated radio advertisement detection technique incorporating advanced speech recognition and text classification algorithms. RadIA's approach surpasses traditional methods by eliminating the need for prior knowledge of the broadcast content. This contribution allows for detecting impromptu and newly introduced advertisements, providing a comprehensive solution for advertisement detection in radio broadcasting. Experimental results show that the resulting model, trained on carefully segmented and tagged text data, achieves an F1-macro score of 87.76 against a theoretical maximum of 89.33. This paper provides insights into the choice of hyperparameters and their impact on the model's performance. This study demonstrates its potential to ensure compliance with advertising broadcast contracts and offer competitive surveillance. This groundbreaking research could fundamentally change how radio advertising is monitored and open new doors for marketing optimization
