    PURPOSE: To investigate genetic, environmental, and systemic risk factors in prospectively identified subjects with the age-related macular degeneration (AMD) phenotypes of (1) reticular pseudodrusen without large soft drusen and (2) large soft drusen without reticular pseudodrusen. DESIGN: Prospective case-case comparison. METHODS: In a clinical practice setting, patients with AMD were sequentially screened using clinical examination and scanning laser ophthalmoscopy imaging to prospectively identify subjects (n = 73) with the phenotypes of (1) reticular pseudodrusen without large soft drusen (n = 30) or (2) large soft drusen without reticular pseudodrusen (n = 43). Subjects were genotyped for 2 alleles associated with AMD, age-related maculopathy susceptibility 2 (ARMS2) and complement factor H (CFH). A questionnaire was administered to collect history of smoking, hypertension, diabetes, and hyperlipidemia, as well as personal and family history of AMD. RESULTS: The reticular pseudodrusen group was older (median age 87 vs 81 years, P = .04) and had more female subjects (83.3% vs 48.8%, P = .003), later ages of AMD onset (83 vs 70 years, P = .0005), and a greater frequency of hypertension (76.7% vs 55.8%, P = .08). No significant differences were found in the distribution of the ARMS2 risk allele (P = .4) between the reticular pseudodrusen (homozygous = 20.0%; heterozygous = 56.7%) and large soft drusen (homozygous = 19.0%; heterozygous = 42.9%) phenotypes, or in the distribution of the CHF risk allele (P = .7) between the reticular pseudodrusen (homozygous = 26.7%; heterozygous = 56.7%) and large soft drusen (homozygous = 21.4%; heterozygous = 66.7%) phenotypes.     Stereotactic radiosurgery for the treatment of recurrent high-grade gliomas: long-term follow-up

    High-grade gliomas (HGG) are the most frequent primary central nervous system tumors; treatment of HCGs includes surgery and post-operative conformal radiotherapy associated with temozolomide (TMZ or procarbazine/lomustine/vincristine [PCV], specifically in patients with anaplastic oligodendrogliomas or anaplastic oligoastrocytomas). However, recurrence is common. Re-irradiation has been utilized in this setting for years and remains a feasible option, although there is always a concern regarding toxicity. Modern high-precision conformal techniques, including stereotactic radiosurgery (SRS), could improve the therapeutic ratio by delivering high biologically equivalent doses while reducing high-dose radiotherapy (RT) to normal brain tissue.     Susceptibility of corythaica cyathicollis costa to a native isolation of beauveria bassiana (vuillemin)

    The research aimed to evaluate a native isolation of B. bassiana under laboratory conditions as a biological alternative control for the eggplant lace bug (C. cyathicollis). The experiments were conducted in the Pathology and Entomology Laboratories at the Universidad de Córdoba, Monteria (Colombia). Initially, the pathogenicity of B. bassiana on adult insects was studied using a concentration of 1·107 spores/mL, in which mortality and intrinsic mortality were determined, in addition, the presented symptomatology was described. Subsequently, the concentrations 1·100 (control treatment), 1·103, 1·105, 1·106, 1·107 and 1·108 spores/mL were evaluated to determine the lethal concentrations of CL50 and CL90 through Probit analysis. The results obtained showed that the native isolation was pathogenic since post-inoculation was observed, between the fifth and sixth days, with 50% mortality (and 87.7% at 12 days), in addition, the intrinsic mortality was 92.4%. The symptomatology showed loss of mobility and lack of appetite between 11 and 12 hours post-inoculation, approximately, and the presence of mycelium 2 days after death. The percentages of mortality for the evaluated concentrations were 0, 46.6, 73.3, 83.3, 90.0 and 96.6% respectively, and the lethal concentrations of CL50 and CL90 were 1.8·103 and 6.5·106 spores/mL respectively. These results indicate the potential of entomopathogenic fungus as an alternative that could be articulated into integrated pest management

    Social policies: inequity, universality and focalization in health within Brazil

    This article aims to contribute to the debate concerning social policies in health and the elaboration of viable strategies for organizing health services, extending them to areas that are not currently covered and consequently, transforming present conditions of population's health. Having these goals in mind, it proposes a reflection on the significance of current policies and their repercussions with respect to the conceptions of universality and focalization. The relevance of health concepts and practices, established over the last few years, towards establishing a decrease in the problems of inequity and poverty is also discussed. This analysis is based on a reflection concerning the experience of implementing the Family Health Program in the central area of the city of Sao Paulo, its repercussions as well as the necessary adjustments made. This, in turn led to broader queries with respect to the characteristics and qualities that should constitute such a program according to the specificities of each region of the country. The paper concludes by affirming that the principle of universality has contributed towards an increase in access to health services, but does not create the conditions for establishing equity, for which a reformulation and adaptation of current health programs and policies are required.O presente artigo tem por finalidade contribuir para o debate sobre as políticas sociais em saúde e para a elaboração de estratégias que viabilizem a organização de serviços de saúde, a extensão da cobertura às regiões desprovidas de serviços e a conseqüente transformação das condições de saúde da população. Para isso, propõe uma reflexão sobre os sentidos das políticas, seus desdobramentos nas concepções da universalização e da focalização, e a pertinência das concepções e práticas em saúde estabelecidas nos últimos anos quanto ao encaminhamento da diminuição dos problemas da desigualdade e da pobreza. Esta análise toma como base a reflexão sobre a experiência de implantação do Programa de Saúde da Família na área central da cidade de São Paulo, seus desdobramentos e ajustes, além do questionamento sobre as características e a qualidade que devem configurar tal programa, de acordo com as especificidades de cada região do país. Conclui-se o estudo com a afirmação de que o princípio da universalidade tem contribuído para o incremento do acesso aos serviços de saúde, porém não tem criado condições para o estabelecimento da eqüidade, necessitando para isso reformulação e adaptação de programas e ações em saúde

    What is the evidence for the clinical value of SBRT in cancer of the cervix?

    AimThe aim of this review is to describe and analyze indications and results of the use of SBRT in uterine cervix cancer, reviewing articles published from January 2010 up to August 2017, for any one of the four indications listed:1Patient refusal or anatomic impediments to interstitial or intracavitary brachytherapy (BCT), i.e. SBRT as an “alternative” for BCT;2Patients with voluminous tumors, or asymmetric tumors where BCT alone would not achieve curative doses, i.e. SBRT as a primary adjunct to BCT;3Pelvic and para aortic adenopathy where SBRT could be used as a boost, i.e. SBRT as a primary adjunct to external beam pelvic radiotherapy;4Small volume recurrences (postoperative or post radiotherapy), i.e. SBRT for salvage.BackgroundCervix cancer standard treatment involves pelvic irradiation and chemotherapy, recent advances in irradiation techniques might offer new possible approaches.Material and methodsSystematic review of the English language literature about Cervix cancer, SBRT, published from January 2010 to January 2018 identified through a database search of PubMed, and Ovid MEDLINE, using pre-defined search phrases.ResultsThe results in the literature, in general, demonstrate rather weak efficacy of SBRT. In this review, we did not find strong evidence to recommend routine SBRT as a primary treatment for cervico-uterine cancers, i.e. as a replacement for BCT; in highly selected cases it might be considered useful as salvage therapy for relapsed cervix cancer.ConclusionThe existing data to not warrant recommending SBRT for the definitive treatment of cervix cancer, but may have some value in the recurrent/relapsed setting

    Concurrent trastuzumab with adjuvant radiotherapy in HER2-positive breast cancer patients: acute toxicity analyses from the French multicentric study

    Background: Trastuzumab (T) combined with chemotherapy has been recently shown to improve outcome in HER2-positive breast cancer (BC). The aim of this study was to evaluate the toxic effects of concurrent radiation therapy (RT) and T administration in the adjuvant setting. Patients and methods: Data of 146 patients with stages II-III HER2-positive BC were recorded. Median age was 46 years. In all, 32 (23%) and 114 (77%) patients received a weekly and a 3-week T schedule, respectively. A median dose of 50 Gy was delivered after surgery. Internal mammary chain (IMC) was irradiated in 103 (71%) patients. Results: Grade >2 dermatitis and esophagitis were noted in 51% and 12%, respectively. According to the Common Toxicity Criteria v3.0 scale and HERA (HERceptin Adjuvant) trial criteria, respectively, 10% and 6% of the patients had a grade ≥2 of left ventricular ejection fraction (LVEF) decrease after RT. Multivariate analyses revealed two independent prognostic factors: weekly T administration (for LVEF decrease) and menopausal status (for dermatitis). Higher level of T cumulative dose (>1600 mg) was only borderline of statistical significance for acute esophagitis toxicity. Conclusion: We showed that weekly concurrent T and RT are feasible in daily clinical practice with, however, a decrease of LVEF. Cardiac volume sparing and patient selections for IMC irradiation are highly recommended. Longer follow-up is warranted to evaluate late toxic effect
