Current status of laboratory and imaging diagnosis of neonatal necrotizing enterocolitis
Necrotizing enterocolitis continues to be a devastating disease process for very low birth weight infants in Neonatal Intensive Care Units. The aetiology and pathogenesis of necrotizing enterocolitis are not definitively understood. It is known that necrotizing enterocolitis is secondary to a complex interaction of multiple factors that results in mucosal damage, which leads to intestinal ischemia and necrosis. Advances in neonatal care, including resuscitation and ventilation support technology, have seen increased survival rates among premature neonates and a concomitant detection in the incidence of this intestinal disease.Diagnosis can be difficult, and identifying infants at the onset of disease remains a challenge. Early diagnosis, which relies on imaging findings, and initiation of prompt therapy are essential to limit morbidity and mortality. Moreover, early management is critical and life-saving.This review summarizes what is known on the laboratory and instrumental diagnostic strategies needed to improve neonatal outcomes and, possibily, to prevent the onset of an overt necrotizing enterocolitis
A bright nanowire single photon source based on SiV centers in diamond
The practical implementation of many quantum technologies relies on the
development of robust and bright single photon sources that operate at room
temperature. The negatively charged silicon-vacancy (SiV-) color center in
diamond is a possible candidate for such a single photon source. However, due
to the high refraction index mismatch to air, color centers in diamond
typically exhibit low photon out-coupling. An additional shortcoming is due to
the random localization of native defects in the diamond sample. Here we
demonstrate deterministic implantation of Si ions with high conversion
efficiency to single SiV- centers, targeted to fabricated nanowires. The
co-localization of single SiV- centers with the nanostructures yields a ten
times higher light coupling efficiency than for single SiV- centers in bulk
diamond. This enhanced photon out-coupling, together with the intrinsic
scalability of the SiV- creation method, enables a new class of devices for
integrated photonics and quantum science.Comment: 15 pages, 5 figure
Update on vaccination of preterm infants: a systematic review about safety and efficacy/effectiveness. Proposal for a position statement by Italian Society of Pediatric Allergology and Immunology jointly with the Italian Society of Neonatology
Preterm infants (PIs) are at increased risk of vaccine-preventable diseases (VPDs). However, delayed vaccination start and low vaccine coverage are still reported. Areas covered: This systematic review includes 37 articles on preterm vaccination published in 2008-2018 in PubMed. Both live attenuated and inactivated vaccines are safe and well tolerated in PIs. Local reactions, apnea, and reactivity changes are the most frequently reported adverse events. Lower gestational age and birth weight, preimmunization apnea, longer use of continuous positive airway pressure (CPAP) are risk factors for apnea. The proportion of PIs who develop protective humoral and cellular immunity is generally similar to full terms although later gestational age is associated with increased antibody IgG concentrations (i.e. against certain pneumococcal serotypes, influenza, hepatitis B virus and poliovirus 1) and increased mononuclear cells proliferation (i.e. after inactivated poliovirus). Expert opinion: PIs can be safely and adequately protected by available vaccines with the same schedule used for full terms. Data at this regard have been retrieved by studies using a 3-dose primary series for pneumococcal and hexavalent vaccines. Further studies are needed regarding the 2 + 1 schedule. Apnea represents a nonspecific stress response in PIs, thus those hospitalized at 2 months should have cardio-respiratory monitoring after their first vaccination
Rhinosinusitis and asthma: a very long engagement
Upper and lower airways may be considered as a unique entity, interested by coexisting inflammatory processes that share common etiopathogenic mechanisms. Previous studies have strongly demonstrated a relationship between rhinosinusitis and asthma. This has led to the introduction of the concept of United Airways , which has also been included in the WHO document Allergic Rhinitis and its Impact on Asthma (ARIA); this concept has important consequences also on the treatment of these disorders. To better summarize the evident connection between upper and lower airway disease we decided to describe it as a multilayered construction, each level pointing out more deeply the relationship between these entities
Ultra-bright and efficient single photon generation based on N-V centres in nanodiamonds on a solid immersion lens
Single photons are fundamental elements for quantum information technologies
such as quantum cryptography, quantum information storage and optical quantum
computing. Colour centres in diamond have proven to be stable single photon
sources and thus essential components for reliable and integrated quantum
information technology. A key requirement for such applications is a large
photon flux and a high efficiency. Paying tribute to various attempts to
maximise the single photon flux we show that collection efficiencies of photons
from colour centres can be increased with a rather simple experimental setup.
To do so we spin-coated nanodiamonds containing single nitrogen-vacancy colour
centres on the flat surface of a ZrO2 solid immersion lens. We found stable
single photon count rates of up to 853 kcts/s at saturation under continuous
wave excitation while having excess to more than 100 defect centres with count
rates from 400 kcts/s to 500 kcts/s. For a blinking defect centre we found
count rates up to 2.4 Mcts/s for time intervals of several ten seconds. It
seems to be a general feature that very high rates are accompanied by a
blinking behaviour. The overall collection efficiency of our setup of up to
4.2% is the highest yet reported for N-V defect centres in diamond. Under
pulsed excitation of a stable emitter of 10 MHz, 2.2% of all pulses caused a
click on the detector adding to 221 kcts/s thus opening the way towards diamond
based on-demand single photon sources for quantum applications

Digital health in the management of allergic diseases
In recent years there has been an important implementation in the medical field of both Mobile Health, such as the use of mobile communication devices, and of other telemedicine tools in general, with the aim of supporting the supervision of diseases from the moment of the first diagnosis to the therapeutic follow-up. In fact, Digital Health can also have a very positive impact on the management of allergic patients, who are known to have the greatest need for regular monitoring, simplifying contact between doctor and patient, but there is still a need to improve implementation regulations, define certification programs and adequate reimbursement systems, as well as to guarantee a high level of attention to the protection of sensitive data. The hope is that one positive outcome of the Covid-19 pandemic will be an acceleration, by all stake-holders involved, of the process of the modernization of health care. (www.actabiomedica.it)
