Schema therapy for emotional dysregulation: Theoretical implication and clinical applications
The term emotional dysregulation refers to an impaired ability to regulate unwanted
emotional states. Scientific evidence supports the idea that emotional dysregulation
underlies several psychological disorders as, for example: personality disorders, bipolar
disorder type II, interpersonal trauma, anxiety disorders, mood disorders and posttraumatic
stress disorder. Emotional dysregulation may derive from early interpersonal
traumas in childhood. These early traumatic events create a persistent sensitization of
the central nervous system in relation to early life stressing events. For this reason,
some authors suggest a common endophenotypical origin across psychopathologies.
In the last 20 years, cognitive behavioral therapy has increasingly adopted an interactiveontogenetic
view to explain the development of disorders associated to emotional
dysregulation. Unfortunately, standard Cognitive Behavior Therapy (CBT) methods are
not useful in treating emotional dysregulation. A CBT-derived new approach called
Schema Therapy (ST), that integrates theory and techniques from psychodynamic and
emotion focused therapy, holds the promise to fill this gap in cognitive literature. In this
model, psychopathology is viewed as the interaction between the innate temperament
of the child and the early experiences of deprivation or frustration of the subject\u2019s
basic needs. This deprivation may lead to develop early maladaptive schemas (EMS),
and maladaptive Modes. In the present paper we point out that EMSs and Modes
are associated with either dysregulated emotions or with dysregulatory strategies that
produce and maintain problematic emotional responses. Thanks to a special focus on
the therapeutic relationship and emotion focused-experiential techniques, this approach
successfully treats severe emotional dysregulation. In this paper, we make several
comparisons between the main ideas of ST and the science of emotion regulation, and
we present how to conceptualize pathological phenomena in terms of failed regulation
and some of the ST strategies and techniques to foster successful regulation in patients
Determining the prevalence of childhood hypertension and its concomitant metabolic abnormalities using data mining methods in the Northeastern region of Hungary
OBJECTIVE: Identifying hypertension in children and providing treatment for it have a marked impact on the patients’ long-term cardiovascular outcomes. The global prevalence of childhood hypertension is increasing, yet its investigation has been rather sporadic in Eastern Europe. Therefore, our goal was to determine the prevalence of childhood hypertension and its concomitant metabolic abnormalities using data mining methods. METHODS: We evaluated data from 3 to 18-year-old children who visited the University of Debrecen Clinical Center’s hospital throughout a 15-year study period (n = 92,198; boys/girls: 48/52%). RESULTS: We identified a total of 3,687 children with hypertension (2,107 boys and 1,580 girls), with a 4% calculated prevalence of hypertension in the whole study population and a higher prevalence in boys (4.7%) as compared to girls (3.2%). Among boys we found an increasing prevalence in consecutive age groups in the study population, but among girls the highest prevalences are identified in the 12-15-year age group. Markedly higher BMI values were found in hypertensive children as compared to non-hypertensives in all age groups. Moreover, significantly higher total cholesterol (4.27 ± 0.95 vs. 4.17 ± 0.88 mmol/L), LDL-C (2.62 ± 0.79 vs. 2.44 ± 0.74 mmol/L) and triglyceride (1.2 (0.85-1.69) vs. 0.94 (0.7-1.33) mmol/L), and lower HDL-C (1.2 ± 0.3 vs. 1.42 ± 0.39 mmol/L) levels were found in hypertensive children. Furthermore, significantly higher serum uric acid levels were found in children with hypertension (299.2 ± 86.1 vs. 259.9 ± 73.3 μmol/L), while glucose levels did not differ significantly. CONCLUSION: Our data suggest that the calculated prevalence of childhood hypertension in our region is comparable to data from other European countries and is associated with early metabolic disturbances. A fluktuáló aszimmetria és a tájhasználat kapcsolatának vizsgálata egy bentikus halfaj populációin
A természetes és természetközeli élőhelyek napjainkban egyre inkább ki vannak téve az antropogén zavarásnak. Tartós emberi beavatkozások eredményeként számos faj élőhelye átalakulhat, fragmentálódhat. A problémát súlyosbítja, hogy az élőhelyek állapotromlása a populációkra közvetlen negatív hatást gyakorol, mely hosszú távon a fajok lokális eltűnéséhez vezethet. Egyes szakmai vélemények szerint a kétoldali szimmetriával rendelkező élőlények esetében az aszimmetria vizsgálata, különös tekintettel a fluktuáló aszimmetriára, korai indikátora lehet a populációkban fellépő negatív hatásoknak (környezeti és genetikai stressz). Megfelelően korai jelzőrendszer kidolgozása létfontosságú felgyorsult világunkban, mely mind több károsodást okoz az élővilágnak, ezért a konzerváció biológia szempontjából döntő jelentőségű a problémák időben történő felismerése. A természetvédelmi szakemberek számáraa fluktuálóaszimmetria ezért hasznos indikátor eszköz lehet a veszélyeztetett helyzetbe került populációk korai észleléséhez.
Kutatásunkban fenékjáró küllő (Gobio gobio) populációk aszimmetriája és az élőhely vízgyűjtőterülete felszínborításának antropogén befolyásoltsága közötti kapcsolatot vizsgáltuk. A küllők morfometriai változóinak manuális mérési adatait rögzítettük, ezeket az orr-szem távolság (DPNE), szemátmérő (ED), bajuszhossz (BL), mellúszó hossza (PecFL) és úszósugár száma (PecFR), az oldalvonal menti (SNLL), az oldalvonal és a hasúszó (SNBLL) valamint az oldalvonal és a hátúszó (SNALL) közötti pikkelyszámok adták. Ezt követően statisztikai modellek segítségével jártuk körül kérdéseinket és kerestük azokra a válaszokat. A mintavételi helyek elkülönülését az aszimmetria érték szerint egyszempontos varianciaanalízisekkel vizsgáltuk (ANOVA és MANOVA). A helyek elkülönülése esetén, hogy mely morfometriai bélyeg aszimmetriája szerint történik az elkülönülés a lineáris diszkriminancia analízis (LDA) adta a választ. A tájhasználat és az aszimmetria közötti összefüggést Pearson-féle korrelációelemzéssel vizsgáltuk
