    Washing practices on the microflora on Georgia-grown cantaloupes.

    In recent years, several foodborne illness outbreaks have been associated with the consumption of cantaloupe. Cantaloupes can be contaminated with pathogens anywhere from the field to the packing line. In the United States, cantaloupes are handled and packed differently in each state. Georgia-grown cantaloupes are brought to sheds, washed, and packed, whereas California-grown cantaloupes are field packed. In this study, the microbiological status of cantaloupes produced by four Georgia growers that use various washing and packing practices was assessed to determine the influence of these different practices. The facilities were visited four times during the harvest season. Aerobic bacteria, Escherichia coli, and coliforms on these Georgia-grown cantaloupes were enumerated in transport trailers, after washing, and after packing. Samples also were analyzed for the presence of Salmonella and E. coli O157:H7. In sheds 1 and 4, a chlorinated dump tank was used to wash melons. In sheds 2 and 3, heated water with chlorine was used in the dump tanks. Although there was a significant reduction (P0.05) in the populations of the aerobic bacteria and E. coli between the transport trailer and the dump tank for sheds 1 and 4, the reduction was less than 0.5 log CFU/cm2. The temperatures of the water in the dump tanks at sheds 2 and 3 were not high enough to effectively reduce the microbial populations evaluated. Populations on the melons increased slightly (0.5 log CFU/cm2) after the melons were removed from the dump tank, suggesting possible contamination after washing

    Platform: What physical attributes separate EM waves, of the enormous band of radio to visible to x-ray, from the high energy narrow band of gamma-ray? From radio to visible to x-ray, telescopes are designed based upon the optical imaging theory; which is an extension of the Huygens-Fresnel diffraction integral. Do we understand the physical properties of gamma rays that defy us to manipulate them similarly? One demonstrated unique property of gamma rays is that they can be converted to elementary particles (electron and positron pair); or a particle-antiparticle pair can be converted into gamma rays. Thus, EM waves and elementary particles, being inter-convertible; we cannot expect to understand the deeper nature of light without succeeding to find structural inter-relationship between photons and particles. This topic is directly relevant to develop a deeper understanding of the nature of light; which will, in turn, help our engineers to invent better optical instruments

    Core sample extractor

    The problem of retrieving and storing core samples from a hole drilled on the lunar surface is addressed. The total depth of the hole in question is 50 meters with a maximum diameter of 100 millimeters. The core sample itself has a diameter of 60 millimeters and will be two meters in length. It is therefore necessary to retrieve and store 25 core samples per hole. The design utilizes a control system that will stop the mechanism at a certain depth, a cam-linkage system that will fracture the core, and a storage system that will save and catalogue the cores to be extracted. The Rod Changer and Storage Design Group will provide the necessary tooling to get into the hole as well as to the core. The mechanical design for the cam-linkage system as well as the conceptual design of the storage device are described

    Perioperative Anticoagulation in Patients with Mechanical Heart Valves Undergoing Elective Surgery: Results of a Survey Conducted among Korean Physicians

    The optimal perioperative anticoagulation management in patients on warfarin therapy is poorly defined due to the lack of randomized trials. Because guidelines are heterogeneous, it was hypothesized that "treatment strategies are not uniform in clinical practice". Between February 2003 and May 2003, a questionnaire with 4 different clinical scenarios was distributed to physicians by e-mail, or direct contact was made by a survey monitor. Two scenarios described the cases of patients with a mechanical heart valve (MHV) in the mitral position, with additional risk factors for a systemic embolism; one undergoing major (scenario 1) and the other minor surgery (scenario 3). Two scenarios described patients with an aortic MHV; one undergoing major (scenario 2) and the other minor (scenario 4) surgery. Different preoperative and postoperative management options were offered. The treatment options for all scenarios were the same. Of the 90 questionnaires distributed, 52 (57.8%) were returned. Hospitalization for full-dose intravenous unfractionated heparin (IV UH) was the most commonly selected strategy in the preoperative phase for scenarios 1 (59%), 2 (42%) and 3 (44%). In scenario 4, 34% chose IV UH. Outpatient, full-dose, subcutaneous UH or low-molecular-weight heparin (LMWH) was the most selected option in the postoperative phase for all scenarios, with the exception of number 4 (52.9% in scenario 1, 34% in scenario 2, 32%, in scenario 3 and 28% in scenario 4). Even among expert clinicians, the management of perioperative anticoagulation is heterogeneous. In particular, the definition of risk categories and the optimal intensity of antithrombotic drugs need to be defined by well-designed prospective studies

    Developing a decision aid to guide public sector health policy decisions: A study protocol

    <p>Abstract</p> <p>Background</p> <p>Decision aids have been developed in a number of health disciplines to support evidence-informed decision making, including patient decision aids and clinical practice guidelines. However, policy contexts differ from clinical contexts in terms of complexity and uncertainty, requiring different approaches for identifying, interpreting, and applying many different types of evidence to support decisions. With few studies in the literature offering decision guidance specifically to health policymakers, the present study aims to facilitate the structured and systematic incorporation of research evidence and, where there is currently very little guidance, values and other non-research-based evidence, into the policy making process. The resulting decision aid is intended to help public sector health policy decision makers who are tasked with making evidence-informed decisions on behalf of populations. The intent is not to develop a decision aid that will yield uniform recommendations across jurisdictions, but rather to facilitate more transparent policy decisions that reflect a balanced consideration of all relevant factors.</p> <p>Methods/design</p> <p>The study comprises three phases: a modified meta-narrative review, the use of focus groups, and the application of a Delphi method. The modified meta-narrative review will inform the initial development of the decision aid by identifying as many policy decision factors as possible and other features of methodological guidance deemed to be desirable in the literatures of all relevant disciplines. The first of two focus groups will then seek to marry these findings with focus group members' own experience and expertise in public sector population-based health policy making and screening decisions. The second focus group will examine issues surrounding the application of the decision aid and act as a sounding board for initial feedback and refinement of the draft decision aid. Finally, the Delphi method will be used to further inform and refine the decision aid with a larger audience of potential end-users.</p> <p>Discussion</p> <p>The product of this research will be a working version of a decision aid to support policy makers in population-based health policy decisions. The decision aid will address the need for more structured and systematic ways of incorporating various evidentiary sources where applicable.</p

    Semantics in active surveillance for men with localized prostate cancer - results of a modified Delphi consensus procedure

    Active surveillance (AS) is broadly described as a management option for men with low-risk prostate cancer, but semantic heterogeneity exists in both the literature and in guidelines. To address this issue, a panel of leading prostate cancer specialists in the field of AS participated in a consensus-forming project using a modified Delphi method to reach international consensus on definitions of terms related to this management option. An iterative three-round sequence of online questionnaires designed to address 61 individual items was completed by each panel member. Consensus was considered to be reached if >= 70% of the experts agreed on a definition. To facilitate a common understanding among all experts involved and resolve potential ambiguities, a face-to-face consensus meeting was held between Delphi survey rounds two and three. Convenience sampling was used to construct the panel of experts. In total, 12 experts from Australia, France, Finland, Italy, the Netherlands, Japan, the UK, Canada and the USA participated. By the end of the Delphi process, formal consensus was achieved for 100% (n = 61) of the terms and a glossary was then developed. Agreement between international experts has been reached on relevant terms and subsequent definitions regarding AS for patients with localized prostate cancer. This standard terminology could support multidisciplinary communication, reduce the extent of variations in clinical practice and optimize clinical decision making.Peer reviewe

    Extremely Red Galaxies at z = 5-9 with MIRI and NIRSpec:Dusty Galaxies or Obscured Active Galactic Nuclei?

    We study a new population of extremely red objects (EROs) recently discovered by the James Webb Space Telescope (JWST) based on their NIRCam colors F277W − F444W &gt; 1.5 mag. We find 37 EROs in the Cosmic Evolution Early Release Science Survey (CEERS) field with F444W &lt; 28 mag and photometric redshifts between 5 &lt; z &lt; 7, with median z = 6.9 − 1.6 + 1.0 . Surprisingly, despite their red long-wavelength colors, these EROs have blue short-wavelength colors (F150W − F200W ∼ 0 mag) indicative of bimodal spectral energy distributions (SEDs) with a red, steep slope in the rest-frame optical, and a blue, flat slope in the rest-frame UV. Moreover, all these EROs are unresolved, point-like sources in all NIRCam bands. We analyze the SEDs of eight of them with MIRI and NIRSpec observations using stellar population models and active galactic nucleus (AGN) templates. We find that dusty galaxies or obscured AGNs provide similarly good SED fits but different stellar properties: massive and dusty, log M ⋆ / M ⊙ ∼ 10 and A V ≳ 3 mag, or low mass and obscured, log M ⋆ / M ⊙ ∼ 7.5 and A V ∼ 0 mag, hosting an obscured quasi-stellar object (QSO). SED modeling does not favor either scenario, but their unresolved sizes are more suggestive of AGNs. If any EROs are confirmed to have log M ⋆ / M ⊙ ≳ 10.5, it would increase the pre-JWST number density at z &gt; 7 by up to a factor ∼60. Similarly, if they are QSOs with luminosities in the L bol &gt; 1045-46 erg s−1 range, their number would exceed that of bright blue QSOs by more than three orders of magnitude. Additional photometry at mid-infrared wavelengths will reveal the true nature of the red continuum emission in these EROs and will place this puzzling population in the right context of galaxy evolution.</p

    Initial Characterization of the FlgE Hook High Molecular Weight Complex of

    The spirochete periplasmic flagellum has many unique attributes. One unusual characteristic is the flagellar hook. This structure serves as a universal joint coupling rotation of the membrane-bound motor to the flagellar filament. The hook is comprised of about 120 FlgE monomers, and in most bacteria these structures readily dissociate to monomers (∼ 50 kDa) when treated with heat and detergent. However, in spirochetes the FlgE monomers form a large mass of over 250 kDa [referred to as a high molecular weight complex (HMWC)] that is stable to these and other denaturing conditions. In this communication, we examined specific aspects with respect to the formation and structure of this complex. We found that the Lyme disease spirochete Borrelia burgdorferi synthesized the HMWC throughout the in vitro growth cycle, and also in vivo when implanted in dialysis membrane chambers in rats. The HMWC was stable to formic acid, which supports the concept that the stability of the HMWC is dependent on covalent cross-linking of individual FlgE subunits. Mass spectrometry analysis of the HMWC from both wild type periplasmic flagella and polyhooks from a newly constructed ΔfliK mutant indicated that other proteins besides FlgE were not covalently joined to the complex, and that FlgE was the sole component of the complex. In addition, mass spectrometry analysis also indicated that the HMWC was composed of a polymer of the FlgE protein with both the N- and C-terminal regions remaining intact. These initial studies set the stage for a detailed characterization of the HMWC. Covalent cross-linking of FlgE with the accompanying formation of the HMWC we propose strengthens the hook structure for optimal spirochete motility

    Cyclic di-GMP is Essential for the Survival of the Lyme Disease Spirochete in Ticks

    Cyclic dimeric GMP (c-di-GMP) is a bacterial second messenger that modulates many biological processes. Although its role in bacterial pathogenesis during mammalian infection has been documented, the role of c-di-GMP in a pathogen's life cycle within a vector host is less understood. The enzootic cycle of the Lyme disease pathogen Borrelia burgdorferi involves both a mammalian host and an Ixodes tick vector. The B. burgdorferi genome encodes a single copy of the diguanylate cyclase gene (rrp1), which is responsible for c-di-GMP synthesis. To determine the role of c-di-GMP in the life cycle of B. burgdorferi, an Rrp1-deficient B. burgdorferi strain was generated. The rrp1 mutant remains infectious in the mammalian host but cannot survive in the tick vector. Microarray analyses revealed that expression of a four-gene operon involved in glycerol transport and metabolism, bb0240-bb0243, was significantly downregulated by abrogation of Rrp1. In vitro, the rrp1 mutant is impaired in growth in the media containing glycerol as the carbon source (BSK-glycerol). To determine the contribution of the glycerol metabolic pathway to the rrp1 mutant phenotype, a glp mutant, in which the entire bb0240-bb0243 operon is not expressed, was generated. Similar to the rrp1 mutant, the glp mutant has a growth defect in BSK-glycerol medium. In vivo, the glp mutant is also infectious in mice but has reduced survival in ticks. Constitutive expression of the bb0240-bb0243 operon in the rrp1 mutant fully rescues the growth defect in BSK-glycerol medium and partially restores survival of the rrp1 mutant in ticks. Thus, c-di-GMP appears to govern a catabolic switch in B. burgdorferi and plays a vital role in the tick part of the spirochetal enzootic cycle.
