Congenital hip dislocation, surgical treatment, evolution for the dysplastic coxarthrosis (A case report)

comparativ cu alte afecţiuni ortopedice congenitale, frecvent (4,
5, 7). Rolul principal în tratamentul acestei patologii, mai ales în
caz de luxaţie de şold, depinde de tactica şi strategia de depistare
precoce şi tratare ortopedică, începând de la cea mai fragedă vârstă
– după naşterea copilului; prin metode funcţionale, gingaşe
şi cruţătoare. Procedeele de reducere şi stabilizare ortezică prin
forţă sunt inadmisibile (2). La nereuşita metodelor conservatoare
de reducere a femurului luxat, se parcurge la intervenţii chirurgicale,
care de asemenea nu trebuie să fie întârziate. Consecinţele
tratamentului tardiv sunt nefavorabile şi, de regulă, nu stopează
dezvoltarea coxartrozei la aceşti pacienţi. Prezentare de caz. Pacienta B., 1968, luxaţie congenitală de
şold pe dreapta(Fig. 1).
La 18. 09. 1973 s-a efectuat intervenţie chirurgicală (I. Marin)
de tip Salter, plastia defectului osului iliac cu alogrefă corticală,
osteotomie de detorsie, varizare şi medializare de femur cu osteosinteza
fragmentelor prin aloştift confecţionat din os cortical (Fig.
2). Postoperator şi la distanţă recuperarea a decurs satisfăcător.
A absolvit şcoala medie, ulterior s-a căsătorit. A născut 2 copii, a
lucrat şi lucrează în gospodărie sătească proprie, la construcţia şi
amenajarea căreia a participat activ personal. 5 ani (1990-1995)
a lucrat fizic la construcţii înafara republicii. De dispensarizare
şi protecţie medicală nu s-a folosit. În a. 2000, treptat, a apărut
disconfort în articulaţia în cauză, dureri, limitarea volumului de
mişcări. Examenul clinic şi radiologic a determinat diagnosticul:
Coxartroză displazică pe dreapta, gr. III (Fig3). În anul 2007 bolnavei
i s-a efectuat endoprotezare totală de şold. În prezent starea
pacientei este satisfăcătoare, activează în gospodărie.
Peste 30 de ani de la prima operaţie pacienta a avut calitatea
vieţii, activitate funcţională şi profesionistă satisfăcătoare. Să fi
fost pe parcurs la evidenţă dispensarială şi supraveghere medicală,
patologia coxartrozică putea fi cu consecinţe mult mai puţin
pronunţate.
Discuţie şi concluzii
Coxartroza constituie 17,8 la 10000 de populaţie matură, din
care 50,0% are geneză displazică (5). Publicaţii de acest gen în
literatura de specialitate sunt numeroase, însă lucrări consacrate
dinamicii evoluţiei proceselor de deteriorare şi recuperare pe
fundalul displaziei în articulaţiile coxofemurale, începând cu nounăscuţii
şi continuând cu persoanele mature, sunt foarte puţine
(1, 2, 3). De facto, specialiştii ortopezi-pediatri sunt la curent cu
dinamica evoluţiei dezvoltării articulaţiilor afectate în perioada
creşterii copilului până la finele perioadei de adolescenţă. Iar ma
Translabyrinthine approach in acoustic neuroma surgery - case report
Introduction. Acoustic neuroma is a benign tumor that arises from the glial Schwann sheath junction of the vestibulocochlear nerve. It has an incidence of 1:100000 population. Despite the fact that this is a rare tumor, it accounts for approximately 6% of all intracranial tumors and 80% of tumors localized in cerebellopontine angle. Treatment of acoustic neuroma is represented by "Wait and Scan" monitoring, radiologic and surgical treatment. The latter remains the primary treatment for acoustic neuroma and consists of 3 main approaches: retrosigmoid approach, middle cranial fossa approach and translabyrinthine approach. Until now, in our country, acoustic neuroma surgery was done only by retrosigmoid approach.
Case presentation. The first translabirinthine surgery for acoustic neuroma in our country was done on 09.12.2021 on a 60-year-old patient who, during preparation for cochlear implant surgery, was accidentally diagnosed with 3rd grade right acoustic neuroma, according to Koos classification. Patient had cophosis on the right ear and moderate hearing loss in the left ear. During the surgery, a gross total resection of the tumor was accomplished. The patient was discharged from the medical institution on 20.12.2021 in a satisfactory condition. Magnetic resonance imaging performed 3 months and 1 year after the surgery showed no complications or tumor remnants.
Conclusion. The current report, which describes an accidental diagnosis of acoustic neuroma during preparation for a cochlear implantation surgery, resulted in acoustic neuroma surgery through the translabyrinthine approach. This serves as an eloquent example of why it is necessary to perform initially a magnetic resonance examination in cases of sensorineural hearing loss or tinnitus. The translabyrinthine approach in acoustic neuroma surgery allows for the removal of tumors of any size without affecting the brain, especially the cerebellum. In our case, where the patient had cophosis on the side of the tumor, this was the most appropriate surgical approach
Evaluation of monitoring of patients with acoustic neurinoma

Obiective. În Statele Unite ale Americii, managementul ”Wait & Scan” al neurinomului de acustic a fost inițiat în anul 1974, în ”Florida Ear and Sinus Center”. Acesta constă în monitorizarea pacienților cu schwanom vestibular, pe parcursul a mai multor ani, prin examen utilizând rezonanță magnetică nucleară. Scopul lucrării a fost de a evalua rezultatele monitorizării pacienților aflați la evidență cu schwanom vestibular. Material și Metode. În cadrul clinicii de Otorinolaringologie a Spitalului Clinic Republican „Timofei Moșneaga”, Chișinău, Republica Moldova, s-au aflat la evidență 6 pacienți cu schwanom vestibular. Pentru stabilirea diagnosticului la acești pacienți, au fost utilizate următoarele metode de diagnostic: audiometria, potențialele evocate auditive precoce, vestibulometria și examenul prin rezonanță magnetică nucleară. Rezultate. Pacienți încadrați în studiu au fost diagnosticați cu următoarele patolgii: în 1 caz – schwanom vestibular intralabirintic, 2 cazuri – schwanom vestibular intracanalicular, 3 cazuri – schwanom vestibular de gradul II. În 5 cazuri, primul simptom a fost hipoacuzia neurosenzorială unilaterală. Conform clasificării Academiei Americane de Otorinolaringologie – Chirugie Cap și Gât, pacienții au fost repartizați în următorul mod: hipoacuzie de clasa D – în 4 cazuri, de clasa C – 1 caz, 1 caz – auzul a fost în limitele normei. Conform examenului prin rezonanță magnetică nucleară, la 2 pacienți cu schwanom de gradul II, formațiunea a fost în creștere, iar în 2 cazuri de schwanom intracanalicular, formațiunea a fost în stagnare. Concluzii. Pacienții cu hipoacuzie neurosensorială unilaterală, de etiologie necunoscută, necesită efectuarea rezonanței magnetice nucleare cu contrast, pentru stabilirea diagnosticului. De asemenea, în urma studiului efectuat s-a stabilit faptul că tumorile ce depășesc conductul auditiv intern manifestă o evoluție negativă, spre deosebire de cele intracanaliculare.Summary.
Objectives. In 1974, in the United States of America, ”Florida Ear and Sinus Center” initiated a ”Wait & Scan” approach for acoustic neurinoma. It consists of monitoring vestibular schwannoma patients, during several years, using magnetic resonance imaging. The aim of our study was to evaluate monitoring results of vestibular schwannoma patients. Material and Methods. In the Otorhinolaryngology clinic of the Republican Clinical Hospital „Timofei Moșneaga”, Chișinău, Republic of Moldova, were monitored 6 patients with vestibular schwannoma. In order to establish the diagnosis in these patients, the following diagnostic methods were used: audiometry, brainstem auditory evoked potential test, vestibulopathy and magnetic resonance imaging exam. Results. In our study, the following diagnoses were established: 1 case of intralabyrinthine vestibular schwannoma, 2 cases of intracanalicular vestibular schwannoma, 3 cases of 2nd grade vestibular schwannoma. Unilateral neurosensorial hypoacusis, in 5 cases, was the first symptom of this pathology. By American Academy of Otolaryngology - Head and Neck Surgery classification system, patients were distributed as follows: 4 cases of class D hypoacusis, 1 case of class C hypoacusis and, in 1 case, hearing was normal. According to the magnetic resonance imaging scan, two 2nd grade vestibular schwannoma patients showed tumor growth, while 2 cases of intracanalicular vestibular schwannoma were in stagnation. Conclusion. It is strongly necessary to perform contrast magnetic resonance imaging scan in patients with unilateral neurosensorial hypoacusis of unknown ethiology, in order to establish the diagnosis. Also, following the study, it was established that tumors that exceed the internal auditory canal, show a higher growth rate than intracanalicular tumors

Open Data from the Third Observing Run of LIGO, Virgo, KAGRA, and GEO
The global network of gravitational-wave observatories now includes five detectors, namely LIGO Hanford, LIGO Livingston, Virgo, KAGRA, and GEO 600. These detectors collected data during their third observing run, O3, composed of three phases: O3a starting in 2019 April and lasting six months, O3b starting in 2019 November and lasting five months, and O3GK starting in 2020 April and lasting two weeks. In this paper we describe these data and various other science products that can be freely accessed through the Gravitational Wave Open Science Center at https://gwosc.org. The main data set, consisting of the gravitational-wave strain time series that contains the astrophysical signals, is released together with supporting data useful for their analysis and documentation, tutorials, as well as analysis software packages
Search for continuous gravitational wave emission from the Milky Way center in O3 LIGO--Virgo data
We present a directed search for continuous gravitational wave (CW) signals
emitted by spinning neutron stars located in the inner parsecs of the Galactic
Center (GC). Compelling evidence for the presence of a numerous population of
neutron stars has been reported in the literature, turning this region into a
very interesting place to look for CWs. In this search, data from the full O3
LIGO--Virgo run in the detector frequency band have been
used. No significant detection was found and 95 confidence level upper
limits on the signal strain amplitude were computed, over the full search band,
with the deepest limit of about at .
These results are significantly more constraining than those reported in
previous searches. We use these limits to put constraints on the fiducial
neutron star ellipticity and r-mode amplitude. These limits can be also
translated into constraints in the black hole mass -- boson mass plane for a
hypothetical population of boson clouds around spinning black holes located in

A Joint Fermi-GBM and Swift-BAT Analysis of Gravitational-wave Candidates from the Third Gravitational-wave Observing Run
We present Fermi Gamma-ray Burst Monitor (Fermi-GBM) and Swift Burst Alert Telescope (Swift-BAT) searches for gamma-ray/X-ray counterparts to gravitational-wave (GW) candidate events identified during the third observing run of the Advanced LIGO and Advanced Virgo detectors. Using Fermi-GBM onboard triggers and subthreshold gamma-ray burst (GRB) candidates found in the Fermi-GBM ground analyses, the Targeted Search and the Untargeted Search, we investigate whether there are any coincident GRBs associated with the GWs. We also search the Swift-BAT rate data around the GW times to determine whether a GRB counterpart is present. No counterparts are found. Using both the Fermi-GBM Targeted Search and the Swift- BAT search, we calculate flux upper limits and present joint upper limits on the gamma-ray luminosity of each GW. Given these limits, we constrain theoretical models for the emission of gamma rays from binary black hole mergers
