Padrão nutricional de doentes cirróticos e de doentes com carcinoma hepatocelular
Tese de mestrado, Nutrição Clínica, Universidade de Lisboa, Faculdade de Medicina, 2016Introdução: A cirrose, de qualquer etiologia, é reconhecida como o principal fator de risco para o desenvolvimento de carcinoma hepatocelular (CHC). Contudo, informações recentes têm vindo a sugerir que determinados estilos de vida e/ou padrões alimentares poderão ter algum impacto na sua prevenção e prognóstico.
Objetivos: Avaliar e comparar o consumo alimentar de doentes cirróticos com e sem carcinoma hepatocelular, por forma a averiguar a existência de diferenças significativas entre o padrão alimentar dos grupos em estudo, procurando determinar se existe uma relação entre os hábitos de consumo alimentar e os estadios de cirrose e carcinoma hepatocelular.
Materiais e Métodos: As informações foram recolhidas utilizando uma Tabela de registo e um Questionário de Frequência Alimentar, sendo os dados posteriormente analisados recorrendo ao software IBM SPSS Statistics.
Resultados: No total foram avaliados 39 indivíduos, divididos em 2 grupos, com 21 cirróticos sem CHC e 18 com carcinoma hepatocelular. Verificou-se que 29 doentes são do género masculino e 10 do género feminino, com uma média de idades de 64 anos, e que a principal etiologia da doença hepática é o álcool, seguida pela hepatite C. Relativamente ao padrão alimentar observaram-se apenas diferenças significativas no consumo de vitamina C e frutas, com os doentes com CHC a consumir quantidades significativamente inferiores aos doentes cirróticos sem carcinoma.
Conclusões: Nesta amostra, apesar da dimensão reduzida, verificamos que num grupo de cirróticos, os doentes que desenvolveram CHC têm menor ingestão de Vitamina C e Frutas.Introduction: Cirrhosis of any etiology is recognized as the major risk factor for the development of hepatocellular carcinoma (HCC). However, recent data have suggested that lifestyle and eating patterns may have an impact on prevention and prognosis.
Objectives: To compare the food intake of cirrhotic patients with and without HCC, in order to evaluate if there were significant differences between the dietary patterns of the studied groups, seeking to determine whether there is a relationship between food habits and the presence of HCC.
Materials and Methods: The information was assembled using a Registration table and a Food Frequency Questionnaire, with the data being analyzed by the IBM SPSS Statistics software.
Results: In total 39 subjects were evaluated, divided into 2 groups, with 21 cirrhotic without evidence of HCC and 18 with HCC. It was found that 29 patients were male and 10 were female, with a mean age of 64 years, and that the main cause of liver disease is alcohol, followed by chronic hepatitis C. Regarding the dietary patterns, there were only observed significant differences in the consumption of vitamin C and fruits, with patients with hepatocellular carcinoma eating significantly lower amounts than cirrhotic patients.
Conclusions: In this sample, despite the small size, we found that in a cirrhotic group, the patients who presented with HCC have a lower intake of Vitamin C and Fruits
Síndrome de Torsades de Pointes: análise de casos: Torsades de Pointes Syndrome: case analysis
A Síndrome de Torsades de Pointes (TdP) é uma taquiarritmia ventricular polimórfica de pacientes com um intervalo QT longo congênito ou induzido por fármacos, cujo eletrocardiograma possui aspecto de “torção das pontas” e os sinais e sintomas característicos são síncope, palpitação ou mesmo evolução para fibrilação ventricular e morte súbita. O sexo mais frequentemente acometido é o feminino, o diagnóstico se baseia no eletrocardiograma e o tratamento preconizado é o sulfato de magnésio (MgSO4) intravenoso, a correção dos distúrbios eletrolíticos, principalmente a hipocalemia e o tratamento da causa base, na TdP farmacoinduzida. O objetivo do estudo é analisar os casos de Síndrome de Torsades de Pointes em pacientes com alterações do intervalo QT no eletrocardiograma. Trata-se de uma revisão bibliográfica integrativa, do tipo quantitativa, que utilizou as plataformas do PubMed, SciELO e Cochrane Library como bases de dados para seleção dos artigos, todos na língua inglesa. Foram utilizadas literaturas publicadas com recorte temporal de 2017 a 2022. De acordo com as literaturas analisadas, conclui-se que a TdP é uma taquiarritmia ventricular polimórfica com um mau prognóstico se não tratada precocemente com o MgSO4 intravenoso e, por ter diversas etiologias, é primordial que o diagnóstico preciso seja estabelecido de forma rápida, devido ao alto índice de mortalidade. Pacientes portadores da síndrome do QT longo congênita, bradicardia sinusal e bloqueio atrioventricular de 1º grau possuem predisposição para o desenvolvimento de TdP. Observa-se escassez na literatura a respeito das formas adequadas de prevenção da TdP, já que muitos pacientes que participam das triagens, muitas das vezes inefetivas, adquirem a síndrome após o uso de drogas que a predispõem, com prolongamento do intervalo QT, ou não sabem que possuem uma SQTL pré-existente, obrigatória para o desenvolvimento da TdP
SARS-CoV-2 introductions and early dynamics of the epidemic in Portugal
Genomic surveillance of SARS-CoV-2 in Portugal was rapidly implemented by
the National Institute of Health in the early stages of the COVID-19 epidemic, in collaboration
with more than 50 laboratories distributed nationwide.
Methods By applying recent phylodynamic models that allow integration of individual-based
travel history, we reconstructed and characterized the spatio-temporal dynamics of SARSCoV-2 introductions and early dissemination in Portugal.
Results We detected at least 277 independent SARS-CoV-2 introductions, mostly from
European countries (namely the United Kingdom, Spain, France, Italy, and Switzerland),
which were consistent with the countries with the highest connectivity with Portugal.
Although most introductions were estimated to have occurred during early March 2020, it is
likely that SARS-CoV-2 was silently circulating in Portugal throughout February, before the
first cases were confirmed.
Conclusions Here we conclude that the earlier implementation of measures could have
minimized the number of introductions and subsequent virus expansion in Portugal. This
study lays the foundation for genomic epidemiology of SARS-CoV-2 in Portugal, and highlights the need for systematic and geographically-representative genomic surveillance.We gratefully acknowledge to Sara Hill and Nuno Faria (University of Oxford) and

ON THE STUDY OF THE ORGANIC CONTENT OF EARLY EMPIRE CERAMICS FROM BEJA’S CASTLE - A PRELIMINARY REPORT
This manuscript describes the analysis of the
organic contents of Early Empire ceramics from Beja’s
Castle.
A combination of extraction procedures was used
in order to extract the highest number of compounds
from the studied ceramics. Pinaceae resin biomarkers
were identified in all the ceramics, confirming the
widely use of these resins by the Romans. Both animal
and plant steroids were identified in the samples,
and Δ5-avenasterol, found in all the samples, is of
particular significance, as it can be used as a marker
for oil seed crops and olive oil

Previous issue date: 2013-01O objetivo deste estudo foi avaliar o efeito
antioxidante do extrato hidroalcoolico da casca do pequi
(EHCP) em ratos após a administração de doxorrubicina
(DOX). Foram utilizados ratos da raça Wistar, distribuídos em
quatro grupos, sendo que os animais do G1 (n=6) receberam
água e solução salina (grupo controle), G2 (n=7) EHCP e
solução salina, G3 (n=7) água e DOX e G4 (n=6) EHCP e
DOX. O EHCP foi administrado por gavagem durante 10 dias
aos ratos dos grupos G2 e G4 e água aos dos G1 e G3. DOX na
dose de 10mg kg-1 e solução salina 0,9% foram administradas
por via intravenosa no dia sete após o início do experimento
aos animais de G3 e G4 e aos de G1 e G2, respectivamente.
Foram avaliados peso e taxa de mortalidade. Dez dias após o
início do experimento, foi avaliada a concentração sérica de
creatina quinase MB (CK-MB), troponina e mioglobina, e os
ratos foram submetidos à eutanásia e à avaliação
anatomopatológica. Não houve diferença entre os tratamentos
quanto ao peso dos animais (P 0,05). Com relação à taxa de
mortalidade, houve aumento no grupo 2 (P<0,05). Os
resultados do teste qualitativo para a detecção de CK-MB,
troponina I e mioglobina nos quatro grupos foram negativos
e não foram observadas alterações macroscópicas nos órgãos
dos ratos dos diferentes grupos. Constatou-se necrose tubular
aguda multifocal de intensidade moderada a acentuada nas
regiões cortical e medular nos rins de todos os ratos avaliados.
A DOX em dose única de 10mg kg-1 e via intravenosa não
promove alterações cardíacas em ratos e o EHCP nas condições
avaliadas aumenta o índice de mortalidade em ratos, o que
pode estar relacionado a substâncias tóxicas presentes na casca
desse fruto.The objective of this study was to evaluate the
antioxidant effect of the hydroalcoholic extract of pequi peel
(HEPP) in rats after administration of doxorubicin (DOX).
Were used 26 Wistar rats divided into four groups, which G1
(n=6) received water and saline solution (control group), G2
(n=7) HEPP and saline solution, G3 (n=7) water and DOX,
and G4 (n=6) HEPP and DOX. The HEPP was administered
by gavage for 10 days to G2 and G4 and water to G1 and G3.
DOX and saline solution were administered intravenously on
day seven after the start of the experiment, with the DOX (10mg
kg-1) applied in G3 and G4, and saline solution 0.9% in G1
and G2. Were evaluated weight and mortality rate. Ten days
after the start of the experiment were evaluated creatina kinase
MB (CK-MB), troponin and myoglobin, and the rats were
euthanized and evaluated morphologically. There was no
difference between treatments in weight of animals (P>0.05).
About the mortality rate an increase in group 2 was showed
(P<0.05). The results of the qualitative test for the detection of
CK-MB, troponin I and myoglobin in the four groups were
negative and there were no macroscopic changes in different
rat’s organs of different groups. Multifocal and moderate to
severe acute tubular necrosis in cortical and medullary regions
of the kidneys was observed in all rats studied. DOX intravenous
and in a one dose of 10mg kg-1 don’t induce cardiac changes
in rats and the HEPP in conditions here evaluated increase the
rate of mortality of rats, which may be related to toxic substances
in the peel of this fruit

Development and validation of the MMCD score to predict kidney replacement therapy in COVID-19 patients
Abstract Background Acute kidney injury (AKI) is frequently associated with COVID-19, and the need for kidney replacement therapy (KRT) is considered an indicator of disease severity. This study aimed to develop a prognostic score for predicting the need for KRT in hospitalised COVID-19 patients, and to assess the incidence of AKI and KRT requirement. Methods This study is part of a multicentre cohort, the Brazilian COVID-19 Registry. A total of 5212 adult COVID-19 patients were included between March/2020 and September/2020. Variable selection was performed using generalised additive models (GAM), and least absolute shrinkage and selection operator (LASSO) regression was used for score derivation. Accuracy was assessed using the area under the receiver operating characteristic curve (AUC-ROC). Results The median age of the model-derivation cohort was 59 (IQR 47–70) years, 54.5% were men, 34.3% required ICU admission, 20.9% evolved with AKI, 9.3% required KRT, and 15.1% died during hospitalisation. The temporal validation cohort had similar age, sex, ICU admission, AKI, required KRT distribution and in-hospital mortality. The geographic validation cohort had similar age and sex; however, this cohort had higher rates of ICU admission, AKI, need for KRT and in-hospital mortality. Four predictors of the need for KRT were identified using GAM: need for mechanical ventilation, male sex, higher creatinine at hospital presentation and diabetes. The MMCD score had excellent discrimination in derivation (AUROC 0.929, 95% CI 0.918–0.939) and validation (temporal AUROC 0.927, 95% CI 0.911–0.941; geographic AUROC 0.819, 95% CI 0.792–0.845) cohorts and good overall performance (Brier score: 0.057, 0.056 and 0.122, respectively). The score is implemented in a freely available online risk calculator ( https://www.mmcdscore.com/ ). Conclusions The use of the MMCD score to predict the need for KRT may assist healthcare workers in identifying hospitalised COVID-19 patients who may require more intensive monitoring, and can be useful for resource allocation

III Encontro Nacional de História das Ciências e da Tecnologia. O Centro de Estudos de História e Filosofia da Ciência, organiza o 3.º Encontro Nacional de História da Ciência e da Técnica, sob o tema «Ciência, Crise e Mudança» que tem lugar na Universidade de Évora, nos dias 26, 27 e 28 de Setembro de 2012.
O Primeiro Encontro Nacional de História da Ciência teve lugar em 21 e 22 Julho de 2009, no seguimento do programa de estímulo ao de¬senvolvimento da História da Ciência em Portugal e de valorização do património cultural e científico do País, lançado pelo Ministério da Ciência, Tecnologia e Ensino Superior (MCTES) em 31 de Janeiro desse ano. A sua organização coube a investigadores do Instituto de História Contemporânea (IHC), da FCSH da UNL, e do Centro Científico e Cultural de Macau (CCCM), em cujas instalações se realizou. De en¬tre as conclusões do Encontro, destacou-se a de realizar periodicamen¬te novos Encontros Nacionais, a serem organizados de forma rotativa por diferentes centros e núcleos de investigadores. Na sequência deste Primeiro Encontro, o Centro Interuniversitário de História das Ciências e da Tecnologia (CIUHCT) organizou, entre 26 e 28 de Julho de 2010, o II Encontro, dedicado ao tema “Comunicação das Ciências e da Tecnologia em Portugal: Agentes, Meios e Audiências”.
Cabe agora ao CEHFCi cumprir o que foi decidido no final deste Encontro. Na situação económica e política que hoje vivemos torna-se particularmente urgente aprofundar o estudo e o debate sobre a interação entre a Sociedade, a Ciência e a sua História.
