    Substitution and sameness: two components of a relational conception of the equals sign

    A sophisticated and flexible understanding of the equals sign is important for arithmetic competence and for learning further mathematics, particularly algebra. Research has identified two common conceptions held by children: the equals sign as an operator, and the equals sign as signalling the same value on both sides of the equation. We argue here that as well as these two conceptions, the notion of substitution is also an important part of a sophisticated understanding of mathematical equivalence. We provide evidence from a cross-cultural study in which English and Chinese children were asked to rate the “cleverness” of operational, sameness and substitutive definitions of the equals sign. A Principle Components Analysis revealed the substitutive items were distinct from the sameness items. Furthermore, Chinese children rated the substitutive items as ‘very clever’, whereas the English children rated them as ‘not so clever’, suggesting that the notion of substitution develops differently across the two countries. Implications for developmental models of children’s understanding of equivalence are discussed

    Mid-latitude continental temperatures through the early Eocene in western Europe

    Branched glycerol dialkyl glycerol tetraethers (brGDGTs) are increasingly used to reconstruct mean annual air temperature (MAAT) during the early Paleogene. However, the application of this proxy in coal deposits is limited and brGDGTs have only been detected in immature coals (i.e. lignites). Using samples recovered from Schöningen, Germany (∼48°N palaeolatitude), we provide the first detailed study into the occurrence and distribution of brGDGTs through a sequence of early Eocene lignites and associated interbeds. BrGDGTs are abundant and present in every sample. In comparison to modern studies, changes in vegetation type do not appear to significantly impact brGDGT distributions; however, there are subtle differences between lignites – representing peat-forming environments – and siliciclastic nearshore marine interbed depositional environments. Using the most recent brGDGT temperature calibration (MATmr) developed for soils, we generate the first continental temperature record from central-western continental Europe through the early Eocene. Lignite-derived MAAT estimates range from 23 to 26 °C while those derived from the nearshore marine interbeds exceed 20 °C. These estimates are consistent with other mid-latitude environments and model simulations, indicating enhanced mid-latitude, early Eocene warmth. In the basal part of the section studied, warming is recorded in both the lignites (∼2 °C) and nearshore marine interbeds (∼2–3 °C). This culminates in a long-term temperature maximum, likely including the Early Eocene Climatic Optimum (EECO). Although this long-term warming trend is relatively well established in the marine realm, it has rarely been shown in terrestrial settings. Using a suite of model simulations we show that the magnitude of warming at Schöningen is broadly consistent with a doubling of CO2, in agreement with late Paleocene and early Eocene pCO2 estimates

    High temperatures in the terrestrial mid-latitudes during the early Palaeogene

    The early Paleogene (56–48 Myr) provides valuable information about the Earth’s climate system in an equilibrium high pCO2 world. High ocean temperatures have been reconstructed for this greenhouse period, but land temperature estimates have been cooler than expected. This mismatch between marine and terrestrial temperatures has been difficult to reconcile. Here we present terrestrial temperature estimates from a newly calibrated branched glycerol dialkyl glycerol tetraether-based palaeothermometer in ancient lignites (fossilized peat). Our results suggest early Palaeogene mid-latitude mean annual air temperatures of 23–29 °C (with an uncertainty of ± 4.7 °C), 5–10 °C higher than most previous estimates. The identification of archaeal biomarkers in these same lignites, previously observed only in thermophiles and hyperthermophilic settings, support these high temperature estimates. These mid-latitude terrestrial temperature estimates are consistent with reconstructed ocean temperatures and indicate that the terrestrial realm was much warmer during the early Palaeogene than previously thought

    Safety and tolerability of donepezil 23 mg in moderate to severe Alzheimer's disease

    <p>Abstract</p> <p>Background</p> <p>Donepezil 23 mg/d, recently approved in the United States for treatment of moderate to severe Alzheimer's disease (AD), was developed to address the need for an additional treatment option for patients with advanced AD. This report, based on a pivotal phase 3 study, presents a detailed analysis of the safety and tolerability of increasing donepezil to 23 mg/d compared with continuing 10 mg/d.</p> <p>Method</p> <p>Safety analyses comprised examination of the incidence, severity, and timing of treatment-emergent adverse events (AEs) and their relationship to treatment initiation; changes in weight, electrocardiogram, vital signs, and laboratory parameters; and the incidence of premature study discontinuation. The analysis population (n = 1434) included all randomized patients who took at least 1 dose of study drug and had a postbaseline safety assessment. To further examine the effect of transition from a lower to a higher donepezil dose, a pooled analysis of safety data from 2 phase 3 trials of donepezil 5 mg/d and 10 mg/d was also performed.</p> <p>Results</p> <p>The safety population comprised 1434 patients: donepezil 23 mg/d (n = 963); donepezil 10 mg/d (n = 471); completion rates were 71.1% and 84.7%, respectively. The most common AEs were nausea, vomiting, and diarrhea (donepezil 23 mg/d: 11.8%, 9.2%, 8.3%; donepezil 10 mg/d: 3.4%, 2.5%, 5.3%, respectively). AEs that contributed most to early discontinuations were vomiting (2.9% of patients in the 23 mg/d group and 0.4% in the 10 mg/d group), nausea (1.9% and 0.4%), diarrhea (1.7% and 0.4%), and dizziness (1.1% and 0.0%). The percentages of patients with AEs in the 23 mg/d group, as well as the timing, type, and severity of these AEs, were similar to those seen in previous donepezil trials with titration from 5 to 10 mg/d. Serious AEs were uncommon (23 mg/d, 8.3%; 10 mg/d, 9.6%).</p> <p>Discussion</p> <p>The 23 mg/d dose of donepezil was associated with typical cholinergic AEs, particularly gastrointestinal-related AEs, similar to those observed in studies with a dose increase from 5 to 10 mg/d.</p> <p>Conclusion</p> <p>The good safety and predictable tolerability profile for donepezil 23 mg/d supports its favorable risk/benefit ratio in patients with moderate to severe AD.</p> <p>Trial Registration</p> <p><a href="http://www.clinicaltrials.gov/ct2/show/NCT00478205">NCT00478205</a></p

    Tourist Photographers and the Promotion of Travel: the Polytechnic Touring Association, 1888–1939

    The Polytechnic Touring Association (PTA) was a London-based, originally philanthropic turned commercial travel firm whose historical origins coincided with the arrival of the Kodak camera in 1888 – thus, of popular (tourist) photography. This article examines the PTA’s changing relationship with tourist photographers, and how this influenced the company’s understanding of what role photography could play in promoting the tours, in the late nineteenth and early twenty century. This inquiry is advanced on the basis of the observation that, during this time, the PTA’s passage from viewing tourists as citizens to educate, to customers to please, paralleled the move from using photography-based images to mixed media. Such a development was certainly a response to unprecedented market demands; this article argues that it should also be considered in relation to the widening of photographic perceptions engendered by the democratization of the medium, to which the PTA responded, first as educator, then as service provider. In doing so, the article raises several questions about the shifting relationship between “high”, or established, and “low”, or emerging, forms of culture, as mass photography and the mass marketing of tourism developed

    Implementation of a diabetes in pregnancy clinical register in a complex setting: findings from a process evaluation

    Background: Rates of diabetes in pregnancy are disproportionately higher among Aboriginal than non-Aboriginal women in Australia. Additional challenges are posed by the context of Aboriginal health including remoteness and disadvantage. A clinical register was established in 2011 to improve care coordination, and as an epidemiological and quality assurance tool. This paper presents results from a process evaluation identifying what worked well, persisting challenges and opportunities for improvement. Methods: Clinical register data were compared to the Northern Territory Midwives Data Collection. A cross-sectional survey of 113 health professionals across the region was also conducted in 2016 to assess use and value of the register; and five focus groups (49 healthcare professionals) documented improvements to models of care. Results: From January 2012 to December 2015, 1,410 women were referred to the register, 48% of whom were Aboriginal. In 2014, women on the register represented 75% of those on the Midwives Data Collection for Aboriginal women with gestational diabetes and 100% for Aboriginal women with pre-existing diabetes. Since commencement of the register, an 80% increase in reported prevalence of gestational diabetes among Aboriginal women in the Midwives Data Collection occurred (2011-2013), prior to adoption of new diagnostic criteria (2014). As most women met both diagnostic criteria (81% in 2012 and 74% in 2015) it is unlikely that the changes in criteria contributed to this increase. Over half (57%) of survey respondents reported improvement in knowledge of the epidemiology of diabetes in pregnancy since establishment of the register. However, only 32% of survey respondents thought that the register improved care-coordination. The need for improved integration and awareness to increase use was also highlighted. Conclusion: Although the register has not been reported to improve care coordination, it has contributed to increased reported prevalence of gestational diabetes among high risk Aboriginal women, in a routinely collected jurisdiction-wide pregnancy dataset. It has therefore contributed to an improved understanding of epidemiology and disease burden and may in future contribute to improved management and outcomes.     Minimum joint space width and tibial cartilage morphology in the knees of healthy individuals: A cross-sectional study

    <p>Abstract</p> <p>Background</p> <p>The clinical use of minimum joint space width (mJSW) and cartilage volume and thickness has been limited to the longitudinal measurement of disease progression (i.e. change over time) rather than the diagnosis of OA in which values are compared to a standard. This is primarily due to lack of establishment of normative values of joint space width and cartilage morphometry as has been done with bone density values in diagnosing osteoporosis. Thus, the purpose of this pilot study is to estimate reference values of medial joint space width and cartilage morphometry in healthy individuals of all ages using standard radiography and peripheral magnetic resonance imaging.</p> <p>Design</p> <p>For this cross-sectional study, healthy volunteers underwent a fixed-flexion knee X-ray and a peripheral MR (pMR) scan of the same knee using a 1T machine (ONI OrthOne™, Wilmington, MA). Radiographs were digitized and analyzed for medial mJSW using an automated algorithm. Only knees scoring ≤1 on the Kellgren-Lawrence scale (no radiographic evidence of knee OA) were included in the analyses. All 3D SPGRE fat-sat sagittal pMR scans were analyzed for medial tibial cartilage morphometry using a proprietary software program (Chondrometrics GmbH).</p> <p>Results</p> <p>Of 119 healthy participants, 73 were female and 47 were male; mean (SD) age 38.2 (13.2) years, mean BMI 25.0 (4.4) kg/m<sup>2</sup>. Minimum JSW values were calculated for each sex and decade of life. Analyses revealed mJSW did not significantly decrease with increasing decade (p > 0.05) in either sex. Females had a mean (SD) medial mJSW of 4.8 (0.7) mm compared to males with corresponding larger value of 5.7 (0.8) mm. Cartilage morphometry results showed similar trends with mean (SD) tibial cartilage volume and thickness in females of 1.50 (0.19) μL/mm<sup>2 </sup>and 1.45 (0.19) mm, respectively, and 1.77 (0.24) μL/mm<sup>2 </sup>and 1.71 (0.24) mm, respectively, in males.</p> <p>Conclusion</p> <p>These data suggest that medial mJSW values do not decrease with aging in healthy individuals but remain fairly constant throughout the lifespan with "healthy" values of 4.8 mm for females and 5.7 mm for males. Similar trends were seen for cartilage morphology. Results suggest there may be no need to differentiate a t-score and a z-score in OA diagnosis because cartilage thickness and JSW remain constant throughout life in the absence of OA.</p
