    Tratamiento de la gonartrosis con osteotomía tibial de penetración y fijación con grapas

    La osteotomía metafisaria proximal de tibia ha sido utilizada en el tratamiento de 96 gonartrosis con desviación axil en varo (87 casos) o en valgo (nueve casos), con componente de flexo en la mitad de los casos y con cierto grado de laxitud ligamentosa en 1/3 de los casos. Se utilizó un cálculo angular preoperatorio en bipedestación con contraste opaco del eje mecánico de la extremidad y Rx-TV. La osteotomía fue cuneiforme de penetración, se estabilizó con grapas y se aplicó calza de yeso para facilitar el apoyo precoz. Proporcionó un efecto antiálgico en el 94 % de los casos, pero la valoración funcional deja el resultado en 25 % de casos excelentes y 51% de casos buenos. Destacan la necesidad de operar ambas rodillas, la bondad del resultado con independencia del grado de angulación, la compensación de las laxitudes moderadas ya referidas y la repercusión favorable del realineamiento axil sobre la artrosis femoropatelar presente en el 75 % de los casos

    Bayesian inference and data cloning in population projection matrices

    Discrete time models are used in Ecology for describing the evolution of an agestructured population. Usually, they are considered from a deterministic viewpoint but, in practice, this is not very realistic. The statistical model we propose in this article is a reasonable model for the case in which the evolution of the population is described by means of a projection matrix. In this statistical model, fertility rates and survival rates are unknown parameters and they are estimated by using a Bayesian approach. Usual Bayesian and data cloning methods (based on Bayesian methodology) are applied to real data from the population of the Steller sea lions located in the Alaska coast since 1978 to 2004. The estimates obtained from these methods show a good behavior when they are compared to the actual value

    Efficacy of Neurofeedback on the Increase of Mindfulness-Related Capacities in Healthy Individuals: a Controlled Trial

    Electroencephalogram (EEG) studies of mindfulness have shown it can lead to increases in alpha power, which are similar to those obtained by alpha-based neurofeedback (NF) interventions. It has been hypothesized there may be relationships between mindfulness and NF in terms of the neural pathways through which they induce salutary outcomes. The aim of the study was to evaluate possible changes in mindfulness and cognitive functioning following an alpha-based NF intervention, and the role of alpha power as a mediator of improvements. A controlled, non-randomized, trial with 50 healthy participants was conducted with two experimental conditions: a six-session NF intervention and a waiting-list control group. Both groups were administered mindfulness questionnaires (Mindful Attention Awareness Scale (MAAS), Five Facet Mindfulness Questionnaire (FFMQ)) and cognitive measures (Paced Auditory Serial Addition Task (PASAT)), at pre- and post-test. The NF intervention focused on the up-regulation of upper alpha power. Differences among groups were estimated using ANCOVAs, and mediation assessment through path analyses. Compared to controls, the NF group showed enhanced task-related upper alpha power (effect size (ES) = 1.16, p < 0.001), mindfulness outcomes (MAAS: ES = 0.94, p = 0.004; FFMQ: ES = 1.38, p < 0.001), and a trend of cognitive functioning (PASAT time: ES = 0.59, p = 0.062). Upper alpha power had a mediating effect for cognitive functioning (PASAT errors: indirect effect = 0.81, 95% CI = 0.21–1.85), but not for mindfulness. These results demonstrate the effectiveness of NF for increasing mindfulness in healthy individuals with no previous experience in mindfulness or neurofeedback training, suggesting that NF may be an acceptable method of augmenting mindfulness-related capacities in the general population

    Incentivos fiscales y planificación fiscal de la empresa familiar española: un estudio empírico

    La planificación fiscal de la empresa familiar es imprescindible para garantizar la supervivencia de la misma y poder hacer frente a los costes derivados de los impuestos. Sin embargo, la evidencia nos indica que las empresas familiares están más preocupadas por otros factores de éxito, tales como las relaciones familiares en el seno empresarial. En este trabajo, analizamos las relaciones existentes entre el establecimiento de incentivos del sistema fiscal y la planificación fiscal de la empresa familiar desde la perspectiva del familiness. Sobre la base de una muestra de 282 empresas familiares del sector industrial, se estudia la consistencia de las relaciones entre las dimensiones poder, experiencia y cultura familiar con la capacidad para planificar fiscalmente de las empresas familiares. Los resultados indican que, en general, las empresas familiares están interesadas por una adecuada planificación fiscal cuando se establecen incentivos fiscales que puedan considerar de interés para la adecuada gestión de la empresa familiar.

    Modification of corneal biomechanics and intraocular pressure following non-penetrating deep sclerectomy

    Changes in the cornea can influence outcomes in patients with primary open-angle glaucoma (POAG). We aimed to evaluate the relevance of changes in corneal biomechanics and intraocular pressure (IOP) in patients undergoing non-penetrating deep sclerectomy (NPDS) with the Esnoper V2000 implant® (AJL Ophthalmic S.A., Gasteiz, Spain). We included 42 eyes of 42 patients with POAG scheduled for NPDS with the Esnoper V2000 implant. Biomechanical properties were measured by Ocular Response Analyzer® G3 (ORA; Reichert Inc., Depew, NY, USA). Corneal hysteresis (CH), corneal resistance factor (CRF), corneal compensated IOP (IOPcc), and Goldmann-correlated IOP (IOPg) were measured the day before surgery and on day 1, 7, and 30 and 2 and 3 months after surgery. CH initially increased, fell below the presurgical value at 30 days after the surgery, and increased again at 2 and 3 months. CRF, IOPcc, and IOPg decreased on the first day after surgery, then followed a trend of increasing but stayed below pre-surgery levels. All values reached statistical significance. While observed changes in corneal biomechanics after NPDS and Esnoper V2000 implant were significant, more studies are needed if we are to understand their influence on corneal biomechanics and their clinical relevance in POAG

    Performance characterization of a PCM storage tank

    This paper presents the experimental results of a versatile latent heat storage tank capable of working with organic phase-change materials within a temperature range from -10 °C to 100 °C. The tank contains a paraffin with a phase-change temperature between 3 °C and 8 °C. Firstly, this study focuses on explaining the design criteria which were followed to build the tank. Secondly, a full experimental characterization of the performance has been carried out. The enthalpy temperature curve, the specific heat and density have been measured for the tested paraffin. The performance of the tank has been analyzed in terms of the vertical stratification within the PCM, the effectiveness, the reacted fraction and the total heat transfer of the tank. The results indicate that up to 78% of the maximum capacity is reached within 4 h. The performance is mainly controlled by the supply temperature and the effect of the mass flow rate is almost negligible given that all the tests are in laminar flow.The authors from the Polytechnic University of Valencia gratefully acknowledge ACCIONA Infraestructuras for the funding of the LHTS installation.     Variation in assignment of the COPD patients into a GOLD group according to symptoms severity

    Introduction: The Global Organization of Lung Disease (GOLD) classifies patients with chronic obstructive pulmonary disease (COPD) taking into account the symptoms. The modified Medical Research Council’s dyspnea scale (mMRC) and the COPD assessment test (CAT) are used to assess these symptoms. In this study, we analyze the concordance of GOLD classification using mMRC and CAT. Patients and Methods: This is an observational study of a cohort of 169 patients with COPD, who were classified following the GOLD 2017 recommendations, using both mMRC and CAT. A concordance analysis was applied, and a ROC curve was generated to identify the CAT score that best concorded with the mMRC scale. Results: The concordance for the GOLD groups classified by CAT and mMRC was moderate (kappa 0.492). For mMRC score of 1 and 2, a CAT score of =9 and =16 showed the maximum value of the Youden index, respectively. By reclassifying the patients with the new cut-off points obtained, the best concordance was obtained between the cut-off point for CAT of 16 and for mMRC of 2, followed by CAT of 9 and mMRC of 1. Conclusion: Because of the deficient concordance between CAT and mMRC, we propose the use of new cut-off points in future updates of the GOLD strategy

    Experimental analysis of a paraffin-based cold storage tank

    [EN] The aim of this study is to characterize a paraffin-based cold storage tank. Novel experimental results are presented for this system which combines a significant amount of paraffin (1450 kg) immersed around 18 spiral-shaped coils disposed in counter-current flow. The paraffin has a phase-change temperature in the range 4 8 °C as measured by a 3-layer calorimeter. Different tests have been carried out with a constant mass flow rate and supply temperature. Around 31% of the paraffin has hardly any contact with the coils and hereby acts as a dead mass. The results show the importance of natural convection within the phase-change-material, particularly during the melting process.
