Cardiac power output accurately reflects external cardiac work over a wide range of inotropic states in pigs
BACKGROUND:
Cardiac power output (CPO), derived from the product of cardiac output and mean aortic pressure, is an important yet underexploited parameter for hemodynamic monitoring of critically ill patients in the intensive-care unit (ICU). The conductance catheter-derived pressure-volume loop area reflects left ventricular stroke work (LV SW). Dividing LV SW by time, a measure of LV SW min- 1 is obtained sharing the same unit as CPO (W). We aimed to validate CPO as a marker of LV SW min- 1 under various inotropic states.
METHODS:
We retrospectively analysed data obtained from experimental studies of the hemodynamic impact of mild hypothermia and hyperthermia on acute heart failure. Fifty-nine anaesthetized and mechanically ventilated closed-chest Landrace pigs (68 ± 1 kg) were instrumented with Swan-Ganz and LV pressure-volume catheters. Data were obtained at body temperatures of 33.0 °C, 38.0 °C and 40.5 °C; before and after: resuscitation, myocardial infarction, endotoxemia, sevoflurane-induced myocardial depression and beta-adrenergic stimulation. We plotted LVSW min- 1 against CPO by linear regression analysis, as well as against the following classical indices of LV function and work: LV ejection fraction (LV EF), rate-pressure product (RPP), triple product (TP), LV maximum pressure (LVPmax) and maximal rate of rise of LVP (LV dP/dtmax).
RESULTS:
CPO showed the best correlation with LV SW min- 1 (r2 = 0.89; p < 0.05) while LV EF did not correlate at all (r2 = 0.01; p = 0.259). Further parameters correlated moderately with LV SW min- 1 (LVPmax r2 = 0.47, RPP r2 = 0.67; and TP r2 = 0.54). LV dP/dtmax correlated worst with LV SW min- 1 (r2 = 0.28).
CONCLUSION:
CPO reflects external cardiac work over a wide range of inotropic states. These data further support the use of CPO to monitor inotropic interventions in the ICU
Postmortem examination of patient H.M.’s brain based on histological sectioning and digital 3D reconstruction
Modern scientific knowledge of how memory functions are organized in the human brain originated from the case of Henry G. Molaison (H.M.), an epileptic patient whose amnesia ensued unexpectedly following a bilateral surgical ablation of medial temporal lobe structures, including the hippocampus. The neuroanatomical extent of the 1953 operation could not be assessed definitively during H.M.’s life. Here we describe the results of a procedure designed to reconstruct a microscopic anatomical model of the whole brain and conduct detailed 3D measurements in the medial temporal lobe region. This approach, combined with cellular-level imaging of stained histological slices, demonstrates a significant amount of residual hippocampal tissue with distinctive cytoarchitecture. Our study also reveals diffuse pathology in the deep white matter and a small, circumscribed lesion in the left orbitofrontal cortex. The findings constitute new evidence that may help elucidate the consequences of H.M.’s operation in the context of the brain’s overall pathology
HAMSTER: visualizing microarray experiments as a set of minimum spanning trees
<p>Abstract</p> <p>Background</p> <p>Visualization tools allow researchers to obtain a global view of the interrelationships between the probes or experiments of a gene expression (<it>e.g. microarray</it>) data set. Some existing methods include hierarchical clustering and k-means. In recent years, others have proposed applying minimum spanning trees (MST) for microarray clustering. Although MST-based clustering is formally equivalent to the dendrograms produced by hierarchical clustering under certain conditions; visually they can be quite different.</p> <p>Methods</p> <p>HAMSTER (Helpful Abstraction using Minimum Spanning Trees for Expression Relations) is an open source system for generating a <b>set </b>of MSTs from the experiments of a microarray data set. While previous works have generated a single MST from a data set for data clustering, we recursively merge experiments and repeat this process to obtain a set of MSTs for data visualization. Depending on the parameters chosen, each tree is analogous to a snapshot of one step of the hierarchical clustering process. We scored and ranked these trees using one of three proposed schemes. HAMSTER is implemented in C++ and makes use of Graphviz for laying out each MST.</p> <p>Results</p> <p>We report on the running time of HAMSTER and demonstrate using data sets from the NCBI Gene Expression Omnibus (GEO) that the images created by HAMSTER offer insights that differ from the dendrograms of hierarchical clustering. In addition to the C++ program which is available as open source, we also provided a web-based version (HAMSTER<sup>+</sup>) which allows users to apply our system through a web browser without any computer programming knowledge.</p> <p>Conclusion</p> <p>Researchers may find it helpful to include HAMSTER in their microarray analysis workflow as it can offer insights that differ from hierarchical clustering. We believe that HAMSTER would be useful for certain types of gradient data sets (e.g time-series data) and data that indicate relationships between cells/tissues. Object-Based Attention
There appear to be three independent systems for allocating attention: space-based, feature based, and object-based. Here, we review the literature of object-based attention to determine its underlying mechanisms. First, findings from unconscious priming and cuing suggest that the pre-attentive targets of object-based attention can be fully developed object representations. Next, the control of object-based attention appears to come from ventral visual areas specialized in object analysis that project downward to early visual areas. Whether feedback from object areas can accurately target the object’s specific locations and features is controversial, but recent work in autoencoding has made this plausible. Finally, we suggest that the three classic modes of attention may not be as independent as is commonly considered, and instead could rely on object-based attention for all three modes of selection. Specifically, studies show that attention can spread over the separated members of a group – without affecting the space between them — matching the defining property of feature-based attention. At the same time, object-based attention directed to a single small item has the properties of space-based attention. Nevertheless, the evidence for a parallel, space-based selection controlled through saccade centers is also convincing. We outline the architecture for this combined system and discuss how it works in parallel with other attention pathways
Validation of the theoretical domains framework for use in behaviour change and implementation research
Background
An integrative theoretical framework, developed for cross-disciplinary implementation and other behaviour change research, has been applied across a wide range of clinical situations. This study tests the validity of this framework.
Methods
Validity was investigated by behavioural experts sorting 112 unique theoretical constructs using closed and open sort tasks. The extent of replication was tested by Discriminant Content Validation and Fuzzy Cluster Analysis.
Results
There was good support for a refinement of the framework comprising 14 domains of theoretical constructs (average silhouette value 0.29): 'Knowledge', 'Skills', 'Social/Professional Role and Identity', 'Beliefs about Capabilities', 'Optimism', 'Beliefs about Consequences', 'Reinforcement', 'Intentions', 'Goals', 'Memory, Attention and Decision Processes', 'Environmental Context and Resources', 'Social Influences', 'Emotions', and 'Behavioural Regulation'.
Conclusions
The refined Theoretical Domains Framework has a strengthened empirical base and provides a method for theoretically assessing implementation problems, as well as professional and other health-related behaviours as a basis for intervention development
Elemental and mineralogical analysis of marine and coastal sediments from Phra Thong Island, Thailand: Insights into the provenance of coastal hazard deposits
Sediment records left by coastal hazards (e.g. tsunami and/or storms) may shed light on the sedimentary and hydrodynamic processes happening during such events. Modern onshore and offshore sediment samples were compared with the 2004 Indian Ocean Tsunami, three palaeotsunami and a 2007 storm deposit from Phra Thong Island, Thailand, to determine provenance relationships between these coastal overwash deposits. Sedimentological and stratigraphic characteristics are generally inadequate to discriminate tsunami and storm deposits so a statistical approach (including cluster analysis, principal component analysis and discriminant function analysis) was used based on grain size, mineralogy and trace element geochemistry. The mineral content and trace element geochemistry are statistically inadequate to distinguish the provenance of the modern storm and tsunami deposits at this site, but the mean grain size can potentially discriminate these overwash deposits. The 2007 storm surge deposits were most likely sourced from the onshore sediment environment whereas all four tsunami units statistically differ from each other indicating diverse sediment sources. Our statistical analyses suggest that the 2004 tsunami deposit was mainly derived from nearshore marine sediments. The uppermost palaeotsunami deposit was possibly derived from both onshore and nearshore materials while the lower palaeotsunami deposits showed no clear evidence of their sediment sources. Such complexity raises questions about the origin of the sediments in the tsunami and storm deposits and strongly suggests that local context and palaeogeography are important aspects that cannot be ignored in tsunami provenance studies
