    Hematological and coagulation parameters as predictors of death by Coronavirus disease in hospitalized patients: a Brazilian follow-up study

    This study aimed to evaluate the hematological and coagulation parameters according to the clinical outcomes of coronavirus disease (COVID-19). We analyzed the hematological and coagulation parameters of hospitalized patients with COVID-19 at admission, and two and three weeks during hospitalization. To assess the performance of these parameters in predicting poor outcomes, receiver operating characteristic (ROC) curves were created. We studied 128 patients with COVID-19 (59.2±17.7 years, 56% male). Non-survivors (n=54, 42%) presented significant alterations in hematological and coagulation parameters at admission, such as increased in white blood cells (WBC), neutrophil, and band cell counts, as well as elevated prothrombin time (PT), activated partial thromboplastin time, and D-dimer levels. During follow-up, the same group presented a gradual increase in D-dimer and PT levels, accompanied by a reduction in PT activity, hemoglobin, and red blood cell count (RBC). ROC curves showed that WBC, neutrophil, and band cell counts presented the best area under the curve (AUC) values with sensitivity and specificity of >70%; however, a logistic regression model combining all the parameters, except for RBC, presented an AUC of 0.89, sensitivity of 84.84%, and specificity of 77.41%. Our study shows that significant alterations in hematological and coagulation tests at admission could be useful predictors of disease severity and mortality in COVID-19

    Prevalência de traumas em membros inferiores de pacientes atendidos pela fisioterapia no Hospital Regional de Ceilândia

    Objetivos: verificar o perfil epidemiológico e a prevalência dos traumatismos de membros inferiores de pacientes atendidos por fisioterapeutas em um hospital público do Distrito Federal, demonstrando essa prevalência quanto à região anatômica, o tipo e o mecanismo de lesão e realizar uma análise da função muscular dos membros inferiores de dados de uma avaliação inicial. Método: Trata-se de um estudo descritivo transversal baseado na análise dos dados de fichas de avaliação realizadas durante o projeto Prevenção de Deformidades Ortopédicas em Pessoas com Deficiência Física do PET/Saúde Redes de Atenção à Saúde entre 2013 a 2015. Resultados: Foram analisadas no total 224 fichas, em que houve predomínio do sexo masculino (72,32%), idade média de 42,47±19,59 anos, etnia parda (43,30%), situação conjugal solteiro (40,63%), escolaridade de ensino fundamental completo (57,14%). A região anatômica mais acometida foi o fêmur (27,68%), a fratura (80,36%) foi o tipo de lesão mais prevalente, e o acidente de trânsito (43,75%) foi o mecanismo de lesão mais predominante.     The complete genome sequence of Chromobacterium violaceum reveals remarkable and exploitable bacterial adaptability

    Chromobacterium violaceum is one of millions of species of free-living microorganisms that populate the soil and water in the extant areas of tropical biodiversity around the world. Its complete genome sequence reveals (i) extensive alternative pathways for energy generation, (ii) ≈500 ORFs for transport-related proteins, (iii) complex and extensive systems for stress adaptation and motility, and (iv) wide-spread utilization of quorum sensing for control of inducible systems, all of which underpin the versatility and adaptability of the organism. The genome also contains extensive but incomplete arrays of ORFs coding for proteins associated with mammalian pathogenicity, possibly involved in the occasional but often fatal cases of human C. violaceum infection. There is, in addition, a series of previously unknown but important enzymes and secondary metabolites including paraquat-inducible proteins, drug and heavy-metal-resistance proteins, multiple chitinases, and proteins for the detoxification of xenobiotics that may have biotechnological applications

    Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial

    Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients.     Cardiac autonomic responses after resistance exercise in treated hypertensive subjects

    The aim of this study was to assess and to compare heart rate variability (HRV) after resistance exercise (RE) in treated hypertensive and normotensive subjects. Nine hypertensive men [HT: 58.0±7.7 years, systolic blood pressure (SBP) =133.6±6.5 mmHg, diastolic blood pressure (DBP) =87.3±8.1 mmHg; under antihypertensive treatment] and eleven normotensive men (NT: 57.1±6.0 years, SBP =127±8.5 mmHg, DBP =82.7±5.5 mmHg) performed a single session of RE (2 sets of 15-20 repetitions, 50% of 1RM, 120 s interval between sets/exercise) for the following exercises: leg extension, leg press, leg curl, bench press, seated row, triceps push-down, seated calf flexion, seated arm curl. HRV was assessed at resting and during 10 min of recovery period by calculating time (SDNN, RMSSD) and frequency domain (LF and HF) indices. Mean values of HRV indices were reduced in the post-exercise period compared to the resting period (HT: lnHF: 4.7±1.4 vs. 2.4±1.2 ms²; NT: lnHF: 4.8±1.5 vs. 2.2±1.1 ms², p<0.01). However, there was no group vs. time interaction in this response (p=0.8). The results indicate that HRV is equally suppressed after RE in normotensive and hypertensive individuals. These findings suggest that a single session of RE does not bring additional cardiac autonomic stress to treated hypertensive subjects

    Effects of metformin on the adrenal cortex of androgenized rats

    Objective: To evaluate the sex steroid profile and histomorphometry of the adrenal cortical zones of androgenized rats (wistar) with polycystic ovary syndrome treated with metformin.Study design: Thirty animals were divided into three groups: GC (regular estrous cycle), GPE (permanent estrus), and GPEM (permanent estrus + metformin 28 mg/kg for 50 days). At the end of this period, blood was collected for hormone measurement. The width of the adrenal cortical zones and the nuclear volumes were analyzed by histomorphometry. The ANOVA test was used in the statistical analysis.Results: The adrenal glands of the androgenized animals were larger and more intensely vascularized than those of the other groups. The concentration of androstenedione in GPE was higher than that in the other groups (0.4 ± 0.1â\u88\u97>= 0.2 ± 0.1 = 0.2 ± 01,â\u88\u97p GPEM = GC,â\u88\u97p < 0.05).Conclusion: Metformin treatment may decrease the serum levels of androstenedione as well as the width and the nuclear volumes of the zona glomerulosa and of the zona reticularis in androgenized animals

    Atenção ao aborto no Sistema Único de Saúde no Nordeste Brasileiro: a estrutura dos serviços

    Objetivos: avaliar a estrutura de maternidades que prestam atenção a mulheres em situação de abortamento no Sistema Único de Saúde, em Salvador, Recife e São Luís. Métodos: foram selecionadas três maternidades em cada capital. Os dados foram obtidos por entrevista com gerente do serviço ou equipe de direção e observação direta. Utilizou-se instrumento com 120 questões pontuadas abrangendo seis componentes - planta física, recursos materiais, recursos humanos, materiais de consumo, educação em saúde e ferramentas de gestão que incorporam quatro dimensões de avaliação - insumo/ambiente físico; qualidade técnica e gerencial do cuidado; acolhimento/orientação e continuidade da atenção. Os resultados foram categorizados pelo percentual obtido em relação ao máximo esperado: suficiente (B a 80%); intermediário (de 50% a 79%); insuficiente (< 50%). Resultados: os componentes melhor pontuados foram planta física, recursos materiais e material de consumo. Educação em saúde e ferramentas de gestão tiveram pior pontuação. Nenhuma unidade atingiu nível considerado suficiente. Sete foram classificadas no nível intermediário e duas insuficiente. Conclusões: as unidades estudadas não apresentam estrutura adequada para o modelo de atenção ao abortamento preconizado. Há necessidade de intervenções para qualificar a estrutura dos serviços para a atenção ao aborto e promover a huma-nização do cuidado

    Neotropical ornithology: Reckoning with historical assumptions, removing systemic barriers, and reimagining the future

    A major barrier to advancing ornithology is the systemic exclusion of professionals from the Global South. A recent special feature, Advances in Neotropical Ornithology, and a shortfalls analysis therein, unintentionally followed a long-standing pattern of highlighting individuals, knowledge, and views from the Global North, while largely omitting the perspectives of people based within the Neotropics. Here, we review current strengths and opportunities in the practice of Neotropical ornithology. Further, we discuss problems with assessing the state of Neotropical ornithology through a northern lens, including discovery narratives, incomplete (and biased) understanding of history and advances, and the promotion of agendas that, while currently popular in the north, may not fit the needs and realities of Neotropical research. We argue that future advances in Neotropical ornithology will critically depend on identifying and addressing the systemic barriers that hold back ornithologists who live and work in the Neotropics: unreliable and limited funding, exclusion from international research leadership, restricted dissemination of knowledge (e.g., through language hegemony and citation bias), and logistical barriers. Moving forward, we must examine and acknowledge the colonial roots of our discipline, and explicitly promote anti-colonial agendas for research, training, and conservation. We invite our colleagues within and beyond the Neotropics to join us in creating new models of governance that establish research priorities with vigorous participation of ornithologists and communities within the Neotropical region. To include a diversity of perspectives, we must systemically address discrimination and bias rooted in the socioeconomic class system, anti-Blackness, anti-Brownness, anti-Indigeneity, misogyny, homophobia, tokenism, and ableism. Instead of seeking individual excellence and rewarding top-down leadership, institutions in the North and South can promote collective leadership. In adopting these approaches, we, ornithologists, will join a community of researchers across academia building new paradigms that can reconcile our relationships and transform science. Spanish and Portuguese translations are available in the Supplementary Material.• Research conducted by ornithologists living and working in Latin America and the Caribbean has been historically and systemically excluded from global scientific paradigms, ultimately holding back ornithology as a discipline.• To avoid replicating systems of exclusion in ornithology, authors, editors, reviewers, journals, scientific societies, and research institutions need to interrupt long-held assumptions, improve research practices, and change policies around funding and publication.• To advance Neotropical ornithology and conserve birds across the Americas, institutions should invest directly in basic field biology research, reward collective leadership, and strengthen funding and professional development opportunities for people affected by current research policies.Peer reviewe
