    Застосування ультразвукової навігації в хірургічному лікуванні вогнепальних ран

    Мета. Поліпшення результатів лікування постраждалих з вогнепальними осколковими і кульовими пораненнями шляхом широкого використання комплексу ультразвукової та рентгенологічної навігації. Матеріали і методи. Наведено досвід лікування 198 постраждалих із сліпими кульовими й осколковими пораненнями м’яких тканин на II рівні надання медичної допомоги в умовах АТО на базі 61, 65 і 66 військово–мобільних госпіталів. У діагностиці і видаленні чужорідних тіл (куль і осколків) під час хірургічної обробки ран використана рентгенологічна й ультразвукова навігація. Переважали пацієнти з множинними осколковими пораненнями. Результати. Застосування сонографії дозволило лоціювати і видалити сторонні тіла у 95,4% поранених, знизити частоту повторних операцій і скоротити терміни лікування. Висновки. Перевагами методу сонографії є: мобільність, висока інформативність щодо структурних порушень м’яких тканин і верифікації чужорідних тіл, особливо не металевих. Видалення сторонніх тіл під контролем ультразвуку дозволяє проводити операцію в режимі реального часу без променевого навантаження на пацієнта і медичний персонал

    Microbiological quality and physicochemical characterization of Brazilian bee pollen

    Bee Research Association. Sixty-two commercial samples of dehydrated bee pollen (Apis mellifera bees) collected in Brazil (eight states and the Federal District) were analyzed for commercial quality (moisture, ash, lipids, proteins, and carbohydrates), and hygiene (aerobic mesophiles, yeasts and molds, coliforms, Escherichia coli, Staphylococcus aureus and sulfite-reducing Clostridium). The analyzed samples were within the regulatory limits established by the legislation for ash, lipid and protein, except for moisture content. The analyzed hygiene parameters evaluated for bee pollen are not regulated by the Brazilian legislation, and the data discussed can contribute to regulatory agencies.     Application of the method of ultrasound imaging in the removal of foreign bodies in gunshot wounds.

    Data on the possibility of using ultrasound diagnostics when removing foreign bodies - bullets and fragments from body tissues during primary or repeated surgical treatment are rather limited. The aim of the research – improving the results of treatment of victims with gunshot fragmentation and bullet wounds by using a complex of ultrasound and X-ray navigation in the process of removing injurious elements during primary and repeated surgical treatment. The study included 214 wounded. In group I, the main group (122 wounded), the removal of fragments was carried out using a complex of X-ray and ultrasound navigation. In group II, the control group (92 wounded), only X-ray navigation was used. In group I during primary surgical treatment, foreign objects were removed in 101 (82.8%) of 122 injured and during secondary – in 17 (13.9%). In group 4 (3.3%) patients, the fragments were not visualized: in 1 - in the lateral region of the neck, in 1 - in the muscles of the leg, in 2 - in the chest wall. Complications were not observed. In group II, during primary surgical treatment, foreign objects were removed in 59 (64.1%), and in the secondary – in 18 (19.6%). The reasons for the failures were: deep lieying of fragments - in 6 (6.5%), location near large vessels - in 5 (5.4%), non-X-ray contrast foreign bodies - in 4 (4.3%). Among the complications, bleeding was observed in 2 wounded. The use of ultrasound navigation allows by 18.7% to increase the frequency of detection and removal of foreign bodies - bullets and fragments during the primary surgical treatment of a wound and to reduce the need for repeated interventions by 5.7%. The advantages of the method are: mobility, high information content in structural damages to soft tissues and verification of foreign bodies, the possibility of performing real-time, the absence of exposure to radiation

Improvement of the treatment results in injured persons with a gun–shot shrapnel woundings, widely using complex of ultrasonic and roentgenologic navigation. Маterials and methods. Experience of treatment of 198 injured persons with blind bullet and shrapnel woundings of soft tissues on the II level of the medical help provision in conditions of antiterrorist operation on base of 61, 65 and 66 military– mobile hospitals is demonstrated. Roentgenologic and ultrasound navigation was applied while doing surgical processing for diagnosis and remove of foreign bodies (bullets and shrapnel). The patients with multiple shrapnel woundings prevailed. Results. Application of sonography have permitted to locate and remove foreign bodies in 95.4% wounded persons, to lower the reoperations rate and shorten the treatment duration. Conclusion. There are following advantages of sonography method: mobility, high informativeness concerning structural disorders of soft tissues and verification of foreign bodies, peculiarly of nonmetallic. Remove of foreign bodies under control of ultrasound permits to conduct operation in regime of real time without radiation load on the patient and medical personnel

    Freqüência de infecção por Toxocara em crianças atendidas em serviço público de Maringá, sul do Brasil

    The lack of specific laboratorial diagnosis methods and precise symptoms makes the toxocariasis a neglected disease in Public Health Services. This study aims to determine the frequency of Toxocara spp. infection in children attended by the Health Public Service of Hospital Municipal de Maringá, South Brazil. To evaluate the association of epidemiological and clinical data, an observational and cross-section study was carried out. From 14,690 attended children/year aged from seven month to 12 years old, 450 serum samples were randomly collected from September/2004 to September/2005. A questionnaire was used to evaluate epidemiological, clinical and hematological data. An ELISA using Toxocara canis larval excretory-secretory products as antigen detected 130 (28.8%) positive sera, mainly between children from seven month to five years old (p = 0.0016). Significant correlation was observed between positive serology for Toxocara, and frequent playing in sandbox at school or daycare center (p = 0.011) and the presence of a cat at home (p = 0.056). From the families, 50% were dog owners which exposed soil backyards. Eosinophilia (p = 0.776), and signs and symptoms analyzed (fever p = 0.992, pneumonia p = 0.289, cold-like symptoms p = 0.277, cough p = 0.783, gastrointestinal problems p = 0.877, migraine p = 0.979, abdominal pain p = 0.965, joint pain p = 0.686 and skin rash p = 0.105) could not be related to the presence of anti-Toxocara antibodies. Therefore, two asthmatics children showed titles of 1:10,240 and accentuated eosinophilia (p = 0.0001). The authors emphasize the needs of prevention activities.A falta de métodos de diagnóstico laboratorial específico e sintomas específicos fazem da toxocaríase uma doença negligenciada nos serviços públicos de saúde. Este estudo teve por objetivo determinar a freqüência de infecção por Toxocara spp. em crianças atendidas no serviço público do Hospital Municipal de Maringá, sul do Brasil, e avaliar a associação com dados epidemiológicos e clínicos, em estudo observacional e transversal. De 14.690 crianças/ano atendidas, com idade entre sete meses a 12 anos, foram coletados 450 soros de setembro/2004 a setembro/2005. Um questionário foi utilizado para avaliar dados epidemiológicos, clínicos e hematológicos. Pelo teste ELISA, com antígeno de excreção/secreção de larvas de Toxocara canis, detectou-se 130 (28,8%) soros positivos, principalmente em crianças entre sete meses e cinco anos (p = 0,0016). Houve significante correlação entre sorologia positiva para Toxocara e freqüente recreação das crianças em caixas de areia da escola ou pré escola (p = 0,011) e presença do gato no domicilio (p = 0,056). Das famílias dessas crianças, 50% possuíam cachorros e o quintal com solo exposto. Eosinofilia (p = 0,776), sinais e sintomas (febre p = 0,992, pneumonia p = 0,289, resfriado p = 0,277, tosse p = 0,783, problema gastrointestinal p = 0877, dor de cabeça p = 0,979, dor abdominal p = 0,965, dores articulares p = 0,686, urticária p = 0,105) não se correlacionaram com a soropositividade. Todavia, duas crianças asmáticas apresentaram títulos de 1:10.240 (>; 1:320) e acentuada eosinofilia (p = 0.0001). Os autores enfatizam a necessidade de atividades preventivas

    Complex Risks from Old Urban Waste Landfills: Sustainability Perspective from Iasi, Romania

    Landfills continue to represent the most frequent managerial practice for municipal solid wastes and an increasing and complex problem globally. In certain countries, a transition to an open society and free market is superimposed on the transition to sustainability, resulting in even higher complexity of management. This paper proposes an approach for problem-structuring of landfills in complex transitions: sustainability or unsustainability of a management approach is determined by a set of sustainability filters that are defined by sets of indicators and prioritized according the systemic concept of sustainability, which says that economy is embedded in society, which is embedded in nature. The writers exercise this approach with an old landfill in Iasi, Romania, and conclude for unsustainability, because the ecological sustainability filter is not successfully passed. Social and economic sustainability filters are also discussed in relation with the ecological sustainability indicators. The described approach allows a coherent, transdisciplinary synthesis of knowledge scattered across various disciplines, a pervasive problem in landfill management. The case study helps distinguish between generally true and context-dependent aspects.Peer reviewe
