The interaction between diabetes, body mass index, hepatic steatosis, and risk of liver resection: insulin dependent diabetes is the greatest risk for major complications
Background. This study aimed to assess the relationship between diabetes, obesity, and hepatic steatosis in patients undergoing liver resection and to determine if these factors are independent predictors of major complications. Materials and Methods. Analysis of a prospectively maintained database of patients undergoing liver resection between 2005 and 2012 was undertaken. Background liver was assessed for steatosis and classified as <33% and ≥33%. Major complications were defined as Grade III–V complications using theindo-Clavien classification. Results. 504 patients underwent liver resection, of whom 56 had diabetes and 61 had steatosis ≥33%. Median BMI was 26kg/m2 (16–54kg/m 2). 94 patients developed a major complication (18.7%). Internal and External Barriers to Effective Supply Chain Management Implementation in Malaysian Manufacturing Companies: A Priority List Based on Varying Demographic Perspectives
Successful implementation of supply-chain management in a developing country such as Malaysia is considered a major challenge due to lack of awareness and unclear missions and goals. Focusing on this idea, this article compares the ranking of various demographic groups in relation to the internal and external barriers experienced by Malaysian manufacturing companies. The identified lists of internal and external barriers are prioritised using the Analytic Hierarchy Process (AHP). Ten respondents with wide ranging experience in SCM provided the necessary information in the prioritisation exercise. The ranking of the respondents from different races, age groups, education levels, employment types, work experience and designations suggests significant variations between these groups in the ranking of the internal and external barriers. The findings of this research provide important information to company managers who desire to implement SCM in a multi-cultural setting such as Malaysia. The onus is on these managers to be cautious in developing a strategic plan for its effective implementation and also in designing programs to overcome SCM obstacles
Assessment of the effect of interval from presentation to surgery on outcome in patients with peri-ampullary malignancy
Background: Delay between diagnosis of peri-ampullary cancer (PC) and surgery may allow tumour progression and affect outcome. This study aims to explore associations of interval to surgery (IS) with pathological outcomes and survival in patients with PC. Method: A database review of all patients undergoing surgery between 2006 and 2014 was undertaken. IS was measured from diagnosis by imaging. Potential association between IS and survival was measured using Cox regression analysis, and between IS and pathological outcome with multivariate logistic analysis. Results: 388 patients underwent surgery. The median IS was 49 days (1-551 days), and was not associated with any of the evaluated outcomes in patients with pancreatic (149) or distal bile duct (46) cancer. For patients with ampullary cancer (71) longer IS was associated with improved survival, with median survival of 1.3, 3.1 and 4.3 years for patients waiting 2 (25) months for surgery (p=0.036). A higher rate of margin positivity (31.4%) was also noted among patients who waited less than the median IS compared to those waiting longer than this interval (11.4%) (p=0.032). Conclusion: For patients with ampullary cancer there is a paradoxical improvement in outcome among those with a longer IS, which may be explained by progression to inoperability of more aggressive lesions
Conflict and Livelihood Decisions in the Chittagong Hill Tracts of Bangladesh

We analyse rural household livelihood and child school enrolment
decisions
in the post-conflict setting of the Chittagong Hill Tracts (CHT) region of
Bangladesh. What makes this paper innovative is the use of current subjective perceptions
regarding the possibility of violence in the future and past actual experiences
of violence in explaining household economic decision-making. Preferences
are endogenous in line with behavioural economics. Regression results show that
heightened subjective perceptions of future violence and past actual experiences
of conflict influence current consumption and child enrolment and could encourage
risky mixed crop cultivation. The trauma emanating from past experiences
combined with current high perceptions of risk of violence may induce bolder
and riskier behaviour in line with prospect theories of risk. Furthermore, a postconflic
Cooperation and Contagion in Web-Based, Networked Public Goods Experiments
A longstanding idea in the literature on human cooperation is that
cooperation should be reinforced when conditional cooperators are more likely
to interact. In the context of social networks, this idea implies that
cooperation should fare better in highly clustered networks such as cliques
than in networks with low clustering such as random networks. To test this
hypothesis, we conducted a series of web-based experiments, in which 24
individuals played a local public goods game arranged on one of five network
topologies that varied between disconnected cliques and a random regular graph.
In contrast with previous theoretical work, we found that network topology had
no significant effect on average contributions. This result implies either that
individuals are not conditional cooperators, or else that cooperation does not
benefit from positive reinforcement between connected neighbors. We then tested
both of these possibilities in two subsequent series of experiments in which
artificial seed players were introduced, making either full or zero
contributions. First, we found that although players did generally behave like
conditional cooperators, they were as likely to decrease their contributions in
response to low contributing neighbors as they were to increase their
contributions in response to high contributing neighbors. Second, we found that
positive effects of cooperation were contagious only to direct neighbors in the
network. In total we report on 113 human subjects experiments, highlighting the
speed, flexibility, and cost-effectiveness of web-based experiments over those
conducted in physical labs
Retinoic Acid Signalling and the Control of Meiotic Entry in the Human Fetal Gonad
The development of mammalian fetal germ cells along oogenic or spermatogenic fate trajectories is dictated by signals from the surrounding gonadal environment. Germ cells in the fetal testis enter mitotic arrest, whilst those in the fetal ovary undergo sex-specific entry into meiosis, the initiation of which is thought to be mediated by selective exposure of fetal ovarian germ cells to mesonephros-derived retinoic acid (RA). Aspects of this model are hard to reconcile with the spatiotemporal pattern of germ cell differentiation in the human fetal ovary, however. We have therefore examined the expression of components of the RA synthesis, metabolism and signalling pathways, and their downstream effectors and inhibitors in germ cells around the time of the initiation of meiosis in the human fetal gonad. Expression of the three RA-synthesising enzymes, ALDH1A1, 2 and 3 in the fetal ovary and testis was equal to or greater than that in the mesonephros at 8–9 weeks gestation, indicating an intrinsic capacity within the gonad to synthesise RA. Using immunohistochemistry to detect RA receptors RARα, β and RXRα, we find germ cells to be the predominant target of RA signalling in the fetal human ovary, but also reveal widespread receptor nuclear localization indicative of signalling in the testis, suggesting that human fetal testicular germ cells are not efficiently shielded from RA by the action of the RA-metabolising enzyme CYP26B1. Consistent with this, expression of CYP26B1 was greater in the human fetal ovary than testis, although the sexually-dimorphic expression patterns of the germ cell-intrinsic regulators of meiotic initiation, STRA8 and NANOS2, appear conserved. Finally, we demonstrate that RA induces a two-fold increase in STRA8 expression in cultures of human fetal testis, but is not sufficient to cause widespread meiosis-associated gene expression. Together, these data indicate that while local production of RA within the fetal ovary may be important in regulating the onset of meiosis in the human fetal ovary, mechanisms other than CYP26B1-mediated metabolism of RA may exist to inhibit the entry of germ cells into meiosis in the human fetal testis
Cruel to be kind but not cruel for cash : harm aversion in the dictator game
People regularly take prosocial actions, making individual sacrifices for the greater good. Similarly, people generally avoid causing harm to others. These twin desires to do good and avoid harm often align, but sometimes they can diverge, creating situations of moral conflict. Here, we examined this moral conflict using a modified dictator game. Participants chose how much money to allocate away from a recipient who was designated as an orphan, creating a sense of harm. This money was then reallocated to either the participant or a charity. People were strongly prosocial: they allocated more money away from the orphan for charity than for themselves. Furthermore, people left more money with the orphan when the harm was framed as a means (taking) than as a side effect (splitting). As is predicted by dual-process theories of moral decision making, response times were longer with the take action and were positively correlated with the amount taken from the orphan. We concluded that just as people take positive actions for the greater good, they are similarly more willing to cause harm when it benefits others rather than themselves
The Association between Pro-Social Attitude and Reproductive Success Differs between Men and Women
The evolution of pro-social attitude and cooperation in humans is under debate. Most of the knowledge on human cooperation results from laboratory experiments and theoretic modeling. Evolutionary explanations, however, rest upon fitness consequences. We therefore examined fitness correlates of pro-social behavior in a real life setting, analyzing data from the Wisconsin Longitudinal Study (n = 2545 men, 2967 women). We investigated whether pro-social attitude, proxied by self reported voluntary work, is associated with lifetime reproductive success. We find a sex difference in the association between pro-social attitude and offspring number. In men, a pro-social attitude was associated with higher offspring number, whereas in women, it was associated with lower offspring count. To our knowledge, this is the first study to demonstrate fitness consequences of pro-social behavior towards strangers. We conclude that analysing real life settings may help to explain the evolutionary forces leading to pro-social behavior in humans and speculate that these factors might differ between the sexes
Tipping Points for Norm Change in Human Cultures
Humans interact with each other on a daily basis by developing and
maintaining various social norms and it is critical to form a deeper
understanding of how such norms develop, how they change, and how fast they
change. In this work, we develop an evolutionary game-theoretic model based on
research in cultural psychology that shows that humans in various cultures
differ in their tendencies to conform with those around them. Using this model,
we analyze the evolutionary relationships between the tendency to conform and
how quickly a population reacts when conditions make a change in norm
desirable. Our analysis identifies conditions when a tipping point is reached

Bone turnover markers for early detection of fracture healing disturbances: A review of the scientific literature
Imaging techniques are the standard method for assessment of fracture healing processes. However, these methods are perhaps not entirely reliable for early detection of complications, the most frequent of these being delayed union and non-union. A prompt diagnosis of such disorders could prevent prolonged patient distress and disability. Efforts should be directed towards the development of new technologies for improving accuracy in diagnosing complications following bone fractures. The variation in the levels of bone turnover markers (BTMs) have been assessed with regard to there ability to predict impaired fracture healing at an early stage, nevertheless the conclusions of some studies are not consensual. In this article the authors have revised the potential of BTMs as early predictors of prognosis in adult patients presenting traumatic bone fractures but who did not suffer from osteopenia or postmenopausal osteoporosis. The available information from the different studies performed in this field was systematized in order to highlight the most promising BTMs for the assessment of fracture healing outcome.As técnicas imagiológicas são o método convencional para a avaliação dos processos de cicatrização das fraturas. No entanto, estes métodos não são talvez totalmente confiáveis para a deteção precoce de complicações, as mais frequentes destas sendo o atraso da união e a não-união. Um diagnóstico eficaz destas desordens poderia prevenir a dor e a incapacidade prolongada do paciente. Esforços devem ser dirigidos no sentido do desenvolvimento de novas tecnologias para melhorar a exatidão no diagnóstico de complicações após fraturas ósseas. A variação nos níveis dos marcadores do turnover ósseo (BTMs) têm sido avaliados com vista à sua capacidade para prever o comprometimento da cicatrização das fraturas numa fase inicial, no entanto, as conclusões de alguns estudos não são consensuais. Neste artigo os autores fizeram uma revisão do potencial dos BTMs como fatores de previsibilidade precoce do prognóstico em doentes adultos que apresentavam fraturas ósseas traumáticas mas que não sofriam de osteopenia ou osteoporose pós-menopausa. A informação disponível nos diferentes estudos realizados neste campo foi sistematizada com vista a evidenciar-se os BTMs mais promissores para a avaliação da evolução da cicatrização das fraturas.SFRH/BD/45018/200
