    L'article és un estudi lingüístic del discurs periodístic de massa. L'anàlisi se centra en els titulars de premsa que contenen metàfores i respon a plantejaments retòrics, pragmàtics i cognitius. Aquesta anàlisi aplicada a les expressions metafòriques dels titulars pretén endinsar- se en el que simbòlicament és denominat per l'autora la caixa negra del pensament social. L'article és, doncs, un estudi pragmàtic de la mètafora com a peca del llenguatge, que reflecteix una determinada manera de veure el món; i un estudi del llenguatge periodístic com a versionador i constructor de la realitat. The analysis focuses on newspaper headlines containing metaphors and is based on pragmatic and cognitive theoretical approaches.This analysis, applied to metaphorical expressions used in newspaper headlines, aims to probe into what the author symbolically calls the «black box" of social thought.The article is, therefore, both a pragmatic study of metaphor as an item of language which reflects a certain way of seeing the world, and a study of journalistic language as shaper and builder of reality.     Background: The economic cost of not breastfeeding in Indonesia is estimated at US1.59.4billionannually,thehighestinSouthEastAsia.Halfofthe33.6millionworkingwomenofreproductiveage(WRA)inIndonesia(1549years)areinformalemployees,meaningtheyareworkingascasualworkersortheyareselfemployed(smallscalebusiness)andassistedbyunpaid/familyworker(s).NospecificmaternityprotectionentitlementsarecurrentlyavailableforWRAworkinginformallyinIndonesia.Thisstudyaimstoestimatethefinancingneedofprovidingmaternityleavecashtransfer(MCT)forWRAworkingintheinformalsectorinIndonesia.Method:ThecostingmethodologyusedistheadaptedversionoftheWorldBankmethodologybyVilarCompteetal,followingpresetstepstoestimatecostsusingnationalsecondarydata.Weusedthe2018IndonesianNationalSocioEconomicSurveytoestimatethenumberofwomenworkinginformallywhogavebirthwithinthelastyear.Thepopulationcovered,potentialcashtransfersunitarycost,theincrementalcoverageofthepolicyintermsoftimeandcoverage,andtheadministrativecostswereusedtoestimatethecostofMCTfortheinformalsector.Result:At1001.5–9.4 billion annually, the highest in South East Asia. Half of the 33.6 million working women of reproductive age (WRA) in Indonesia (15–49 years) are informal employees, meaning they are working as casual workers or they are self-employed (small scale business) and assisted by unpaid/family worker(s). No specific maternity protection entitlements are currently available for WRA working informally in Indonesia. This study aims to estimate the financing need of providing maternity leave cash transfer (MCT) for WRA working in the informal sector in Indonesia. Method: The costing methodology used is the adapted version of the World Bank methodology by Vilar-Compte et al, following pre-set steps to estimate costs using national secondary data. We used the 2018 Indonesian National Socio-Economic Survey to estimate the number of women working informally who gave birth within the last year. The population covered, potential cash transfer’s unitary cost, the incremental coverage of the policy in terms of time and coverage, and the administrative costs were used to estimate the cost of MCT for the informal sector. Result: At 100% coverage for 13 weeks of leave, the yearly financing need of MCT ranged from US175million (US152/woman)toUS152/woman) to US669million (US$583/woman). The share of the yearly financing need did not exceed 0.5% of Indonesian Gross Domestic Product (GDP). Conclusions: The yearly financing need of providing MCT for eligible WRA working in the informal sector is economically attractive as it amounts to less than 0.5% of GDP nominal of Indonesia. While such a program would be perceived as a marked increase from current public health spending at the onset, such an investment could substantially contribute to the success of breastfeeding and substantial corresponding public health savings given that more than half of working Indonesian WRA are employed in the informal sector. Such policies should be further explored while taking into consideration realistic budget constraints and implementation capacity

Aquesta informació s'ha consensuat entre farmacèutics i metges nefròlegs a partir de la informació disponible en diferents bases de dades nacionals i internacionals. Per agilitzar l'ús de la Guia s'ha consensuat categoritzar en nivell baix, moderat o alt el risc que suposa pel pacient l’ús d'aquests medicaments segons el seu filtrat glomerular. A més la Guia recull els ajustos de dosi a realitzar, les interaccions medicamentoses i la simptomatologia per sobre dosificació en pacients amb funció renal disminuïda. A partir d'aquesta Base de Dades, s'ha dissenyat una aplicació web que facilita al professional sanitari la presa de decisions per a l'ajust de dosis de medicaments en funció del filtrat glomerular del pacient

    Increased Lysis of Stem Cells but Not Their Differentiated Cells by Natural Killer Cells; De-Differentiation or Reprogramming Activates NK Cells

    The aims of this study are to demonstrate the increased lysis of stem cells but not their differentiated counterparts by the NK cells and to determine whether disturbance in cell differentiation is a cause for increased sensitivity to NK cell mediated cytotoxicity. Increased cytotoxicity and augmented secretion of IFN-γ were both observed when PBMCs or NK cells were co-incubated with primary UCLA oral squamous carcinoma stem cells (UCLA-OSCSCs) when compared to differentiated UCLA oral squamous carcinoma cells (UCLA-OSCCs). In addition, human embryonic stem cells (hESCs) were also lysed greatly by the NK cells. Moreover, NK cells were found to lyse human Mesenchymal Stem Cells (hMSCs), human dental pulp stem cells (hDPSCs) and human induced pluripotent stem cells (hiPSCs) significantly more than their differentiated counterparts or parental lines from which they were derived. It was also found that inhibition of differentiation or reversion of cells to a less-differentiated phenotype by blocking NFκB or targeted knock down of COX2 in monocytes significantly augmented NK cell cytotoxicity and secretion of IFN-γ. Taken together, these results suggest that stem cells are significant targets of the NK cell cytotoxicity. However, to support differentiation of a subset of tumor or healthy untransformed primary stem cells, NK cells may be required to lyse a number of stem cells and/or those which are either defective or incapable of full differentiation in order to lose their cytotoxic function and gain the ability to secrete cytokines (split anergy). Therefore, patients with cancer may benefit from repeated allogeneic NK cell transplantation for specific elimination of cancer stem cells
