    Validation of the steady state hover formulation for accurate performance predictions

    This paper shows accurate predictions for hover performance regardless of planform geometry, blade-tip Mach number, or disk loading. To prove this statement, sensitivity analyses were performed along with performance predictions for four rotor designs. Planform effects were also studied, such as the blade anhedral, showing the strong sensitivity of the rotor blade performance due to geometric features. The steady-state solution methodology with imposed Froude boundary conditions is shown to give accurate results for relatively coarse grid sizes. This approach leads to reduced computational costs as compared to time-dependent simulations. It is also recognized that, given the current accuracy of the available experimental data, the use of more advanced computational fluid dynamics methods may not be fully justified. To advance the accuracy of modern computational fluid dynamics methods, a comprehensive experimental dataset is required

    Potential benefits of melatonin in organ transplantation: a review

    Organ transplantation is a useful therapeutic tool for patients with end-stage organ failure; however, graft rejection is a major obstacle in terms of a successful treatment. Rejection is usually a consequence of a complex immunological and nonimmunological antigen-independent cascade of events, including free radical-mediated ischemia-reperfusion injury (IRI). To reduce the frequency of this outcome, continuing improvements in the efficacy of antirejection drugs are a top priority to enhance the long-term survival of transplant recipients. Melatonin (N-acetyl-5-methoxytryptamine) is a powerful antioxidant and ant-inflammatory agent synthesized from the essential amino acid L-tryptophan; it is produced by the pineal gland as well as by many other organs including ovary, testes, bone marrow, gut, placenta, and liver. Melatonin has proven to be a potentially useful therapeutic tool in the reduction of graft rejection. Its benefits are based on its direct actions as a free radical scavenger as well as its indirect antioxidative actions in the stimulation of the cellular antioxidant defense system. Moreover, it has significant anti-inflammatory activity. Melatonin has been found to improve the beneficial effects of preservation fluids when they are enriched with the indoleamine. This article reviews the experimental evidence that melatonin is useful in reducing graft failure, especially in cardiac, bone, otolaryngology, ovarian, testicular, lung, pancreas, kidney, and liver transplantation

    Methodology to improve water and energy use by proper irrigationscheduling in pressurised networks

    With the aim of reducing energy consumption and improving water use in pressurised irrigation systems, the methodology to minimise energy consumption by grouping intakes of pressurised irrigation networks into sectors, as developed by Jimenez Bello et al. (2010a), was modified to enable irrigation intakes to operate during the scheduled period for each intake instead of operating during restricted irrigation periods of the same length. Moreover, a method was developed to detect the maximum number of intakes that can operate without extra energy if the source has sufficient head to feed at least some of the intakes. These methods were applied to a Mediterranean irrigation system, where the total cropped area was mainly citrus orchards. In this case study, water was allocated to two groups of intakes, one fed by gravity and the other by pumps. A saving of 36.3 % was achieved by increasing the total volume supplied by gravity, decreasing the injection pump head, and improving the pump performance. Therefore, all the intakes only operated during the irrigation periods at the minimum required pressure.This research was supported by funds from Climate-KIC AGADAPT and from EU 7th Framework Programme FIGARO projects.     Hypofractionated radiation therapy and temozolomide in patients with glioblastoma and poor prognostic factors. A prospective, single-institution experience

    Background: Hypofractionated radiation therapy is a feasible and safe treatment option in elderly and frail patients with glioblastoma. The aim of this study was to evaluate the effectiveness of hypofractionated radiation therapy with concurrent temozolomide in terms of feasibility and disease control in primary glioblastoma patients with poor prognostic factors other than advanced age, such as post-surgical neurological complications, high tumor burden, unresectable or multifocal lesions, and potential low treatment compliance due to social factors or rapidly progressive disease. Material and methods: GTV included the surgical cavity plus disease visible in T1WI-MRI, FLAIR-MRI and in the MET-uptake. The CTV was defined as the GTV plus 1.5-2 cm margin; the PTV was the CTV+0.3 cm margin. Forty, fourty-five, and fifty grays in 15 fractions were prescribed to 95% of PTV, CTV, and GTV, respectively. Treatment was delivered using IMRT or the VMAT technique. Simultaneously, 75 mg/m2/day of temozolomide were administered. Results: Between January 2010 and November 2017, we treated a total of 17 patients. The median age at diagnosis was 68-years; median KPS was 50-70%. MGMT-methylation status was negative in 5 patients, and 8 patients were IDH-wildtype. Eight of 18 patients were younger than 65-years. Median tumor volume was 26.95cc; median PTV volume was 322cc. Four lesions were unresectable; 6 patients underwent complete surgical resection. Median residual volume was 1.14cc. Progression-free survival was 60% at 6 months, 33% at 1-year and 13% at 2-years (median OS = 7 months). No acute grade 3-5 toxicities were documented. Symptomatic grade 3 radiation necrosis was observed in one patient. Conclusions: Patients with poor clinical factors other than advanced age can be selected for hypofractionated radiotherapy. The OS and PFS rates obtained in our series are similar to those in patients treated with standard fractionation, assuring good treatment adherence, low rates of toxicity and probable improved cost-effectiveness

    Does accelerating universe indicates Brans-Dicke theory

    The evolution of universe in Brans-Dicke (BD) theory is discussed in this paper. Considering a parameterized scenario for BD scalar field ϕ=ϕ0aα\phi=\phi_{0}a^{\alpha} which plays the role of gravitational "constant" GG, we apply the Markov Chain Monte Carlo method to investigate a global constraints on BD theory with a self-interacting potential according to the current observational data: Union2 dataset of type supernovae Ia (SNIa), high-redshift Gamma-Ray Bursts (GRBs) data, observational Hubble data (OHD), the cluster X-ray gas mass fraction, the baryon acoustic oscillation (BAO), and the cosmic microwave background (CMB) data. It is shown that an expanded universe from deceleration to acceleration is given in this theory, and the constraint results of dimensionless matter density Ω0m\Omega_{0m} and parameter α\alpha are, Ω0m=0.2860.0390.047+0.037+0.050\Omega_{0m}=0.286^{+0.037+0.050}_{-0.039-0.047} and α=0.00460.01710.0206+0.0149+0.0171\alpha=0.0046^{+0.0149+0.0171}_{-0.0171-0.0206} which is consistent with the result of current experiment exploration, α0.132124\mid\alpha\mid \leq 0.132124. In addition, we use the geometrical diagnostic method, jerk parameter jj, to distinguish the BD theory and cosmological constant model in Einstein's theory of general relativity.Comment: 16 pages, 3 figure

    El futuro Centro Nacional de Tecnologías para la Fusión, TechnoFusión, tiene como objetivo desarrollar aquellas tecnologías, relacionadas con materiales, metales líquidos, manipulación remota y simulación, que permitan avanzar en los actuales retos que supone el uso de la fusión nuclear. Sus instalaciones, abiertas al servicio de la comunidad científica externa, garantizarán una destacada participación de empresas y grupos de investigación españoles. En esta contribución se describirán y analizarán las instalaciones que TechnoFusión construirá con capacidad para abordar: la fabricación y procesado de nuevas aleaciones a escala semiindustrial; el comportamiento de materiales, simulando las condiciones durante operación mediante una instalación de triple irradiación (iones pesados, hidrógeno y helio), generadores de plasma lineal (continuo y pulsado) y un circuito de litio líquido; y la caracterización del efecto que las severas condiciones de experimentación producen en las propiedades, en la composición y en la microestructura de materiales estructurales y funcionales (técnicas convencionales e in-situ)

    Clinical phenotypes of acute heart failure based on signs and symptoms of perfusion and congestion at emergency department presentation and their relationship with patient management and outcomes

    Objective To compare the clinical characteristics and outcomes of patients with acute heart failure (AHF) according to clinical profiles based on congestion and perfusion determined in the emergency department (ED). Methods and results Overall, 11 261 unselected AHF patients from 41 Spanish EDs were classified according to perfusion (normoperfusion = warm; hypoperfusion = cold) and congestion (not = dry; yes = wet). Baseline and decompensation characteristics were recorded as were the main wards to which patients were admitted. The primary outcome was 1-year all-cause mortality; secondary outcomes were need for hospitalisation during the index AHF event, in-hospital all-cause mortality, prolonged hospitalisation, 7-day post-discharge ED revisit for AHF and 30-day post-discharge rehospitalisation for AHF. A total of 8558 patients (76.0%) were warm+ wet, 1929 (17.1%) cold+ wet, 675 (6.0%) warm+ dry, and 99 (0.9%) cold+ dry; hypoperfused (cold) patients were more frequently admitted to intensive care units and geriatrics departments, and warm+ wet patients were discharged home without admission. The four phenotypes differed in most of the baseline and decompensation characteristics. The 1-year mortality was 30.8%, and compared to warm+ dry, the adjusted hazard ratios were significantly increased for cold+ wet (1.660; 95% confidence interval 1.400-1.968) and cold+ dry (1.672; 95% confidence interval 1.189-2.351). Hypoperfused (cold) phenotypes also showed higher rates of index episode hospitalisation and in-hospital mortality, while congestive (wet) phenotypes had a higher risk of prolonged hospitalisation but decreased risk of rehospitalisation. No differences were observed among phenotypes in ED revisit risk. Conclusions Bedside clinical evaluation of congestion and perfusion of AHF patients upon ED arrival and classification according to phenotypic profiles proposed by the latest European Society of Cardiology guidelines provide useful complementary information and help to rapidly predict patient outcomes shortly after ED patient arrival

    Atomic X-ray Spectroscopy of Accreting Black Holes

    Current astrophysical research suggests that the most persistently luminous objects in the Universe are powered by the flow of matter through accretion disks onto black holes. Accretion disk systems are observed to emit copious radiation across the electromagnetic spectrum, each energy band providing access to rather distinct regimes of physical conditions and geometric scale. X-ray emission probes the innermost regions of the accretion disk, where relativistic effects prevail. While this has been known for decades, it also has been acknowledged that inferring physical conditions in the relativistic regime from the behavior of the X-ray continuum is problematic and not satisfactorily constraining. With the discovery in the 1990s of iron X-ray lines bearing signatures of relativistic distortion came the hope that such emission would more firmly constrain models of disk accretion near black holes, as well as provide observational criteria by which to test general relativity in the strong field limit. Here we provide an introduction to this phenomenon. While the presentation is intended to be primarily tutorial in nature, we aim also to acquaint the reader with trends in current research. To achieve these ends, we present the basic applications of general relativity that pertain to X-ray spectroscopic observations of black hole accretion disk systems, focusing on the Schwarzschild and Kerr solutions to the Einstein field equations.     Effectiveness of an mHealth intervention combining a smartphone app and smart band on body composition in an overweight and obese population: Randomized controlled trial (EVIDENT 3 study)

    Background: Mobile health (mHealth) is currently among the supporting elements that may contribute to an improvement in health markers by helping people adopt healthier lifestyles. mHealth interventions have been widely reported to achieve greater weight loss than other approaches, but their effect on body composition remains unclear. Objective: This study aimed to assess the short-term (3 months) effectiveness of a mobile app and a smart band for losing weight and changing body composition in sedentary Spanish adults who are overweight or obese. Methods: A randomized controlled, multicenter clinical trial was conducted involving the participation of 440 subjects from primary care centers, with 231 subjects in the intervention group (IG; counselling with smartphone app and smart band) and 209 in the control group (CG; counselling only). Both groups were counselled about healthy diet and physical activity. For the 3-month intervention period, the IG was trained to use a smartphone app that involved self-monitoring and tailored feedback, as well as a smart band that recorded daily physical activity (Mi Band 2, Xiaomi). Body composition was measured using the InBody 230 bioimpedance device (InBody Co., Ltd), and physical activity was measured using the International Physical Activity Questionnaire. Results: The mHealth intervention produced a greater loss of body weight (–1.97 kg, 95% CI –2.39 to –1.54) relative to standard counselling at 3 months (–1.13 kg, 95% CI –1.56 to –0.69). Comparing groups, the IG achieved a weight loss of 0.84 kg more than the CG at 3 months. The IG showed a decrease in body fat mass (BFM; –1.84 kg, 95% CI –2.48 to –1.20), percentage of body fat (PBF; –1.22%, 95% CI –1.82% to 0.62%), and BMI (–0.77 kg/m2, 95% CI –0.96 to 0.57). No significant changes were observed in any of these parameters in men; among women, there was a significant decrease in BMI in the IG compared with the CG. When subjects were grouped according to baseline BMI, the overweight group experienced a change in BFM of –1.18 kg (95% CI –2.30 to –0.06) and BMI of –0.47 kg/m2 (95% CI –0.80 to –0.13), whereas the obese group only experienced a change in BMI of –0.53 kg/m2 (95% CI –0.86 to –0.19). When the data were analyzed according to physical activity, the moderate-vigorous physical activity group showed significant changes in BFM of –1.03 kg (95% CI –1.74 to –0.33), PBF of –0.76% (95% CI –1.32% to –0.20%), and BMI of –0.5 kg/m2 (95% CI –0.83 to –0.19). Conclusions: The results from this multicenter, randomized controlled clinical trial study show that compared with standard counselling alone, adding a self-reported app and a smart band obtained beneficial results in terms of weight loss and a reduction in BFM and PBF in female subjects with a BMI less than 30 kg/m2 and a moderate-vigorous physical activity level. Nevertheless, further studies are needed to ensure that this profile benefits more than others from this intervention and to investigate modifications of this intervention to achieve a global effect
