    The Development of an Application Conceived for the Design, Feasibility Study and Data Analysis of Photovoltaic Pumping Systems

    Because of the rise in diesel and butane prices widely used for pumping, added to their negative impact on both Morocco's environment and trade balance, the use of renewable energies should sound obvious, practical and cost effective. This study offers the transformation of a traditional butane pumping system (BPS) and diesel pumping system (DPS), located on a farm nearby the city of Agadir, into an optimized solar pumping system (SPS). The suggested method is based on a techno-economic study according to the “Business-As-usual” scenario. As a first step, we have dimensioned our pumping system and chosen the elements that constitute it. As a second step, we carried out an economic analysis, based on the calculation of all costs, which makes it possible to ensure the viability of the components of our SPS over its life cycle and brought it to a discounted value. The processing of the different data is made possible thanks to the computer application “PVDesign” which we have developed. This application has allowed us to carry out a comparative study of several techniques of pumping systems. The result of the study is that the SPS beats the other systems at various levels, namely economic, environmental and technical

    The mass trapping technique based on the use of the female-targeted attractant lure Tri-pack® as an alternative to malathion bait-spraying (control treatment) was tested in two citrus orchards in the North of Tunisia against the Mediterranean fruit fly Ceratitis capitata during 2006 and 2007. Results of mass trapping trials in 2006 and 2007 indicated that adult males Medfly captures showed reductions respect to control of 37.62% and 40.2% respectively in mandarin orange variety (Citrus reticulata) orchard compared to 36.48% and 47.29% in Washington navel orange variety (Citrus sinensis) field. Fruit damage assessment showed significant differences between the mass trapping with Tripack® and malathion bait-spraying techniques in the reduction of the percentage of fruit punctures. The percentage of punctured fruit at harvest was significantly different between the treated and the control field in 2006 and in 2007 in the mandarin orange orchard. Nevertheless, in the Washington navel orange orchard, the percentage of punctured fruit at harvest was significantly different between the treated and the control field only in 2006. Thus, results obtained from this study showed that the mass trapping technique based on the use of the female-targeted lure Tri-pack® could be involved as an appropriate strategy for the control of the Medfly and is as effective as malathion bait spraying treatment without leaving pesticide residues on fruit.Se probó en dos huertos de cítricos, en el norte de Túnez en 2006 y 2007, la técnica del trampeo masivo utilizando el atrayente de hembras Tri-Pack® contra la mosca mediterránea de la fruta, Ceratitis capitata como alternativa a los tratamientos cebo con malatión, que constituye la forma convencional de control utilizada por los agricultores y que se utilizó como tratamiento control. En 2006 y 2007, en los ensayos de trampeo masivo las capturas de machos adultos del insecto se redujeron respecto el control, respectivamente, un 37,62% y un 40,2% en la variedad de mandarina (Citrus reticulata) en comparación con un 36,48% y un 47,29% en la variedad de naranja Washington navel (Citrus sinensis). Al evaluar los daños en los frutos se observaron diferencias significativas entre el trampeo masivo con Tripack® y los tratamientos cebo con malatión con una reducción de las picaduras en los frutos El porcentaje de frutos con picaduras en el momento de la cosecha fue significativamente diferente en las parcelas control y tratada en el huerto de mandarina tanto en 2006 como en 2007. Sin embargo, en el huerto de naranja Washington navel, el porcentaje de frutos con picaduras recolectados fue significativamente diferente entre el tratado y el control sólo en 2006. Por tanto, los resultados obtenidos en este estudio muestran que la técnica de trampeo masivo utilizando el atrayente de hembras Tri-Pack® podría ser una estrategia adecuada para el control de la mosca mediterránea de la fruta y es tan efectiva como el tratamiento cebo con malatión, pero sin dejar residuos de plaguicidas en la fruta

    La dysplasie fibreuse est une affection bénigne rare dont l’étiologie est inconnue. Elle représente une anomalie dans le développement normal de l’os. L’os temporal est rarement atteint, donnant des complications telles qu’une surdité et une paralysie faciale périphérique. L’imagerie, principalement la tomodensitométrie est capitale pour le diagnostic positif. On rapporte un cas de dysplasie fibreuse retrouvé chez une femme de 43 ans présentant une surdité brutale.Mots clés : os temporal, dysplasie, scanner.Fibrous dysplasia is an uncommon benign disorder of unknown etiology. It most likely represents a disorder of bone normal development. The temporal bone is rarely involved, giving complications such as hearing loss and facial nerve palsy. The imaging, mainly performed with computerized tomography, plays a major role in positive diagnosis. We report a case of fibrous dysplasia found on a 43 year-old women presenting an acute sensorineural hearing loss.Key words: temporal bone, dysplasia, CT

    Predictors of Return to Work 12 Months After Solid Organ Transplantation: Results from the Swiss Transplant Cohort Study.

    Background Return to work with or after a chronic disease is not a very well understood process, influenced by a variety of personal, professional, societal and medical factors. The aim of this study is to identify predictors for return to work 12 months after a solid organ transplant applying a bio-psycho-social model. Methods This study is based on patients included in the Swiss Transplant Cohort Study, a national prospective multicentre cohort, who underwent a first solid organ transplant (kidney, liver, heart, lung). Bio-psycho-social factors were tested and predictors of return to work identified using logistic regression models. Results Among the 636 patients included in the study, 49.8% (317) were employed 12 months post-transplant. The major predictor for returning to work 12 months posttransplant was pre-transplant employment status (OR 10.8). Accordingly, the population was stratified in employed and not employed pre-transplant groups. Age, self-perceived health (6 months post-transplant) and the transplanted organ were significantly associated with post-transplant employment status in both groups. Return to work was influenced by education, depression (6 month post-transplant) and waiting time in the employed pre-transplant group and by invalidity pension in the not employed pre-transplant group. Conclusion Employment status pre-transplant being highly associated with employment status post-transplant, the process promoting return to work should be started well before surgery. Biomedical, psychological and social factors must be taken into account to promote return to work in transplanted patients

    Le syndrome de pourfour du petit : une manifestation rare des cellulites cervico-mediastinales

    Le syndrome de Pourfour du Petit (SPdP) relève de diverses étiologies. Nous rapportons le premier cas de SPdP secondaire à une cellulite cervico-médiastinale, a travers lequel nous décrivons l’étiopathogénie, la présentation clinique et les aspects évolutifs de cette entité clinique rare. un homme de 37 ans a été hospitalisé pour cellulite cervico-médiatinale. Outre le blindage inflammatoire cervical, l’examen a objectivé une mydriase gauche associée à une exophtalmie et un élargissement de la fente palpébrale d’installation récente. Le reste de l’examen neurologique et ORL a été sans particularité. Nous avons retenu le diagnostic du SPdP d’origine infectieuse. Le patient a été mis sous antibiothérapie et a eu un drainage et nettoyage de toute la loge viscérale cervicale et du médiastin supérieur. L’évolution a été favorable avec régression partielle des signes ophtalmiques. Le syndrome de Pourfour du Petit est une entité rare. Sa connaissance offre au clinicien des pistes diagnostiques notamment dans les pathologies mettant en jeu le pronostic vital.Mots clés : syndrome de Pourfour du Petit, système nerveux sympathique cervical, mydriase.The Pourfour de Petit syndrome (PdPS) is of various etiologies. We report the first case of SPdP secondary to cervicomediastinal cellulitis through which we describe the etiopathogeny, clinical presentation and evolution aspects of this rare clinical entity. A 37 years old male was hospitalized for cervico-mediastinal cellulitis. Besides the cervical inflammation, examination objectified a recent left mydriasis, exophthalmia and a widening of the palpebral fissure. The rest of the neurological examination was unremarkable. We retained the diagnostic of PdPS of an infectious origin. Beside antibiotherapy, the patient had drainage and cleaning of the visceral lodge and upper mediastinum. The evolution was favorable with partial regression of ophthalmic signs. The PdPS is a rare entity. its knowledge provides the clinician with diagnostic tracks including diseases involving lifethreatening.Key words : Pourfour du Petit syndrome, cervical sympathetic system, mydriasi

    Mucocèle sphénoïdale bilatérale : a propos d’un cas

    Les mucocèles sphénoïdales sont des formations pseudokystiques expansives, d’évolution lente, qui se développent au sein de la cavité sinusienne sphénoïdale. Elles possèdent la capacité d’éroder les parois osseuses et présentent une évolution pseudotumorale. Les rapports neuroméningés et orbitaires font la gravité de cette localisation sinusienne  la présentation clinique qui est souvent d’apparition retardée par rapport à la constitution de la mucocèle. L’imagerie (TDM et IRM) permet le diagnostic positif et le diagnostic différentiel avec certains processus développés au niveau de l’étage moyen de la base du crâne. La marsupialisation chirurgicale par voie endoscopique endonasale permet l’effondrement de la mucocèle.Mots-clés : mucocèle, sinus sphénoïda

    Tumeurs primitives de l’orbite traitement chirurgical

    Les tumeurs primitives de l’orbite sont des affections rares, leurs  manifestation cliniques sont tardives et variées pouvant êtres trompeuses. L’imagerie TDM et surtout IRM est capitale pour le diagnostic et la prise en charge. Le traitement est souvent chirurgical, le choix de la technique dépend de la taille et la topographie tumorale. Nous rapportons 13 cas de tumeurs orbitaires colligés entre 2004 et 2010. L’âge moyen de nos patients est de 36 ans, l’exophtalmie a été le maitre symptôme. Tous nos patients ont été opérés. Après étude histologique de la pièce d’exérèse opératoire il s’agissait de: 5 hémangiomes caverneux, 4 tumeurs mixtes, 2 neurinomes, 1 kyste hydatique et 1 angiome.     Protocol for evaluating a workplace intervention within the framework of consultations for suffering at work in French-speaking Switzerland.

    Psychosocial suffering involves diverse human, social and economic costs. Some 34.4% of workers in Switzerland report chronic stress related to their jobs. Medical consultations for suffering at work aim to maintain-or renew-patients' abilities to make decisions and act following a diagnosis of psychological suffering related to their work; they also aim to help workers return to their workstations or remain there. Workplace interventions by consulting occupational physicians can go beyond the subjective issues: they can be offered to employees, in anticipation of a return to work when this appears feasible from the outset. To qualitatively evaluate perceptions of workplace interventions and identify their effects by collecting the verbatim statements of employees and their employers. Qualitative single-centre study of workplace interventions conducted by the Consultation Service for Suffering at Work's occupational physicians for patients seen between January 2015 to December 2017. Nineteen workplace interventions took place, out of 184 different consultations. The verbatim statements of employees and their employers will be collected over a variable timeframe, using semi-structured face-to-face interviews. These will then be recorded, transcribed and analysed. Fourteen patients refused the workplace intervention. Their professional path will be collected for comparison and exploratory purposes. This exploratory research project will provide a better understanding of the issues surrounding work-related psychological suffering and of which strategies support patients most effectively
