    METAROUTE - Gestion de la qualité des données du repérage spatial et de la géométrie des axes routiers

    Le système d’information de la route, anciennement appelé banque de données routières, repose principalement sur un système de repérage linéaire : le système de repérage spatial de base (SRB). Alors que la plupart des systèmes d’information géographique (SIG) reposent sur un système de repérage planaire, basé sur des coordonnées nationales, il est indispensable de faire cohabiter ces deux vues de l’espace. Les projets SYRROU et AGRAM ont montré la complémentarité des deux systèmes de repérage et recommandé une meilleure intégration des différentes méthodes d’acquisition de données spatiales et des différentes sources d’information géographique. Face à la multitude des sources d’information géographique et à la nature intrinsèque des deux modes de repérage, il s’est avéré nécessaire de mieux documenter les données routières de façon à connaître leur origine et leur qualité. Cette documentation s’appuie sur le principe des métadonnées qui sont littéralement des données relatives à des données. Elles permettent ainsi à l’utilisateur d’avoir un aperçu sur le contenu d’un jeu de données. Le projet METAROUTE a donc défini les types d’information nécessaires à la description de l’origine et de la qualité des données du repérage spatial et de la géométrie des axes de route. Il comprend en particulier une analyse des besoins en matière de métadonnées pour la gestion de la qualité du repérage spatial des données routières. Il propose un catalogue de métadonnées spécifique pour la documentation des caractéristiques du repérage spatial des données routières, et en particulier de la géométrie des axes de route. Parmi les éléments de qualité du repérage spatial, la précision de la position est la caractéristique principale et elle fait l’objet d’une définition plus détaillée. Par analogie à la mensuration officielle, on propose des classes de précision (planimétrique et altimétrique) ou niveaux de tolérance. L’avantage de cette classification réside dans une interprétation facilitée pour l’utilisateur et permet aisément de comparer des jeux de données de sources différentes. L’évaluation de la qualité est faite par les niveaux de tolérance du repérage des objets de la route et du système de repérage spatial. Lors de la combinaison d’informations de qualités différentes, on propose de qualifier le résultat de la transformation en prenant le niveau de tolérance le plus bas

    Targeted colonic release formulations of mesalazine - A clinical pharmaco-scintigraphic proof-of-concept study in healthy subjects and patients with mildly active ulcerative colitis.

    Colonic targeting of orally applied therapeutic drugs remains a challenge. Tablet coatings relying on gastrointestinal pH and colonic bacterial enzymes as triggers in association with an inner alkaline layer are expected to improve targeting efficiency. Mesalazine release from three differently coated tablets labelled with 1 MBq 153Sm was characterised in a single centre, open-label, parallel group study in nineteen healthy subjects and seven patients with mildly active ulcerative colitis. Two semi-organic and one aqueous-based outer coating with different ratios of enteric polymer and resistant starch were tested. All coatings showed comparable release lagtimes in biorelevant dissolution media and were not affected by neutron-activation of the samarium tracer. Mesalazine pharmacokinetics and gamma scintigraphy were used to characterise drug release, anatomical site of tablet disintegration and gastrointestinal transit. Initial tablet disintegration occurred at the ileo-caecal junction or beyond in 92% of the subjects Time to initial tablet disintegration was inversely correlated with maximal plasma concentrations and systemic mesalazine exposure. Although high inter-subject variability precluded detection of differences between solvent types and different enteric polymer to polysaccharide ratios, the dual pH and enzymatic triggered release system in combination with an inner alkaline layer promoted mesalazine release at the target site with high accuracy

    When asked what the top three issues of the Las Vegas region were, the reply was “water, water, water! This was the result of a survey done a few years ago of Las Vegas Valley TV anchors. The reason for this response is that sustainability of the urban environments requires sufficient water resources as does population growth. With the advent of global climate change, this resource is in danger. Water flow and mountainous ice packs are impacted by this change in climate there by impacting the amount of water the the region. This is compounded over time as the population increases and the water supply decreases. Even as the flow of water to the Las Vegas Valley is decreasing, many people in the area do not fully appreciate the severity of this crisis. This knowledge is at many times not brought to fruition as many people do not even understand some possible ways to contribute to water conservation. With the idea of educating young Nevadans, future initiatives can be put into practice to further alleviate a dire situation. Research has shown that imagery is important for a students\u27 attention and enables changes in their thought process. With this approach, the goal of this project is to create an engaging environment to help awareness for the young and old alike

    Assessment of explanatory models of mental illness: effects of patient and interviewer characteristics

    Background: Explanatory models (EMs) refer to patients’ causal attributions of illness and have been shown to affect treatment preference and outcome. Reliable and valid assessment of EMs may be hindered by interviewer and respondent disparities on certain demographic characteristics, such as ethnicity. The present study examined (a) whether ethnic minority patients reported different EMs to ethnically similar interviewers in comparison with those with a different ethnicity, and (b) whether this effect was related to respondents’ social desirability, the perceived rapport with the interviewer and level of uncertainty toward their EMs. Methods: A total of 55 patients of Turkish and Moroccan origins with mood and anxiety disorders were randomly assigned to ethnically similar or dissimilar interviewers. EMs were assessed, using a semi-structured interview, across 11 different categories of causes. Results: Participants who were interviewed by an ethnically similar interviewer perceived interpersonal, victimization and religious/mystical causes as more important, whereas interviews by ethnically dissimilar interviewers generated higher scores on medical causes. These effects were not mediated by the perceived rapport with the interviewer, and social desirability had a modest impact on the results. Higher uncertainty among participants toward medical and religious/mystical causes seemed to be associated with greater adjustment in the report of these EMs. Conclusion: The findings have significant implications for interviewer selection in epidemiological research and clinical practice

    Reliability of the interRAI suite of assessment instruments: a 12-country study of an integrated health information system

    <p>Abstract</p> <p>Background</p> <p>A multi-domain suite of instruments has been developed by the interRAI research collaborative to support assessment and care planning in mental health, aged care and disability services. Each assessment instrument comprises items common to other instruments and specialized items exclusive to that instrument. This study examined the reliability of the items from five instruments supporting home care, long term care, mental health, palliative care and post-acute care.</p> <p>Methods</p> <p>Paired assessments on 783 individuals across 12 nations were completed within 72 hours of each other by trained assessors who were blinded to the others' assessment. Reliability was tested using weighted kappa coefficients.</p> <p>Results</p> <p>The overall kappa mean value for 161 items which are common to 2 or more instruments was 0.75. The kappa mean value for specialized items varied among instruments from 0.63 to 0.73. Over 60% of items scored greater than 0.70.</p> <p>Conclusion</p> <p>The vast majority of items exceeded standard cut-offs for acceptable reliability, with only modest variation among instruments. The overall performance of these instruments showed that the interRAI suite has substantial reliability according to conventional cut-offs for interpreting the kappa statistic. The results indicate that interRAI items retain reliability when used across care settings, paving the way for cross domain application of the instruments as part of an integrated health information system.</p

    Good practice in health care for migrants: views and experiences of care professionals in 16 European countries

    <p>Abstract</p> <p>Background</p> <p>Health services across Europe provide health care for migrant patients every day. However, little systematic research has explored the views and experiences of health care professionals in different European countries. The aim of this study was to assess the difficulties professionals experience in their service when providing such care and what they consider constitutes good practice to overcome these problems or limit their negative impact on the quality of care.</p> <p>Methods</p> <p>Structured interviews with open questions and case vignettes were conducted with health care professionals working in areas with high proportion of migrant populations in 16 countries. In each country, professionals in nine primary care practices, three accident and emergency hospital departments, and three community mental health services (total sample = 240) were interviewed about their views and experiences in providing care for migrant patients, i.e. from first generation immigrant populations. Answers were analysed using thematic content analysis.</p> <p>Results</p> <p>Eight types of problems and seven components of good practice were identified representing all statements in the interviews. The eight problems were: language barriers, difficulties in arranging care for migrants without health care coverage, social deprivation and traumatic experiences, lack of familiarity with the health care system, cultural differences, different understandings of illness and treatment, negative attitudes among staff and patients, and lack of access to medical history. The components of good practice to overcome these problems or limit their impact were: organisational flexibility with sufficient time and resources, good interpreting services, working with families and social services, cultural awareness of staff, educational programmes and information material for migrants, positive and stable relationships with staff, and clear guidelines on the care entitlements of different migrant groups. Problems and good care components were similar across the three types of services.</p> <p>Conclusions</p> <p>Health care professionals in different services experience similar difficulties when providing care to migrants. They also have relatively consistent views on what constitutes good practice. The degree to which these components already are part of routine practice varies. Implementing good practice requires sufficient resources and organisational flexibility, positive attitudes, training for staff and the provision of information.</p

    The Synergistic Effect of Concomitant Schistosomiasis, Hookworm, and Trichuris Infections on Children's Anemia Burden

    Polyparasitic infections have been recognized as the norm in many tropical developing countries, but the significance of this phenomenon for helminth-associated morbidities is largely unexplored. Earlier studies have suggested that multi-species, low-intensity parasitic infections were associated with higher odds of anemia among school-age children relative to their uninfected counterparts or those with one low-intensity infection. However, specific studies of the nature of interactions between helminth species in the mediation of helminth-associated morbidities are lacking. This study quantifies the extent to which polyparasitic infections have more than the sum of adverse effects associated with individual infections in the context of childhood anemia. This study found that the risk of anemia is amplified beyond the sum of risks for individual infections in children simultaneously exposed to 1) hookworm and schistosomiasis, and 2) hookworm and trichuris, and suggests that combined treatment for some geohelminth species and schistosomiasis could yield greater than additive benefits for the reduction of childhood anemia in helminth-endemic areas. However, more studies to understand the full range of interactions between parasitic species in their joint effects on helminth-associated morbidities will be necessary to better predict the impact of any future public health intervention

    Spatial and Genetic Epidemiology of Hookworm in a Rural Community in Uganda

    There are remarkably few contemporary, population-based studies of intestinal nematode infection for sub-Saharan Africa. This paper presents a comprehensive epidemiological analysis of hookworm infection intensity in a rural Ugandan community. Demographic, kinship, socioeconomic and environmental data were collected for 1,803 individuals aged six months to 85 years in 341 households in a cross-sectional community survey. Hookworm infection was assessed by faecal egg count. Spatial variation in the intensity of infection was assessed using a Bayesian negative binomial spatial regression model and the proportion of variation explained by host additive genetics (heritability) and common domestic environment was estimated using genetic variance component analysis. Overall, the prevalence of hookworm was 39.3%, with the majority of infections (87.7%) of light intensity (≤1000 eggs per gram faeces). Intensity was higher among older individuals and was associated with treatment history with anthelmintics, walking barefoot outside the home, living in a household with a mud floor and education level of the household head. Infection intensity also exhibited significant household and spatial clustering: the range of spatial correlation was estimated to be 82 m and was reduced by a half over a distance of 19 m. Heritability of hookworm egg count was 11.2%, whilst the percentage of variance explained by unidentified domestic effects was 17.8%. In conclusion, we suggest that host genetic relatedness is not a major determinant of infection intensity in this community, with exposure-related factors playing a greater role
