    Spanish Version of the Scale 'Eventos Adversos Associados às Práticas de Enfermagem' (EAAPE): Validation in Nursing Students

    Healthcare carried out by different health professionals, including nurses, implies the possible appearance of adverse events that affect the safety of the patient and may cause damage to the patient. In clinical practice, it is necessary to have measurement instruments that allow for the evaluation of the presence of these types of events in order to prevent them. This study aims to validate the 'Eventos adversos associados às práticas de enfermagem' (EAAPE) scale in Spanish and evaluate its reliability. The validation was carried out through a cross-sectional study with a sample of 337 nursing students from the University of Valencia recruited during the 2018-19 academic year. An exploratory factor analysis was carried out using principal components and varimax rotation. The factor analysis extracted two factors that explained 32.10% of the total variance. Factor 1 explains 22.19% and refers to the 'adverse results' of clinical practice (29 items), and factor 2 explains 9.62% and refers to 'preventive practices' (24 items). Both factors presented high reliability (Cronbach's alpha 0.902 and 0.905, respectively). The Spanish version of the EAAPE is valid and reliable for measuring the perception of adverse events associated with nursing practice and the presence of prevention measures

    El trabajo que se desarrolló durante este PAP está enfocado en la vivienda emergente, específicamente en el caso del señor Maximiliano, quien habita en la comunidad de Santa Maria Tequepexpan y quien vive en una situación precaria. Por lo cual se tomó como modelo y usuario principal, para generar un prototipo de vivienda emergente que sea funcional y responda a las necesidades básicas que requiere nuestro usuario, así como gran parte de la población en condiciones similares o peores. Este proyecto ya ha sido desarrollado desde el semestre pasado, en el cual se presentaron circunstancias que no nos permitieron construir el proyecto por problemas legales con el predio. Se hizo un replanteamiento del proyecto en donde se propuso construir sobre la estructura ya existente donde vive el señor Maximiliano, utilizando otro terreno distinto al proyecto ya propuesto con anterioridad. La nueva propuesta se enfoca en mejorar la calidad de vida y las condiciones de vivienda de la estructura ya existente, brindando un techo, baño, regadera y cubriendo las necesidades básicas principalmente con un bajo presupuesto y sin el requerimiento de de muchos recursos

    DNA Hydroxymethylation in the Regulation of Gene Expression in Human Solid Cancer

    Chromatin in cancer undergoes chemical and structural changes that alter gene expression patterns. One of the chemical modifications that impacts gene regulation is 5-hydroxymethylcytosine (5hmC), also called DNA hydroxymethylation. 5hmC is a stable mark that is commonly associated with transcriptional activation. In cancer, the global loss of 5hmC is a hallmark. In addition, the deregulation of 5hmC in specific regions of the genome, such as enhancers, promoters, and body of the gene, alters the expression of genes in cancer. These alterations have been detected by the improvement in the mapping of 5hmC at genomic scale, which has allowed us to evaluate the sites where 5hmC alterations occur and the genes that are affected. In this chapter, the recent knowledge about the status of 5hmC in genome specific sites of human solid cancers, the relationship with enzymes ten-eleven translocation (TET) and isocitrate dehydrogenase (IDH) involved in the dynamic regulation of 5hmC levels, and the impact of the 5hmC aberrant changes on the genic expression in these malignances is reviewed

    The combination of sirolimus plus tacrolimus improves outcome after reduced-intensity conditioning, unrelated donor hematopoietic stem cell transplantation compared with cyclosporine plus mycofenolate

    Different types of graft-versus-host disease prophylaxis have been proposed in the setting of reduced intensity and non-myeloablative allogeneic stem cell transplantation. An alternative combination with sirolimus and tacrolimus has recently been tested although comparative studies against the classical combination of a calcineurin inhibitor and mycophenolate mofetil or methotrexate are lacking. We describe the results of a prospective, multicenter trial using sirolimus + tacrolimus as immunoprophylaxis, and compare this approach with our previous experience using cyclosporine + mycophenolate in the setting of unrelated donor transplantation setting after reduced-intensity conditioning. Forty-five patients received cyclosporine + mycophenolate between 2002 and mid-2007, while the subsequent 50 patients, who were transplanted from late 2007, were given sirolimus + tacrolimus. No significant differences were observed in terms of hematopoietic recovery or acute graft-versus-host disease overall, although gastrointestinal acute graft-versus-host disease grade ≥2 was more common in the cyclosporine + mycophenolate group (55% versus 21%, respectively, P=0.003). The 1-year cumulative incidence of chronic graftversus-host disease was 50% versus 90% for the patients treated with the sirolimus- versus cyclosporine-based regimen, respectively (P<0.001), while the incidence of extensive chronic disease was 27% versus 49%, respectively (P=0.043). The 2-year non-relapse mortality rate was 18% versus 38% for patients receiving the sirolimus- versus the cyclosporine-based regimen, respectively (P=0.02). The event-free survival and overall survival at 2 years were 53% versus 29% (P=0.028) and 70% versus 45% (P=0.018) among patients receiving the sirolimus- versus the cyclosporine-based regimen, respectively. In conclusion, in the setting of reduced intensity transplantation from an unrelated donor, promising results can be achieved with the combination of sirolimus + tacrolimus, due to a lower risk of chronic graft-versus-host disease and non-relapse mortality, which translates into better event-free and over all survival rates, in comparison with those achieved with cyclosporine + mycophenolate

    El ejercicio del liderazgo docente en Colombia: voces de los maestros

    119 PáginasEsta investigación se inicia a partir de la ausencia de estudios relacionados con el tema del liderazgo docente en Colombia. Los objetivos se centran en: (1) interpretar las creencias que subyacen a los conceptos de liderazgo de un grupo de docentes, (2) identificar motivaciones, dificultades y logros en su ejercicio como líderes y (3) determinar, desde su perspectiva, las transformaciones en su ejercicio de liderazgo propiciado por los programas de cualificación de posgrado en los que participaron. El estudio de tipo cualitativo, descriptivo e interpretativo empleó entrevistas a profundidad con 21 docentes, participantes de programas posgraduales de la Ruta de Formación de la Universidad de La Sabana. Los resultados indican que el docente-líder se concibe como un visionario con un buen manejo de habilidades comunicativas y sociales que lo convierten en modelo ejemplarizante dentro de la comunidad. La investigación resalta su rol como gestor de innovaciones y como agente en la movilización de acciones y grupos de personas y pone de relieve la injerencia del relacionamiento apropiado con directivos docentes y pares en la realización de sus propuestas.This research responds to the absence of studies related to the topic of teacher leadership in Colombia. The main objectives were: (1) interpreting the beliefs about the concept of leadership from a group of teachers, (2) identifying motivations, difficulties and achievements in their role as leaders and (3) determining, from their perspective, the transformations in their leadership fostered by the graduate qualification programs in which they participated. This qualitative, descriptive and interpretative study collected data through in-depth interviews from 21 teachers who were participants of postgraduate programs of the “Ruta de Formación”, a program of Universidad de La Sabana. The results indicate teacher-leaders believed they were visionaries characterized by solid communication and social skills and were exemplary models within the community. The research highlights their role as innovators and as agents for the mobilization of actions and groups of people, and shows the influence of healthy relationships with principals and peers in the realization of their proposals. The research also reveals that qualification programs are opportunities to generate reflections that lead them to take actions based on the theory and to propose alternatives for the development of their skills and talents. The research emphasizes the need to generate synergies between the university and the student to respond more assertively to their qualification needs and support their process

    Exposing and Overcoming Limitations of Clinical Laboratory Tests in COVID-19 by Adding Immunological Parameters; A Retrospective Cohort Analysis and Pilot Study

    BackgroundTwo years since the onset of the COVID-19 pandemic no predictive algorithm has been generally adopted for clinical management and in most algorithms the contribution of laboratory variables is limited. ObjectivesTo measure the predictive performance of currently used clinical laboratory tests alone or combined with clinical variables and explore the predictive power of immunological tests adequate for clinical laboratories. Methods: Data from 2,600 COVID-19 patients of the first wave of the pandemic in the Barcelona area (exploratory cohort of 1,579, validation cohorts of 598 and 423 patients) including clinical parameters and laboratory tests were retrospectively collected. 28-day survival and maximal severity were the main outcomes considered in the multiparametric classical and machine learning statistical analysis. A pilot study was conducted in two subgroups (n=74 and n=41) measuring 17 cytokines and 27 lymphocyte phenotypes respectively. Findings1) Despite a strong association of clinical and laboratory variables with the outcomes in classical pairwise analysis, the contribution of laboratory tests to the combined prediction power was limited by redundancy. Laboratory variables reflected only two types of processes: inflammation and organ damage but none reflected the immune response, one major determinant of prognosis. 2) Eight of the thirty variables: age, comorbidity index, oxygen saturation to fraction of inspired oxygen ratio, neutrophil-lymphocyte ratio, C-reactive protein, aspartate aminotransferase/alanine aminotransferase ratio, fibrinogen, and glomerular filtration rate captured most of the combined statistical predictive power. 3) The interpretation of clinical and laboratory variables was moderately improved by grouping them in two categories i.e., inflammation related biomarkers and organ damage related biomarkers; Age and organ damage-related biomarker tests were the best predictors of survival, and inflammatory-related ones were the best predictors of severity. 4) The pilot study identified immunological tests (CXCL10, IL-6, IL-1RA and CCL2), that performed better than most currently used laboratory tests. ConclusionsLaboratory tests for clinical management of COVID 19 patients are valuable but limited predictors due to redundancy; this limitation could be overcome by adding immunological tests with independent predictive power. Understanding the limitations of tests in use would improve their interpretation and simplify clinical management but a systematic search for better immunological biomarkers is urgent and feasible
