    Fall-Related Emergency Department Traumas at Thomas Jefferson University Hospital: A Retrospective Analysis of Elderly Adult Trends

    Falls are the most common reason for non-fatal injuries treated at Emergency Departments (EDs) in the United States (US), excluding those ages 15-24. Pennsylvania (PA) spends 2.7billiontreatingfall−relatedinjuriesyearly,withanaverageper−hospitalizationcostof2.7 billion treating fall-related injuries yearly, with an average per-hospitalization cost of 58,529. PA also has the 5th highest state senior population, and Philadelphia has the 5th oldest senior population among the major American cities. Examining the 2016 Thomas Jefferson University (TJU) ED trauma database, we analysed fall-related traumas in adults ages 65 and over. The sample was stratified into 3 age groups (65-74, 75-84, and ³85) and SAS 9.4 was used to investigate age group trends for fall type, average length of stay (LOS), post-ED destination, loss of consciousness (LOC), incidents by month, and if LOC impacted patient LOS. We found a significant association between LOC and LOS, with those experiencing an LOC staying in hospital an average of 1.7 days. We did not observe significant differences across age groups for fall type, length of stay, post-ED destination, LOC, or number of incidents by month. While results are not significant they are suggestive and may reveal patients being more likely to experience a simple fall (defined as a fall without a slip or trip) as age increases, and less likely to experience a fall with a trip as age increases. All patients, regardless of age group, have average LOS of approximately 7 days. Analysis of our study sample (n=400) describes trends in elderly adult fall-related trauma patients at the TJU ED. We observed that LOC due to a fall is associated with an increased LOS, and for several utilization comparisons there are no significant differences between age groups. This study helps to better understand the patients we serve and identify trends in utilization that may contribute to ongoing fall-prevention efforts at TJU

    Development of an online database of typical food portion sizes in Irish population groups

    The Irish Food Portion Sizes Database (available at www.iuna.net) describes typical portion weights for an extensive range of foods and beverages for Irish children, adolescents and adults. The present paper describes the methodologies used to develop the database and some key characteristics of the portion weight data contained therein. The data are derived from three large, cross-sectional food consumption surveys carried out in Ireland over the last decade: the National Children's Food Survey (2003–2004), National Teens' Food Survey (2005–2006) and National Adult Nutrition Survey (2008–2010). Median, 25th and 75th percentile portion weights are described for a total of 545 items across the three survey groups, split by age group or sex as appropriate. The typical (median) portion weights reported for adolescents and adults are similar for many foods, while those reported for children are notably smaller. Adolescent and adult males generally consume larger portions than their female counterparts, though similar portion weights may be consumed where foods are packaged in unit amounts (for example, pots of yoghurt). The inclusion of energy under-reporters makes little difference to the estimation of typical portion weights in adults. The data have wide-ranging applications in dietary assessment and food labelling, and will serve as a useful reference against which to compare future portion size data from the Irish population. The present paper provides a useful context for researchers and others wishing to use the Irish Food Portion Sizes Database, and may guide researchers in other countries in establishing similar databases of their own

    Objective: To describe relationships between the portion sizes of a range of foods commonly consumed by Irish children and adolescents and key indicators of dietary quality on the days they were consumed. Design: Cross-sectional data from the Irish National Children’s Food Survey (2003–2004; 7 d weighed record) and National Teens’ Food Survey (2005–2006; 7 d semi-weighed record) were used to compare mean values for a number of dietary quality indicators (e.g. energy-adjusted intakes of saturated fat, dietary fibre and Na) across portion size tertiles for a range of foods, on the days the foods were consumed. Setting: The Republic of Ireland. Subjects: Nationally representative samples of children aged 5–12 years (n 594) and adolescents aged 13–17 years (n 441). Results: Relationships between food portion sizes and indicators of dietary quality on the days the foods were consumed were similar in both children and adolescents. Lower dietary energy density and saturated fat intakes, and higher dietary fibre intakes, were observed on the days larger portions of fruit and boiled potatoes were consumed. Higher dietary energy density and lower micronutrient intakes were observed on the days larger portions of sugar-sweetened beverages were consumed. Higher Na intakes were observed on the days larger portions of frying meats were consumed. Conclusions: The current work identifies foods for which larger portion sizes may be associated with positive dietary attributes, as well as the opposite. Findings will form an evidence base from which more specific dietary guidance relating to portion size may be developed for Irish children and adolescents

    Molecular dynamics simulation of the phosphorylation-induced conformational changes of a tau peptide fragment

    Aggregation of the microtubule associated protein tau (MAPT) within neurons of the brain is the leading cause of tauopathies such as Alzheimer's disease. MAPT is a phospho-protein that is selectively phosphorylated by a number of kinases in vivo to perform its biological function. However, it may become pathogenically hyperphosphorylated, causing aggregation into paired helical filaments and neurofibrillary tangles. The phosphorylation induced conformational change on a peptide of MAPT (htau225-250) was investigated by performing molecular dynamics simulations with different phosphorylation patterns of the peptide (pThr231 and/or pSer235) in different simulation conditions to determine the effect of ionic strength and phosphate charge. All phosphorylation patterns were found to disrupt a nascent terminal ß-sheet pattern (226VAVVR230 and 244QTAPVP249), replacing it with a range of structures. The double pThr231/pSer235 phosphorylation pattern at experimental ionic strength resulted in the best agreement with NMR structural characterization, with the observation of a transient α-helix (239AKSRLQT245). PPII helical conformations were only found sporadically throughout the simulations

These proceedings contain written versions of nearly all the talks, several of which were given by professional statisticians. The talks varied from general overviews, to those describing searches for specific particles. The treatment of background uncertainties figured prominently. Many of the talks describing search strategies for new effects should be of interest not only to particle physicists but also to scientists in other fields

    An unprecedented NH2-directed Pd(II)-catalytic carbonylation of quaternary aromatic α -amino esters to yield 6-membered 10 benzolactams has been developed. The reaction shows a strong bias to 6-membered lactams over 5-membered ones. The steric hindrance around the amino group seems to be pivotal for the success of the process

    Counting spanning trees in a small-world Farey graph

    The problem of spanning trees is closely related to various interesting problems in the area of statistical physics, but determining the number of spanning trees in general networks is computationally intractable. In this paper, we perform a study on the enumeration of spanning trees in a specific small-world network with an exponential distribution of vertex degrees, which is called a Farey graph since it is associated with the famous Farey sequence. According to the particular network structure, we provide some recursive relations governing the Laplacian characteristic polynomials of a Farey graph and its subgraphs. Then, making use of these relations obtained here, we derive the exact number of spanning trees in the Farey graph, as well as an approximate numerical solution for the asymptotic growth constant characterizing the network.     Integration of magnetic bearings in the design of advanced gas turbine engines

    Active magnetic bearings provide revolutionary advantages for gas turbine engine rotor support. These advantages include tremendously improved vibration and stability characteristics, reduced power loss, improved reliability, fault-tolerance, and greatly extended bearing service life. The marriage of these advantages with innovative structural network design and advanced materials utilization will permit major increases in thrust to weight performance and structural efficiency for future gas turbine engines. However, obtaining the maximum payoff requires two key ingredients. The first key ingredient is the use of modern magnetic bearing technologies such as innovative digital control techniques, high-density power electronics, high-density magnetic actuators, fault-tolerant system architecture, and electronic (sensorless) position estimation. This paper describes these technologies. The second key ingredient is to go beyond the simple replacement of rolling element bearings with magnetic bearings by incorporating magnetic bearings as an integral part of the overall engine design. This is analogous to the proper approach to designing with composites, whereby the designer tailors the geometry and load carrying function of the structural system or component for the composite instead of simply substituting composites in a design originally intended for metal material.     Chorus acceleration of radiation belt relativistic electrons during March 2013 geomagnetic storm

    Abstract The recent launching of Van Allen probes provides an unprecedent opportunity to investigate variations of the radiation belt relativistic electrons. During the 17-19 March 2013 storm, the Van Allen probes simultaneously detected strong chorus waves and substantial increases in fluxes of relativistic (2 - 4.5 MeV) electrons around L = 4.5. Chorus waves occurred within the lower band 0.1-0.5fce (theelectron equatorial gyrofrequency), with a peak spectral density ∼10-4 nT 2/Hz. Correspondingly, relativistic electron fluxes increased by a factor of 102-103 during the recovery phase compared to the main phase levels. By means of a Gaussian fit to the observed chorus spectra, the drift and bounce-averaged diffusion coefficients are calculated and then used to solve a 2-D Fokker-Planck diffusion equation. Numerical simulations demonstrate that the lower-band chorus waves indeed produce such huge enhancements in relativistic electron fluxes within 15 h, fitting well with the observation.     Fruit and vegetable intakes, sources and contribution to total diet in very young children (1–4 years): the Irish National Pre-School Nutrition Survey

    Although the importance of fruit and vegetable (F&V) intakes in the prevention of chronic diseases is well established, there are limited data on intakes in very young children. This study estimates F&V intakes and sources and the contribution to the total diet using data from the National Pre-School Nutrition Survey, a nationally representative sample (n 500) of Irish children aged 1–4 years. A 4-d weighed food record was used to collect food intake data. Of 1652 food codes consumed, 740 had a fruit/vegetable component. The percentage of edible fruits and/or vegetables in each food code was calculated. Intakes (g/d), sources (g/d) and the contribution of F&V to the weight of the total diet (%) were estimated, split by age. All children consumed F&V. Intakes of total fruits, in particular fruit juice, increased with age. The contribution to total fruit intake was discrete fruits (47–56 % range across age), 100 % fruit juice, smoothies and pureés (32–45 %) as well as fruits in composite dishes (7–13 %). Total vegetable intake comprised of discrete vegetables (48–62 % range across age) and vegetables in composite dishes (38–52 %). F&V contributed on average 20 % (15 % fruit; 5 % vegetables) to the weight of the total diet and was <10 % in sixty-one children (12 %). F&V contributed 50 % of vitamin C, 53 % of carotene, 34 % of dietary fibre and 42 % of non-milk sugar intakes from the total diet. F&V are important components of the diet of Irish pre-school children; however, some aspects of F&V intake patterns could be improved in this age group
