    The study and analysis of the kaolin clay tobacco pipe collection from the seventeenth-century archaeological site of Port Royal, Jamaica

    The examination and analysis of the kaolin clay pipe collection from Port Royal, Jamaica, revealed several trends. From a database of 21,575 pipes recovered from 1981-1990, 61 bowl types were identified and arranged in an expandable typology. Thirty-nine makers' marks were also identified, many of them ascribed to Bristol pipemakers, where most of Port Royal's pipes were manufactured and exported. Findings from the Bristol Port Books for 1682 and 1694-1695, and from entries listed in the Jamaica Probate Records verify that enormous quantities of clay pipes were being shipped to Port Royal. The heavy concentrations of pipes found in Room 5, Building 1, and Room 2, Building 3, also support these findings and strongly suggest that both buildings had storage facilities which contained the current retail stock in pipes to be sold in the shops and taverns of Port Royal. In applying Stanley South's model, the "Brunswick Pattern of adjacent secondary refuse disposal" to the yard areas at Port Royal, it was found that the accumulation of pipes and other artifacts in the yards reflects areas of multiple activities rather than the disposal behavior commonly associated with English colonial sites. Applying Binford's straight-line regression formula to the Port Royal pipes resulted in date ranges close to the Port Royal earthquake. A comparison of the Binford and Heighton/Deagan methods of formula dating also confirmed that the Binford method was more reliable, because Heighton/Deagan dates were consistently off by 20 years or more. The Port Royal pipes also reflect other trends within the context of 17th-century English economy and society. The desire for tobacco fueled a tobacco-growing economy in the Chesapeake colonies, which necessitated the manufacture of clay pipes in England. The production of clay pipes also reflects pre-industrial manufacturing and consumerism fueled by English colonization and trade, which eventually propelled England into a position of dominance in the world system. The desire for novel types of food and drink, as well as tobacco, resulted in the adoption of new customs and habits into English society, particularly in public institutions like the tavern

    Leading wellness in healthcare:A qualitative study of leadership practices for wellness in hospital settings

    Ways of dealing with workplace stress and enhancing healthcare workers wellness are sought globally. The aim of this study was to explore healthcare leaders\u27 practice in relation to the implementation of a workplace wellness program called SEED in the context of multiple crises (bushfires and COVID-19) affecting a local health district in New South Wales, Australia. Practice theory informed interviews (n = 23), focus groups (n = 2) and co-analysis reflexive discussions (n = 2) that were conducted with thirteen leaders and twenty healthcare workers. A pragmatic approach to program implementation for healthcare workers\u27 wellness explored the process and actions that resulted from leadership practice in an inductive thematic analysis. Preliminary themes were presented in the co-analysis sessions to ensure the lived experiences of the SEED program were reflected and co-interpretation of the data was included in the analysis. Three key themes were identified. 1) Leading change-implementing a wellness program required leaders to try something new and be determined to make change happen. 2) Permission for wellness-implicit and explicit permission from leaders to engage in wellness activities during worktime was required. 3) Role-modelling wellness-leaders viewed SEED as a way to demonstrate leadership in supporting and caring for healthcare workers. SEED provided a platform for leaders who participated to demonstrate their leadership practices in supporting wellness activities. Leadership practices are critical to the implementation of healthcare wellness programs. The implementation of SEED at a time of unprecedented crisis gave leaders and healthcare workers opportunities to experience something new including leadership that was courageous, responsive and authentic. The study highlighted the need for workplace wellness programs to intentionally include leaders rather than only expect them to implement them. The practices documented in this study provide guidance to others developing, implementing and researching workplace wellness programs

    The SEED Wellness Model: A Workplace Approach to Address Wellbeing Needs of Healthcare Staff During Crisis and Beyond

    Workplace wellness has gained new meaning and significance in the healthcare workforce in the face of the COVID-19 pandemic. Healthcare workers across the world have carried the burden of responding to the public health crisis by having to work under new pressures and constantly changing environments, take on additional shifts, risk their own health and lives, and cope with the ongoing psychological and emotional strain. The purpose of this paper is to articulate a workplace wellness model applied across hospitals in the Illawarra Shoalhaven Local Health District, a regional area in New South Wales, Australia. The description of the development, components, and lessons learned from the SEED Wellness Model illustrates one possible solution about how to provide better care for the staff thus not only preventing staff burnout and turnover, but also creating lasting organizational benefits. The detailed model description can assist in developing a larger and more rigorous evidence-base to improve staff wellness in healthcare settings, both within Australia and internationally

    Faculty and Administrative Partnerships: Disciplinary Differences in Perceptions of Civic Engagement and Service-Learning at a Large, Research-Extensive University

    In recent years, considerable energy has been expended attempting to define, evaluate and promote active learning pedagogies such as civic engagement and service-learning. Yet much of this scholarship treats civic engagement and service-learning at either a macroscopic level (studying an entire university system) or microscopic level (studying a particular course or project). There has been comparably less research examining how different disciplinary cultures influence the conceptualization and implementation of active learning pedagogies within individual institutions. This study draws on quantitative survey methodologies to examine faculty perceptions of civic engagement and service-learning at a major public research university within and across four disciplines: the Humanities, Behavioral and Social Sciences, Science, Technology, Engineering, and Mathematics (STEM), and the Applied Professions. Quantitative results reveal significant variance in disciplinary approaches to civic engagement and service-learning across a variety of measures including advocacy, concerns, and goals for active learning pedagogies. The findings suggest several strategies for recognizing disciplinary differences and encouraging collaboration among faculty and between disciplines on civic engagement and service-learning approaches in higher education

    Mammographic density change in a cohort of premenopausal women receiving tamoxifen for breast cancer prevention over 5 years.

    BACKGROUND: A decrease in breast density due to tamoxifen preventive therapy might indicate greater benefit from the drug. It is not known whether mammographic density continues to decline after 1 year of therapy, or whether measures of breast density change are sufficiently stable for personalised recommendations. METHODS: Mammographic density was measured annually over up to 5 years in premenopausal women with no previous diagnosis of breast cancer but at increased risk of breast cancer attending a family-history clinic in Manchester, UK (baseline 2010-2013). Tamoxifen (20 mg/day) for prevention was prescribed for up to 5 years in one group; the other group did not receive tamoxifen and were matched by age. Fully automatic methods were used on mammograms over the 5-year follow-up: three area-based measures (NN-VAS, Stratus, Densitas) and one volumetric (Volpara). Additionally, percentage breast density at baseline and first follow-up mammograms was measured visually. The size of density declines at the first follow-up mammogram and thereafter was estimated using a linear mixed model adjusted for age and body mass index. The stability of density change at 1 year was assessed by evaluating mean squared error loss from predictions based on individual or mean density change at 1 year. RESULTS: Analysis used mammograms from 126 healthy premenopausal women before and as they received tamoxifen for prevention (median age 42 years) and 172 matched controls (median age 41 years), with median 3 years follow-up. There was a strong correlation between percentage density measures used on the same mammogram in both the tamoxifen and no tamoxifen groups (all correlation coeficients > 0.8). Tamoxifen reduced mean breast density in year 1 by approximately 17-25% of the inter-quartile range of four automated percentage density measures at baseline, and from year 2, it decreased further by approximately 2-7% per year. Predicting change at 2 years using individual change at 1 year was approximately 60-300% worse than using mean change at 1year. CONCLUSIONS: All measures showed a consistent and large average tamoxifen-induced change in density over the first year, and a continued decline thereafter. However, these measures of density change at 1 year were not stable on an individual basis

    Reverberation Mapping of the Kepler-Field AGN KA1858+4850

    KA1858+4850 is a narrow-line Seyfert 1 galaxy at redshift 0.078 and is among the brightest active galaxies monitored by the Kepler mission. We have carried out a reverberation mapping campaign designed to measure the broad-line region size and estimate the mass of the black hole in this galaxy. We obtained 74 epochs of spectroscopic data using the Kast Spectrograph at the Lick 3-m telescope from February to November of 2012, and obtained complementary V-band images from five other ground-based telescopes. We measured the H-beta light curve lag with respect to the V-band continuum light curve using both cross-correlation techniques (CCF) and continuum light curve variability modeling with the JAVELIN method, and found rest-frame lags of lag_CCF = 13.53 (+2.03, -2.32) days and lag_JAVELIN = 13.15 (+1.08, -1.00) days. The H-beta root-mean-square line profile has a width of sigma_line = 770 +/- 49 km/s. Combining these two results and assuming a virial scale factor of f = 5.13, we obtained a virial estimate of M_BH = 8.06 (+1.59, -1.72) x 10^6 M_sun for the mass of the central black hole and an Eddington ratio of L/L_Edd ~ 0.2. We also obtained consistent but slightly shorter emission-line lags with respect to the Kepler light curve. Thanks to the Kepler mission, the light curve of KA1858+4850 has among the highest cadences and signal-to-noise ratios ever measured for an active galactic nucleus; thus, our black hole mass measurement will serve as a reference point for relations between black hole mass and continuum variability characteristics in active galactic nuclei

    HER3 and downstream pathways are involved in colonization of brain metastases from breast cancer

    Introduction: Metastases to the brain from breast cancer have a high mortality, and basal-like breast cancers have a propensity for brain metastases. However, the mechanisms that allow cells to colonize the brain are unclear.Methods: We used morphology, immunohistochemistry, gene expression and somatic mutation profiling to analyze 39 matched pairs of primary breast cancers and brain metastases, 22 unmatched brain metastases of breast cancer, 11 non-breast brain metastases and 6 autopsy cases of patients with breast cancer metastases to multiple sites, including the brain.Results: Most brain metastases were triple negative and basal-like. the brain metastases over-expressed one or more members of the HER family and in particular HER3 was significantly over-expressed relative to matched primary tumors. Brain metastases from breast and other primary sites, and metastases to multiple organs in the autopsied cases, also contained somatic mutations in EGFR, HRAS, KRAS, NRAS or PIK3CA. This paralleled the frequent activation of AKT and MAPK pathways. in particular, activation of the MAPK pathway was increased in the brain metastases compared to the primary tumors.Conclusions: Deregulated HER family receptors, particularly HER3, and their downstream pathways are implicated in colonization of brain metastasis. the need for HER family receptors to dimerize for activation suggests that tumors may be susceptible to combinations of anti-HER family inhibitors, and may even be effective in the absence of HER2 amplification (that is, in triple negative/basal cancers).
