    Suicide in post agreement Northern Ireland: A Study of the Role of Paramilitary Intimidation 2007-2009

    Since the end of the ‘Troubles’ in Northern Ireland, there has been a rise in the number of acts of intimidation and Paramilitary Punishment Attacks (PPA). Anecdotally, some suicides have been linked to such incidents. To date, there has been a lack of research examining this association. This article details a case series study exploring how individual deaths by suicide in Northern Ireland were connected to intimidation. Data from Coroners, GP records and interviews with family members were examined to identify experiences of intimidation among a two-year cohort who died by suicide. Further case based analysis was conducted using a modified version of the psychological autopsy method. Our results indicate that in 19 male suicides there were incidents of intimidation in the twelve months prior to death. The suicides of these men are discussed in relation to the suicide model of entrapment. Our findings highlight the continuing problem of intimidation in Northern Ireland and suggest further research into the connection between these incidents and suicide is warranted

    Spying and Surveillance in Shakespeare’s Dramatic Courts

    This thesis examines representations of spying and surveillance in Shakespearean drama in conjunction with historical practices of espionage in later sixteenth-century England. The introductory chapter outlines how spying operations were conducted in Elizabethan England, with specific attention to the complex attitudes and behaviour of individual agents working in the broader context of the religious wars, both hot and cold, taking place between Protestant England and the Catholic powers of continental Europe. It also provides some analysis of the organisational structures within which those agents worked and examines a wide range of particular cases to illustrate how surveillance operations might play out in practice. The memory of Sir Francis Walsingham, often described as the ‘spymaster’ of Elizabeth’s government and noted for his skill in intelligence work, would have loomed large for any dramatist thinking about espionage at the turn of the seventeenth century. Subsequent chapters each examine a specific play in light of the material presented in the introduction, comprising Much Ado About Nothing, The Tempest, Measure for Measure, Henry V and Hamlet. Each chapter seeks to elucidate how Shakespeare draws upon the world of Elizabethan espionage to provide vital structural components in his dramatic plotting, especially as regards inter-personal relationships between courtiers, secretaries and agents on the ground. Real individuals and the spies depicted in Shakespeare’s plays all behave in a manner that is personally inflected to a profound degree, and it is this particular aspect of early-modern espionage that provides the single most important connection between history and drama. Periodically, this thesis also reflects upon the metatheatrical relationship between characters’ schemes and Shakespeare’s own plotting as a dramatist

    When health services are powerless to prevent suicide: results from a linkage study of suicide among men with no service contact in the year prior to death

    Aims: To investigate cases of suicide in which there was no healthcare contact, by looking at history of help-seeking and evidence of previous mental health vulnerability. To identify any life events associated with suicide for which individuals did not seek help. Background: Previous research has suggested that non-consultation is the main barrier to suicide prevention among men. Estimates suggest approximately 22% of men who die by suicide have not consulted their GP in the year before their death. Little is known about the lifetime pattern of engagement with services among these individuals and whether or not this may influence their help-seeking behaviour before death. Methods: Coroner records of suicide deaths in Northern Ireland over 2 years were linked to general practice (GP) records. This identified 63 individuals who had not attended health services in the 12 months before death. Coroner’s data were used to categorise life events associated with the male deaths. Lifetime mental health help-seeking at the GP was assessed. Findings: The vast majority of individuals who did not seek help were males (n=60, 15% of all suicide deaths). Lack of consultation in the year before suicide was consistent with behaviour over the lifespan; over two-thirds had no previous consultations for mental health. In Coroner’s records, suicides with no prior consultation were primarily linked to relationship breakdown and job loss. These findings highlight the limitations of primary care in suicide prevention as most had never attended GP for mental health issues and there was a high rate of supported consultation among those who had previously sought help. Public health campaigns that promote service use among vulnerable groups at times of crisis might usefully be targeted at those likely to be experiencing financial and relationship issues

    The failure of suicide prevention in primary care: family and GP perspectives - a qualitative study

    Background Although Primary care is crucial for suicide prevention, clinicians tend to report completed suicides in their care as non-preventable. We aimed to examine systemic inadequacies in suicide prevention from the perspectives of bereaved family members and GPs.Methods Qualitative study of 72 relatives or close friends bereaved by suicide and 19 General Practitioners who have experienced the suicide of patients.Results Relatives highlight failures in detecting symptoms and behavioral changes and the inability of GPs to understand the needs of patients and their social contexts. A perceived overreliance on anti-depressant treatment is a major source of criticism by family members. GPs tend to lack confidence in the recognition and management of suicidal patients, and report structural inadequacies in service provision.Conclusions Mental health and primary care services must find innovative and ethical ways to involve families in the decision-making process for patients at risk of suicide

    The need for a global drought early warning framework. Drought has had a significant impact on civilization throughout history in terms of reductions in agricultural productivity, potable water supply, and economic activity, and in extreme cases this has led to famine. Every continent has semiarid areas, which are especially vulnerable to drought. The Intergovernmental Panel on Climate Change has noted that average annual river runoff and water availability are projected to decrease by 10%–13% over some dry and semiarid regions in mid and low latitudes, increasing the frequency, intensity, and duration of drought, along with its associated impacts. The sheer magnitude of the problem demands efforts to reduce vulnerability to drought by moving away from the reactive, crisis management approach of the past toward a more proactive, risk management approach that is centered on reducing vulnerability to drought as much as possible while providing early warning of evolving drought conditions and possible impacts. Many countries, unfortunately, do not have adequate resources to provide early warning, but require outside support to provide the necessary early warning information for risk management. Furthermore, in an interconnected world, the need for information on a global scale is crucial for understanding the prospect of declines in agricultural productivity and associated impacts on food prices, food security, and potential for civil conflict

This paper highlights the recent progress made toward a Global Drought Early Warning Monitoring Framework (GDEWF), an underlying partnership and framework, along with its Global Drought Early Warning System (GDEWS), which is its interoperable information system, and the organizations that have begun working together to make it a reality. The GDEWF aims to improve existing regional and national drought monitoring and forecasting capabilities by adding a global component, facilitating continental monitoring and forecasting (where lacking), and improving these tools at various scales, thereby increasing the capacity of national and regional institutions that lack drought early warning systems or complementing existing ones. A further goal is to improve coordination of information delivery for drought-related activities and relief efforts across the world. This is especially relevant for regions and nations with low capacity for drought early warning. To do this requires a global partnership that leverages the resources necessary and develops capabilities at the global level, such as global drought forecasting combined with early warning tools, global real-time monitoring, and harmonized methods to identify critical areas vulnerable to drought. Although the path to a fully functional GDEWS is challenging, multiple partners and organizations within the drought, forecasting, agricultural, and water-cycle communities are committed to working toward its success

    A double-blind, placebo-controlled study of the short term effects of a spring water supplemented with magnesium bicarbonate on acid/base balance, bone metabolism and cardiovascular risk factors in postmenopausal women

    <p>Abstract</p> <p>Background</p> <p>A number of health benefits including improvements in acid/base balance, bone metabolism, and cardiovascular risk factors have been attributed to the intake of magnesium rich alkaline mineral water. This study was designed to investigate the effects of the regular consumption of magnesium bicarbonate supplemented spring water on pH, biochemical parameters of bone metabolism, lipid profile and blood pressure in postmenopausal women.</p> <p>Findings</p> <p>In this double-blind, placebo-controlled, parallel-group, study, 67 postmenopausal women were randomised to receive between 1500 mL and 1800 mL daily of magnesium bicarbonate supplemented spring water (650 mg/L bicarbonate, 120 mg/L magnesium, pH 8.3-8.5) (supplemented water group) or spring water without supplements (control water group) over 84 days. Over this period biomarkers of bone turnover (serum parathyroid hormone (PTH), 1,25-dihydroxyvitamin D, osteocalcin, urinary telopeptides and hydroxyproline), serum lipids (total cholesterol, HDL-cholesterol, LDL-cholesterol and triglycerides), venous and urinary pH were measured together with measurements of standard biochemistry, haematology and urine examinations.</p> <p>Serum magnesium concentrations and urinary pH in subjects consuming the magnesium bicarbonate supplemented water increased significantly at Day 84 compared to subjects consuming the spring water control (magnesium - p = 0.03; pH - p = 0.018). The consumption of spring water led to a trend for an increase in parathyroid hormone (PTH) concentrations while the PTH concentrations remained stable with the intake of the supplemented spring water. However there were no significant effects of magnesium bicarbonate supplementation in changes to biomarkers of bone mineral metabolism (n-telopeptides, hydroxyproline, osteocalcin and 1,25-dihydroxyvitamin D) or serum lipids or blood pressure in postmenopausal women from Day 0 to Day 84.</p> <p>Conclusions</p> <p>Short term regular ingestion of magnesium bicarbonate supplemented water provides a source of orally available magnesium. Long term clinical studies are required to investigate any health benefits.</p> <p>Trial registration</p> <p>ACTRN12609000863235</p

    Theory of mind in emerging reading comprehension: a longitudinal study of early indirect and direct effects

    The relation between children’s theory of mind (ToM) and emerging reading comprehension was investigated in a longitudinal study over 2.5 years. 80 children were tested for ToM, decoding, language skills and executive function (EF) at Time 1 (mean age = 3:10 years). At Time 2 (mean age = 6:03 years) children's word reading efficiency, language skills and reading comprehension were measured. Mediation analysis showed that ToM at Time 1, when children were around four years old, indirectly predicted Time 2 reading comprehension, when children were six years old, via language ability, after controlling for age, non-verbal ability, decoding, EF and earlier language ability. Importantly, ToM at four years old also directly predicted reading comprehension two and a half years later at six years. This is the first longitudinal study to show a direct contribution of theory of mind to reading comprehension in typical development. Findings are discussed in terms of the Simple View of Reading (SVR); ToM not only supports reading comprehension indirectly by facilitating language, but also directly contributes to it over and above the SVR. The potential role of metacognition is considered when accounting for the direct contribution of early ToM to later reading comprehension

    Nephele: genotyping via complete composition vectors and MapReduce

    <p>Abstract</p> <p>Background</p> <p>Current sequencing technology makes it practical to sequence many samples of a given organism, raising new challenges for the processing and interpretation of large genomics data sets with associated metadata. Traditional computational phylogenetic methods are ideal for studying the evolution of gene/protein families and using those to infer the evolution of an organism, but are less than ideal for the study of the whole organism mainly due to the presence of insertions/deletions/rearrangements. These methods provide the researcher with the ability to group a set of samples into distinct genotypic groups based on sequence similarity, which can then be associated with metadata, such as host information, pathogenicity, and time or location of occurrence. Genotyping is critical to understanding, at a genomic level, the origin and spread of infectious diseases. Increasingly, genotyping is coming into use for disease surveillance activities, as well as for microbial forensics. The classic genotyping approach has been based on phylogenetic analysis, starting with a multiple sequence alignment. Genotypes are then established by expert examination of phylogenetic trees. However, these traditional single-processor methods are suboptimal for rapidly growing sequence datasets being generated by next-generation DNA sequencing machines, because they increase in computational complexity quickly with the number of sequences.</p> <p>Results</p> <p>Nephele is a suite of tools that uses the complete composition vector algorithm to represent each sequence in the dataset as a vector derived from its constituent k-mers by passing the need for multiple sequence alignment, and affinity propagation clustering to group the sequences into genotypes based on a distance measure over the vectors. Our methods produce results that correlate well with expert-defined clades or genotypes, at a fraction of the computational cost of traditional phylogenetic methods run on traditional hardware. Nephele can use the open-source Hadoop implementation of MapReduce to parallelize execution using multiple compute nodes. We were able to generate a neighbour-joined tree of over 10,000 16S samples in less than 2 hours.</p> <p>Conclusions</p> <p>We conclude that using Nephele can substantially decrease the processing time required for generating genotype trees of tens to hundreds of organisms at genome scale sequence coverage.</p
