Utilizing Pharmacist-Led Telehealth Services in Ambulatory Patients with Heart Failure
Background: Heart failure is one of the leading causes of hospital admissions. Non-adherence to medications and poor dietary management for patients who suffer from this condition can lead to worsening of symptoms and hospitalization. Pharmacist interventions via telehealth have demonstrated a beneficial impact on disease management and adherence outcomes in patients with chronic conditions.
Methods: This retrospective, descriptive cohort study reviewed subjects from a single-centered primary care office. Data was collected via electronic chart review between January and December 2021. Subjects eligible for inclusion were adults who were diagnosed with heart failure by their primary care provider and referred to a pharmacist for a telehealth visit. The primary outcome was the number of heart failure-related hospitalizations post-pharmacist intervention via telehealth. The secondary outcome was the number of cardiovascular-related hospitalizations post-pharmacist intervention via telehealth.
Results: 37 patients were included for analysis. Only two patients were admitted for heart failure post-pharmacist intervention. Fifteen patients were admitted post-pharmacist intervention for a cardiovascular-related hospitalization.
Conclusion: This report illustrates the employment of pharmacist-led telehealth services in the chronic heart failure population. This study encourages pharmacist-based interventions via telehealth in the ambulatory setting as few HF-related hospitalizations occurred in this cohort
Rab27a controls HIV-1 assembly by regulating plasma membrane levels of phosphatidylinositol 4,5-bisphosphate
During the late stages of the HIV-1 replication cycle, the viral polyprotein Pr55Gag is recruited to the plasma membrane (PM), where it binds phosphatidylinositol 4,5-bisphosphate (PI(4,5)P2) and directs HIV-1 assembly. We show that Rab27a controls the trafficking of late endosomes carrying phosphatidylinositol 4-kinase type 2 α (PI4KIIα) toward the PM of CD4+ T cells. Hence, Rab27a promotes high levels of PM phosphatidylinositol 4-phosphate and the localized production of PI(4,5)P2, therefore controlling Pr55Gag membrane association. Rab27a also controls PI(4,5)P2 levels at the virus-containing compartments of macrophages. By screening Rab27a effectors, we identified that Slp2a, Slp3, and Slac2b are required for the association of Pr55Gag with the PM and that Slp2a cooperates with Rab27a in the recruitment of PI4KIIα to the PM. We conclude that by directing the trafficking of PI4KIIα-positive endosomes toward the PM, Rab27a controls PI(4,5)P2 production and, consequently, HIV-1 replication.Fil: Pereyra Gerber, Federico PehuĂ©n. Wonder, education, and human flourishing:Theoretical, empirical, and practical perspectives
The premise that underlies this volume is that there are strong interconnections between wonder, education and human flourishing. And more specifically, that wonder can make a significant difference to how well oneâs education progresses and how well oneâs life goes. The contributors to this volume â both senior, well-known and beginning researchers and students of wonder â variously explore aspects of these connections from philosophical, empirical, theoretical and practical perspectives. The three chapters that comprise Part I of the book are devoted to the importance of wonder for education and for human flourishing. Part II contains four chapters offering conceptual analyses of wonder and perspectives from developmental psychology and philosophy (Spinoza, Wittgenstein, philosophy of religion). The seven chapters that form Part III contain a wealth of ideas and educational strategies to promote wonder in education and teacher education. This volume not only underlines and articulates the importance of wonder in education and in life but also offers fresh perspectives, allowing us to look with renewed wonder at wonder itself
Bidirectional cross-regulation between the eNOS and Ă-catenin signalling pathways
AIMS: ÎČ-catenin has been shown to be regulated by inducible nitric oxide synthase (NOS) in endothelial cells. We investigated here whether ÎČ-catenin interacts with and regulates endothelial NOS (eNOS) and whether eNOS activation promotes ÎČ-catenin signalling. METHODS AND RESULTS: We identified ÎČ-catenin as a novel eNOS binding protein in human umbilical vein endothelial cells (HUVECs) by mass spectroscopy and western blot analyses of ÎČ-catenin and eNOS immunoprecipitates. This was confirmed by in situ proximity ligation assay. eNOS activity, assessed by cGMP production and eNOS phosphorylation (Ser1177), was enhanced in ÎČ-catenin(-/-) mouse pulmonary endothelial cells (MPECs) relative to wild-type MPECs. eNOS activation (using adenosine, salbutamol, thrombin, or histamine), or application of an NO donor (spermine NONOate) or cGMP-analogue (8-bromo-cGMP) caused nuclear translocation of ÎČ-catenin in HUVEC as shown by western blotting of nuclear extracts. Exposure to spermine NONOate, 8-bromo-cGMP, or sildenafil (a phosphodiesterase type 5 inhibitor) also increased the expression of ÎČ-catenin-dependent transcripts, IL-8, and cyclin D1. Stimulation of wild-type MPECs with basic fibroblast growth factor (bFGF), vascular endothelial growth factor (VEGF), spermine NONOate, 8-bromo-cGMP, or sildenafil increased tube length relative to controls in an angiogenesis assay. These responses were abrogated in ÎČ-catenin(-/-) MPECs, with the exception of that to bFGF which is NO-independent. In C57BL/6 mice, subcutaneous VEGF-supplemented Matrigel plugs containing ÎČ-catenin(-/-) MPECs exhibited reduced angiogenesis compared with plugs containing wild-type MPECs. Angiogenesis was not altered in bFGF-supplemented Matrigel. CONCLUSION: These data reveal bidirectional cross-talk and regulation between the NO-cGMP and ÎČ-catenin signalling pathways
Using formative research to develop the healthy eating component of the CHANGE! school-based curriculum intervention
Background: Childhood obesity is a significant public health concern. Many intervention studies have attempted
to combat childhood obesity, often in the absence of formative or preparatory work. This study describes the
healthy eating component of the formative phase of the Childrenâs Health Activity and Nutrition: Get Educated!
(CHANGE!) project. The aim of the present study was to gather qualitative focus group and interview data
regarding healthy eating particularly in relation to enabling and influencing factors, barriers and knowledge in
children and adults (parents and teachers) from schools within the CHANGE! programme to provide populationspecific
evidence to inform the subsequent intervention design.
Methods: Semi-structured focus group interviews were conducted with children, parents and teachers across 11
primary schools in the Wigan borough of North West England. Sixty children (N = 24 boys), 33 parents (N = 4 male)
and 10 teachers (N = 4 male) participated in the study. Interview questions were structured around the PRECEDE
phases of the PRECEDE-PROCEED model. Interviews were transcribed verbatim and analysed using the pen-profiling
technique.
Results: The pen-profiles revealed that childrenâs knowledge of healthy eating was generally good, specifically
many children were aware that fruit and vegetable consumption was âhealthyâ (N = 46). Adultsâ knowledge was also
good, including restricting fatty foods, promoting fruit and vegetable intake, and maintaining a balanced diet. The
important role parents play in childrenâs eating behaviours and food intake was evident. The emerging themes
relating to barriers to healthy eating showed that external drivers such as advertising, the preferred sensory
experience of âunhealthyâ foods, and food being used as a reward may play a role in preventing healthy eating.
Conclusions: Data suggest that; knowledge related to diet composition was not a barrier per se to healthy eating,
and education showing how to translate knowledge into behavior or action is required. The key themes that
emerged through the focus groups and pen-profiling data analysis technique will be used to inform and tailor the
healthy eating component of the CHANGE! intervention study.

Finishing the euchromatic sequence of the human genome
The sequence of the human genome encodes the genetic instructions for human physiology, as well as rich information about human evolution. In 2001, the International Human Genome Sequencing Consortium reported a draft sequence of the euchromatic portion of the human genome. Since then, the international collaboration has worked to convert this draft into a genome sequence with high accuracy and nearly complete coverage. Here, we report the result of this finishing process. The current genome sequence (Build 35) contains 2.85 billion nucleotides interrupted by only 341 gaps. It covers âŒ99% of the euchromatic genome and is accurate to an error rate of âŒ1 event per 100,000 bases. Many of the remaining euchromatic gaps are associated with segmental duplications and will require focused work with new methods. The near-complete sequence, the first for a vertebrate, greatly improves the precision of biological analyses of the human genome including studies of gene number, birth and death. Notably, the human enome seems to encode only 20,000-25,000 protein-coding genes. The genome sequence reported here should serve as a firm foundation for biomedical research in the decades ahead
Painting the Body: Feminist Musings on Visual Autographies
In this paper I look at autographical depictions of the body in the work of Mato Ioannidou, a Greek woman artist, who participated in a wider narrative-based project on visual and textual entanglements between life and art. The paper unfolds in three parts: first, I give an overview of Ioannidouâs artwork, making connections with significant events in her life; then I discuss feminist theorizations of embodiment and visual auto/biography; and finally I draw on insights from Spinozist feminist philosophers to discuss the artistâs portrayal of womenâs bodies in three cycles of her work. What I argue is that the body becomes a centerpiece in the attempt to perceive connections between life and art through expressionism rather than representation
