    Quantitative and Qualitative Research on the Fear of Recurrence and the Belief in Prevention of Recurrence in Elderly Patients with Coronary Heart Disease

    Objective: To explore the fear of recurrence and the belief in prevention of recurrence in elderly patients with coronary heart disease. Methods: A total of 178 elderly patients with coronary heart disease were included, and quantitative and qualitative research methods were used to explore the fear of recurrence and the belief in prevention of recurrence in elderly patients with coronary heart disease. The quantitative research tool was a self-made fear of coronary heart disease recurrence questionnaire; the qualitative research was guided by the phenomenological method, semi-structured in-depth interviews with 11 elderly patients with coronary heart disease, and the data were collected by recording, and the Colaizzi method was used to analyze the data. Results Elderly patients with coronary heart disease were afraid of recurrence; they knew the etiology of coronary heart disease, but the controllable rate of the cause and effect was extremely low. Conclusion: Due to the belief in fatalism and the lack of control over the etiology of coronary heart disease, elderly patients with coronary heart disease are afraid of recurrence

    Integrating spatial and non-spatial dimensions to evaluate access to rural primary healthcare service: a case study of Songzi, China

    Access to rural primary healthcare services has been broadly studied in the past few decades. However, most earlier studies that focused on examining access to rural healthcare services have conventionally treated spatial and non-spatial access as separate factors. This research aims to measure access to primary healthcare services in rural areas with the consideration of both spatial and non-spatial dimensions. The methodology of study is threefold. First, the Gaussian two-step floating catchment area (G-2SFCA) method was adopted to measure spatial access to primary healthcare services. Then, a questionnaire survey was conducted to investigate non-spatial access factors, including demographic condition, patient’s household income, healthcare insurance, education level, and patient satisfaction level with the services. After that, a comprehensive evaluation index system was employed to integrate both spatial and non-spatial access. The empirical study showed a remarkable disparity in spatial access to primary healthcare services. In total, 78 villages with 185,137 local people had a “low” or “very low” level of spatial access to both clinics and hospitals. For the non-spatial dimension, the results depicted that Songzi had significant inequalities in socioeconomic status (e.g., income, education) and patient satisfaction level for medical service. When integrating both spatial and non-spatial factors, the disadvantaged areas were mainly located in the eastern and middle parts. In addition, this study found that comprehensively considering the spatial and non-spatial access had a significant impact on results in healthcare access. In conclusion, this study calls for policymakers to pay more attention to primary healthcare inequalities within rural areas. The spatial and non-spatial access should be considered comprehensively when the long-term rural medical support policy is designated

    Endoplasmic Reticulum Targeting Ratiometric Fluorescent Probe for Carboxylesterase 2 Detection in Drug-Induced Acute Liver Injury

    Carboxylesterase 2 (CES2), an endoplasmic reticulum (ER) located phase I enzyme, plays a vital role in the metabolism of various endogenous and exogenous substances, and is regarded as an important target for the design of prodrugs. Unfortunately, superior highly selective ER targeting fluorescent probes for monitoring of CES2 are not currently available. Herein, we report an ER targeting CES2 selective and sensitive ratiometric fluorescent probe ERNB based on the ER localizing group p-toluenesulfonamide. ERNB possessed high specificity, sensitivity, and exhibited excellent subcellular localization when compared to commercial ER tracker, and was used to image CES2 in the ER of living cells. Additionally, using ERNB we evaluated the CES2 regulation under d,l-dithiothreitol and tunicamycin-induced ER stress. Furthermore, we determined the down regulation of CES2 activity and expression in the acetaminophen-induced acute liver injury model. On the basis of these results, we conclude that ERNB is a promising tool for highlighting the role of CES2 in the ER and in exploring the role of CES2 in the development of diseases associated with ER stress.</p

    Study of the Effect of Drying and Wetting Cycles and Water Content on the Shear Characteristics of Tailing Sands

    The mechanical characteristics of tailing sands have an important impact on the safety and stability of the tailing dams. Fully understanding the effect of drying and wetting cycles (DWC) and water content on the characteristics of tailing sands is urgently needed. In this study, direct shear tests were first carried out to analyze the effect of DWC and water content on the macroscopic mechanical characteristics of tailing sands. Then, the mesoscopic mechanical behavior of tailing sands with different water contents under the action of DWC was studied by using PFC2D particle flow software. The results showed that the effect of DWC on the shear properties of tailing sands is more pronounced than water content. The cohesive force and the internal friction angle increase first and then decrease with the increasing water content. With the increasing number of DWC, the cohesive force and the internal friction angle all decreased to varying degrees. The results of the mesoscopic mechanical analysis indicated that after experiencing the DWC, the force chain of the sample gradually thickens to form a coarse force chain network area, and the number of cracks inside the sample is significantly larger than that of the sample that has not experienced the DWC. The results of this study are of great significance for understanding the macroscopic and mesoscopic shear failure mechanism of tailing sands under the effects of DWCs and water content

    Quantification of the Arctic Sea ice‐driven atmospheric circulation variability in coordinated large ensemble simulations

    A coordinated set of large ensemble atmosphere‐only simulations is used to investigate the impacts of observed Arctic sea ice‐driven variability (SIDV) on the atmospheric circulation during 1979–2014. The experimental protocol permits separating Arctic SIDV from internal variability and variability driven by other forcings including sea surface temperature and greenhouse gases. The geographic pattern of SIDV is consistent across seven participating models, but its magnitude strongly depends on ensemble size. Based on 130 members, winter SIDV is ~0.18 hPa2 for Arctic‐averaged sea level pressure (~1.5% of the total variance), and ~0.35 K2 for surface air temperature (~21%) at interannual and longer timescales. The results suggest that more than 100 (40) members are needed to separate Arctic SIDV from other components for dynamical (thermodynamical) variables, and insufficient ensemble size always leads to overestimation of SIDV. Nevertheless, SIDV is 0.75–1.5 times as large as the variability driven by other forcings over northern Eurasia and Arctic

    A VMD and LSTM based hybrid model of load forecasting for power grid security

    As the basis for the static security of the power grid, power load forecasting directly affects the safety of grid operation, the rationality of grid planning, and the economy of supply-demand balance. However, various factors lead to drastic changes in short-term power consumption, making the data more complex and thus more difficult to forecast. In response to this problem, a new hybrid model based on Vari-ational mode decomposition (VMD) and Long Short-Term Memory (LSTM) with seasonal factors elimination and error correction is proposed in this paper. Comprehensive case studies on four real-world load datasets from Singapore and the United States are employed to demonstrate the effectiveness and practicality of the proposed hybrid model. The experimental results show that the prediction accuracy of the proposed model is significantly higher than that of the contrast models.     The effects of smoke-free legislation on acute myocardial infarction: A systematic review and meta-analysis

    Background: Comprehensive smoke-free legislation has been implemented in many countries. The current study quantitatively examined the reduction in risk of acute myocardial infarction (MI) occurrence following the legislations and the relationship with the corresponding smoking prevalence decrease. Methods. PubMed, EMBASE, and Google Scholar databases and bibliographies of relevant studies and reviews were searched for potential original studies published from January 1, 2004, through October 31, 2011. Meta-analysis was performed using a random effect model to estimate the overall effects of the smoking-free legislations. Meta-regression was used to investigate possible causes of heterogeneity in risk estimates. Results: A total of 18 eligible studies with 44 estimates of effect size were used in this study. Meta-analysis produced a pooled estimate of the relative risk of 0.87 (95% confidence interval (CI): 0.84 to 0.91). There was significant heterogeneity in the risk estimates (overall I2 = 96.03%, p<0.001). In meta-regression analysis, studies with greater smoking prevalence decrease produced larger relative risk (adjusted coefficient -0.027, 95% CI: -0.049 to -0.006, p=0.014). Conclusion: Smoke-free legislations in public and work places were associated with significant reduction in acute MI risk, which might be partly attributable to reduced smoking prevalence. © 2013 Lin et al.; licensee BioMed Central Ltd.link_to_subscribed_fulltex

    Pressure-tunable magnetic topological phases in magnetic topological insulator MnSb4Te7

    Magnetic topological insulators, possessing both magnetic order and topological electronic structure, provides an excellent platform to research unusual physical properties. Here, we report a high-pressure study on the anomalous Hall effect of magnetic TI MnSb4Te7 through transports measurements combined with first-principle theoretical calculations. We discover that the ground state of MnSb4Te7 experiences a magnetic phase transition from the A-type antiferromagnetic state to ferromagnetic dominating state at 3.78 GPa, although its crystal sustains a rhombohedral phase under high pressures up to 8 GPa. The anomalous Hall conductance {\sigma}xyA keeps around 10 {\Omega}-1 cm-1, dominated by the intrinsic mechanism even after the magnetic phase transition. The results shed light on the intriguing magnetism in MnSb4Te7 and pave the way for further studies of the relationship between topology and magnetism in topological materials.Comment: 10 pages, 4 figure
