    This reasearch started from the writer's pre-observation in a local institution of Padang City administration, Social and Human Resource Office of Padang City. Thepre-observation shows problems in leadership and motivation of public servants. Theproblem derives from the lack of support of the leaders about the needs of the workersthat can motivate them to do their jobs, and the lack of guidance by the leaders inconducting tasks. It can be seen from the empty office after lunch because most workersgets home early, especially when the office heads are going out. Theworkers' lack ofmotivation affect the performance and lack of responsibility for their job assignment,and many workers do their job carelessly when the heads are not present. Thisquantitative reasearch is correlational. The population is the 72 public servants in theSocial and Human Resource Office of Padang City. The sample is measured using slovinformula, and resulted in 68 samples. Descriptive analysis of the paternalistic leadershipstyle shows that 73,03% respondents agree, while the public servants' motivation showsrespondents agree 78,10%. The result of simple regression analysis shows thatpaternalistic leadership style influences the workers' motivation as high as 18,2%, with44,1% strength where tcount> t; which is 3,990 > 1,682

    Sentinel lymph node biopsy (SLNB) is the gold standard procedure for breast cancer staging and axillary lymph node dissection (AD) is recommended when metastasis are found. Two recent practice-changing studies altered this paradigm. They brought into discussion old unanswered questions, such as the impact of AD on morbidity, survival and choice of adjuvant therapy as well as new questions, like the role of radiotherapy and the omission of AD after positive SLNB.     The Chaco-Paraná basin (CPB) is located mostly in the north-eastern Argentina, covering an area of approximately 700,000 km2 in a lowland region (~100 m a.s.l.), known as Chacopampean plain. The average sediment thickness of the basin is 4,000 m, and along the Las Breñas fault zone the sediment thickness may reach more than 6,000 m. The Paraná basin (PB) is located in south Brazil, covering an area of over 1,000,000 km2 and its cumulative sediment and basalt thickness reaches up to 7,000 m.     Lessons learnt from the introduction of the contraceptive implant in South Africa

    In 2014, South Africa (SA) introduced the subdermal contraceptive implant with the aim of expanding the contraceptive method mix and availability of long-acting reversible methods in the public sector. Three years on, concerns have been raised about the decline in uptake, early implant removals and challenges in service delivery. This article explores the lessons learnt from the introduction of contraceptive technologies elsewhere and applies these to the SA context. Drawing on the World Health Organization’s conceptual framework for the introduction of new contraceptive methods, and subsequent literature on the topic, lessons are classified into six cross-cutting themes. Recommendations highlight the need for SA to review and explore strategies to strengthen current implant services, including the provision of improved provider training aimed at sensitive, client-centred approaches; increased community engagement; and improved systems for programmatic monitoring and evaluation. With implementation of these recommendations, worrying trends in the provision of implants could be reversed

    Did Patagonia collide with Gondwana in the Late Paleozoic? Some insights from a multidisciplinary study of magmatic units of the North Patagonian Massif

    The origin of Patagonia and its relations with the South American crustal blocks to the north have been a matter of debate for decades. We report results from a multidisciplinary study centered on Paleozoic granitoids exposed in the northeastern corner of the North Patagonian Massif. Microstructural and magnetofabric studies reveal two suites of granitoids. Late Carboniferous (?) granitoids (Yaminué Complex, Tardugno Granodiorite, Cabeza de Vaca leucogranite) were emplaced and subsequently deformed in a major NNE-SSW compressive stress regime that also provoked top-to-the-SW thrust deformation in shallow crustal levels. Gravity and geobarometric studies show that the same major deformation event has been recorded at different crustal levels. The age and type of deformation of this event recorded across the northern boundary of Patagonia strongly supports a Late Carboniferous - Early Permian frontal collision between Patagonia and Gondwana. This major deformation event ceased by 281 Ma when the Navarrete Plutonic Complex, which shows mainly magmatic fabrics, was emplaced under a far-field WNW-ESE stress regime. Crustal continuity between the North Patagonian Massif and the Pampia and Arequipa- Antofalla terranes is suggested by similar Late Paleoproterozoic crustal model ages, comparable detrital zircon ages in Early Paleozoic successions, the apparent continuity of an Early Ordovician continental magmatic arc and paleomagnetic data. Reconciliation of this evidence with the Late Paleozoic frontal collision is obtained in a tectonic model that suggests that the North Patagonian Massif is a parautochthonous crustal block

    Alzheimer's disease (AD) is a neurodegenerative disorder characterized by progressively disabling impairments in memory, cognition, and non-cognitive behavioural symptoms. Sporadic AD is multifactorial and genetically complex. While several monogenic mutations cause early-onset AD and gene alleles have been suggested as AD susceptibility factors, the only extensively validated susceptibility gene for late-onset AD is the apolipoprotein E (APOE) epsilon4 allele. Alleles of the APOE gene do not account for all of the genetic load calculated to be responsible for AD predisposition. Recently, polymorphisms across the neuronal sortilin-related receptor (SORL1) gene were shown to be significantly associated with AD in several cohorts.     A fresh look at the evolution and diversification of photochemical reaction centers

    In this review, I reexamine the origin and diversification of photochemical reaction centers based on the known phylogenetic relations of the core subunits, and with the aid of sequence and structural alignments. I show, for example, that the protein folds at the C-terminus of the D1 and D2 subunits of Photosystem II, which are essential for the coordination of the water-oxidizing complex, were already in place in the most ancestral Type II reaction center subunit. I then evaluate the evolution of reaction centers in the context of the rise and expansion of the different groups of bacteria based on recent large-scale phylogenetic analyses. I find that the Heliobacteriaceae family of Firmicutes appears to be the earliest branching of the known groups of phototrophic bacteria; however, the origin of photochemical reaction centers and chlorophyll synthesis cannot be placed in this group. Moreover, it becomes evident that the Acidobacteria and the Proteobacteria shared a more recent common phototrophic ancestor, and this is also likely for the Chloroflexi and the Cyanobacteria. Finally, I argue that the discrepancies among the phylogenies of the reaction center proteins, chlorophyll synthesis enzymes, and the species tree of bacteria are best explained if both types of photochemical reaction centers evolved before the diversification of the known phyla of phototrophic bacteria.     Surgical and medical second trimester abortion in South Africa: A cross-sectional study

    <p>Abstract</p> <p>Background</p> <p>A high percentage of abortions performed in South Africa are in the second trimester. However, little research focuses on women's experiences seeking second trimester abortion or the efficacy and safety of these services.</p> <p>The objectives are to document clinical and acceptability outcomes of second trimester medical and surgical abortion as performed at public hospitals in the Western Cape Province.</p> <p>Methods</p> <p>We performed a cross-sectional study of women undergoing abortion at 12.1-20.9 weeks at five hospitals in Western Cape Province, South Africa in 2008. Two hundred and twenty women underwent D&E with misoprostol cervical priming, and 84 underwent induction with misoprostol alone. Information was obtained about the procedure and immediate complications, and women were interviewed after recovery.</p> <p>Results</p> <p>Median gestational age at abortion was earlier for D&E clients compared to induction (16.0 weeks vs. 18.1 weeks, p < 0.001). D&E clients reported shorter intervals between first clinic visit and abortion (median 17 vs. 30 days, p < 0.001). D&E was more effective than induction (99.5% vs. 50.0% of cases completed on-site without unplanned surgical procedure, p < 0.001). Although immediate complications were similar (43.8% D&E vs. 52.4% induction), all three major complications occurred with induction. Early fetal expulsion occurred in 43.3% of D&E cases. While D&E clients reported higher pain levels and emotional discomfort, most women were satisfied with their experience.</p> <p>Conclusions</p> <p>As currently performed in South Africa, second trimester abortions by D&E were more effective than induction procedures, required shorter hospital stay, had fewer major immediate complications and were associated with shorter delays accessing care. Both services can be improved by implementing evidence-based protocols.</p

    Pelvic organ prolapse and collagen-associated disorders

    Contains fulltext : 109010.pdf (publisher's version ) (Open Access)INTRODUCTION AND HYPOTHESIS: Pelvic organ prolapse (POP) and other disorders, such as varicose veins and joint hypermobility, have been associated with changes in collagen strength and metabolism. We hypothesized that these various disorders were more prevalent in both POP patients and their family members. METHODS: In this study, the prevalence of various collagen-associated disorders, including POP, was compared between POP patients (n = 110) and control patients (n = 100) and their first and second degree family members. RESULTS: POP patients reported a higher prevalence of varicose veins, joint hypermobility and rectal prolapse and were more likely to have family members with POP as compared to the control group (p < 0.01). In contrast, the family members of the POP group did not report a higher prevalence of collagen-associated disorders compared to the family members of the control group (p = 0.82). CONCLUSIONS: POP and other collagen-associated disorders may have a common aetiology, originating at the molecular level of the collagens.1 maart 201
