Population-based proband-oriented pedigree information system: application to hypertension with population-based screening data (KCIS No. 25)
Objective To develop a population-based probandoriented pedigree information system that can be easily applied to various diseases in genetic epidemiological studies, making allowance for the capture of theoretical family relationships.
Designs and Measurements A population-based proband-oriented pedigree information system with ties of consanguinity based on both population-based household registry data and Keelung Community Integrated Screening data was proposed to build a comprehensive extended family pedigree structure to accommodate a series of genetic studies on different diseases. We also developed an algorithm to efficiently assess how well theoretical family relationships affecting the occurrence of diseases across three generations with respect to the relative relationship score, a quantitative indicator of genetic influence, were captured.
Results We applied this population-based probandoriented pedigree information system to estimate the rate of hypertension with various relative relationships given the selection of probands. The degree of capturing complete familial relationships was assessed for three generations. The risk for early onset of hypertension was proportional to the proband-oriented relative relationship score with 2% increased risk and 1% correction for incomplete capture.
Conclusions The population-based proband-oriented pedigree information system is powerful and can support various genetic descriptive and analytic epidemiological studies
Dynamic neural networks for real-time water level predictions of sewerage systems-covering gauged and ungauged sites
[[abstract]]In this research, we propose recurrent neural networks (RNNs) to build a relationship between rainfalls and water level patterns of an urban sewerage system based on historical torrential rain/storm events. The RNN allows signals to propagate in both forward and backward directions, which offers the network dynamic memories. Besides, the information at the current time-step with a feedback operation can yield a time-delay unit that provides internal input information at the next time-step to effectively deal with time-varying systems. The RNN is implemented at both gauged and ungauged sites for 5-, 10-, 15-, and 20-min-ahead water level predictions. The results show that the RNN is capable of learning the nonlinear sewerage system and producing satisfactory predictions at the gauged sites. Concerning the ungauged sites, there are no historical data of water level to support prediction. In order to overcome such problem, a set of synthetic data, generated from a storm water management model (SWMM) under cautious verification process of applicability based on the data from nearby gauging stations, are introduced as the learning target to the training procedure of the RNN and moreover evaluating the performance of the RNN at the ungauged sites. The results demonstrate that the potential role of the SWMM coupled with nearby rainfall and water level information can be of great use in enhancing the capability of the RNN at the ungauged sites. Hence we can conclude that the RNN is an effective and suitable model for successfully predicting the water levels at both gauged and ungauged sites in urban sewerage systems.[[incitationindex]]SCI[[booktype]]紙
Scalable photonic sources using two-dimensional lead halide perovskite superlattices
Miniaturized photonic sources based on semiconducting two-dimensional (2D) materials offer new technological opportunities beyond the modern III-V platforms. For example, the quantum-confined 2D electronic structure aligns the exciton transition dipole moment parallel to the surface plane, thereby outcoupling more light to air which gives rise to high-efficiency quantum optics and electroluminescent devices. It requires scalable materials and processes to create the decoupled multi-quantum-well superlattices, in which individual 2D material layers are isolated by atomically thin quantum barriers. Here, we report decoupled multi-quantum-well superlattices comprised of the colloidal quantum wells of lead halide perovskites, with unprecedentedly ultrathin quantum barriers that screen interlayer interactions within the range of 6.5 Å. Crystallographic and 2D k-space spectroscopic analysis reveals that the transition dipole moment orientation of bright excitons in the superlattices is predominantly in-plane and independent of stacking layer and quantum barrier thickness, confirming interlayer decoupling
Effectiveness of influenza vaccination in patients with end-stage renal disease receiving hemodialysis: a population-based study.
BackgroundLittle is known on the effectiveness of influenza vaccine in ESRD patients. This study compared the incidence of hospitalization, morbidity, and mortality in end-stage renal disease (ESRD) patients undergoing hemodialysis (HD) between cohorts with and without influenza vaccination.MethodsWe used the insurance claims data from 1998 to 2009 in Taiwan to determine the incidence of these events within one year after influenza vaccination in the vaccine (N = 831) and the non-vaccine (N = 3187) cohorts. The vaccine cohort to the non-vaccine cohort incidence rate ratio and hazard ratio (HR) of morbidities and mortality were measured.ResultsThe age-specific analysis showed that the elderly in the vaccine cohort had lower hospitalization rate (100.8 vs. 133.9 per 100 person-years), contributing to an overall HR of 0.81 (95% confidence interval (CI) 0.72-0.90). The vaccine cohort also had an adjusted HR of 0.85 [95% CI 0.75-0.96] for heart disease. The corresponding incidence of pneumonia and influenza was 22.4 versus 17.2 per 100 person-years, but with an adjusted HR of 0.80 (95% CI 0.64-1.02). The vaccine cohort had lowered risks than the non-vaccine cohort for intensive care unit (ICU) admission (adjusted HR 0.20, 95% CI 0.12-0.33) and mortality (adjusted HR 0.50, 95% CI 0.41-0.60). The time-dependent Cox model revealed an overall adjusted HR for mortality of 0.30 (95% CI 0.26-0.35) after counting vaccination for multi-years.ConclusionsESRD patients with HD receiving the influenza vaccination could have reduced risks of pneumonia/influenza and other morbidities, ICU stay, hospitalization and death, particularly for the elderly
DEVELOPMENT AND VALIDATION OF THE TAIWAN CHILDREN’S ENVIRONMENTAL ACTION INDEX
In this study, we use Smith-Sebasto & Fortner’s (1994) Environmental Action Internal Control Index (EAICI) as a framework to develop, and validate a useful instrument for assessing environmental attitudes and behavior among elementary and middle school children within the Taiwanese context. We dub the new instrument the Taiwan Children’s Environmental Action Index (TCEAI). Our findings suggest that the TCEAI displays substantial internal consistency (Cronbach's alpha=.92), moderately positive correlations with self-report measures of environmentally responsible behavior (R = .35 to .46, p < .01), and few threats to validity by age or gender. The results suggest that the TCEAI may be used to elicit important dimensions of environmental attitudes and to predict environmentally responsible behavior for elementary and middle school children in Taiwan. Practical implications are discussed
High ERCC1 expression predicts cisplatin-based chemotherapy resistance and poor outcome in unresectable squamous cell carcinoma of head and neck in a betel-chewing area
<p>Abstract</p> <p>Background</p> <p>This study was to evaluate the effect of excision repair cross-complementation group 1(ERCC1) expression on response to cisplatin-based induction chemotherapy (IC) followed by concurrent chemoradiation (CCRT) in locally advanced unresectable head and neck squamous cell carcinoma (HNSCC) patients.</p> <p>Methods</p> <p>Fifty-seven patients with locally advanced unresectable HNSCC who received cisplatin-based IC followed by CCRT from January 1, 2006 through January 1, 2008. Eligibility criteria included presence of biopsy-proven HNSCC without a prior history of chemotherapy or radiotherapy. Immunohistochemistry was used to assess ERCC1 expression in pretreatment biopsy specimens from paraffin blocks. Clinical parameters, including smoking, alcohol consumption and betel nuts chewing, were obtained from the medical records.</p> <p>Results</p> <p>The 12-month progression-free survival (PFS) and 2-year overall survival (OS) rates of fifty-seven patients were 61.1% and 61.0%, respectively. Among these patients, thirty-one patients had low ERCC1 expression and forty-one patients responded to IC followed by CCRT. Univariate analyses showed that patients with low expression of ERCC1 had a significantly higher 12-month PFS rates (73.3% vs. 42.3%, p < 0.001) and 2-year OS (74.2 vs. 44.4%, p = 0.023) rates. Multivariate analysis showed that for patients who did not chew betel nuts and had low expression of ERCC1 were independent predictors for prolonged survival.</p> <p>Conclusions</p> <p>Our study suggest that a high expression of ERCC1 predict a poor response and survival to cisplatin-based IC followed by CCRT in patients with locally advanced unresectable HNSCC in betel nut chewing area.</p
Modification of thermally activated delayed fluorescence emitters comprising acridan– pyrimidine moieties for efficient sky-blue to greenish-blue OLEDs
Thermally activated delayed fluorescence (TADF) is a promising approach to harvest triplet excitons and achieve high-performance organic light-emitting diodes (OLEDs) for displays. In this study, we synthesized two new TADF emitters, 4Ac25CzPy and 4Ac35CzPy, featuring acridan–pyrimidine–carbazole moieties. Remarkably, a slight modification in the carbazole group position enables precise control of luminous color, resulting in emissions at 483 nm and 494 nm for 4Ac25CzPy and 4Ac35CzPy, respectively, in the electroluminescent device. Both compounds exhibit small energy difference between their singlet and triplet states (DEST) of 0.14 eV and 0.15 eV, confirming their TADF characteristics. Notably, OLEDs utilizing 4Ac35CzPy achieve outstanding performance with the maximum external quantum efficiency (ZEQE) of 21.2% and a photoluminescence quantum yield of 65.1%. This high efficiency is attributed to efficient energy transfer from the host to the emitter. Moreover, the 4Ac35CzPy device exhibits a high light outcoupling efficiency of 0.3, further enhancing its remarkable performance
Partial Nephrectomy in the Treatment of Localized Renal Cell Carcinoma — Experience of Taichung Veterans General Hospital
BackgroundPartial nephrectomy has been considered an effective and efficient method in the treatment of localized renal cell carcinoma. Herein, we retrospectively review our experience with partial nephrectomy in the treatment of localized renal cell carcinoma and compared it with patients who received radical nephrectomy.MethodsFrom 1982 to 2005, 35 patients who received partial nephrectomy for localized renal cell carcinoma were enrolled in this study. Ten patients were female (28.6%). The median age was 70 years (range, 42–82 years). Sixteen (45.7%) patients had pathologic T1a tumors; 17 (48.6%) patients had pathologic T1b tumors and 2 (5.7%) patients had pathologic T2 tumor (7 cm). In the meantime, 128 patients who had T1N0M0 renal cell carcinoma and who received radical nephrectomy were assigned to a control group. Thirty-nine patients (30.5%) were female in this group. The median age was 62 years (range, 30–83 years). The tumor characteristics, location, surgical techniques and patient survival were subsequently compared.ResultsThe median tumor size in the partial nephrectomy group was 3.9 cm (range, 1.5–7.0 cm), and it was 4.5 cm (range, 1–6.5 cm) in radical nephrectomy group. The tumor size was smaller in the partial nephrectomy group (p = 0.003). The median follow-up period was 4.38 years (range, 0.05–17.99 years) in the partial nephrectomy group and 5.66 years (range, 0.01–22.25 years) in the radical nephrectomy group. There was no local recurrence or distant metastasis in the partial nephrectomy group. The 5-year overall survival was 85.0% compared with 91.4% in the radical nephrectomy group (p = 0.126). The 5-year disease-specific survival in the partial nephrectomy group was 100%. The postoperative serum creatinine level increased to > 2.0 mg/dL in 5 (14.3%) patients in the partial nephrectomy group, but no patient needed hemodialysis during follow-up.ConclusionFrom our review, partial nephrectomy is safe and provides excellent disease control in the treatment of localized renal cell carcinoma in selected patients. Renal function preservation was observed in the partial nephrectomy group, while the operated kidney showed functioning in the follow-up nuclear medicine survey
New Plasma Separation Glucose Oxidase-based Glucometer in Monitoring of Blood With Different PO2 Levels
BackgroundThe PalmLab glucometer is a newly designed plasma separation glucose oxidase (GO)-based glucometer. Past studies have shown that the accuracy of GO-based glucometers is compromised when measurements are taken in patients with high PO2 levels. We performed a two-arm study comparing the fitness of the PalmLab blood glucometer with that of a standard glucose analyzer in monitoring blood glucose levels in pediatric patients, especially when arterial partial pressure of oxygen (PO2) was high.MethodsIn the first arm of the study, arterial blood samples from pediatric patients were measured by the PalmLab blood glucometer and the YSI 2302 Plus Glucose/Lactate analyzer. In the second arm of the study, venous blood samples from adult volunteers were spiked with glucose water to prepare three different levels of glucose (65, 150, and 300mg/dL) and then oxygenated to six levels of PO2 (range, 40–400mmHg). The biases of the PalmLab glucometer were calculated.ResultsA total of 162 samples were collected in the first arm of the study. Results of linear regression showed that the coefficient of determination (R2) between PalmLab glucometer and standard glucose analyzer was 0.9864. Error grid analysis revealed that all the results were within Zone A (clinically accurate estimate zone). The biases between the two systems were low at different PO2 levels. In the second arm of the study, the results were also unaffected by changes in PO2.ConclusionThe PalmLab glucometer provides accurate results in samples with high PO2 and is suitable for measuring arterial glucose levels in pediatric patients
