CT colonography: avoiding traps and pitfalls

Computed tomographic colonography (CTC) is a reliable technique for detecting tumoral lesions in the colon. However, good performance of polyp detection is only achieved if experienced CTC radiologists combine meticulous interpretation with state-of-the-art CTC technique. To reach this experience level, CTC training is mandatory. With a considerably long and steep learning curve, it has been demonstrated that in inexperienced hands both technical failure and observer errors stand for the majority of missed lesions. The purpose of this pictorial review is to give an overview of traps and pitfalls in CTC imaging resulting in false negative and positive findings, and how to avoid them by application of state-of-the-art CTC technique and interpretation

Tiempo de silencio, de Luis Martín Santos, y la innovación de la novela española bajo el Franquismo

La conferencia va a centrarse en la relación compleja que construye Tiempo de silencio (1962), de Luis Martín Santos, con la tradición del realismo de la novela española y en particular con el realismo social bajo la dictadura de Franco. Se pondrá de relieve que, si bien el autor elabora una visión –pesimista– de la España de los años 50, lo hace de una manera que trasciende los moldes de la poética realista al uso (la decimonónica, algo renovada por el llamado "realismo social" de la posguerra, tipo La colmena –una metáfora que por cierto se remonta por lo menos a Galdós—) para trascenderlo hacia un realismo que se puede calificar como “dialéctico”, "existencial", "simbólico", incluso "mítico" (como hicieron los diversos comentaristas, pero se tratará de ponderar y articular estas diferentes perspectivas), pero también hacia una superación de la misma estética realista en el sentido de una realismo lingüístico (triunfo de la "función poética" de Jakobson sobre la función expresiva).Universidad de Málaga. Lighter serum copper isotopic composition in patients with early non-alcoholic fatty liver disease

Objective The occurrence of non-alcoholic fatty liver disease (NAFLD) is globally increasing. To challenge the current incidence of NAFLD, non-invasive markers that could identify patients at risk or monitor disease progression are an important need. Copper intake and organ copper concentrations have earlier been linked to NAFLD progression, but serum copper does not adequately represent the disease state. Cu atoms occur under the form of two stable isotopes, Cu-63 and Cu-65, and the ratio of both (expressed as delta Cu-65, in parts per thousand) in blood serum has been shown to be altered in chronic liver disease. To assess whether the Cu isotope ratio might predict disease occurrence and progression of NAFLD, the serum Cu isotopic composition of patients with different stages of NAFLD was determined. Results Our results showed that serum delta Cu-65 values were lower in NAFLD patients, already at the level of simple steatosis, and remained stable during further disease progression. ROC analysis shows an almost perfect diagnostic ability of serum delta Cu-65 values for NAFLD, but no discrimination between different severity degrees could be made. Therefore, the serum Cu isotopic composition might show potential for early diagnosis of NAFLD patients

Comparison of the diagnostic performance of CT colonography interpreted by radiologists and radiographers

OBJECTIVE: To compare computed tomographic colonography (CTC) performance of four trained radiographers with the CTC performance of two experienced radiologists. METHODS: Four radiographers and two radiologists interpreted 87 cases with 40 polyps ≥6 mm. Sensitivity, specificity, and positive predictive value (PPV) were assessed on a per-patient basis. On a per-polyp basis, sensitivity was calculated according to the respective size categories (polyps ≥6 mm as well as polyps ≥10 mm). RESULTS: Overall per-patient sensitivity for polyps ≥6 mm was 76.2 % (95 % CI 61.4–91.0) and 76.2 % (95 % CI 61.7–90.6), for the radiographers and radiologists, respectively. Overall per-patient specificity for polyps ≥6 mm were 81.4 % (95 % CI 73.7–89.2) and 81.1 % (95 % CI 73.8–88.3) for the radiographers and the radiologists, respectively. For the radiographers, overall per-polyp sensitivity was 60.3 % (95 % CI 50.3–70.3) and 60.7 % (95 % CI 42.2–79.2) for polyps ≥6 mm and ≥10 mm, respectively. For the radiologists, overall per polyp sensitivity was 59.2 % (95 % CI 46.4–72.0) and 69.0 % (95 % CI 48.1–89.6) for polyps ≥6 mm and ≥10 mm, respectively. CONCLUSION: Radiographers with training in CT colonographic evaluation achieved sensitivity and specificity in polyp detection comparable with that of experienced radiologists. MAIN MESSAGES: • The diagnostic accuracy of trained radiographers was comparable to that of experienced radiologists. • The use of radiographers in reading CTC examinations is acceptable, however radiologists would still be necessary for the evaluation of extracolonic findings. • Skilled non-radiologists may play a vital role as a second reader of intraluminal findings or by performing quality control of examinations before patient dismissal

Theatrum mundi y exaltación del héroe trágico en La visita en el tiempo de Uslar Pietri

Esta contribución se centra en la última novela histórica de A. Uslar Pietri, La visita en el tiempo (1990), para poner de relieve la especificidad semántica del discurso novelesco de la Historia y, en especial, para analizar el proceso de ficcionalización. En este marco, reflexiona sobre la tematización de la figura de Don Juan de Austria (desmitificado como héroe histórico y exaltado como héroe trágico) y, en particular, sobre las funciones y el alcance del topos literario (principalmente el theatrum mundi) en el proceso de tematización de la Historia.Aquesta contribució se centra en l'última novel·la històrica de A. Uslar Pietri, La visita en el tiempo (1990), per posar en relleu l'especificitat semàntica del discurs novel·lesc de la Història i, especialment, per analitzar el procés de ficcionalizació. En aquest marc, reflexiona sobre la tematització de la figura de Don Juan de Austria (desmitificat com a heroi històric i exaltat com a heroi tràgic) i, en particular, sobre les funcions i l'abast del talps literari (principalment el theatrum mundi) en el procés de tematització de la Història.This contribution focuses on A. It aims at highlighting the semantic specificity of the novelistic discourse of History, and more particularly at analyzing the process of fictionalization. Within this framework, the article reflects on the thematization of the Don Juan de Austria's figure (demythified as an historical hero and exalted as a tragic hero) and more specifically on the functions and the scope of the literary topos (mainly the theatrum mundi) in the process of History thematization
