    Delays in starting antiretroviral therapy in patients with HIV-associated tuberculosis accessing non-integrated clinical services in a South African township

    BACKGROUND: Delays in the initiation of antiretroviral therapy (ART) in patients with HIV-associated tuberculosis (TB) are associated with increased mortality risk. We examined the timing of ART among patients receiving care provided by non-integrated TB and ART services in Cape Town, South Africa. METHODS: In an observational cohort study, we determined the overall time delay between starting treatment for TB and starting ART in patients treated in Gugulethu township between 2002 and 2008. For patients referred from TB clinics to the separate ART clinic, we quantified and identified risk factors associated with the two component delays between starting TB treatment, enrolment in the ART clinic and subsequent initiation of ART. RESULTS: Among 893 TB patients studied (median CD4 count, 81 cells/Ī¼L), the delay between starting TB treatment and starting ART was prolonged (median, 95 days; IQR = 49-155). Delays were shorter in more recent calendar periods and among those with lower CD4 cell counts. However, the median delay was almost three-fold longer for patients referred from separate TB clinics compared to patients whose TB was diagnosed in the ART clinic (116 days versus 41 days, respectively; P < 0.001). In the most recent calendar period, the proportions of patients with CD4 cell counts < 50 cells/Ī¼L who started ART within 4 weeks of TB diagnosis were 11.1% for patients referred from TB clinics compared to 54.6% of patients with TB diagnosed in the ART service (P < 0.001). CONCLUSIONS: Delays in starting ART were prolonged, especially for patients referred from separate TB clinics. Non-integration of TB and ART services is likely to be a substantial obstacle to timely initiation of ART

    Congenital disorders (often also called birth defects) are an important cause of mortality and disability. They encompass a wide range of disorders with differing severity that can affect any aspect of structure or function. Understanding their epidemiology is important in developing appropriate services both for their prevention and treatment. The need for epidemiological data on congenital disorders has been recognised for many decades. Here, we provide a historical overview of work that has led to the development of the Modell Global Database of Congenital Disorders (MGDb)-a tool that can be used to generate evidence-based country, regional and global estimates of the birth prevalence and outcomes of congenital disorders

    Drug and non-drug reward processing in cigarette and cannabis users

    Most people who try psychoactive drugs never become addicted. Theoretically, hypersensitivity to drug rewards and hyposensitivity to non-drug rewards may contribute to the development of drug addiction. In chapter 1, I review this literature, focusing on the psychology and neuroscience of reward processing, in nicotine and cannabis addictions. In chapter 2, using a novel task (the DReaM-Choice), I demonstrate that dependent (n=20), compared with occasional smokers (n=20), had greater motivation for and liking of cigarettes, but displayed little evidence of a difference in non-drug reward processing. Surprisingly, I also show the effects of 12 hour abstinence on reward processing were similar in dependent and occasional smokers. I then report a functional magnetic-resonance-imaging (fMRI) experiment (chapter 3), in which dependent smokers (n=22) had greater behavioural motivation for cigarettes and a stronger neural response to winning cigarettes than occasional smokers (n=20). However, there were no differences between the groups in behavioural or neural processing of the non-drug reward (music). I attempted to lessen the motivation to smoke cigarettes in the study reported in chapter 4, by administering a dopamine D2/3 receptor agonist (0.5mg pramipexole) to both dependent (n=20) and occasional (n=20) smokers. Pramipexole had no impact on motivation to smoke cigarettes, though it did impair reward learning and effort-related decision-making for monetary reward. In chapter 5, I found that, in non-dependent cannabis users (n=17), acutely administered cannabis reduced motivation for monetary reward; an effect which was moderated by the presence of cannabidiol in the cannabis. In a separate study, I demonstrate that dependent cannabis users (n=20) had impaired reward learning, but were not amotivated, relative to non-dependent, drug-using controls (n=20). Finally, in chapter 6, I summarise my findings, discuss their theoretical and clinical implications, consider their limitations and suggest future research directions for the field of reward processing in addiction

    The Influence of Fallback Foods on Great Ape Tooth Enamel

    Lucas and colleagues recently proposed a model based on fracture and deformation concepts to describe how mammalian tooth enamel may be adapted to the mechanical demands of diet (Lucas et al.: Bioessays 30[2008] 374-385). Here we review the applicability of that model by examining existing data on the food mechanical properties and enamel morphology of great apes (Pan, Pongo, and Gorilla). Particular attention is paid to whether the consumption of fallback foods is likely to play a key role in influencing great ape enamel morphology. Our results suggest that this is indeed the case. We also consider the implications of this conclusion on the evolution of the dentition of extinct hominins

    Fracture in teethā€”a diagnostic for inferring bite force and tooth function

    Teeth are brittle and highly susceptible to cracking. We propose that observations of such cracking can be used as a diagnostic tool for predicting bite force and inferring tooth function in living and fossil mammals. Laboratory tests on model tooth structures and extracted human teeth in simulated biting identify the principal fracture modes in enamel. Examination of museum specimens reveals the presence of similar fractures in a wide range of vertebrates, suggesting that cracks extended during ingestion or mastication. The use of ā€˜fracture mechanicsā€™ from materials engineering provides elegant relations for quantifying critical bite forces in terms of characteristic tooth size and enamel thickness. The role of enamel microstructure in determining how cracks initiate and propagate within the enamel (and beyond) is discussed. The picture emerges of teeth as damage-tolerant structures, full of internal weaknesses and defects and yet able to contain the expansion of seemingly precarious cracks and fissures within the enamel shell.     Improved memory for information learnt before alcohol use in social drinkers tested in a naturalistic setting

The final version is available from Springer Nature via the DOI in this record.Alcohol is known to facilitate memory if given after learning information in the laboratory; we aimed to investigate whether this effect can be found when alcohol is consumed in a naturalistic setting. Eighty-eight social drinkers were randomly allocated to either an alcohol self-dosing or a sober condition. The study assessed both retrograde facilitation and alcohol induced memory impairment using two independent tasks. In the retrograde task, participants learnt information in their own homes, and then consumed alcohol ad libitum. Participants then undertook an anterograde memory task, of alcohol impairment when intoxicated. Both memory tasks were completed again the following day. Mean amount of alcohol consumed was 82.59 grams over the evening. For the retrograde task, as predicted, both conditions exhibited similar performance on the memory task immediately following learning (before intoxication) yet performance was better when tested the morning after encoding in the alcohol condition only. The anterograde task did not reveal significant differences in memory performance post-drinking. Units of alcohol drunk were positively correlated with the amount of retrograde facilitation the following morning. These findings demonstrate the retrograde facilitation effect in a naturalistic setting, and found it to be related to the self-administered grams of alcohol.The study was funded by an MRC grant to CJAM (MR/L023032/1), we would like to thank all the participants for their time

    To What Extent is Blood a Reasonable Surrogate for Brain in Gene Expression Studies: Estimation from Mouse Hippocampus and Spleen

    Microarrays are designed to measure genome-wide differences in gene expression. In cases where a tissue is not accessible for analysis (e.g. human brain), it is of interest to determine whether a second, accessible tissue could be used as a surrogate for transcription profiling. Surrogacy has applications in the study of behavioural and neurodegenerative disorders. Comparison between hippocampus and spleen mRNA obtained from a mouse recombinant inbred panel indicates a high degree of correlation between the tissues for genes that display a high heritability of expression level. This correlation is not limited to apparent expression differences caused by sequence polymorphisms in the target sequences and includes both cis and trans genetic effects. A tissue such as blood could therefore give surrogate information on expression in brain for a subset of genes, in particular those co-expressed between the two tissues, which have heritably varying expression

    Phase Transition in a Random Fragmentation Problem with Applications to Computer Science

    We study a fragmentation problem where an initial object of size x is broken into m random pieces provided x>x_0 where x_0 is an atomic cut-off. Subsequently the fragmentation process continues for each of those daughter pieces whose sizes are bigger than x_0. The process stops when all the fragments have sizes smaller than x_0. We show that the fluctuation of the total number of splitting events, characterized by the variance, generically undergoes a nontrivial phase transition as one tunes the branching number m through a critical value m=m_c. For m<m_c, the fluctuations are Gaussian where as for m>m_c they are anomalously large and non-Gaussian.     Making stillbirths count, making numbers talk - issues in data collection for stillbirths.

    BACKGROUND: Stillbirths need to count. They constitute the majority of the world's perinatal deaths and yet, they are largely invisible. Simply counting stillbirths is only the first step in analysis and prevention. From a public health perspective, there is a need for information on timing and circumstances of death, associated conditions and underlying causes, and availability and quality of care. This information will guide efforts to prevent stillbirths and improve quality of care. DISCUSSION: In this report, we assess how different definitions and limits in registration affect data capture, and we discuss the specific challenges of stillbirth registration, with emphasis on implementation. We identify what data need to be captured, we suggest a dataset to cover core needs in registration and analysis of the different categories of stillbirths with causes and quality indicators, and we illustrate the experience in stillbirth registration from different cultural settings. Finally, we point out gaps that need attention in the International Classification of Diseases and review the qualities of alternative systems that have been tested in low- and middle-income settings. SUMMARY: Obtaining high-quality data will require consistent definitions for stillbirths, systematic population-based registration, better tools for surveys and verbal autopsies, capacity building and training in procedures to identify causes of death, locally adapted quality indicators, improved classification systems, and effective registration and reporting systems

    Psilocybin with psychological support improves emotional face recognition in treatment-resistant depression

    RATIONALE: Depressed patients robustly exhibit affective biases in emotional processing which are altered by SSRIs and predict clinical outcome. OBJECTIVES: The objective of this study is to investigate whether psilocybin, recently shown to rapidly improve mood in treatment-resistant depression (TRD), alters patients' emotional processing biases. METHODS: SeventeenĀ patients with treatment-resistant depression completed a dynamic emotional face recognition task at baseline and 1Ā month later after two doses of psilocybin with psychological support. Sixteen controls completed the emotional recognition task over the same time frame but did not receive psilocybin. RESULTS: We found evidence for a group Ɨ time interaction on speed of emotion recognition (pĀ =Ā .035). At baseline, patients were slower at recognising facial emotions compared with controls (pĀ <Ā .001). After psilocybin, this difference was remediated (pĀ =Ā .208). Emotion recognition was faster at follow-up compared with baseline in patients (pĀ =Ā .004, dĀ =Ā .876) but not controls (pĀ =Ā .263, dĀ =Ā .302). In patients, this change was significantly correlated with a reduction in anhedonia over the same time period (rĀ =Ā .640, pĀ =Ā .010). CONCLUSIONS: Psilocybin with psychological support appears to improve processing of emotional faces in treatment-resistant depression, and this correlates with reduced anhedonia. Placebo-controlled studies are warranted to follow up these preliminary findings
