“SENSIBILIDAD Y ESPECIFICIDAD DE LA ESCALA DEL CENTER FOR DISEASE CONTROL AND PREVENTION PARA EL DIAGNÓSTICO DE NEUMONIA ASOCIADA A VENTILACIÓN MECÁNICA NEONATAL”.
RESUMEN.
INTRODUCCIÓN
La Neumonía Asociada a Ventilación Mecánica ocupa cerca de un 30 % de las infecciones nosocomiales en las terapias neonatales, el diagnóstico temprano en el recién nacido ha venido sustentando diversos problemas, ya que no se han logrado unificar criterios para un diagnóstico certero y así prevenir las complicaciones de la misma.
OBJETIVOS
Conocer la sensibilidad y especificidad de escala de la CDC para el diagnóstico de Neumonía Asociada a Ventilación Mecánica.
METODOS
Se hizo un análisis retrolectivo desde enero de 2013 a Julio de 2015, obteniendo información del expediente en el servicio de neonatología del Hospital Para el Niño IMIEM y mediante medidas de tendencia central y dispersión, de las diferentes variables demográficas; Se calculó la sensibilidad y especificidad de la escala CDC para el diagnóstico de neumonía asociada a ventilación mecánica neonatal.
RESULTADOS
La población total del estudio fue de 27 casos (n=27); calculando una sensibilidad 72.7%, y una especificidad de 25 % con un valor predictivo positivo de 40% y un valor predictivo negativo 57% para el diagnóstico de neumonía asociada a ventilación mecánica de la escala de la CDC.
CONCLUSIONES
La escala de la CDC para el diagnóstico de Neumonía Asociada a Ventilación Mecánica no es altamente sensible y específica para el diagnóstico de Neumonía asociada a ventilación mecánica en la población neonatal. Sin embargo, este estudio fue limitado por lo que un estudio más extenso creemos que arrojara mejores resultados para el diagnóstic
De la idea al concepto en la calidad en los servicios de salud
The definition of quality in healthcare is not a static or
universal concept; there are diverse conceptualizations
that enrich the term based on the elements added as
time goes by. Some of the concepts of quality that have
been used in the industry are presented, as well as the
way in which some of them have been the origin to
the concepts used in healthcare services. Finally, detailed
concepts regarding the quality in healthcare are presented
from different perspectives and taking into account
diverse components.La definición de calidad en la atención a la salud no es
un concepto estático ni universal, existen conceptualizaciones
que enriquecen el término en función de elementos
que se han agregado en el tiempo. Se presentan
algunos conceptos de calidad que se han utilizado en la
industria y cómo varios de ellos han dado origen a los
que se emplean en los servicios de salud. Finalmente se
presentan conceptos explícitos de la calidad en la atención
a la salud, desde diferentes perspectivas y considerando
distintos componentes
Characterization of titania inverse opals prepared by two distinct infiltration approaches
Titanium dioxide inverse opals were prepared by the typical method of infiltration with a liquid precursor (LP-IOs) and by an improved method in which the infiltration step is done with a suspension of TiO2 nanoparticles (NP-IOs). A thorough characterization study was performed on both types of preparations in order to highlight the differences between the resulting inverse opal samples. The formation of a compact overlayer in LP-IO samples inherent to the use of a liquid precursor proved to be detrimental for the optical and photonic properties of the inverse opals. NP-IO samples, on the other hand, showed improved optical, photonic, and mechanical properties. The photocatalytic degradation of a model compound (a well-known dye, methylene blue) revealed that NP-IO samples are able to degrade the model compound at a higher rate due to the absence of the compact overlayer which acts as a mass and light blocking barrier.Fil: Curti, Mariano. El Sistema de Gestión de Calidad en la CONAMED, a 12 años de su implantación
Many private companies implement a quality management
system because by setting up this system they generate
an additional value that directly impacts the manufacturing,
marketing, and distribution processes; and
this impact is also felt in their provision of services and
the image the organization gives to its customers.
By addressing the dispositions for improving management
in public administration and given the nature of
the Medical Arbitration National Commission (attention
to non-conformances derived from medical assistance), it becomes necessary to provide a service according to
international standards, with the purpose of providing
the users with a quality service, that is also honest, fair
and impartial; and, at the same time, it is necessary that
these experiences give feedback to the Health National
System, by means of elements contributing to improve
the quality of medical assistance.
The Quality Management System implemented in this
Commission supports the operation processes, which
allows to promote a quality provision of services to users
and successfully face the contingencies that occur in its
environment, apart form duly complying with legal and
regulating dispositions.La implementación de un sistema de gestión de calidad
es realizada por un gran número de empresas privadas,
ya que su puesta en marcha genera un valor adicional
que impacta directamente en los procesos de fabricación,
comercialización y distribución de los productos;
así como en la prestación de sus servicios y en la imagen
de la organización hacia sus clientes.
Atendiendo las disposiciones para la mejora de la
gestión en la administración pública y dada la naturaleza
de la Comisión Nacional de Arbitraje Médico (atención
de inconformidades derivadas de la atención médica),
resulta necesario brindar una atención acorde a los estándares
internacionales, con el objeto de brindar a los
usuarios una atención de calidad, honesta, justa e imparcial;
y que a su vez estas experiencias retroalimenten al
Sistema Nacional de Salud con elementos que contribuyan
a mejorar la calidad de la atención médica.
El Sistema de Gestión de Calidad implementado en
esta Comisión, da sustento a los procesos de operación,
lo cual permite promover la prestación de servicios a los
usuarios con calidad y hacer frente de forma exitosa a las
eventualidades que se presentan en el entorno, además
de dar cabal cumplimiento con disposiciones legales y
reglamentarias
A step forward in antibiotic use and resistance monitoring: a quarterly surveillance system pilot in 11 European Union/European Economic Area countries, September 2017 to May 2020.
BackgroundSurveillance of antimicrobial resistance (AMR) and antimicrobial use (AMU) in Europe is currently annual.AimTo study the feasibility and scalability of a quarterly AMR/AMU surveillance system in the European Union/European Economic Area (EU/EEA).MethodsWe conducted a longitudinal study within the scope of the EU-JAMRAI project. Seventeen partners from 11 EU/EEA countries prospectively collected 41 AMU and AMR indicators quarterly from September 2017 to May 2020 for the hospital sector (HS) and primary care (PC). Descriptive statistics and coefficients of variation (CV) analysis were performed.ResultsData from 8 million hospital stays and 45 million inhabitants per quarter were collected at national (n = 4), regional (n = 6) and local (n = 7) levels. Of all partners, five were able to provide data within 3 months after each preceding quarter, and eight within 3-6 months. A high variability in AMU was found between partners. Clinical Experience of 6 Years with Rabbit Anti-Thymocyte Globulin as Induction Therapy in Renal Transplant Receptors
The rabbit-derived anti-thymocyte globulin (rATG/ThymoglobulineÒ) is currently the most commonly used drug in induction therapy regimens in kidney transplantation worldwide.
OBJECTIVESuccess rate in the survival of the renal graft and adverse effects at one year with Thymoglobuline as inducer in the immediate post-transplant.
MATERIAL AND METHODSIn a retrospective cohort of post-transplant kidney patients who received Thymoglobuline as induction, 12 months were followed. We include recipients of a related live donor, unrelated and deceased. We obtained information about accumulated dose, cytopenias, renal function, rejection, adverse effect, modification of maintenance immunosuppression due to leukopenia, lymphoproliferative disorders, bacterial, mycotic and viral infections as well as graft and patient survival.
RESULTSWe included 165 kidney transplant recipients, 38.7% related live donor, 30.9% unrelated live, 30.3% deceased. The cumulative dose of Thymoglobuline was 5.3 ± 1.61 mg/kg. Absence of rejection 92.72% at 12 months, 7.27% presented rejection and graft loss 0.6% of the population. Serious adverse effects to Thymoglobuline 1.2%. Thrombocytopenia 26.06%, 95.34% grade 1 (114.01 ± 0.20 cells/mm3). Leukopenia 7 days 8.4%, most grade 1 (92.8%) average 3.48 ± 0.28 cells/mm3, at 3 months in 33.93% grade 1 in 50 % 3.60 ± 0.21 cells/mm3, severe 1.78%. No patient had lymphoproliferative disease and 53.93% bacterial infections.
CONCLUSIONSThe use of rATG as post-transplant induction therapy has an excellent success rate with 95.9% patient survival and 95.4% of the graft per year, a lower rejection rate of 7.27%, with few adverse effects and none serious
Visible-Light Photocatalysis with Mullite-Type Bi2(Al1- xFex)4O9: Striking the Balance between Bandgap Narrowing and Conduction Band Lowering
The rich crystal chemistry of mullite-type Bi2M4O9 (M = Fe, Al, Ga) offers multiple potential applications. In particular, the strong absorption of visible light shown by Bi2Fe4O9 has led to an influx of research on its photocatalytic properties. However, most of the published studies involve the decolorization of dyes and take it as proof of its photocatalytic activity; furthermore, there are no reports on its conduction and valence band edges, and, thus, the actual redox characteristics of the photogenerated charge carriers have not been determined. Here, we evaluate the photocatalytic activity toward methanol oxidation under monochromatic visible light (= 450 nm) irradiation of 12 different members of the Bi2(Al1-xFex)4O9 (x = 0-1) series of compounds. The reaction rate reaches its highest value at an iron fraction of x = 0.1, while the compounds with the highest iron fractions present negligible activity. Based on an extensive characterization, which included the Rietveld refinement of the XRD patterns, the measurement of the specific surface areas by the BET method, and the determination of the flat-band potentials by the Mott-Schottky method, we rationalize the results on the basis of two opposing factors: the incorporation of iron narrows the fundamental bandgap and thus improves light capture, but at the same time it lowers the conduction band edge, hindering the oxygen reduction half-reaction and thus promoting electron-hole recombination. Our work highlights the importance of a proper band edge engineering for photocatalytic applications and underlines the inadequacy of dye decolorization tests for visible-light-active materials.Fil: Curti, Mariano. "Cora 2012": híbrido intervarietal de maíz para Nayarit y regiones similares
The intervarietal Cora hybrid of maize 2012 was developed
by the method of Reciprocal Recurrent Selection (RRS).
A heterotic pattern of P * A populations is used. The first
two cycles were with half sib progeny supported by corn
farmers from Nayarit, participatory breeding. From the
third cycle, progeny were from reciprocal full siblings
through crosses between lines S1
, type P S1 * A S1
. Cora
2012 was derived from the third cycle. The formation of
populations (1999-2001) and the implementation of RRS
(2002-2010) were performed in Nayarit. The evaluation was
conducted in Nayarit (2005-2010) and validation included
the states of Jalisco, Michoacán and Sinaloa (2010-2013).
The characterization was made in Nayarit (2010- 2011) and
was assigned as 'MAZ-1477-010313' in the record from the
Catálogo Nacional de Variedades Vegetales (CNVV). Is has
a similar behavior to current maize hybrids of 7 t ha-1. On
average, also have similar agronomic characteristics than
commercial hybrids. Its greatest attribute is that it yields well
even under changing climate factors, technology used and
soil. Seed production is relatively easy and yields around 4.5
t ha-1 of seed. Associated farmers and small seed companies

El Cora 2012 se derivó de este tercer ciclo de avance. La
formación de las poblaciones (1999-2001) y la aplicación
de la SRR (2002-2010) se han realizado en Nayarit. La
evaluación se realizó en Nayarit (2005-2010) y la validación
también incluyó los estados de Jalisco, Michoacán y Sinaloa
(2010-2013). La caracterización se realizó en Nayarit
(2010- 2011) y fue asignado como �MAZ-1477 - 010313�en
el registro del Catálogo Nacional de Variedades Vegetales
(CNVV). Tiene un comportamiento similar a los híbridos
de maíz de actual uso; de 7 t ha-1. En promedio, también
tiene similares características agronómicas que los híbridos
comerciales. Su mayor atributo es que rinde bien aún en
factores cambiantes de clima, tecnología usada y suelo. La
producción de semilla es relativamente fácil y se obtienen como 4.5 t ha-1 de semilla. Agricultores asociados y pequeñas
empresas productoras de semilla pueden disponer de la
semilla de los progenitores
