    Influencia del apego en la toma de decisiones profesionales durante la adolescencia: Una revisión sistemática

    The first career decision making we face occurs in the course of adolescence, however, the primary caregivers and the attachment relationship with them seems that it could influence it, although it is not clear in what way. The aim of this paper is to critically review studies that analyse the relationship between attachment style between caregivers and adolescents and career decision-making. Following the PRISMA methodology, 20 studies were selected. The results showed that there is a relationship between attachment style and career decision-making and involved other variables as mediators. Specifically, an insecure attachment style negatively influences decision-making while a secure attachment style functions as a protective and facilitating factor.La primera toma de decisiones profesionales a la que nos enfrentamos se da en el transcurso de la adolescencia, sin embargo, los cuidadores principales y la relación de apego con ellos parece que podría influir en la misma, aunque no está claro en qué forma. El objetivo de este trabajo es analizar críticamente los estudios que analizan la relación entre el estilo de apego entre cuidadores y adolescentes y la toma de decisiones profesionales. Siguiendo la metodología PRISMA, se seleccionaron 20 estudios. Los resultados mostraron que existe una relación entre el estilo de apego y la toma de decisiones profesionales e involucraron a otras variables como mediadoras. Concretamente un estilo de apego inseguro influye negativamente en la toma de decisiones mientras que un estilo de apego seguro funciona como factor de protección y facilitador

    Synthetic approaches to multifunctional indenes

    The synthesis of multifunctional indenes with at least two different functional groups has not yet been extensively explored. Among the plausible synthetic routes to 3,5-disubstituted indenes bearing two different functional groups, such as the [3-(aminoethyl)inden-5-yl)]amines, a reasonable pathway involves the (5-nitro-3-indenyl)acetamides as key intermediates. Although several multistep synthetic approaches can be applied to obtain these advanced intermediates, we describe herein their preparation by an aldol-type reaction between 5-nitroindan-1-ones and the lithium salt of N,N-disubstituted acetamides, followed immediately by dehydration with acid. This classical condensation process, which is neither simple nor trivial despite its apparent directness, permits an efficient entry to a variety of indene-based molecular modules, which could be adapted to a range of functionalized indanones

    Prevalence of altered passive eruption in orthodontically treated and untreated patients

    Background: some authors have reported that after orthodontic treatment (OT), a 'gummy smile' might develop. Nevertheless, there are no studies in the literature that investigate whether OT increases the presence of altered passive eruption (APE). The primary aim of this cross-sectional study is to evaluate the prevalence of APE after OT (OT group) and compare it with patients who never received OT (control group). A secondary aim is to identify which variables are related to APE. Methods: the study population consisted of 190 patients (95 patients each in the control and OT groups), providing 1,140 anterior teeth for the clinical examination. The following clinical parameters were assessed: presence or absence of APE, clinical crown length, and gingival biotype, which was divided into three categories: thin-scalloped, thick-flat, and thick-scalloped. Results: twenty-eight patients (29.5%) were diagnosed with APE in the control group and 40 (42.1%) in the OT group, although this difference was not statistically significant (P = 0.07). Furthermore, 34 (75.6%) patients with thick-flat biotype were diagnosed with APE, whereas 30 (31.3%) and four (8.2%) with thick-scalloped and thin-scalloped biotypes, respectively, had APE. These differences were statistically significant (P <0.001). Conclusions: it was concluded that: 1) the prevalence of APE is higher after OT but not to a statistically significant degree and 2) APE is more common in individuals with a thick-flat gingival biotype

    A randomized controlled trial: branched‐chain amino acid levels and glucose metabolism in patients with obesity and sleep apnea

    There is evidence that changes in branched‐chain amino acid (BCAA) levels may correlate with the efficacy of therapeutic interventions for affecting improvement in metabolic control. The objective of this study was to evaluate whether serum concentrations of BCAAs (leucine, isoleucine, valine) could mediate in insulin sensitivity and glucose tolerance after continuous positive airway pressure (CPAP) treatment in patients with obstructive sleep apnea (OSA). A prospective randomized controlled trial of OSA patients with morbid obesity was conducted. Eighty patients were randomized into two groups: 38 received conservative treatment and 42 received CPAP treatment for 12 weeks. Plasma levels of BCAA, glucose tolerance and insulin resistance were evaluated at baseline and after treatment. After treatment, significant decreases of leucine levels were observed in both groups when compared with baseline levels (P < 0.005). With respect to patients with normal glucose tolerance (NGT), patients with impaired glucose tolerance (IGT) had higher baseline levels of isoleucine (78 ± 16 versus 70 ± 13 μmol L−1, P = 0.014) and valine (286 ± 36 versus 268 ± 41 μmol L−1, P = 0.049), respectively. Changes in levels of leucine and isoleucine after treatment were related negatively to changes in fasting plasma glucose and glycosylated haemoglobin values only in the conservative group (P < 0.05). In summary, we found that the treatment with CPAP for 12 weeks caused similar changes in circulating BCAAs concentrations to conservative treatment and a differential metabolic response of CPAP and conservative treatment was observed between the relationship of BCAAs and glucose homeostasis. Additional studies are needed to determine the interplay between branched‐chain amino acids and glucose metabolism in patients with sleep apnea

    The recent neophyte Opuntia aurantiaca (Cactaceae): distribution and potential invasion in the Iberian Peninsula

    The Cactaceae, and especially its most emblematic genus, Opuntia, is one of the groups of plants with greater invasion potential in the Iberian Peninsula. One of the most recently detected species is Opuntia aurantiaca, a small cactus with an enormous capacity of dispersion. Probably native to Argentina and Uruguay, it behaves as a very aggressive invader in Australia and South Africa. In Europe, it only occurs on the Mediterranean coast of the Iberian Peninsula (Catalonia and Valencian Community). In this study, the geographic range of the species is accurately delineated at the peninsular level. Detected firstly at the beginning of the last decade in Navajas (Castelló Province), it has been subsequently observed in other places of Castelló, but also in Valencia, Tarragona and Barcelona, and since 2017 in Girona. With all gathered occurrence data, the potential distribution of O. aurantiaca is estimated (for the current climatic conditions as well as for different scenarios of global warming).     Differential contribution of the two major polygalacturonases from Penicillium digitatum to virulence towards citrus fruit

    The fungus Penicillium digitatum is the causal agent of the citrus green mould, the major postharvest diseases of citrus fruit. Lesions on the surface of infected fruits first appear as soft areas around the inoculation site, due to maceration of fruit. The macerating activity has been associated with pectinases secreted by the fungus during infection. In order to evaluate the contribution to virulence and macerating activity of the two major polygalacturonases (PGs) secreted by P. digitatum, we have obtained and characterized mutants lacking either pg1 or pg2, the genes encoding PG1 and PG2, respectively. Disease incidence of deletants in either gene was not different from that of the parental strain or ectopic transformants. However, disease progressed more slowly in deletants, especially in those lacking the pg2 gene. The lesions originated by the Δpg2 deletants were not as soft and the pH was not as acid as those originated by either the wild type strain or the ectopic transformants. Total PG activity in the macerated tissue was also lower in fruits infected with the Δpg2 deletants. Interestingly, the macerated tissue of oranges infected with Δpg2 deletants showed around 50% reduction in galacturonic acid content with respect to lesions caused by any other strain.The technical assistance of Ana Izquierdo is gratefully acknowledged. In fact, this area is considered one of the three global hotspots of invasion of cacti, together with South Africa and Australia. In our country, one of the most recently detected species is Opuntia aurantiaca Lindl, a small cactus with an enormous capacity of dispersion by means of cladodes and (sterile) fruits, which are easily detached and adhere to animals, clothes, and footwear. Probably native to the Southern Cone (Argentina and Uruguay), it behaves as a very aggressive invader in Australia and South Africa. In Europe, it only occurs on the Mediterranean coast of the Iberian Peninsula (Catalonia and Valencian Community). In this study, the geographic range of the species is updated at the peninsular level. Firstly detected at the beginning of the last decade in Navajas (Castellón), it has been subsequently observed in other places of the province of Castellón, but also in Valencia, Tarragona and Barcelona. With all occurrence data, its potential distribution is estimated using the algorithm of maximum entropy implemented in the program MaxEnt. Distribution models are built for the current climatic conditions as well as for different scenarios of global warming. Due to its possible invasive nature, it is necessary to eradicate the existing populations at this initial stage. Legislation should consider the prohibition of trade and transport of this plant to avoid new introductions

    Endovascular treatment improves cognition after stroke. A secondary analysis of REVASCAT trial

    Objective: To investigate the effect of endovascular treatment on cognitive function as a prespecified secondary analysis of the REVASCAT (Endovascular Revascularization With Solitaire Device Versus Best Medical Therapy in Anterior Circulation Stroke Within 8 Hours) trial. Methods: REVASCAT randomized 206 patients with anterior circulation proximal arterial occlusion stroke to Solitaire thrombectomy or best medical treatment alone. Patients with established dementia were excluded from enrollment. Cognitive function was assessed in person with Trail Making Test (TMT) Parts A and B at 3 months and 1 year after randomization by an investigator masked to treatment allocation. Test completion within 5 minutes, time of completion (seconds), and number of errors were recorded. Results: From November 2012 to December 2014, 206 patients were enrolled in REVASCAT. TMT was assessed in 82 of 84 patients undergoing thrombectomy and 86 of 87 control patients alive at 3 months and in 71 of 79 patients undergoing thrombectomy and 72 of 78 control patients alive at 1 year. Rates of timely TMT-A completion were similar in both treatment arms, although patients undergoing thrombectomy required less time for TMT-A completion and had higher rates of error-free TMT-A performance. Thrombectomy was also associated with a higher probability of timely TMT-B completion (adjusted odds ratio 3.17, 95% confidence interval 1.51-6.66 at 3 months; and adjusted ratio 3.66, 95% confidence interval 1.60-8.35 at 1 year) and shorter time for TMT-B completion. Differences in TMT completion times between treatment arms were significant in patients with good functional outcome but not in those who were functionally dependent (modified Rankin Scale score >2). Poorer cognitive outcomes were significantly associated with larger infarct volume, higher modified Rankin Scale scores, and worse quality of life. Conclusions: Thrombectomy improves TMT performance after stroke, especially among patients who reach good functional recovery. Classification of evidence: This study provides Class I evidence that for patients with stroke from acute anterior circulation proximal arterial occlusion, thrombectomy improves performance on the TMT at 3 months

    Importance of immunometabolic markers for the classification of patients with major depressive disorder using machine learning

    Background: Although there is scientific evidence of the presence of immunometabolic alterations in major depression, not all patients present them. Recent studies point to the association between an inflammatory phenotype and certain clinical symptoms in patients with depression. The objective of our study was to classify major depression disorder patients using supervised learning algo-rithms or machine learning, based on immunometabolic and oxidative stress biomarkers and lifestyle habits.Methods: Taking into account a series of inflammatory and oxidative stress biomarkers (C-reactive protein (CRP), tumor necrosis factor (TNF), 4-hydroxynonenal (HNE) and glutathione), metabolic risk markers (blood pressure, waist circumference and glucose, triglyceride and cholesterol levels) and lifestyle habits of the participants (physical activity, smoking and alcohol consumption), a study was carried out using machine learning in a sample of 171 participants, 91 patients with depression (71.42% women, mean age = 50.64) and 80 healthy subjects (67.50% women, mean age = 49.12).The algorithm used was the support vector machine, performing cross validation, by which the subdivision of the sample in training (70%) and test (30%) was carried out in order to estimate the precision of the model. The prediction of belonging to the patient group (MDD patients versus control subjects), melancholic type (melancholic versus non-melancholic patients) or resistant depression group (treatment-resistant versus non -treatment-resistant) was based on the importance of each of the immunometabolic and lifestyle variables.Results: With the application of the algorithm, controls versus patients, such as patients with melancholic symptoms versus non-melancholic symptoms, and resistant versus non-resistant symptoms in the test phase were optimally classified.The variables that showed greater importance, according to the results of the area under the ROC curve, for the discrimination between healthy subjects and patients with depression were current alcohol consumption (AUC = 0.62), TNF-alpha levels (AUC = 0.61), glutathione redox status (AUC = 0.60) and the performance of both moderate (AUC = 0.59) and vigorous physical exercise (AUC = 0.58). On the other hand, the most important variables for classifying melancholic patients in relation to lifestyle habits were past (AUC = 0.65) and current (AUC = 0.60) tobacco habit, as well as walking routinely (AUC = 0.59) and in relation to immunometabolic markers were the levels of CRP (AUC = 0.62) and glucose (AUC = 0.58).In the analysis of the importance of the variables for the classification of treatment-resistant patients versus non-resistant patients, the systolic blood pressure (SBP) variable was shown to be the most relevant (AUC = 0.67). Other immunometabolic variables were also among the most important such as TNF-alpha (AUC = 0.65) and waist circumference (AUC = 0.64). In this case, sex (AUC = 0.59) was also relevant along with alcohol (AUC = 0.58) and tobacco (AUC = 0.56) consumption.Conclusions: The results obtained in our study show that it is possible to predict the diagnosis of depression and its clinical typology from immunometabolic markers and lifestyle habits, using machine learning techniques. The use of this type of methodology could facilitate the identification of patients at risk of presenting depression and could be very useful for managing clinical heterogeneity
