    Implementation of a dermatoscopy curriculum during residency at Augsburg University Hospital in Germany

    Background and objectives To date, there is no structured program for dermatoscopy training during residency in Germany. Whether and how much dermatoscopy training is acquired is left to the initiative of each resident, although dermatoscopy is one of the core competencies of dermatological training and daily practice. The aim of the study was to establish a structured dermatoscopy curriculum during residency at the University Hospital Augsburg. Patients and methods An online platform with dermatoscopy modules was created, accessible regardless of time and place. Practical skills were acquired under the personal guidance of a dermatoscopy expert. Participants were tested on their level of knowledge before and after completing the modules. Test scores on management decisions and correct dermatoscopic diagnosis were analyzed. Results Results of 28 participants showed improvements in management decisions from pre- to posttest (74.0% vs. 89.4%) and in dermatoscopic accuracy (65.0% vs. 85.6%). Pre- vs. posttest differences in test score (7.05/10 vs. 8.94/10 points) and correct diagnosis were significant (p < 0.001). Conclusions The dermatoscopy curriculum increases the number of correct management decisions and dermatoscopy diagnoses. This will result in more skin cancers being detected, and fewer benign lesions being excised. The curriculum can be offered to other dermatology training centers and medical professionals

    The 2017 Update of the German Clinical Guideline on Epidemiology, Diagnostics, Therapy, Prevention, and Management of Uncomplicated Urinary Tract Infections in Adult Patients: Part 1

    Objectives: We aimed to update the 2010 evidence- and consensus-based national clinical guideline on the diagnosis and management of uncomplicated urinary tract infections (UTIs) in adult patients. Materials and Methods: An interdisciplinary group consisting of 17 representatives of 12 medical societies and a patient representative was formed. Systematic literature searches were conducted in MEDLINE, ­EMBASE, and the Cochrane Library to identify literature published in 2010–2015. Results: We provide 75 recommendations and 68 statements in the updated evidence- and consensus-based national clinical guideline. The diagnostics part covers practical recommendations on cystitis and pyelonephritis for each defined patient group. Clinical examinations, as well as laboratory testing and microbiological pathogen assessment, are addressed. Conclusion: In accordance with the global antibiotic stewardship initiative and considering new insights in scientific research, we updated our German clinical UTI guideline to promote a responsible antibiotic use and to give clear hands-on recommendations for the diagnosis and management of UTIs in adults in Germany for healthcare providers and patients

    Background: We aimed to update the 2010 evidence- and consensus-based national clinical guideline on the diagnosis and management of uncomplicated urinary tract infections (UTIs) in adult patients. Results are published in 2 parts. Part 1 covers methods, the definition of patient groups, and diagnostics. This second publication focuses on treatment of acute episodes of cystitis and pyelonephritis as well as on prophylaxis of recurrent UTIs. Materials and Methods: An interdisciplinary group consisting of 17 representatives of 12 medical societies and a patient representative was formed. Systematic literature searches were conducted in MEDLINE, EMBASE, and the Cochrane Library to identify literature published in 2010–2015. Results: For the treatment of acute uncomplicated cystitis (AUC), fosfomycin-trometamol, nitrofurantoin, nitroxoline, pivmecillinam, and trimethoprim (depending on the local rate of resistance) are all equally recommended. Cotrimoxazole, fluoroquinolones, and cephalosporins are not recommended as antibiotics of first choice, for concern of an unfavorable impact on the microbiome. Mild to moderate uncomplicated pyelonephritis should be treated with oral cefpodoxime, ceftibuten, ciprofloxacin, or levofloxacin. For AUC with mild to moderate symptoms, instead of antibiotics symptomatic treatment alone may be considered depending on patient preference after discussing adverse events and outcomes. Primarily non-antibiotic options are recommended for prophylaxis of recurrent urinary tract infection.     The Influence of pCO2 and Temperature on Gene Expression of Carbon and Nitrogen Pathways in Trichodesmium IMS101

    Growth, protein amount, and activity levels of metabolic pathways in Trichodesmium are influenced by environmental changes such as elevated pCO2 and temperature. This study examines changes in the expression of essential metabolic genes in Trichodesmium grown under a matrix of pCO2 (400 and 900 µatm) and temperature (25 and 31°C). Using RT-qPCR, we studied 21 genes related to four metabolic functional groups: CO2 concentrating mechanism (bicA1, bicA2, ccmM, ccmK2, ccmK3, ndhF4, ndhD4, ndhL, chpX), energy metabolism (atpB, sod, prx, glcD), nitrogen metabolism (glnA, hetR, nifH), and inorganic carbon fixation and photosynthesis (rbcL, rca, psaB, psaC, psbA). nifH and most photosynthetic genes exhibited relatively high abundance and their expression was influenced by both environmental parameters. A two to three orders of magnitude increase was observed for glnA and hetR only when both pCO2 and temperature were elevated. CO2 concentrating mechanism genes were not affected by pCO2 and temperature and their expression levels were markedly lower than that of the nitrogen metabolism and photosynthetic genes. Many of the CO2 concentrating mechanism genes were co-expressed throughout the day. Our results demonstrate that in Trichodesmium, CO2 concentrating mechanism genes are constitutively expressed. Co-expression of genes from different functional groups were frequently observed during the first half of the photoperiod when oxygenic photosynthesis and N2 fixation take place, pointing at the tight and complex regulation of gene expression in Trichodesmium. Here we provide new data linking environmental changes of pCO2 and temperature to gene expression in Trichodesmium. Although gene expression indicates an active metabolic pathway, there is often an uncoupling between transcription and enzyme activity, such that transcript level cannot usually be directly extrapolated to metabolic activity

    Dissecting the Shared Genetic Architecture of Suicide Attempt, Psychiatric Disorders, and Known Risk Factors

    Background Suicide is a leading cause of death worldwide, and nonfatal suicide attempts, which occur far more frequently, are a major source of disability and social and economic burden. Both have substantial genetic etiology, which is partially shared and partially distinct from that of related psychiatric disorders. Methods We conducted a genome-wide association study (GWAS) of 29,782 suicide attempt (SA) cases and 519,961 controls in the International Suicide Genetics Consortium (ISGC). The GWAS of SA was conditioned on psychiatric disorders using GWAS summary statistics via multitrait-based conditional and joint analysis, to remove genetic effects on SA mediated by psychiatric disorders. We investigated the shared and divergent genetic architectures of SA, psychiatric disorders, and other known risk factors. Results Two loci reached genome-wide significance for SA: the major histocompatibility complex and an intergenic locus on chromosome 7, the latter of which remained associated with SA after conditioning on psychiatric disorders and replicated in an independent cohort from the Million Veteran Program. This locus has been implicated in risk-taking behavior, smoking, and insomnia. SA showed strong genetic correlation with psychiatric disorders, particularly major depression, and also with smoking, pain, risk-taking behavior, sleep disturbances, lower educational attainment, reproductive traits, lower socioeconomic status, and poorer general health. After conditioning on psychiatric disorders, the genetic correlations between SA and psychiatric disorders decreased, whereas those with nonpsychiatric traits remained largely unchanged. Conclusions Our results identify a risk locus that contributes more strongly to SA than other phenotypes and suggest a shared underlying biology between SA and known risk factors that is not mediated by psychiatric disorders.Peer reviewe

    Aufgrund des zunehmenden Einsatzes von Schnittbildgebungen werden immer mehr Nebennieren-Raumforderungen zufällig entdeckt. Häufig ist bei diesen Zufallsbefunden („Inzidentalome“) laborchemisch eine „autonome Cortisol-Sekretion“ auffällig, ohne dass jedoch klinische Zeichen eines Glukokortikoid-Exzesses sichtbar wären. Ein florides Cushing-Syndrom führt bekanntermaßen zu einer erhöhten kardiovaskulären Morbidität und Mortalität. Ziel unserer Untersuchung war es, die Prävalenz kardiovaskulärer Risikofaktoren und Ereignisse bei Patienten mit einem Nebennieren-Inzidentalom zu ermitteln. Hierbei wurden die Adenome hinsichtlich ihrer sekretorischen Aktivität in drei Subgruppen eingeteilt („nicht-funktionell“, „mögliche autonome Cortisol-Sekretion“ und „autonome Cortisol-Sekretion“) und getrennt voneinander betrachtet. Die vorliegende Einzelzenter-Studie umfasst einen Zeitraum von 20 Jahren und beinhaltet sowohl retro- als auch prospektive Elemente. Insgesamt konnten 260 Patienten mit einem Nachsorgeintervall von durchschnittlich fast 9 Jahren inkludiert werden. Die Raten von arterieller Hypertonie, Diabetes mellitus und Dyslipidämie stiegen mit zunehmender Cortisol-Sekretion an; dies war sowohl bei der Erstdiagnose als auch bei der letzten erfassten Nachsorge zu beobachten. Patienten mit einem nicht-funktionellen Adenom wiesen nach der Erstdiagnose eine signifikant geringere Inzidenz kardiovaskulärer Ereignisse auf als Patienten mit einer autonomen Cortisol-Sekretion. Mittels einer multivariaten Cox-Regression wurden die Höhe des Serumcortisols im Dexamethason-Suppressionstest und eine positive Eigenanamnese als signifikante Einflussfaktoren für das Auftreten kardiovaskulärer Ereignisse ermittelt. Eine klinisch unterschwellige, jedoch chronische Cortisol-Exposition im Sinne einer autonomen Cortisol-Sekretion erhöht demnach das Risiko der betroffenen Patienten für die Entwicklung von Herz-Kreislauf-Erkrankungen.Due to the increased use of cross-sectional imaging, more and more adrenal masses are discovered incidentally. Frequently, these incidental findings ("incidentalomas") reveal "autonomic cortisol secretion" by laboratory tests, but without clinical signs of glucocorticoid excess. Florid Cushing's syndrome is known to lead to increased cardiovascular morbidity and mortality. The aim of our study was to determine the prevalence of cardiovascular risk factors and events in patients with adrenal incidentaloma. For this purpose, the adenomas were divided into three subgroups ("nonfunctional", "possible autonomous cortisol secretion" and "autonomous cortisol secretion") with respect to their secretory activity and are considered separately. The present single-center study covers a 20-year period and includes both retrospective and prospective elements. A total of 260 patients with an average follow-up period of almost 9 years were included. Rates of arterial hypertension, diabetes mellitus, and dyslipidemia increased with an elevated cortisol secretion; this was detected both at initial diagnosis and at the last follow-up. Patients with nonfunctional adenoma had a significantly lower incidence of cardiovascular events after initial diagnosis than patients with autonomic cortisol secretion. Using multivariate Cox regression, the level of serum cortisol in the dexamethasone suppression test and a positive self-reported history of cardiovascular events were found to be significant factors influencing the occurrence of cardiovascular events. Subclinical but chronic cortisol exposure in the sense of autonomic cortisol secretion thus increases the risk of affected patients for the development of cardiovascular diseases

    Riparian vegetation provides crucial shelter for resting otters in a human-dominated landscape

    The riparian vegetation belt is one of the few remaining structures that provide coverage for wildlife in many anthropogenic landscapes. It provides shelter for many species and functions as corridors for dispersal. However, this landscape is increasingly utilised by humans for leisure activities. The loss of riparian vegetation with a concurrent increase of human disturbance in these habitats can pose a serious threat to wildlife. One of the species potentially affected is the Eurasian otter Lutra lutra. In the Alps, otters are nocturnal and rest during the day when human activity is high. To study the impact of human presence on resting site selection in otters, we radio-tracked nine otters for up to 30 months in the eastern Central Alps of Austria. We analysed resting site selection in relation to human disturbance. Altogether, we identified 285 resting sites scattered throughout the territories of the individual otters at an average distance of 144 m between sites. Almost all the resting sites (95%) were located in the natural riparian vegetation, making this the most important habitat for resting otters. We found evidence that human disturbance within these riparian habitats shapes resting site selection. Otters preferred narrow riparian strips when there was no human presence, but selected areas with a wider vegetation belt when the disturbance level increased. Outside the vegetation period, animals rested below ground more often than above, which suggests that vegetation also functions as visual cover. Our study highlights the importance of the restoration of the natural riparian vegetation belt for the protection of wildlife, particularly where human activities are intense. We provide information for the effective conservation management of otters considering the combination of spatial distribution of resting sites and specific habitat requirements in the presence of human disturbance. Areas of conservation importance chosen in this way are likely to benefit other wildlife that utilise the riparian vegetation belt
