Discontinuation of standard first-line antiretroviral therapy in a cohort of 1434 Malawian children
The standard first-line antiretroviral (ART) regimen in Malawi for both adults and children is a fixed-dose combination tablet containing stavudine (d4T), lamivudine (3TC) and nevirapine (NVP). This regimen has been shown to yield satisfactory virologic and immunologic outcomes in children. Published studies have described insights into discontinuation of first-line regimen and toxicities of ART in adults, but similar studies in paediatric populations are lacking
Inflammation and changes in cytokine levels in neurological feline infectious peritonitis.
Feline infectious peritonitis (FIP) is a progressive, fatal, predominantly Arthus-type immune-mediated disease that is triggered when cats are infected with a mutant enteric coronavirus. The disease presents variably with multiple organ failure, seizures, generalized effusion, or shock. Neurological FIP is clinically and pathologically more homogeneous than systemic 'wet' or 'dry' FIP; thus, comparison of cytokine profiles from cats with neurological FIP, wet FIP, and non-FIP neurological disease may provide insight into some baseline characteristics relating to the immunopathogenesis of neurological FIP. This study characterizes inflammation and changes in cytokines in the brain tissue of FIP-affected cats. Cellular infiltrates in cats with FIP included lymphocytes, plasma cells, neutrophils, macrophages, and eosinophils. IL-1 beta, IL-6, IL-12, IL-18, TNF-alpha, macrophage inhibitory protein (MIP)-1 alpha, and RANTES showed no upregulation in the brains of control cats, moderate upregulation in neurological FIP cats, and very high upregulation in generalized FIP cats. Transcription of IFN-gamma appeared upregulated in cats with systemic FIP and slightly downregulated in neurological FIP. In most cytokines tested, variance was extremely high in generalized FIP and much less in neurological FIP. Principal components analysis was performed in order to find the least number of 'components' that would summarize the cytokine profiles in cats with neurological FIP. A large component of the variance (91.7%) was accounted for by levels of IL-6, MIP-1 alpha, and RANTES. These findings provide new insight into the immunopathogenesis of FIP and suggest targets for immune therapy of this disease
Gravitational Repulsion within a Black-Hole using the Stueckelberg Quantum Formalism
We wish to study an application of Stueckelberg's relativistic quantum theory
in the framework of general relativity. We study the form of the wave equation
of a massive body in the presence of a Schwarzschild gravitational field. We
treat the mathematical behavior of the wavefunction also around and beyond the
horizon (r=2M). Classically, within the horizon, the time component of the
metric becomes spacelike and distance from the origin singularity becomes
timelike, suggesting an inevitable propagation of all matter within the horizon
to a total collapse at r=0. However, the quantum description of the wave
function provides a different understanding of the behavior of matter within
the horizon. We find that a test particle can almost never be found at the
origin and is more probable to be found at the horizon. Matter outside the
horizon has a very small wave length and therefore interference effects can be
found only on a very small atomic scale. However, within the horizon, matter
becomes totally "tachionic" and is potentially "spread" over all space. Small
location uncertainties on the atomic scale become large around the horizon, and
different mass components of the wave function can therefore interfere on a
stellar scale. This interference phenomenon, where the probability of finding
matter decreases as a function of the distance from the horizon, appears as an

Pulmonary Dysfunction in Patients with Femoral Shaft Fracture Treated with Intramedullary Nailing
Background: This study was undertaken to determine whether alveolar dead space increases during intramedullary nailing of femoral shaft fractures and whether alveolar dead space predicts postoperative pulmonary dysfunction in patients undergoing intramedullary nailing of a femoral shaft fracture.
Methods: All patients with a femoral shaft fracture were prospectively enrolled in the study unless there was evidence of acute myocardial infarction, shock, or heart failure. Arterial blood gases were measured at three consecutive time-periods after induction of general anesthesia: before intramedullary nailing and ten and thirty minutes after intramedullary nailing. The end-tidal carbon-dioxide level, minute ventilation, positive end‐expiratory pressure, and percent of inspired and expired inhalation agent were recorded simultaneously with the blood-gas measurement. Postoperatively, all subjects were monitored for evidence of pulmonary dysfunction, defined as the need for mechanical ventilation or supplemental oxygen (at a fraction of inspired oxygen of >40%) in the presence of clinical signs of a respiratory rate of >20 breaths/min or the use of accessory muscles of respiration.
Results: Seventy‐four patients with a total of eighty femoral shaft fractures completed the study. Fifty fractures (62.5%) underwent nailing after reaming, and thirty fractures (37.5%) underwent nailing with minimal or no reaming. The mean alveolar dead-space measurements before canal opening and at ten and thirty minutes after canal opening were 14.5%, 15.8%, and 15.2% in the total series of seventy‐four patients (general linear model, p = 0.2) and 20.5%, 22.7%, and 24.2% in the twenty patients with postoperative pulmonary dysfunction (general linear model, p = 0.05). Of the twenty‐one patients with an alveolar dead-space measurement of >20% thirty minutes after nailing, sixteen had postoperative pulmonary dysfunction. According to univariate and multivariate analysis, the alveolar dead-space measurement was strongly associated with postoperative pulmonary dysfunction.
Conclusions: According to our data, intramedullary nailing of femoral shaft fractures did not significantly increase alveolar dead space, and the amount of alveolar dead space can predict which patients will have pulmonary dysfunction postoperatively
Irritability and parenting practices as mediational variables between temperament and affective, anxiety, and oppositional defiant problems
Irritability and parenting are potential targets for transdiagnostic studies to identify the common and core dysfunctional characteristics underlying several diagnostic pictures with the goal of addressing these issues in treatment. Our objective was to investigate the different paths from temperament to child psychopathology (affective, anxiety, and oppositional problems) through irritability and parenting using a prospective design from ages 3 to 7. A sample of 614 3-year-old preschoolers was followed at ages 4, 6, and 7. Parents answered questionnaires about temperament (age 3), irritability (age 4), parenting practices (age 6), and psychopathology (age 7). Statistical analyses were carried out through structural equation modeling (SEM) to test the mediation effect of irritability and parenting practices from temperament (negative affectivity and effortful control) through to affective, anxious, and oppositional problems. The proposed model fit the data well. SEM showed (a) an indirect effect from temperament to affective problems, via irritability and positive parenting; (b) a direct effect from negative affectivity to anxiety, plus an indirect effect from both temperament dimensions, via irritability and autonomy parenting practices; and (c) an indirect effect from temperament to oppositional problems, via irritability and punitive parenting. Irritability and parenting are transdiagnostic mediational variables that should be focused on in intervention programs for affective, anxiety, and oppositional problem
Diffractive effects in spin-flip pp amplitudes and predictions for relativistic energies
We analyze the diffractive (Pomeron) contribution to pp spin-flip amplitude
and discuss the possible scenarios for energies available at the Relativistic
Heavy-Ion Collider (RHIC). In particular, we show that RHIC data will be
instrumental in assessing the real contribution of diffraction to spin

How much laser power can propagate through fusion plasma?
Propagation of intense laser beams is crucial for inertial confinement
fusion, which requires precise beam control to achieve the compression and
heating necessary to ignite the fusion reaction. The National Ignition Facility
(NIF), where fusion will be attempted, is now under construction. Control of
intense beam propagation may be ruined by laser beam self-focusing. We have
identified the maximum laser beam power that can propagate through fusion
plasma without significant self-focusing and have found excellent agreement
with recent experimental data, and suggest a way to increase that maximum by
appropriate choice of plasma composition with implication for NIF designs. Our
theory also leads to the prediction of anti-correlation between beam spray and
backscatter and suggests the indirect control of backscatter through
