    Prospective parents’ perspectives on antenatal decision making for the anticipated birth of a periviable infant

    Objective: To examine prospective parents’ perceptions of management options and outcomes in the context of threatened periviable delivery, and the values they apply in making antenatal decisions during this period. Study design: Qualitative analysis of 46 antenatal interviews conducted at three tertiary-care hospitals with 54 prospective parents (40 pregnant women, 14 partners) who had received counseling for threatened periviable delivery (40 cases). Results: Participants most often recalled being involved in resuscitation, cerclage, and delivery mode decisions. Over half (63.0%) desired a shared decision-making role. Most (85.2%) recalled hearing about morbidity and mortality, with many reiterating terms like “brain damage”, “disability”, and “handicap”. The potential for disability influenced decision making to variable degrees. In describing what mattered most, participant spoke of giving their child a “fighting chance”; others voiced concerns about “best interest”, a “healthy baby”, “pain and suffering”, and religious faith. Conclusions: Our findings underscore the importance of presenting clear information on disability and eliciting the factors that parents deem most important in making decisions about periviable birth

    Adultos diante do bullying escolar na série Thirteen reasons why: uma análise bioecológica

    School bullying is a serious public health problem and television drama series can contribute to shedding more light on its numerous facets. Hence, this study sought to analyze the discourses and practices of adults when they learn about or witness bullying situations in the television series Thirteen Reasons Why. This exploratory study employed a qualitative approach based on Bronfenbrenner’s Bioecological Theory. The methodological procedures comprised analyzing and selecting the episodes, identifying the scenes that contained bullying, classifying the adults’ perspectives and actions, and data analysis. We found an insufficient understanding of the adults in situations of bullying that were witnessed or known. Coercive behaviors, punishments, and transference of responsibility between contexts were the most identified actions. This study contributes to raising awareness among managers and civil society about the role of adults in managing and preventing cases of bullying in schools.El acoso escolar es un grave problema de salud pública y las series dramáticas pueden ayudar a iluminar sus múltiples facetas. El objetivo de este estudio analizar los discursos y las prácticas de los adultos que conocen situaciones de acoso escolar en la serie Thirteen reasons why. Se trata de un estudio exploratorio y cualitativo fundamentado en la teoría bioecológica de Bronfenbrenner. Los procedimientos metodológicos fueron: análisis y selección de episodios; identificación de las escenas que contienen acoso; clasificación de las perspectivas y acciones de los adultos; análisis de los datos. Se verificó una insuficiente comprensión de los adultos frente a situaciones de acoso presenciadas o conocidas. Las conductas de coerción y castigo y la transferencia de responsabilidad entre los contextos que se presentan en la serie fueron las acciones más identificadas. El estudio contribuye a sensibilizar a los directivos y a la sociedad civil sobre el papel de los adultos en la gestión y prevención de los casos de acoso escolar.Bullying escolar é um grave problema de saúde pública e séries dramáticas podem contribuir para iluminar suas diversas facetas. Este estudo teve como objetivo analisar os discursos e práticas de adultos ao tomarem conhecimento ou testemunharem situações de bullying na série Thirteen reasons why. Trata-se de estudo exploratório de abordagem qualitativa fundamentado na Teoria Bioecológica de Bronfenbrenner. Os procedimentos metodológicos foram: análise e seleção de episódios; identificação de cenas que continham bullying; classificação das perspectivas e ações dos adultos; análise dos dados. Notou-se uma compreensão insuficiente dos adultos frente às situações de bullying presenciadas ou conhecidas. Comportamentos de coerção, punições e transferência de responsabilidade entre os contextos foram as ações mais identificadas.     Nucleoplasmic lamin C rapidly accumulates at sites of nuclear envelope rupture with BAF and cGAS

    In mammalian cell nuclei, the nuclear lamina (NL) underlies the nuclear envelope (NE) to maintain nuclear structure. The nuclear lamins, the major structural components of the NL, are involved in the protection against NE rupture induced by mechanical stress. However, the specific role of the lamins in repair of NE ruptures has not been fully determined. Our analyses using immunofluorescence and live-cell imaging revealed that the nucleoplasmic pool of lamin C rapidly accumulated at sites of NE rupture induced by laser microirradiation in mouse embryonic fibroblasts. The accumulation of lamin C at the rupture sites required both the immunoglobulin-like fold domain that binds to barrier-to-autointegration factor (BAF) and a nuclear localization signal. The accumulation of nuclear BAF and cytoplasmic cyclic GMP-AMP synthase (cGAS) at the rupture sites was in part dependent on lamin A/C. These results suggest that nucleoplasmic lamin C, BAF, and cGAS concertedly accumulate at sites of NE rupture for rapid repair

    Pulmonary Hypertension and Other Potentially Fatal Pulmonary Complications in Systemic Juvenile Idiopathic Arthritis

    Objective Systemic juvenile idiopathic arthritis (JIA) is characterized by fevers, rash, and arthritis, for which interleukin‐1 (IL‐1) and IL‐6 inhibitors appear to be effective treatments. Pulmonary arterial hypertension (PAH), interstitial lung disease (ILD), and alveolar proteinosis (AP) have recently been reported with increased frequency in systemic JIA patients. Our aim was to characterize and compare systemic JIA patients with these complications to a larger cohort of systemic JIA patients. Methods Systemic JIA patients who developed PAH, ILD, and/or AP were identified through an electronic Listserv and their demographic, systemic JIA, and pulmonary disease characteristics as well as their medication exposure information were collected. Patients with these features were compared to a cohort of systemic JIA patients enrolled in the Childhood Arthritis and Rheumatology Research Alliance (CARRA) registry. Results The patients (n = 25) were significantly ( P < 0.05) more likely than the CARRA registry cohort (n = 389) to be female; have more systemic features; and have been exposed to an IL‐1 inhibitor, tocilizumab, corticosteroids, intravenous immunoglobulin, cyclosporine, and cyclophosphamide. Twenty patients (80%) were diagnosed with pulmonary disease after 2004. Twenty patients (80%) had macrophage activation syndrome (MAS) during their disease course and 15 patients (60%) had MAS at pulmonary diagnosis. Sixteen patients had PAH, 5 had AP, and 7 had ILD. Seventeen patients (68%) were taking or recently discontinued (<1 month) a biologic agent at pulmonary symptom onset; 12 patients (48%) were taking anti–IL‐1 therapy (primarily anakinra). Seventeen patients (68%) died at a mean of 10.2 months from the diagnosis of pulmonary complications. Conclusion PAH, AP, and ILD are underrecognized complications of systemic JIA that are frequently fatal.     Divergent Views of Hope Influencing Communications Between Parents and Hospital Providers

    This study evaluates parents’ and health care providers’ (HCPs) descriptions of hope following counseling of parents at risk of delivering an extremely premature infant. Data came from a longitudinal multiple case study investigation that examined the decision-making and support needs of 40 families and their providers. Semi-structured interviews were conducted before and after delivery. Divergent viewpoints of hope were found between parents and many HCPs and were subsequently coded using content analysis. Parents relied on hope as an emotional motivator, whereas most HCPs described parents’ notions of hope as out of touch with reality. Parents perceived that such divergent beliefs about the role of hope negatively shaped communicative interactions and reduced trust with some of their providers. A deeper understanding of how varying views of hope might shape communications will uncover future research questions and lead to theory-based interventions aimed at improving the process of discussing difficult news with parents
