Usefulness of an Intrapartum Ultrasound Simulator (IUSim™) for Midwife Training: Results from an RCT
Introduction: We conducted a randomized study to determine whether a training session on a dedicated simulator (IUSim™) would facilitate the midwives in learning the technique of transperineal intrapartum ultrasound. Methods: Following a 30-min multimedia presentation including images and videos on how to obtain and measure the angle of progression (AoP) and the head-perineum distance (HPD), 6 midwives with no prior experience in intrapartum ultrasound were randomly split into 2 groups: 3 of them were assigned to the "training group"and 3 to the "control group."The midwives belonging to the former group were taught to measure the 2 sonographic parameters during a 3-h practical session conducted on IUSim™ under the supervision of an expert obstetrician. In the following 3 months, all the 6 midwives were asked to independently perform transperineal ultrasound during their clinical practice and to measure on the acquired images either the AoP or the HPD. The sonographic images were examined in blind by the teaching obstetrician who assigned a 0-3 score to the image quality (IQS) and to the measurement quality (MQS). Results: A total of 48 ultrasound images (24 patients) from 5 midwives were acquired and included in the study analysis. A midwife of the "training group"declined participation after the practical session. Independently from the randomization group, the image quality score (IQS + MQS) was significantly higher for the HPD compared with the AoP (2.5 ± 0.66 vs. 1.79 ± 1.14; p = 0.01). In the training group, the MQS of either AoP (2.66 ± 0.5 vs.1.46 ± 1.45. p = 0.038) and the HPD (2.9 ± 0.33 vs. 1.87 ± 0.83 p = 0.002) was significantly higher in comparison with the control group, while the IQS of both measurements was comparable between the 2 groups (1.91 ± 1.24 vs. 2.25 ± 0.865; p = 0.28). Conclusion: The use of a dedicated simulator may facilitate the midwives in learning how to measure the AoP and the HPD on transperineal ultrasound images
Search for new resonant states in 10C and 11C and their impact on the cosmological lithium problem
The observed primordial 7Li abundance in metal-poor halo stars is found to be
lower than its Big-Bang nucleosynthesis (BBN) calculated value by a factor of
approximately three. Some recent works suggested the possibility that this
discrepancy originates from missing resonant reactions which would destroy the
7Be, parent of 7Li. The most promising candidate resonances which were found
include a possibly missed 1- or 2- narrow state around 15 MeV in the compound
nucleus 10C formed by 7Be+3He and a state close to 7.8 MeV in the compound
nucleus 11C formed by 7Be+4He. In this work, we studied the high excitation
energy region of 10C and the low excitation energy region in 11C via the
reactions 10B(3He,t)10C and 11B(3He,t)11C, respectively, at the incident energy
of 35 MeV. Our results for 10C do not support 7Be+3He as a possible solution
for the 7Li problem. Concerning 11C results, the data show no new resonances in
the excitation energy region of interest and this excludes 7Be+4He reaction
channel as an explanation for the 7Li deficit.Comment: Accepted for publication in Phys. Rev. C (Rapid Communication
Vaccine hesitancy and HPV vaccine uptake among male and female youth in Switzerland: a cross-sectional study
OBJECTIVES: Identifying factors associated with human papillomavirus (HPV) vaccine uptake is essential for designing successful vaccination programmes. We aimed to examine the association between vaccine hesitancy (VH) and HPV vaccine uptake among male and female youth in Switzerland. DESIGN: With a cross-sectional study, an interview-based questionnaire was used to collect information on sociodemographic factors, vaccination records and to measure the prevalence of VH using the Youth Attitudes about Vaccines scale (YAV-5), a modified version of the Parent Attitudes about Childhood Vaccinations survey instrument. SETTING AND PARTICIPANTS: Eligible male and female participants, 15-26 years of age, were recruited through physicians' offices and military enlistment in all three language regions of Switzerland. Of 1001 participants, we included 674 participants with a vaccination record available (415 males and 259 females) in this study. PRIMARY AND SECONDARY OUTCOME MEASURES: The outcome was uptake for HPV vaccine (having received >/=1 dose of HPV vaccine). Covariates were VH, sex, age and other sociodemographics. RESULTS: 151 (58%) female and 64 (15%) male participants received >/=1 dose of HPV vaccine. 81 (31%) female and 92 (22%) male participants were VH (YAV-5-Score >50). The odds for being unvaccinated were higher for VH women than non-VH women, adjusted OR=4.90 (95% CI 2.53 to 9.50), but similar among VH and non-VH men, OR=1.90 (95% CI 0.84 to 4.31). The odds for being unvaccinated were lower for younger men (born on or after 1 July 2002) than older men (born before 1 July 2002), OR=0.34 (95% CI 0.14 to 0.81), but we found no association between age and vaccine uptake for female youth, OR=0.97 (95% CI 0.48 to 1.97). CONCLUSIONS: VH was associated with lower HPV vaccine uptake in female youth but not male youth in our study population in Switzerland. Our findings suggest that issues other than VH contribute to HPV underimmunisation in male youth in Switzerland
Dual role of immune cells in the testis: protective or pathogenic for germ cells?
The purpose of this review is to describe how the immune cells present in the testis interact with the germinal epithelium contributing to survival or apoptosis of germ cells (GCs). Physiologically, the immunosuppressor testicular microenvironment protects GCs from immune attack, whereas in inflammatory conditions, tolerance is disrupted and immune cells and their mediators respond to GC self antigens, inducing damage of the germinal epithelium. Considering that experimental models of autoimmune orchitis have clarified the local immune mechanisms by which protection of the testis is compromised, we described the following topics in the testis of normal and orchitic rats: (1) cell adhesion molecule expression of seminiferous tubule specialized junctions and modulation of blood-testis barrier permeability by cytokines (2) phenotypic and functional characteristics of testicular dendritic cells, macrophages, effector and regulatory T cells and mast cells and (3) effects of pro-inflammatory cytokines (TNF-α, IL-6 and FasL) and the nitric oxide-nitric oxide synthase system on GC apoptosis.Fil: Pérez, Cecilia Valeria. Probing Nuclear forces beyond the drip-line using the mirror nuclei N and F
Radioactive beams of O and O were used to populate the resonant
states 1/2, 5/2 and in the unbound F and F
nuclei respectively by means of proton elastic scattering reactions in inverse
kinematics. Based on their large proton spectroscopic factor values, the
resonant states in F can be viewed as a core of O plus a proton
in the 2s or 1d shell and a neutron in 1p. Experimental
energies were used to derive the strength of the 2s-1p and
1d-1p proton-neutron interactions. It is found that the former
changes by 40% compared with the mirror nucleus N, and the second by
10%. This apparent symmetry breaking of the nuclear force between mirror nuclei
finds explanation in the role of the large coupling to the continuum for the
states built on an proton configuration.Comment: 6 pages, 3 figures, 2 tables, accepted for publication as a regular

New pathway to bypass the 15O waiting point
We propose the sequential reaction process
O(,)O as a new pathway to bypass of the
O waiting point. This exotic reaction is found to have a surprisingly
high cross section, approximately 10 times higher than the
O(,)O. These cross sections were calculated after
precise measurements of energies and widths of the proton-unbound F low
lying states, obtained using the H(O,p)O reaction. The large
cross section can be understood to arise from the more
efficient feeding of the low energy wing of the ground state resonance by the
gamma decay. The implications of the new reaction in novae explosions and X-ray
bursts are discussed.Comment: submitte
Spectroscopy of Na: Bridging the two-proton radioactivity of Mg
The unbound nucleus Na, the intermediate nucleus in the two-proton
radioactivity of Mg, was studied by the measurement of the resonant
elastic scattering reaction Ne(p,Ne)p performed at 4 A.MeV.
Spectroscopic properties of the low-lying states were obtained in a R-matrix
analysis of the excitation function. Using these new results, we show that the
lifetime of the Mg radioactivity can be understood assuming a sequential
emission of two protons via low energy tails of Na resonances
Comparison of low--energy resonances in 15N(alpha,gamma)19F and 15O(alpha,gamma)19Ne and related uncertainties
A disagreement between two determinations of Gamma_alpha of the astro-
physically relevant level at E_x=4.378 MeV in 19F has been stated in two recent
papers by Wilmes et al. and de Oliveira et al. In this work the uncertainties
of both papers are discussed in detail, and we adopt the value
Gamma_alpha=(1.5^{+1.5}_{-0.8})10^-9eV for the 4.378 MeV state. In addition,
the validity and the uncertainties of the usual approximations for mirror
nuclei Gamma_gamma(19F) approx Gamma_gamma(19Ne), theta^2_alpha(19F) approx
theta^2_alpha(19Ne) are discussed, together with the resulting uncertainties on
the resonance strengths in 19Ne and on the 15O(alpha,gamma)19Ne rate.Comment: 9 pages, Latex, To appear in Phys. Rev.
Low energy measurement of the 7Be(p,gamma)8B cross section
We have measured the cross section of the 7Be(p,gamma)8B reaction for E_cm =
185.8 keV, 134.7 keV and 111.7 keV using a radioactive 7Be target (132 mCi).
Single and coincidence spectra of beta^+ and alpha particles from 8B and 8Be^*
decay, respectively, were measured using a large acceptance spectrometer. The
zero energy S factor inferred from these data is 18.5 +/- 2.4 eV b and a
weighted mean value of 18.8 +/- 1.7 eV b (theoretical uncertainty included) is
deduced when combining this value with our previous results at higher energies.Comment: Accepted for publication in Phys. Rev. Let
