Implications for registry-based vaccine effectiveness studies from an evaluation of an immunization registry: A cross-sectional study
<p>Abstract</p> <p>Background</p> <p>Population-based electronic immunization registries create the possibility of using registry data to conduct vaccine effectiveness studies which could have methodological advantages over traditional observational studies. For study validity, the base population would have to be clearly defined and the immunization status of members of the population accurately recorded in the registry. We evaluated a city-wide immunization registry, focusing on its potential as a tool to study pertussis vaccine effectiveness, especially in adolescents.</p> <p>Methods</p> <p>We conducted two evaluations – one in sites that were active registry participants and one in sites that had implemented an electronic medical record with plans for future direct data transfer to the registry – of the ability to match patients' medical records to registry records and the accuracy of immunization records in the registry. For each site, records from current pediatric patients were chosen randomly. Data regarding pertussis-related immunizations, clinic usage, and demographic and identifying information were recorded; for 11–17-year-old subjects, information on MMR, hepatitis B, and varicella immunizations was also collected. Records were then matched, when possible, to registry records. For records with a registry match, immunization data were compared.</p> <p>Results</p> <p>Among 350 subjects from sites that were current registry users, 307 (87.7%) matched a registry record. Discrepancies in pertussis-related data were common for up-to-date status (22.6%), number of immunizations (34.7%), dates (10.2%), and formulation (34.4%). Among 442 subjects from sites that planned direct electronic transfer of immunization data to the registry, 393 (88.9%) would have matched a registry record; discrepancies occurred frequently in number of immunizations (11.9%), formulation (29.1%), manufacturer (94.4%), and lot number (95.1%.) Inability to match and immunization discrepancies were both more common in subjects who were older at their first visit to the provider site. For 11–17-year-old subjects, discrepancies were also common for MMR, hepatitis B, and varicella vaccination data.</p> <p>Conclusion</p> <p>Provider records frequently could not be matched to registry records or had discrepancies in key immunization data. These issues were more common for older children and were present even with electronic data transfer. These results highlight general challenges that may face investigators wishing to use registry-based immunization data for vaccine effectiveness studies, especially in adolescents.</p
DigitalCrust - a 4D Data System of Material Properties for Transforming Research on Crustal Fluid Flow
Fluid circulation in the Earth\u27s crust plays an essential role in surface, near surface, and deep crustal processes. Flow pathways are driven by hydraulic gradients but controlled by material permeability, which varies over many orders of magnitude and changes over time. Although millions of measurements of crustal properties have been made, including geophysical imaging and borehole tests, this vast amount of data and information has not been integrated into a comprehensive knowledge system. A community data infrastructure is needed to improve data access, enable large‐scale synthetic analyses, and support representations of the subsurface in Earth system models. Here, we describe the motivation, vision, challenges, and an action plan for a community‐governed, four‐dimensional data system of the Earth\u27s crustal structure, composition, and material properties from the surface down to the brittle–ductile transition. Such a system must not only be sufficiently flexible to support inquiries in many different domains of Earth science, but it must also be focused on characterizing the physical crustal properties of permeability and porosity, which have not yet been synthesized at a large scale. The DigitalCrust is envisioned as an interactive virtual exploration laboratory where models can be calibrated with empirical data and alternative hypotheses can be tested at a range of spatial scales. It must also support a community process for compiling and harmonizing models into regional syntheses of crustal properties. Sustained peer review from multiple disciplines will allow constant refinement in the ability of the system to inform science questions and societal challenges and to function as a dynamic library of our knowledge of Earth\u27s crust
Child abuse and neglect: Training needs of student teachers
During the exercise of their activity, education professionals are well placed to make an early detection of child abuse and to identify possible risk cases. However, sometimes, teachers and educators in general claim lack of knowledge and training to perform these tasks. That is why, in this work we wish to insist on the necessity of analysing the training of future education professionals in child abuse, both within and outside the family, and regardless of whether it is caused by adults or other children. It should be noted that early identification of violent behaviour and, obviously, the implementation of strong prevention strategies presuppose good training in this sense. For this reason, we have performed a pilot study to analyse the formation that students of the degree in Pedagogy have on child abuse, using a questionnaire that we have designed specifically for such purpose. We carried out the pilot study of the instrument with a sample of 24 students (girls and boys) enrolled in the 4th year of the degree in Pedagogy. Among the conclusions reached we highlight that, after analysing the data derived from the pilot study, it can be concluded that future pedagogues consider this specific training necessary, because at present they consider it insufficient and too limited to cope with their professional responsibilities in the future.Los profesionales de la educación se encuentran, en su ejercicio profesional, en una posición privilegiada para realizar una detección precoz del maltrato infantil y para identificar posibles casos de riesgo. Sin embargo, en ocasiones, maestros y educadores en general aducen falta de conocimiento y formación para realizar dichas tareas. Es por ello que, en este trabajo deseamos insistir en la necesidad de analizar la formación de los futuros profesionales de la educación en torno al maltrato infantil, tanto en el seno de la familia como fuera de ella, y ya sea ejercido por un adulto o por otros menores. No olvidemos que la identificación temprana de comportamientos violentos y, por supuesto, la puesta en marcha de estrategias sólidas para su prevención requieren disponer de una buena capacitación. Por esta razón, hemos realizado un estudio piloto que nos permitiera conocer la formación que los estudiantes del Grado de Pedagogía tienen sobre el maltrato infantil, utilizando un cuestionario que hemos diseñado específicamente para alcanzar tal propósito. En la realización de un estudio piloto contamos con una muestra de 24 alumnos y alumnas del 4º curso del Grado de Pedagogía. Entre las conclusiones alcanzadas destacamos que, tras analizar los datos derivados del pase piloto, podemos concluir que los futuros pedagogos consideran necesario tener formación específica al respecto, una preparación que, mayoritariamente, consideran insuficiente y muy limitada para poder afrontar sus responsabilidades profesionales en el futuro
