    Why latrines are not used : communities' perceptions and practices regarding latrines in a Taenia solium endemic rural area in Eastern Zambia

    Taenia solium cysticercosis is a neglected parasitic zoonosis occurring in many developing countries. Socio-cultural determinants related to its control remain unclear. Studies in Africa have shown that the underuse of sanitary facilities and the widespread occurrence of free-roaming pigs are the major risk factors for porcine cysticercosis. The study objective was to assess the communities' perceptions, practices and knowledge regarding latrines in a T. solium endemic rural area in Eastern Zambia inhabited by the Nsenga ethno-linguistic group, and to identify possible barriers to their construction and use. A total of 21 focus group discussions on latrine use were organized separately with men, women and children, in seven villages of the Petauke district. The themes covered were related to perceived latrine availability (absence-presence, building obstacles) and perceived latrine use (defecation practices, latrine management, socio-cultural constraints). The findings reveal that latrines were not constructed in every household because of the convenient use of existing latrines in the neighborhood. Latrines were perceived to contribute to good hygiene mainly because they prevent pigs from eating human feces. Men expressed reluctance to abandon the open-air defecation practice mainly because of toilet-associated taboos with in-laws and grown-up children of the opposite gender. When reviewing conceptual frameworks of people's approach to sanitation, we found that seeking privacy and taboos hindering latrine use and construction were mainly explained in our study area by the fact that the Nsenga observe a traditionally matrilineal descent. These findings indicate that in this local context latrine promotion messages should not only focus on health benefits in general. Since only men were responsible for building latrines and mostly men preferred open defecation, sanitation programs should also be directed to men and address related sanitary taboos in order to be effective

    This paper describes the optimization of the optics and the collimator geometry for the momentum cleaning insertion of the LHC. To collimate the off-momentum secondary halo without disturbing the circ ulating beam, the normalized dispersion in IR3 is made as large as in the arcs. The jaw locations and orientations are numerically optimized to reduce the momentum-dependent halo amplitude.     Ecology of the Ocean Sunfish, Mola mola, in the southern California Current System

    The common ocean sunfish, Mola mola, occupies a unique position in the eastern Pacific Ocean and the California Current Large Marine Ecosystem (CCLME) as the world\u27s heaviest, most fecund bony fish, and one of the most abundant gelativores. M. mola frequently occur as bycatch in fisheries worldwide and comprise the greatest portion of the bycatch in California\u27s large-mesh drift gillnet fishery. In this first long-term tagging study of any ocean sunfish species in the eastern Pacific, 15 M. mola (99 cm to 200 cm total length) were tagged in the southern California Bight (SCB) between 2003 and 2010 using 14 satellite pop-off archival tags (PATs) and one Fastloc Mk10 GPS tag. Ten tags provided positional data for a cumulative dataset of 349 tracking days during the months of july through March. Thirteen tags provided temperature and depth data. All M. mola remained within similar to 300 km of the coast, and nearly all exhibited seasonal movement between the SCB and adjacent waters off northern and central Baja California, Mexico. No tagged individuals were tracked north of the SCB. Tag depth data showed diel vertical migration and occasional deep (\u3e500 m) dives. Data from the Fastloc GPS tag allowed close examination of the relationship between the movements of the largest tagged ocean sunfish (2 m TL) and fine-scale oceanographic features. Near-instantaneous satellite sea surface temperature images showed this individual associated with upwelling fronts along its migration path, which exceeded 800 km and ranged from 6 to 128 km from the coast. Tag depth data showed active use of the water column within the frontal zones. Synthetic aperture radar (SAR) images demonstrated that surface slicks, which often indicate convergent circulation, coincided with this type of front. Zooplankton tows in the southern region of tracking off central Baja California, Mexico revealed dense populations of salps toward the warm side of these fronts. Satellite tag and ecosystem data suggest that bio-physical interactions in coastal upwelling fronts create favorable foraging habitat. (C) 2015 The Authors. Published by Elsevier B.V

    Physical Activity Characteristics across GOLD Quadrants Depend on the Questionnaire Used

    BACKGROUND:The GOLD multidimensional classification of COPD severity combines the exacerbation risk with the symptom experience, for which 3 different questionnaires are permitted. This study investigated differences in physical activity (PA) in the different GOLD quadrants and patient's distribution in relation to the questionnaire used. METHODS:136 COPD patients (58±21% FEV1 predicted, 34F/102M) completed COPD assessment test (CAT), clinical COPD questionnaire (CCQ) and modified Medical Research Council (mMRC) questionnaire. Exacerbation history, spirometry and 6MWD were collected. PA was objectively measured for 2 periods of 1 week, 6 months apart, in 5 European centres; to minimise seasonal and clinical variation the average of these two periods was used for analysis. RESULTS:GOLD quadrants C+D had reduced PA compared with A+B (3824 [2976] vs. 5508 [4671] steps.d-1, p<0.0001). The choice of questionnaire yielded different patient distributions (agreement mMRC-CAT Îș = 0.57; CCQ-mMRC Îș = 0.71; CCQ-CAT Îș = 0.72) with different clinical characteristics. PA was notably lower in patients with an mMRC score ≄2 (3430 [2537] vs. 5443 [3776] steps.d-1, p <0.001) in both the low and high risk quadrants. CONCLUSIONS:Using different questionnaires changes the patient distribution and results in different clinical characteristics. Therefore, standardization of the questionnaire used for classification is critical to allow comparison of different studies using this as an entry criterion.     Performance of the TOTEM Detectors at the LHC

    The TOTEM Experiment is designed to measure the total proton-proton cross-section with the luminosity-independent method and to study elastic and diffractive pp scattering at the LHC. To achieve optimum forward coverage for charged particles emitted by the pp collisions in the interaction point IP5, two tracking telescopes, T1 and T2, are installed on each side of the IP in the pseudorapidity region 3.1 < = |eta | < = 6.5, and special movable beam-pipe insertions - called Roman Pots (RP) - are placed at distances of +- 147 m and +- 220 m from IP5. This article describes in detail the working of the TOTEM detector to produce physics results in the first three years of operation and data taking at the LHC.Comment: 40 pages, 31 figures, submitted to Int.     Sphingolipid dysregulation due to lack of functional KDSR impairs proplatelet formation causing thrombocytopenia.

    Sphingolipids are fundamental to membrane trafficking, apoptosis, and cell differentiation and proliferation. KDSR or 3-keto-dihydrosphingosine reductase is an essential enzyme for de novo sphingolipid synthesis, and pathogenic mutations in KDSR result in the severe skin disorder erythrokeratodermia variabilis et progressiva-4 Four of the eight reported cases also had thrombocytopenia but the underlying mechanism has remained unexplored. Here we expand upon the phenotypic spectrum of KDSR deficiency with studies in two siblings with novel compound heterozygous variants associated with thrombocytopenia, anemia, and minimal skin involvement. We report a novel phenotype of progressive juvenile myelofibrosis in the propositus, with spontaneous recovery of anemia and thrombocytopenia in the first decade of life. Examination of bone marrow biopsies showed megakaryocyte hyperproliferation and dysplasia. Megakaryocytes obtained by culture of CD34+ stem cells confirmed hyperproliferation and showed reduced proplatelet formation. The effect of KDSR insufficiency on the sphingolipid profile was unknown, and was explored in vivo and in vitro by a broad metabolomics screen that indicated activation of an in vivo compensatory pathway that leads to normalization of downstream metabolites such as ceramide. Differentiation of propositus-derived induced pluripotent stem cells to megakaryocytes followed by expression of functional KDSR showed correction of the aberrant cellular and biochemical phenotypes, corroborating the critical role of KDSR in proplatelet formation. Finally, Kdsr depletion in zebrafish recapitulated the thrombocytopenia and showed biochemical changes similar to those observed in the affected siblings. These studies support an important role for sphingolipids as regulators of cytoskeletal organization during megakaryopoiesis and proplatelet formation

    Continuing to Confront COPD International Patient Survey:Economic Impact of COPD in 12 Countries

    BACKGROUND:The Continuing to Confront COPD International Patient Survey estimated the prevalence and burden of COPD across 12 countries. Using data from this survey we evaluated the economic impact of COPD. METHODS:This cross-sectional, population-based survey questioned 4,343 subjects aged 40 years and older, fulfilling a case definition of COPD based on self-reported physician diagnosis or symptomatology. Direct cost measures were based on exacerbations of COPD (treated and those requiring emergency department visits and/or hospitalisation), contacts with healthcare professionals, and COPD medications. Indirect costs were calculated from work loss values using the Work Productivity and Activity Impairment scale. Combined direct and indirect costs estimated the total societal costs per patient. RESULTS:The annual direct costs of COPD ranged from 504(SouthKorea)to504 (South Korea) to 9,981 (USA), with inpatient hospitalisations (5 countries) and home oxygen therapy (3 countries) being the key drivers of direct costs. The proportion of patients completely prevented from working due to their COPD ranged from 6% (Italy) to 52% (USA and UK) with 8 countries reporting this to be ≄20%. Total societal costs per patient varied widely from 1,721(Russia)to1,721 (Russia) to 30,826 (USA) but a consistent pattern across countries showed greater costs among those with increased burden of COPD (symptoms, health status and more severe disease) and a greater number of comorbidities. CONCLUSIONS:The economic burden of COPD is considerable across countries, and requires targeted resources to optimise COPD management encompassing the control of symptoms, prevention of exacerbations and effective treatment of comorbidities. Strategies to allow COPD patients to remain in work are important for addressing the substantial wider societal costs
