    Sensitivity of histological chorioaminionitis and premature rupture of membranes for neonatal sepsis and its risk factors

    Set the sensitivity of the histopathological diagnosis of chorioamnionitis (CAMH) for early diagnosis of neonatal sepsis and the relationship between histological chorioamnionitis and premature rupture of membranes and neonatal sepsis. Materials and methods: Prospective, observational study and diagnostic test performed in the Neonatology Service of the ‘‘Dr. José Eleuterio González’’ University Hospital. Epidemiological variables were collected from mothers and newborns. The relationship between histological chorioamnionitis with premature rupture of membranes and early neonatal sepsis was established. Results: We recorded 3694 births. Of these, 122 patients were studied as potentially infected, of whom 37 patients were excluded (2 by transfer to another hospital and 35 by not finding a histopathological study of the placenta). The study included 85 newborns. Of these, 43 (50.5%) developed clinical and laboratory data of early neonatal sepsis, the rest (n = 42, 49.5%) were healthy newborns. The sensitivity of histological chorioamnionitis with premature rupture of membranes (PRM) of more than 24 h was 81% for neonatal sepsis and 51% without. The risk factors for neonatal sepsis were: Mother with infection (p < 0.001), weight <1500 g (<0.001), gestational age <28 weeks (<0.05), APGAR score <6 in 5 min (p < 0.05). Conclusions: Placental chorioamnionitis with premature rupture of membranes > 24 h has an 81% sensitivity for neonatal sepsis. A newborn with histological chorioamnionitis has a 51% sensitivity for neonatal sepsis

    We apply the Schr\"odinger factorization to construct the ladder operators for hydrogen atom, Mie-type potential, harmonic oscillator and pseudo-harmonic oscillator in arbitrary dimensions. By generalizing these operators we show that the dynamical algebra for these problems is the su(1,1)su(1,1) Lie algebra.Comment: 10 page

    An integrated approach for evaluating climate change risks: a case study in Suriname

    This paper combines long-term state-of-the-art climate projections and indices to provide detailed insights into the future climate of Suriname to facilitate comprehensive information of areas and sectors at high climate risk for political decision-making. The study analyses Suriname's historical climate (1990-2014) and provides climate projections for three time horizons (2020-2044, 2045-2069, 2070-2094) and two emissions scenarios (intermediate/SSP2-4.5 and severe/SSP5-8.5). Coupled Model Intercomparison Project (CMIP6) modeling is used to analyze changes in sea level, temperature, precipitation, relative humidity, and winds. In addition, risk impact chains were produced for the country's four most important socio-economic sectors: agriculture and fisheries, forestry, water, and infrastructure. Results show the temperature is expected to increase for all regions and timeframes, reaching warming up to 6 degrees C in the southern region in the long-term future (2070-2094). Projections point towards a reduction in precipitation in the southwest and coastal regions and a rise in mean sea level. Regarding risk, Paramaribo and Wanica face the highest climate risk. Coronie and Nickerie face the least climate risk. These regions remain the most and least vulnerable in both the SSP2-4.5 and SSP5-8.5 scenarios, but overall values of their risk indices increase substantially over time

    Elastic scattering of 8He + 208Pb at 22MeV

    The dynamics of 8He around the Coulomb barrier is investigated by measuring the energy and angular distribution of the elastic 8He and the 6He and 4He fragment yields in the collision process with a 208Pb target. The experiment was carried out at SPIRAL/GANIL in October 2010. The experimental results will provide information about the relevant reaction mechanisms and the validity of the optical model (OM) when applied to exotic nuclei.     Radiographers supporting radiologists in the interpretation of screening mammography: a viable strategy to meet the shortage in the number of radiologists.

    BackgroundAn alternative approach to the traditional model of radiologists interpreting screening mammography is necessary due to the shortage of radiologists to interpret screening mammograms in many countries.MethodsWe evaluated the performance of 15 Mexican radiographers, also known as radiologic technologists, in the interpretation of screening mammography after a 6 months training period in a screening setting. Fifteen radiographers received 6 months standardized training with radiologists in the interpretation of screening mammography using the Breast Imaging Reporting and Data System (BI-RADS) system. A challenging test set of 110 cases developed by the Breast Cancer Surveillance Consortium was used to evaluate their performance. We estimated sensitivity, specificity, false positive rates, likelihood ratio of a positive test (LR+) and the area under the subject-specific Receiver Operating Characteristic (ROC) curve (AUC) for diagnostic accuracy. A mathematical model simulating the consequences in costs and performance of two hypothetical scenarios compared to the status quo in which a radiologist reads all screening mammograms was also performed.ResultsRadiographer's sensitivity was comparable to the sensitivity scores achieved by U.S. radiologists who took the test but their false-positive rate was higher. Median sensitivity was 73.3 % (Interquartile range, IQR: 46.7-86.7 %) and the median false positive rate was 49.5 % (IQR: 34.7-57.9 %). The median LR+ was 1.4 (IQR: 1.3-1.7 %) and the median AUC was 0.6 (IQR: 0.6-0.7). A scenario in which a radiographer reads all mammograms first, and a radiologist reads only those that were difficult for the radiographer, was more cost-effective than a scenario in which either the radiographer or radiologist reads all mammograms.ConclusionsGiven the comparable sensitivity achieved by Mexican radiographers and U.S. radiologists on a test set, screening mammography interpretation by radiographers appears to be a possible adjunct to radiologists in countries with shortages of radiologists. Further studies are required to assess the effectiveness of different training programs in order to obtain acceptable screening accuracy, as well as the best approaches for the use of non-physician readers to interpret screening mammography

    The recent advances in asymmetric organocatalysis using eutectic mixtures as a reaction medium are revised in this mini‐review. In addition, the first enantioselective transformations using chiral eutectic solvents, which play the role of a green medium and organocatalyst, are described.     Reconstructing native American migrations from whole-genome and whole-exome data.

    There is great scientific and popular interest in understanding the genetic history of populations in the Americas. We wish to understand when different regions of the continent were inhabited, where settlers came from, and how current inhabitants relate genetically to earlier populations. Recent studies unraveled parts of the genetic history of the continent using genotyping arrays and uniparental markers. The 1000 Genomes Project provides a unique opportunity for improving our understanding of population genetic history by providing over a hundred sequenced low coverage genomes and exomes from Colombian (CLM), Mexican-American (MXL), and Puerto Rican (PUR) populations. Here, we explore the genomic contributions of African, European, and especially Native American ancestry to these populations. Estimated Native American ancestry is 48% in MXL, 25% in CLM, and 13% in PUR. Native American ancestry in PUR is most closely related to populations surrounding the Orinoco River basin, confirming the Southern American ancestry of the Taíno people of the Caribbean. We present new methods to estimate the allele frequencies in the Native American fraction of the populations, and model their distribution using a demographic model for three ancestral Native American populations. These ancestral populations likely split in close succession: the most likely scenario, based on a peopling of the Americas 16 thousand years ago (kya), supports that the MXL Ancestors split 12.2kya, with a subsequent split of the ancestors to CLM and PUR 11.7kya. The model also features effective populations of 62,000 in Mexico, 8,700 in Colombia, and 1,900 in Puerto Rico. Modeling Identity-by-descent (IBD) and ancestry tract length, we show that post-contact populations also differ markedly in their effective sizes and migration patterns, with Puerto Rico showing the smallest effective size and the earlier migration from Europe. Finally, we compare IBD and ancestry assignments to find evidence for relatedness among European founders to the three populations

    Identification and characterization of a nodH ortholog from the alfalfa-nodulating Or191-like rhizobia

    Nodulation of Medicago sativa (alfalfa) is known to be restricted to Sinorhizobium meliloti and a few other rhizobia that include the poorly characterized isolates related to Rhizobium sp. strain Or191. Distinctive features of the symbiosis between alfalfa and S. meliloti are the marked specificity from the plant to the bacteria and the strict requirement for the presence of sulfated lipochitooligosaccharides (Nod factors [NFs]) at its reducing end. Here, we present evidence of the presence of a functional nodH-encoded NF sulfotransferase in the Or191-like rhizobia. The nodH gene, present in single copy, maps to a high molecular weight megaplasmid. As in S. meliloti, a nodF homolog was identified immediately upstream of nodH that was transcribed in the opposite direction (local synteny). This novel nodH ortholog was cloned and shown to restore both NF sulfation and the Nif +Fix+ phenotypes when introduced into an S. meliloti nodH mutant. Unexpectedly, however, nodH disruption in the Or191-like bacteria did not abolish their ability to nodulate alfalfa, resulting instead in a severely delayed nodulation. In agreement with evidence from other authors, the nodH sequence analysis strongly supports the idea that the Or191-like rhizobia most likely represent a genetic mosaic resulting from the horizontal transfer of symbiotic genes from a sinorhizobial megaplasmid to a not yet clearly identified ancestor.Instituto de Biotecnologia y Biologia Molecula
