    Conductividad hidráulica en diferentes suelos del Municipio de San José de los Remates, Boaco 2009

    La conductividad hidráulica es un factor importante que determina el movimiento del agua en el suelo, estrechando una íntima relación con el escurrimiento superficial. Este estudio tiene como objetivo evaluar el desempeño de la metodología del infiltrómetro de tensión deminidiscos, comparado con el infiltrómetro de doble cilindro para determinar la conductividad hidráulica (K) en condiciones de laderas en el municipio de san José de los Remates, Boaco. La obtención de conductividad hidráulica no saturada se realizo por medio de dos metodologías Ankenyet al; (1991) y Zhang (1997). La conductividad hidráulica a saturación se obtuvo mediante el infiltrómetro de doble cilindro utilizando la ecuación de Reynolds (2002). Los diversos valores obtenidos, remarcan el carácter variable de este parámetro. El doble cilindro presentó valores más altos al compararse con los datos de la tensión más cercana a saturación debido a que es un suelo franco arcilloso. Los datos de conductividad generados por las ecuaciones de Ankeny y Zhang presentaron diferencias ya que los primeros fueron en promedio mayor. El método infiltrómetro de tensión de minidisco resulta ser más práctico en términos de su facilidad, tiempo, número de operarios, número de repeticiones, consumo de agua especialmente en zonas de difícil acceso, ambos se realizaron en condiciones naturales del suelo (Campo) lo cual permite mayor confiabilidad del método

    Economic evaluation of a novel community-based diabetes care model in rural Mexico: a cost and cost-effectiveness study.

    OBJECTIVES: Diabetes is the leading cause of disability-adjusted life years in Mexico, and cost-effective care models are needed to address the epidemic. We sought to evaluate the cost and cost-effectiveness of a novel community-based model of diabetes care in rural Mexico, compared with usual care. DESIGN: We performed time-driven activity-based costing to estimate annualised costs associated with typical diabetes care in Chiapas, Mexico, as well as a novel diabetes care model known as Compañeros En Salud Programa de Enfermedades Crónicas (CESPEC). We conducted Markov chain analysis to estimate the cost-effectiveness of CESPEC compared with usual care from a societal perspective. We used patient outcomes from CESPEC in 2016, as well as secondary data from existing literature. SETTING: Rural primary care clinics in Chiapas, Mexico. PARTICIPANTS: Adults with diabetes. INTERVENTIONS: CESPEC is a novel, comprehensive, diabetes care model that integrates community health workers, provider education, supply chain management and active case finding. OUTCOME MEASURE: The primary outcome was the incremental cost-effectiveness of CESPEC compared with care as usual, per quality-adjusted life year (QALY) gained, expressed in 2016 US dollars. RESULTS: The economic cost of the CESPEC diabetes model was US144perpatientperyear,comparedwithUS144 per patient per year, compared with US125 for diabetes care as usual. However, CESPEC care was associated with 0.13 additional years of health-adjusted life expectancy compared with usual care and 0.02 additional years in the first 5 years of treatment. This translated to an incremental cost-effectiveness ratio (ICER) of US2981perQALYgainedoverapatient′slifetimeandanICERofUS2981 per QALY gained over a patient's lifetime and an ICER of US10 444 over the first 5 years. Findings were robust to multiple sensitivity analyses. CONCLUSIONS: CESPEC is a cost-effective, community-based model of diabetes care for patients in rural Mexico. Given the high prevalence and significant morbidity associated with diabetes in Mexico and other countries in Central America, this model should be considered for broader scale up and evaluation

    Canagliflozin and Renal Outcomes in Type 2 Diabetes and Nephropathy

    BACKGROUND Type 2 diabetes mellitus is the leading cause of kidney failure worldwide, but few effective long-term treatments are available. In cardiovascular trials of inhibitors of sodium–glucose cotransporter 2 (SGLT2), exploratory results have suggested that such drugs may improve renal outcomes in patients with type 2 diabetes. METHODS In this double-blind, randomized trial, we assigned patients with type 2 diabetes and albuminuric chronic kidney disease to receive canagliflozin, an oral SGLT2 inhibitor, at a dose of 100 mg daily or placebo. All the patients had an estimated glomerular filtration rate (GFR) of 30 to 300 to 5000) and were treated with renin–angiotensin system blockade. The primary outcome was a composite of end-stage kidney disease (dialysis, transplantation, or a sustained estimated GFR of <15 ml per minute per 1.73 m 2), a doubling of the serum creatinine level, or death from renal or cardiovascular causes. Prespecified secondary outcomes were tested hierarchically. RESULTS The trial was stopped early after a planned interim analysis on the recommendation of the data and safety monitoring committee. At that time, 4401 patients had undergone randomization, with a median follow-up of 2.62 years. The relative risk of the primary outcome was 30% lower in the canagliflozin group than in the placebo group, with event rates of 43.2 and 61.2 per 1000 patient-years, respectively (hazard ratio, 0.70; 95% confidence interval [CI], 0.59 to 0.82; P=0.00001). The relative risk of the renal-specific composite of end-stage kidney disease, a doubling of the creatinine level, or death from renal causes was lower by 34% (hazard ratio, 0.66; 95% CI, 0.53 to 0.81; P<0.001), and the relative risk of end-stage kidney disease was lower by 32% (hazard ratio, 0.68; 95% CI, 0.54 to 0.86; P=0.002). The canagliflozin group also had a lower risk of cardiovascular death, myocardial infarction, or stroke (hazard ratio, 0.80; 95% CI, 0.67 to 0.95; P=0.01) and hospitalization for heart failure (hazard ratio, 0.61; 95% CI, 0.47 to 0.80; P<0.001). There were no significant differences in rates of amputation or fracture. CONCLUSIONS In patients with type 2 diabetes and kidney disease, the risk of kidney failure and cardiovascular events was lower in the canagliflozin group than in the placebo group at a median follow-up of 2.62 years

    Impact of COVID-19 on cardiovascular testing in the United States versus the rest of the world

    Objectives: This study sought to quantify and compare the decline in volumes of cardiovascular procedures between the United States and non-US institutions during the early phase of the coronavirus disease-2019 (COVID-19) pandemic. Background: The COVID-19 pandemic has disrupted the care of many non-COVID-19 illnesses. Reductions in diagnostic cardiovascular testing around the world have led to concerns over the implications of reduced testing for cardiovascular disease (CVD) morbidity and mortality. Methods: Data were submitted to the INCAPS-COVID (International Atomic Energy Agency Non-Invasive Cardiology Protocols Study of COVID-19), a multinational registry comprising 909 institutions in 108 countries (including 155 facilities in 40 U.S. states), assessing the impact of the COVID-19 pandemic on volumes of diagnostic cardiovascular procedures. Data were obtained for April 2020 and compared with volumes of baseline procedures from March 2019. We compared laboratory characteristics, practices, and procedure volumes between U.S. and non-U.S. facilities and between U.S. geographic regions and identified factors associated with volume reduction in the United States. Results: Reductions in the volumes of procedures in the United States were similar to those in non-U.S. facilities (68% vs. 63%, respectively; p = 0.237), although U.S. facilities reported greater reductions in invasive coronary angiography (69% vs. 53%, respectively; p < 0.001). Significantly more U.S. facilities reported increased use of telehealth and patient screening measures than non-U.S. facilities, such as temperature checks, symptom screenings, and COVID-19 testing. Reductions in volumes of procedures differed between U.S. regions, with larger declines observed in the Northeast (76%) and Midwest (74%) than in the South (62%) and West (44%). Prevalence of COVID-19, staff redeployments, outpatient centers, and urban centers were associated with greater reductions in volume in U.S. facilities in a multivariable analysis. Conclusions: We observed marked reductions in U.S. cardiovascular testing in the early phase of the pandemic and significant variability between U.S. regions. The association between reductions of volumes and COVID-19 prevalence in the United States highlighted the need for proactive efforts to maintain access to cardiovascular testing in areas most affected by outbreaks of COVID-19 infection

    Ultraviolet Irradiation Effect on Apple Juice Bioactive Compounds during Shelf Storage

    Clarified and standardized apple juice was ultraviolet-irradiated to inactivate polyphenol oxidase enzyme and microbiota, and its effect on bioactive compounds and stability during storage was also evaluated. Apple juice was irradiated with 345.6 J/cm2 and treatment effect was evaluated in terms of color, antioxidant capacity, polyphenol content, pH, titratable acidity and total soluble solids. Using a linear regression design, inactivation kinetic of polyphenol oxidase enzyme was also described. In addition, a repeated measures design was carried out to evaluate apple juice during 24 days of storage at 4 °C and 20 °C. After irradiation, reduction of antioxidant capacity was observed while during storage, ascorbic acid content decreased up to 40% and total polyphenol content remain stable. Ultraviolet irradiation achieved a complete inactivation of polyphenol oxidase enzyme and microbiota, keeping apple juice antioxidants during ultraviolet treatment and storage available until juice consumption. UV-treated apple juice can be used as a regular beverage, ensuring antioxidant intake

    Efecto de dos condiciones de humedad del suelo y tiempo de cosecha sobre el rendimiento de malanga (Colocasia esculenta L. Schott) para exportacion, Boaco - Nicaragua 2011

    La malanga ( Colocasia esculenta. (L.) Schott) , familia Aráceae, es el tubérculo de mayor exportación en Nicaragua, pero es escasa la información sobre su agrotecnia. Se evaluó el efecto que tienen el tiempo de cosecha y dos condiciones de humedad del suelo sobre el rendimiento de Malanga Lila para exportación. Se establecieron dos ensayos. En el ensayo I se evaluó el efecto de cosechar las plantas a los 7 meses (7M) y 9 meses (9M) en suelo seco. Se establecieron 2 lotes de plantas de 0.7 ha cada uno, el primero en noviembre 2009 y el otro en enero 2010, ambos lotes cosechados en agosto 2010. En el ensayo II se evaluó el efecto de siembra en secano (7MS) y fangoso (7MF) sobre el rendimiento. En ambos ensayos se establecieron cuatro submuestras de 10 plantas (observaciones) por lote. En el Ensayo I se evaluaron variables morfológicas a 127, 150, 180 y 215 días después de la siembra (dds) en relación a plantas 9MS y a los 96, 120, 150 y 185 dds a plantas 7M. El ensayo II se evaluó de igual forma que las plastas 7M del ensayo I. A los datos de las variables morfológicas se realizó la prueba de T-Student y las de rendimiento un ANDEVA. Se determinó la incidencia del Virus del Mosaico del Dasheen (DsMV) mediante la prueba ELISA, se evaluó la concentración de materia seca de los cormos de plantas madres. Las plantas 9M y 7M registraron valores de las variables morfológicas y rendimiento similares (45.15 y 42.47 t ha-1 respectivamente), el momento ó ptimo de cosecha es 9 meses. Las plantas 7MF crecieron más lentamente que las platas 7MS, las que no lograron el máximo desarrollo foliar y presentaron mayor incidencia de enfermedades fungosas y bacterianas en cormos, raíces y hojas. Las plantas de ambos ensayos mostraron síntomas del virus. Las plantas 7MF presentaron cormos alargados y menos anchos que las plantas 7MS

    Development of a community-based intervention for the control of Chagas disease based on peridomestic animal management: an eco-bio-social perspective

    Background: Integrated vector management strategies depend on local eco-bio-social conditions, community participation, political will and inter-sectorial partnership. Previously identified risk factors for persistent Triatoma dimidiata infestation include the presence of rodents and chickens, tiled roofs, dirt floors, partial wall plastering and dog density.Methods: A community-based intervention was developed and implemented based on cyclical stakeholder and situational analyses. Intervention implementation and evaluation combined participatory action research and cluster randomized pre-test post-test experimental designs. The intervention included modified insecticide application, education regarding Chagas disease and risk factors, and participatory rodent control.Results: At final evaluation there was no significant difference in post-test triatomine infestation between intervention and control, keeping pre-test rodent and triatomine infestations constant. Knowledge levels regarding Chagas disease and prevention practices including rodent control, chicken management and health service access increased significantly only in intervention communities. The odds of nymph infection and rat infestation were 8.3 and 1.9-fold higher in control compared to intervention communities, respectively.Conclusion: Vector control measures without reservoir control are insufficient to reduce transmission risk in areas with persistent triatomine infestation. This integrated vector management program can complement house improvement initiatives by prioritizing households with risk factors such as tiled roofs. Requirement for active participation and multi-sectorial coordination poses implementation challenges

    CONFORME A LAS EVIDENCIAS ofrecidas por varios historiadores el 30 de septiembre y el 22 de diciembre de 2015 se conmemoro el 250 aniversario del natalicio y el bicentenario de la muerte, respectivamente, de quien fuera la persona más extraordinario y de mayor trascendencia durante la Guerra de Independencia de México: don José María Morelos y Pavón. Estas fechas nos son propicias no sólo para conmemorar o los próceres que nos dieron patrio y libertad sino también para reflexionar qué hemos hecho los y los ciudadanos actuales con el legado y herencia que recibimos de quienes consideramos nuestros padres de lo noción mexicano. Lo Universidad Autónomo del Estado de México se suma con este libro y otros acciones o lo serie de conmemoraciones que diversas personas, instituciones y entidades del país están realizando este año al líder supremo de la insurgencia del Sur
