Development and clinical evaluation of a simple optical method to detect and measure patient external motion
A simple and independent system to detect and measure the position of a number
of points in space was devised and implemented. Its application aimed to detect
patient motion during radiotherapy treatments, alert of out-of-tolerances motion, and
record the trajectories for subsequent studies. The system obtains the 3D position
of points in space, through its projections in 2D images recorded by two cameras.
It tracks black dots on a white sticker placed on the surface of the moving object.
The system was tested with linear displacements of a phantom, circular trajectories
of a rotating disk, oscillations of an in-house phantom, and oscillations of a
4D phantom. It was also used to track 461 trajectories of points on the surface of
patients during their radiotherapy treatments. Trajectories of several points were
reproduced with accuracy better than 0.3 mm in the three spatial directions. The
system was able to follow periodic motion with amplitudes lower than 0.5 mm,
to follow trajectories of rotating points at speeds up to 11.5 cm/s, and to track
accurately the motion of a respiratory phantom. The technique has been used to
track the motion of patients during radiotherapy and to analyze that motion. The
method is flexible. Its installation and calibration are simple and quick. It is easy
to use and can be implemented at a very affordable price. Data collection does
not involve any discomfort to the patient and does not delay the treatment, so the
system can be used routinely in all treatments. It has an accuracy similar to that of
other, more sophisticated, commercially available systems. It is suitable to implement
a gating system or any other application requiring motion detection, such as
4D CT, MRI or PET

Fundamentos: El Registro Nacional de Fracturas de Cadera (RNFC) es un registro español multicéntrico, prospectivo y continuo, que comenzó en 2017. El objetivo de este artículo fue presentar los datos del primer informe anual y compararlos con los registros autonómicos y los estudios multicéntricos realizados recientemente en España. Métodos: Se incluyeron las personas de 75 años o más atendidas con el diagnóstico de fractura de cadera por fragilidad en alguno de los hospitales participantes en el RNFC, entre enero y octubre de 2017. En el análisis estadístico se utilizó la media y desviación estándar o mediana y rangos intercuartílicos para las variables numéricas y los porcentajes para las variables categóricas. Se realizó un análisis descriptivo global de la casuística y se comparó con los datos disponibles de los estudios previos mencionados. Resultados: Se registraron 7.208 personas de 54 hospitales, con una edad media de 86,7 años (DE 5,6). El 75,4% fueron mujeres y el 36,4% presentaron deterioro cognitivo previo. La demora quirúrgica media fue de 75,7 horas (DE 63,6) y la estancia media fue de 10,9 días (DE 6,7). De las personas que vivían en un domicilio antes de la fractura (75,4%), menos de la mitad (37,0%) volvieron a él tras el alta hospitalaria. Al mes, había fallecido el 7,1%. La comparación con los otros estudios mostró algunas diferencias importantes, sobre todo en la ubicación previa, en el porcentaje de pacientes institucionalizados de novo (7,7-29,4%) y en el porcentaje con tratamiento antiosteoporótico al alta (14,5-36,7%). Conclusiones: El RNFC es la mayor base de datos prospectiva que aporta datos sobre el perfil de los pacientes hospitalizados por fractura de cadera en España. Comparison with other studies showed important differences, especially regarding patients newly sent to nursing homes (7.7-29.4%) and with antiosteoporotic treatment at discharge (14.5-36.7%). CONCLUSIONS: The RNFC is the largest prospective database to date that offers data regarding the characteristics of patients hospitalized for hip fractures in Spain. Écho des travaux du séminaire commun d’histoire sociale des institutions que l’École française de Rome et la Casa de Velázquez ont mis sur pied en 2001 et qui a commencé ses activités en 2002, ce dossier examine les liens entre les magistratures citadines et la Couronne espagnole. Les sept articles rassemblés tentent d’analyser, soit par l’approche globale comparative, soit à partir d’études de cas, l’importance de la vie municipale dans différents espaces de la Couronne espagnole : la péninsule Ibérique bien sûr, mais aussi le Royaume de Naples et les terres américaines.
