    A zooarchaeological study of butchery and bone fat processing practices among early Neolithic farming societies in central Europe

    This thesis presents the results of zooarchaeological investigations into diet in Neolithic central Europe. The aim of these investigations was to gain a better understanding of animal carcass processing, particularly dietary decisions made concerning intensity of exploitation of meat and fat resources. The primary focus was the Linearbandkeramik (LBK) culture, a sedentary community of farmers dating from c. 5500-4900BC in central Europe suspected to be the first society to utilise milk and its products in this region. The adoption of dairying, currently under scrutiny by the NeoMilk project, would have increased the availability of fat on settlements, and could have affected the way in which people utilised primary animal products. Using in-depth zooarchaeological analysis of butchery, fracture and fragmentation, this thesis presents a snapshot of Neolithic meat and fat exploitation. Patterns of butchery and heat exposure suggest differential cooking practices between sites, with a possible focus on nutrient retention at some, contrasted with a cultural preference for roasting at others. Intensive processing of bone fats, namely bone grease, was not detected at any site and it is likely that the domesticated LBK diet rendered this practice unnecessary to subsistence. Bone marrow was a much more commonly exploited resource, but variation was considerable between sites. It is possible that the intensification of dairying had a significant effect on the utilisation of bone marrow. Sites with the most evidence for milk use, detected through lipid residue and osteoarchaeological evidence, show less intensive exploitation of bone marrow than those with little or no evidence of dairying. This thesis therefore presents evidence of zooarchaeologically detectable dietary decisions being made in the face of adoption of new foodstuffs

    Investigating moderators of daily marital to parent–child spillover: Individual and family systems approaches

    Objective: We tested whether cognitive reappraisal and coparenting quality moderate marital to parent–child spillover in mothers and fathers. Background: The influence of marital relationship quality on parent–child relationships, referred to as the spillover effect, is well documented. Factors that may attenuate the occurrence of spillover, however, remain unclear. Cognitive reappraisal, an emotion regulation strategy that promotes the reframing of emotional situations as neutral or positive, and coparenting—the intermediate subsystem between the marital and parent–child relationships—may buffer the effects of marital to parent–child spillover. Method: Using daily diary data from mother–father couples (N = 96) of young children (Mage = 3.22 years), we investigated coparenting quality and cognitive reappraisal as moderators of marital and parent–child spillover within and between days. Results: Dyadic multilevel models revealed within-day spillover of marital emotional climate and parent–child emotional climate for both mothers and fathers. Whereas cognitive reappraisal moderated spillover for fathers, no significant moderators emerged for mothers. Fathers also experienced next-day associations between marital emotional climate and parent–child emotional climate the following day, whereas mothers did not. Coparenting quality accounted for next-day associations between fathers’ marital emotional climate and parent–child climate. Conclusion: Overall, our results evince that although spillover can be attenuated by both cognitive reappraisal and coparenting quality for fathers, the same is not true for mothers. Implications: These results signify the importance of considering mother and father differences in empirical investigations of spillover effects and processes within the family system, and the clinical implications recommended to marriage and family therapists

    The cellular and synaptic architecture of the mechanosensory dorsal horn

    The deep dorsal horn is a poorly characterized spinal cord region implicated in processing low-threshold mechanoreceptor (LTMR) information. We report an array of mouse genetic tools for defining neuronal components and functions of the dorsal horn LTMR-recipient zone (LTMR-RZ), a role for LTMR-RZ processing in tactile perception, and the basic logic of LTMR-RZ organization. We found an unexpectedly high degree of neuronal diversity in the LTMR-RZ: seven excitatory and four inhibitory subtypes of interneurons exhibiting unique morphological, physiological, and synaptic properties. Remarkably, LTMRs form synapses on between four and 11 LTMR-RZ interneuron subtypes, while each LTMR-RZ interneuron subtype samples inputs from at least one to three LTMR classes, as well as spinal cord interneurons and corticospinal neurons. Thus, the LTMR-RZ is a somatosensory processing region endowed with a neuronal complexity that rivals the retina and functions to pattern the activity of ascending touch pathways that underlie tactile perception

    PROTEUS Study: A Prospective Randomised Controlled Trial Evaluating the Use of Artificial Intelligence in Stress Echocardiography.

    BACKGROUND Stress echocardiography (SE) is one of the most commonly used diagnostic imaging tests for coronary artery disease (CAD) but requires clinicians to visually assess scans to identify patients who may benefit from invasive investigation and treatment. EchoGo Pro provides an automated interpretation of SE based on artificial intelligence (AI) image analysis. In reader studies, use of EchoGo Pro when making clinical decisions improves diagnostic accuracy and confidence. Prospective evaluation in real world practice is now important to understand the impact of EchoGo Pro on the patient pathway and outcome. METHODS/DESIGN PROTEUS is a randomised, multicentre, two-armed, non-inferiority study aiming to recruit 2,500 participants from National Health Service (NHS) hospitals in the UK referred to SE clinics for investigation of suspected CAD. All participants will undergo a stress echocardiogram protocol as per local hospital policy. Participants will be randomised 1:1 to a control group, representing current practice, or an intervention group, in which clinicians will receive an AI image analysis report (EchoGo Pro, Ultromics Ltd, Oxford, UK) to use during image interpretation, indicating the likelihood of severe CAD. The primary outcome will be appropriateness of clinician decision to refer for coronary angiography. Secondary outcomes will assess other health impacts including appropriate use of other clinical management approaches, impact on variability in decision making, patient and clinician qualitative experience and a health economic analysis. DISCUSSION This will be the first study to assess the impact of introducing an AI medical diagnostic aid into the standard care pathway of patients with suspected CAD being investigated with SE

    Re-reading of OraQuick HIV-1/2 rapid antibody test results: quality assurance implications for HIV self-testing programmes.

    INTRODUCTION: Scale-up of HIV self-testing (HIVST) will play a key role in meeting the United Nation's 90-90-90 targets. Delayed re-reading of used HIVST devices has been used by early implementation studies to validate the performance of self-test kits and to estimate HIV positivity among self-testers. We investigated the stability of results on used devices under controlled conditions to assess its potential as a quality assurance approach for HIVST scale-up. METHODS: 444 OraQuick® HIV-1/2 rapid antibody tests were conducted using commercial plasma from two HIV-positive donors and HIV-negative plasma (high-reactive n = 148, weak-reactive n = 148 and non-reactive n = 148) and incubated them for six months under four conditions (combinations of high and low temperatures and humidity). Devices were re-read daily for one week, weekly for one subsequent month and then once a month by independent readers unaware of the previous results. We used multistage transition models to investigate rates of change in device results, and between storage conditions. RESULTS AND DISCUSSION: There was a high incidence of device instability. Forty-three (29%) of 148 initially non-reactive results became false weak-reactive results. These changes were observed across all incubation conditions, the earliest on Day 4 (n = 9 kits). No initially HIV-reactive results changed to a non-reactive result. There were no significant associations between storage conditions and hazard of results transition. We observed substantial statistical agreement between independent re-readers over time (agreement range: 0.74 to 0.96). CONCLUSIONS: Delayed re-reading of used OraQuick® HIV-1/2 rapid antibody tests is not currently a valid methodological approach to quality assurance and monitoring as we observed a high incidence (29%) of true non-reactive tests changing to false weak-reactive and therefore its use may overestimate true HIV positivity

    Effects of antiplatelet therapy on stroke risk by brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases: subgroup analyses of the RESTART randomised, open-label trial

    Background Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage.
