THE EFFETIVENESS OF USING TWO STAY TWO STRAY TECHNIQUE TO TEACH READING COMPREHENSION ON FACTUAL REPORT TEXT AT TENTH GRADE STUDENTS OF MAN 4 KEDIRI
Reading comprehension is an important part of reading process. In learning English, students may face some difficulties in learning reading, even though they are students of Senior High School. Therefore, the teacher should teach them by appropriate technique to solve their difficulties. This study is aimed to investigate the effectiveness of using Two Stay Two Stray Techniques to teach reading comprehension on factual report text. The research design used is a static group comparison design of pre-experimental design. The study is conducted by applying Two Stay Two Stray (TS-TS) technique to the experimental class only. There are 34 students in experimental class and 34 students in control class. The result of data analysis shows that the mean score of post-test for experimental class is 81.06 while the post-test of control class is 71.88. It can be seen that the experimental class outperformed the control class in reading comprehension with t = 2.028, df = 66 and P = .005 and 95% confidence interval ranging from 143 to 18.210. The result of t-test shows that the mean difference is 9.176 with sig. (2-tailed) value 0.047. It can be seen that the value 0.047 is lower than 5% (0.047 < 0.05). It can be concluded that the t-value is significant in 5% significant level. It means that there is significant difference between control and experimental class. Thus, there is a significant effect of using Two Stay Two Stray technique to teach reading comprehension at tenth grade students of MAN 4 Kediri. It can be seen from the sig (2-tailed). Characterisation of insulin analogues therapeutically available to patients
The structure and function of clinical dosage insulin and its analogues were assessed. This included ‘native insulins’ (human recombinant, bovine, porcine), ‘fast-acting analogues’ (aspart, glulisine, lispro) and ‘slow-acting analogues’ (glargine, detemir, degludec). Analytical ultracentrifugation, both sedimentation velocity and equilibrium experiments, were employed to yield distributions of both molar mass and sedimentation coefficient of all nine insulins. Size exclusion chromatography, coupled to multi-angle light scattering, was also used to explore the function of these analogues. On ultracentrifugation analysis, the insulins under investigation were found to be in numerous conformational states, however the majority of insulins were present in a primarily hexameric conformation. This was true for all native insulins and two fast-acting analogues. However, glargine was present as a dimer, detemir was a multi-hexameric system, degludec was a dodecamer (di-hexamer) and glulisine was present as a dimer-hexamer-dihexamer system. However, size-exclusion chromatography showed that the two hexameric fast-acting analogues (aspart and lispro) dissociated into monomers and dimers due to the lack of zinc in the mobile phase. This comprehensive study is the first time all nine insulins have been characterised in this way, the first time that insulin detemir have been studied using analytical ultracentrifugation and the first time that insulins aspart and glulisine have been studied using sedimentation equilibrium. The structure and function of these clinically administered insulins is of critical importance and this research adds novel data to an otherwise complex functional physiological protein
Implementation tells us more beyond pooled estimates: Secondary analysis of a multicountry mHealth trial to reduce blood pressure
Background: The uptake of an intervention aimed at improving health-related lifestyles may be influenced by the participant’s stage of readiness to change behaviors. Objective: We conducted secondary analysis of the Grupo de Investigación en Salud Móvil en América Latina (GISMAL) trial according to levels of uptake of intervention (dose-response) to explore outcomes by country, in order to verify the consistency of the trial’s pooled results, and by each participant’s stage of readiness to change a given lifestyle at baseline. The rationale for this secondary analysis is motivated by the original design of the GISMAL study that was independently powered for the primary outcome—blood pressure—for each country. Methods: We conducted a secondary analysis of a mobile health (mHealth) multicountry trial conducted in Argentina, Guatemala, and Peru. The intervention consisted of monthly motivational phone calls by a trained nutritionist and weekly tailored text messages (short message service), over a 12-month period, aimed to enact change on 4 health-related behaviors: salt added to foods when cooking, consumption of high-fat and high-sugar foods, consumption of fruits or vegetables, and practice of physical activity. Results were stratified by country and by participants’ stage of readiness to change (precontemplation or contemplation; preparation or action; or maintenance) at baseline. Exposure (intervention uptake) was the level of intervention (<50%, 50%-74%, and ≥75%) received by the participant in terms of phone calls. Linear regressions were performed to model the outcomes of interest, presented as standardized mean values of the following: blood pressure, body weight, body mass index, waist circumference, physical activity, and the 4 health-related behaviors. Results: For each outcome of interest, considering the intervention uptake, the magnitude and direction of the intervention effect differed by country and by participants’ stage of readiness to change at baseline. Among those in the high intervention uptake category, reductions in systolic blood pressure were only achieved in Peru, whereas fruit and vegetable consumption also showed reductions among those who were at the maintenance stage at baseline in Argentina and Guatemala. Conclusions: Designing interventions oriented toward improving health-related lifestyle behaviors may benefit from recognizing baseline readiness to change and issues in implementation uptake. Countdown to 2030 : tracking progress towards universal coverage for reproductive, maternal, newborn, and child health
Building upon the successes of Countdown to 2015, Countdown to 2030 aims to support the monitoring and measurement of women's, children's, and adolescents' health in the 81 countries that account for 95% of maternal and 90% of all child deaths worldwide. To achieve the Sustainable Development Goals by 2030, the rate of decline in prevalence of maternal and child mortality, stillbirths, and stunting among children younger than 5 years of age needs to accelerate considerably compared with progress since 2000. Such accelerations are only possible with a rapid scale-up of effective interventions to all population groups within countries (particularly in countries with the highest mortality and in those affected by conflict), supported by improvements in underlying socioeconomic conditions, including women's empowerment. Three main conclusions emerge from our analysis of intervention coverage, equity, and drivers of reproductive, maternal, newborn, and child health (RMNCH) in the 81 Countdown countries. First, even though strong progress was made in the coverage of many essential RMNCH interventions during the past decade, many countries are still a long way from universal coverage for most essential interventions. Furthermore, a growing body of evidence suggests that available services in many countries are of poor quality, limiting the potential effect on RMNCH outcomes. Second, within-country inequalities in intervention coverage are reducing in most countries (and are now almost non-existent in a few countries), but the pace is too slow. Third, health-sector (eg, weak country health systems) and non-health-sector drivers (eg, conflict settings) are major impediments to delivering high-quality services to all populations. Although more data for RMNCH interventions are available now, major data gaps still preclude the use of evidence to drive decision making and accountability. Countdown to 2030 is investing in improvements in measurement in several areas, such as quality of care and effective coverage, nutrition programmes, adolescent health, early childhood development, and evidence for conflict settings, and is prioritising its regional networks to enhance local analytic capacity and evidence for RMNCH
Policies and clinical practices relating to the management of gestational diabetes mellitus in the public health sector, South Africa – a qualitative study
BACKGROUND: Women with a prior gestational diabetes have an increased lifetime risk of developing type 2
diabetes. Although post-partum follow-up for GDM women is essential to prevent progression to type 2 diabetes, it
is poorly attended. The need for health systems interventions to support postpartum follow-up for GDM women is
evident, but there is little knowledge of actual current practice. The aim of this study was to explore current policies
and clinical practices relating to antenatal and post-natal care for women with GDM in South Africa, as well as
health sector stakeholders’ perspectives on the barriers to – and opportunities for – delivering an integrated
mother - baby health service that extends beyond the first week post-partum, to the infant’s first year of life.
METHODS: Following a document review of policy and clinical practice guidelines, in-depth interviews were
conducted with 11 key informants who were key policy makers, health service managers and clinicians working in
the public health services in South Africa’s two major cities (Johannesburg and Cape Town). Data were analysed
using qualitative content analysis procedures.
RESULTS: The document review and interviews established that it is policy that health services adhere to
international guidelines for GDM diagnosis and management, in addition to locally developed guidelines and
protocols for clinical practice. All key informants confirmed that lack of postpartum follow-up for GDM women is a
significant problem. Health systems barriers include fragmentation of care and the absence of standardised
postnatal care for post-GDM women. Key informants also raised patient - related challenges including lack of
perceived future risk of developing type 2 diabetes and non-attendance for postpartum follow up, as barriers to
postnatal care for GDM women. All participants supported integrated primary health services but cautioned against
overloading health workers.
CONCLUSION: Although there is alignment between international guidelines, local policy and reported clinical
practice in the management of GDM, there is a gap in continuation of care in the postpartum period. Health
systems interventions that support and facilitate active follow-up for women with prior GDM are needed if high
rates of progression to type 2 diabetes are to be avoided
