Objectives: Metal and Zirconia cantilever resin bonded fixed dental prosthesis (RBFDPs) are extensively used when missing anterior teeth. Lithium disilicate is not used a lot as it is not indicated by the manufacturers. The aim of this in vitro study was to investigate the fracture strength of lithium disilicate cantilever RBFDPs with different configurations and compare them to metal and zirconium RBFDPs. Methods: Sound extracted human canines (N = 60) were divided into six groups, to be restored with a cantilever RBFDP. Specimen were randomly divided over 6 groups (n = 10): Full crown of lithium disilicate (FCL); Veneer wing of lithium disilicate (VL); Connector of lithium disilicate (CL); Palatal wing of lithium disilicate (PL); Palatal wing of zirconia (PZ) and Palatal wing of metal ceramic (PM). All bridges were bonded with an adhesive system. After thermalcyclic ageing (20 x 10(3)x, 5-55 degrees C) all samples were loaded until fracture occurred. Failure types were classified and representative SEM done. Results: The mean fracture strength results per group were: 588N (FCL) 588N (PM), 550N (CL), 534N (PL), 465N (VL), 38N (PZ). A significant (p = 0.001) difference was found between the groups, all groups had a higher fracture strength than the zirconia RBFDPs. Failure type analysis showed some trends among the groups. Irrepairable fractures of the root were only seen in samples restored with lithium disilicate. Metal and zirconia RBFDPs predominantly failed on the adhesive interface, where 60% of the zirconia samples had pretest debondings. Significance: No differences in fracture strength were found between cantilever RBFDPs made from metal or lithium disilicate. Metal (0% pre-test failures) and zirconium (60% pretest failures) RBFDPs failed predominantly on the adhesive interface whereas the lithium disilicate (0% pre-test failures) samples showed fractures in the contact area. The least invasive connector (CL) and Metal (PM) RBFDP obtained a high fracture strength and optimal fracture pattern

International audienceNon-invasive and accurate measurements are essential to study the reactive flows in aeronautical engines. This paper reports the results of a unique measurement campaign providing the temperature flowfield in a large scale facility turbomachine test rig using spontaneous rotational Raman scattering technique. Different planes of interest and operating conditions are probed, showing good agreement with thermocouple measurements. Fast temperature variations (>7.7 kHz) could be probed thanks to synchronization of the laser pulse with the rotor clock. Results outline the performance of in situ Raman technique to investigate steady and unsteady flows in turbine's conditions

International audienceBACKGROUND AND OBJECTIVES: High-dose chemotherapy with allogeneic stem cell transplantation (SCT) has proven to be a successful treatment for low-grade lymphoma (LGL), but is associated with considerable transplant-related mortality (TRM). In an effort to reduce toxic mortality while maintaining the graft-versus-leukemia effect, allogeneic SCT has been combined with a reduced-intensity conditioning (RIC) regimen. The aim of this study was to determine the outcome of patients with LGL treated with RIC allogeneic SCT. DESIGN AND METHODS: This retrospective multicenter study included 73 patients with relapsed or refractory LGL allografted after a RIC regimen between 1998 and 2005 whose data were recorded in a French registry. RESULTS: Patients received a median of three lines of therapy prior to RIC allogeneic SCT. The most widely used conditioning regimens were fludarabine + busulfan + antithymocyte globulin (n=43) and fludarabine + total body irradiation (n=21). Prior to allografting, patients were in complete response (CR; n=21), partial response (PR; n=33) or had chemoresistant disease (n=19). The median follow-up was 37 months (range, 16 to 77 months). In patients in CR, PR and chemoresistant disease, the 3-year overall survival rates were 66%, 64% and 32%, respectively, while the 3-year event-free survival rates were 66%, 52% and 32%, respectively. The 3-year cumulative incidences of TRM were 32%, 28% and 63%, respectively. The incidence of relapse was 9.6%. INTERPRETATION AND CONCLUSIONS: Although associated with significant TRM, RIC allogeneic SCT in advanced chemosensitive disease leads to long-term survival

Chronic pain associated with the Chikungunya Fever: long lasting burden of an acute illness

Abstract

Background

Chikungunya virus (CHIKV) is responsible for major epidemics worldwide. Autochthonous cases were recently reported in several European countries. Acute infection is thought to be monophasic. However reports on chronic pain related to CHIKV infection have been made. In particular, the fact that many of these patients do not respond well to usual analgesics suggests that the nature of chronic pain may be not only nociceptive but also neuropathic. Neuropathic pain syndromes require specific treatment and the identification of neuropathic characteristics (NC) in a pain syndrome is a major step towards pain control.

Methods

We carried out a cross-sectional study at the end of the major two-wave outbreak lasting 17 months in Réunion Island. We assessed pain in 106 patients seeking general practitioners with confirmed infection with the CHIK virus, and evaluated its impact on quality of life (QoL).

Results

The mean intensity of pain on the visual-analogical scale (VAS) was 5.8 ± 2.1, and its mean duration was 89 ± 2 days. Fifty-six patients fulfilled the definition of chronic pain. Pain had NC in 18.9% according to the DN4 questionnaire. Conversely, about two thirds (65%) of patients with NC had chronic pain. The average pain intensity was similar between patients with or without NC (6.0 ± 1.7 vs 6.1 ± 2.0). However, the total score of the Short Form-McGill Pain Questionnaire (SF-MPQ)(15.5 ± 5.2 vs 11.6 ± 5.2; p < 0.01) and both the affective (18.8 ± 6.2 vs 13.4 ± 6.7; p < 0.01) and sensory subscores (34.3 ± 10.7 vs 25.0 ± 9.9; p < 0.01) were significantly higher in patients with NC. The mean pain interference in life activities calculated from the Brief Pain Inventory (BPI) was significantly higher in patients with chronic pain than in patients without it (6.8 ± 1.9 vs 5.9 ± 1.9, p < 0.05). This score was also significantly higher in patients with NC than in those without such a feature (7.2 ± 1.5 vs 6.1 ± 1.9, p < 0.05).

Conclusions

There exists a specific chronic pain condition associated to CHIKV. Pain with NC seems to be associated with more aggressive clinical picture, more intense impact in QoL and more challenging pharmacological treatment.</p
