    The impact of the physical environment on depressive symptoms of older residents living in care homes : a mixed methods study

    Background and Objectives: Forty percent of residents living in care homes in the United Kingdom have significant depressive symptoms. Care homes can appear to be depressing places, but whether the physical environment of homes directly affects depression in care home residents is unknown. This study explores the relationship between the physical environment and depressive symptoms of older people living in care homes. Research Design and Methods: In a prospective cohort study the physical environment of 50 care homes were measured using the Sheffield Care Environment Assessment Matrix (SCEAM) and depressive symptoms of 510 residents measured using the Geriatric Depression Scale (GDS-15). The study was supplemented with semi-structured interviews with residents living in the care homes. Quantitative data were analyzed using multi-level modeling, and qualitative data analyzed using a thematic framework approach. Results: The overall physical environment of care homes (overall SCEAM score) did not predict depressive symptoms. Controlling for dependency, social engagement, and home type, having access to outdoor space was the only environmental variable to significantly predict depressive symptoms. Residents interviewed reported that access to outdoor space was restricted in many ways: locked doors, uneven foot paths, steep steps, and needing permission or assistance to go outside. Discussion and Implications: We provide new evidence to suggest that access to outdoor space predicts depressive symptoms in older people living in care home. Interventions aimed at increasing access to outdoor spaces could positively affect depressive symptoms in older people

    Mandibular fractures in short-finned pilot whales, \u3ci\u3eGlobicephala macrorhynchus\u3c/i\u3e

    This study’s objective was to investigate mandibular fractures in 50 short-finned pilot whales, Globicephala macrorhynchus, from two mass strandings. Based on current theories that this species is sexually dimorphic and polygynous, hypotheses were: (1) males should suffer more frequent or more substantial mandibular fractures than should females, and (2) fracture occurrence should increase with male reproductive maturity and potential correlates of maturity, such as age and length. Fractures were described and correlated with physical characteristics to infer possible explanations for injuries. Mandibular fractures were surprisingly common in males and females, being found in more than half of the animals examined (27/50, or 54% overall; 17/36 or 47% of females and 10/14 or 71% of males). Length was the only correlate of fracture presence; the proportion of animals showing evidence of fracture increased with length. These results offer some support to initial hypotheses, but there must be another set of consequences that contribute to mandibular fractures in females. A combination of intra- and interspecific interactions and life history characteristics may be responsible for fractures. Further research from a larger sample of this and other cetacean species are suggested to help elucidate both the causes and implications of mandibular fractures

    Ab Initio No Core Shell Model - Recent Results and Further Prospects

    There has been significant recent progress in solving the long-standing problems of how nuclear shell structure and collective motion emerge from underlying microscopic inter-nucleon interactions. We review a selection of recent significant results within the ab initio No Core Shell Model (NCSM) closely tied to three major factors enabling this progress: (1) improved nuclear interactions that accurately describe the experimental two-nucleon and three-nucleon interaction data; (2) advances in algorithms to simulate the quantum many-body problem with strong interactions; and (3) continued rapid development of high-performance computers now capable of performing 20×101520 \times 10^{15} floating point operations per second.     Measuring coordination between women's self-help groups and local health systems in rural India:A social network analysis

    Objectives: To assess how the health coordination and emergency referral networks between women\u27s self-help groups (SHGs) and local health systems have changed over the course of a 2-year learning phase of the Uttar Pradesh Community Mobilization Project, India. Design: A pretest, post-test programme evaluation using social network survey to analyse changes in network structure and connectivity between key individuals and groups. Setting: The study was conducted in 18 villages located in three districts in Uttar Pradesh, India. Intervention: To improve linkages and coordination between SHGs and government health providers by building capacity in leadership, management and community mobilisation skills of the SHG federation. Participants: A purposeful sampling that met inclusion criteria. 316 respondents at baseline and 280 respondents at endline, including SHG members, village-level and block-level government health workers, and other key members of the community (traditional birth attendants, drug sellers, unqualified rural medical providers, pradhans or elected village heads, and religious leaders). Main outcome measures: Social network analysis measured degree centrality, density and centralisation to assess changes in health services coordination networks at the village and block levels. Results: The health services coordination and emergency referral networks increased in density and the number of connections between respondents as measured by average degree centrality have increased, along with more diversity of interaction between groups. The network expanded relationships at the village and block levels, reflecting the rise of bridging social capital. The accredited social health activist, a village health worker, occupied the central position in the network, and her role expanded to sharing information and coordinating services with the SHG members. Conclusions: The creation of new partnerships between traditionally under-represented communities and local government can serve as vehicle for building social capital that can lead to a more accountable and accessible community health delivery system

    Hubble Space Telescope NICMOS Observations of T Dwarfs: Brown Dwarf Multiplicity and New Probes of the L/T Transition

    We present the results of a Hubble Space Telescope NICMOS imaging survey of 22 T-type field brown dwarfs. Five are resolved as binary systems with angular separations of 0"05-0"35, and companionship is established on the basis of component F110W-F170M colors (indicative of CH4 absorption) and low probabilities of background contamination. Prior ground-based observations show 2MASS 1553+1532AB to be a common proper motion binary. The properties of these systems - low multiplicity fraction (11[+7][-3]% resolved, as corrected for sample selection baises), close projected separations (a = 1.8-5.0 AU) and near-unity mass ratios - are consistent with previous results for field brown dwarf binaries. Three of the binaries have components that span the poorly-understood transition between L dwarfs and T dwarfs. Spectral decomposition analysis of one of these, SDSS 1021-0304AB, reveals a peculiar flux reversal between its components, as its T5 secondary is ~30% brighter at 1.05 and 1.27 micron than its T1 primary. This system, 2MASS 0518-2828AB and SDSS 1534+1615AB all demonstrate that the J-band brightening observed between late-type L to mid-type T dwarfs is an intrinsic feature of this spectral transition, albeit less pronounced than previously surmised. We also find that the resolved binary fraction of L7 to T3.5 dwarfs is twice that of other L and T dwarfs, an anomaly that can be explained by a relatively rapid evolution of brown dwarfs through the L/T transition, perhaps driven by dynamic (nonequilibrium) depletion of photospheric condensates.Comment: ~40 pages, 17 figures, accepted for publication to ApJ.     Capsule endoscopy compatible fluorescence imager demonstrated using bowel cancer tumours

    We demonstrate a proof of concept highly miniaturised fluorescence imager and its application to detecting cancer in resected human colon cancer tissues. Fluorescence imaging modalities have already been successfully implemented in traditional endoscopy. However, the procedure still causes discomfort and requires sedation. Wireless fluorescence capsule endoscopy has the potential to improve diagnostic accuracy with less inconvenience for patients. In this paper we present a 5 mm x 6 mm x 5 mm optical block that is small enough to integrate into a capsule endoscope. The block integrates ultrathin filters for optical isolation and was successfully integrated with a sensitive CMOS SPAD array to detect green fluorescence from Flavin Adenine Dinucleotide (FAD), which is an endogenous fluorophore responsible for autofluorescence in human tissues, and fluorescence from the cancer selective molecular probe ProteoGREENTM-gGlu used to label colorectal cancer cells. In vitro studies were validated using a commercial ModulusTM Microplate reader. The potential use of the device in capsule endoscopy was further validated by imaging healthy and malignant resected human tissues from the colon to detect changes in autofluorescence signal that are crucial for cancer diagnosis

    Stimulus-evoked high frequency oscillations are present in neuronal networks on microelectrode arrays

    Pathological high frequency oscillations (250–600 Hz) are present in the brains of epileptic animals and humans. The etiology of these oscillations and how they contribute to the diseased state remains unclear. This work identifies the presence of microstimulation-evoked high frequency oscillations (250–400 Hz) in dissociated neuronal networks cultured on microelectrode arrays (MEAs). Oscillations are more apparent with higher stimulus voltages. As with in vivo studies, activity is isolated to a single electrode, however, the MEA provides improved spatial resolution with no spread of the oscillation to adjacent electrodes 200 μm away. Oscillations develop across four weeks in vitro. Oscillations still occur in the presence of tetrodotoxin and synaptic blockers, and they cause no apparent disruption in the ability of oscillation-presenting electrodes to elicit directly evoked action potentials (dAPs) or promote the spread of synaptic activity throughout the culture. Chelating calcium with ethylene glycol tetraacetic acid (EGTA) causes a temporal prolongation of the oscillation. Finally, carbenoxolone significantly reduces or eliminates the high frequency oscillations. Gap junctions may play a significant role in maintaining the oscillation given the inhibitory effect of carbenoxolone, the propagating effect of reduced calcium conditions and the isolated nature of the activity as demonstrated in previous studies. This is the first demonstration of stimulus-evoked high frequency oscillations in dissociated cultures. Unlike current models that rely on complex in vivo recording conditions, this work presents a simple controllable model in neuronal cultures on MEAs to further investigate how the oscillations occur at the molecular level and how they may contribute to the pathophysiology of disease
