Experiences following cataract surgery - patient perspectives
PURPOSE: Most patients report being highly satisfied with the outcome of cataract surgery but there are variable reports regarding the impact of cataract surgery on some real-world activities, such as fall rates. We hypothesised that adaptations to changed refractive correction and visual function may cause difficulties in undertaking everyday activities for some patients and used a series of focus groups to explore this issue. METHOD: Qualitative methods were used to explore patients' experiences of their vision following cataract surgery, including adaptation to vision changes and their post-surgical spectacle prescription. Twenty-six participants took part in five focus groups (Mean age = 68.2 ± 11.4 years), and the data were analysed using thematic analysis. RESULTS: We identified three themes. 'Changes to Vision' explores participants' adaptation following cataract surgery. While several had problems with tasks relying on binocular vision, few found them bothersome and they resolved following second eye surgery. Participants described a trial and error approach to solving these problems rather than applying solutions suggested by their eyecare professionals. 'Prescription Restrictions' describes the long-term vision problems that pre-surgery myopic patients experienced as a consequence of becoming emmetropic following surgery and thus needing spectacles for reading and other close work activities, which they did not need before surgery. Very few reported that they had the information or time to make a decision regarding their post-operative correction. 'Information Needs' describes participant's responses to the post-surgical information they were given, and the unmet information need regarding when they can drive following surgery. CONCLUSION: The findings highlight the need for clinicians to provide information on adaptation effects, assist patients to select the refractive outcome that best suits their lifestyle, and provide clear advice about when patients can start driving again. Patients need to be provided with better guidance from clinicians and prescribing guidelines for clinicians would be beneficial, particularly for the period between first- and second-eye surgery
Effects of invisible particle emission on global inclusive variables at hadron colliders
We examine the effects of invisible particle emission in conjunction with QCD
initial state radiation (ISR) on quantities designed to probe the mass scale of
new physics at hadron colliders, which involve longitudinal as well as
transverse final-state momenta. This is an extension of our previous treatment,
arXiv:0903.2013, of the effects of ISR on global inclusive variables. We
present resummed results on the visible invariant mass distribution and compare
them to parton-level Monte Carlo results for top quark and gluino
pair-production at the LHC. There is good agreement as long as the visible
pseudorapidity interval is large enough (eta ~ 3). The effect of invisible
particle emission is small in the case of top pair production but substantial
for gluino pair production. This is due mainly to the larger mass of the
intermediate particles in gluino decay (squarks rather than W-bosons). We also
show Monte Carlo modelling of the effects of hadronization and the underlying
event. The effect of the underlying event is large but may be approximately

Resummation of transverse energy in vector boson and Higgs boson production at hadron colliders
We compute the resummed hadronic transverse energy (E_T) distribution due to
initial-state QCD radiation in vector boson and Higgs boson production at
hadron colliders. The resummed exponent, parton distributions and coefficient
functions are treated consistently to next-to-leading order. The results are
matched to fixed-order calculations at large E_T and compared with
parton-shower Monte Carlo predictions at Tevatron and LHC energies.Comment: 24 pages, 15 figure
Amblyopia and quality of life: a systematic review
Background/Aims
Amblyopia is a common condition which can affect up to 5% of the general population. The health-related quality of life (HRQoL) implications of amblyopia and/or its treatment have been explored in the literature.
Methods
A systematic literature search was undertaken (16th-30th January 2007) to identify the HRQoL implications of amblyopia and/or its treatment.
Results
A total of 25 papers were included in the literature review. The HRQoL implications of amblyopia related specifically to amblyopia treatment, rather than the condition itself. These included the impact upon family life; social interactions; difficulties undertaking daily activities; and feelings and behaviour. The identified studies adopted a number of methodologies. The study populations included; children with the condition; parents of children with amblyopia; and adults who had undertaken amblyopia treatment as a child. Some studies developed their own measures of HRQoL, and others determined HRQoL through proxy measures.
Conclusions
The reported findings of the HRQoL implications are of importance when considering the management of cases of amblyopia. Further research is required to assess the immediate and long-term effects of amblyopia and/or its treatment upon HRQoL using a more standardised approach
Very Cold Gas and Dark Matter
We have recently proposed a new candidate for baryonic dark matter: very cold
molecular gas, in near-isothermal equilibrium with the cosmic background
radiation at 2.73 K. The cold gas, of quasi-primordial abundances, is condensed
in a fractal structure, resembling the hierarchical structure of the detected
interstellar medium.
We present some perspectives of detecting this very cold gas, either directly
or indirectly. The H molecule has an "ultrafine" structure, due to the
interaction between the rotation-induced magnetic moment and the nuclear spins.
But the lines fall in the km domain, and are very weak. The best opportunity
might be the UV absorption of H in front of quasars. The unexpected cold
dust component, revealed by the COBE/FIRAS submillimetric results, could also
be due to this very cold H gas, through collision-induced radiation, or
solid H grains or snowflakes. The -ray distribution, much more
radially extended than the supernovae at the origin of cosmic rays

QCD Coherence and the Top Quark Asymmetry
Coherent QCD radiation in the hadroproduction of top quark pairs leads to a
forward--backward asymmetry that grows more negative with increasing transverse
momentum of the pair. This feature is present in Monte Carlo event generators
with coherent parton showering, even though the production process is treated
at leading order and has no intrinsic asymmetry before showering. In addition,
depending on the treatment of recoils, showering can produce a positive
contribution to the inclusive asymmetry. We explain the origin of these
features, compare them in fixed-order calculations and the Herwig++, Pythia and
Sherpa event generators, and discuss their implications.Comment: 28 pages, 11 figures, 2 table
Diffuse Gamma Rays: Galactic and Extragalactic Diffuse Emission
"Diffuse" gamma rays consist of several components: truly diffuse emission
from the interstellar medium, the extragalactic background, whose origin is not
firmly established yet, and the contribution from unresolved and faint Galactic
point sources. One approach to unravel these components is to study the diffuse
emission from the interstellar medium, which traces the interactions of high
energy particles with interstellar gas and radiation fields. Because of its
origin such emission is potentially able to reveal much about the sources and
propagation of cosmic rays. The extragalactic background, if reliably
determined, can be used in cosmological and blazar studies. Studying the
derived "average" spectrum of faint Galactic sources may be able to give a clue

Strategic planning as the intentional production of a “Trading Zone”
For a long period of time there has been a kind of mirroring between the development of decision making models in Political science and the concepts and paradigms used in Planning theory. The dialogue has been interrupted when the "Garbage Can" model was proposed by Choen, March and Olsen in 1972: a model that emphasized the irreducible complexity of the policy processes. The article reconstructs this relationship and its evolution showing the influence of decision-making models on planning theory, and showing the difficulties of managing complexity for planners. The paper holds that interpreting planning processes through the lens of the "Trading zone" concept proposed by Peter Galison in the field of history of science could be an interesting way of dealing with the extreme complexity of contemporary planning problems. The conclusion is that the use of the Trading Zone concept is very promising to solve some dilemmas of planning theory and that it is particularly useful if we employ it in the growing area of strategic spatial planning
Acidic microenvironment plays a key role in human melanoma progression through a sustained exosome mediated transfer of clinically relevant metastatic molecules
Background: Microenvironment cues involved in melanoma progression are largely unknown. Melanoma is highly influenced in its aggressive phenotype by the changes it determinates in its microenvironment, such as pH decrease, in turn influencing cancer cell invasiveness, progression and tissue remodelling through an abundant secretion of exosomes, dictating cancer strategy to the whole host. A role of exosomes in driving melanoma progression under microenvironmental acidity was never described. Methods: We studied four differently staged human melanoma lines, reflecting melanoma progression, under microenvironmental acidic pHs pressure ranging between pH 6.0-6.7. To estimate exosome secretion as a function of tumor stage and environmental pH, we applied a technique to generate native fluorescent exosomes characterized by vesicles integrity, size, density, markers expression, and quantifiable by direct FACS analysis. Functional roles of exosomes were tested in migration and invasion tests. Then we performed a comparative proteomic analysis of acid versus control exosomes to elucidate a specific signature involved in melanoma progression. Results: We found that metastatic melanoma secretes a higher exosome amount than primary melanoma, and that acidic pH increases exosome secretion when melanoma is in an intermediate stage, i.e. metastatic non-invasive. We were thus able to show that acidic pH influences the intercellular cross-talk mediated by exosomes. In fact when exposed to exosomes produced in an acidic medium, pH naïve melanoma cells acquire migratory and invasive capacities likely due to transfer of metastatic exosomal proteins, favoring cell motility and angiogenesis. A Prognoscan-based meta-analysis study of proteins enriched in acidic exosomes, identified 11 genes (HRAS, GANAB, CFL2, HSP90B1, HSP90AB1, GSN, HSPA1L, NRAS, HSPA5, TIMP3, HYOU1), significantly correlating with poor prognosis, whose high expression was in part confirmed in bioptic samples of lymph node metastases. Conclusions: A crucial step of melanoma progression does occur at melanoma intermediate -stage, when extracellular acidic pH induces an abundant release and intra-tumoral uptake of exosomes. Such exosomes are endowed with pro-invasive molecules of clinical relevance, which may provide a signature of melanoma advancement
